The early--late onset distinction in DSM-III-R dysthymia.

DSM-III-R recently introduced early-late and primary-secondary subtypes of dysthymia. The present study explored the validity of the DSM-III-R early-late onset distinction by comparing early- and late-onset primary dysthymics on demographic, clinical and familial variables and short-term outcome. Compared to the late-onset dysthymics, the early-onset group had higher lifetime rates of superimposed major depressive episodes and anxiety disorders, had sought treatment significantly more frequently, had a higher rate of major affective disorders in first-degree relatives, and exhibited higher lev

A comparison of the familiality of chronic depression in recurrent early-onset depression pedigrees using different definitions of chronicity.

BACKGROUND: The study of chronicity in the course of major depression has been complicated by varying definitions of this illness feature. Because familial clustering is one component of diagnostic validity we compared family clustering of chronicity as defined in the DSM-IV to that of chronicity determined by an assessment of lifetime course of depressive illness. METHODS: In 1750 affected subjects from 652 families recruited for a genetic study of recurrent, early-onset depression, we applied several definitions of chronicity. Odds ratios were determined for the likelihood of chronicity in

Subtypes of depression among patients with Alzheimer's disease and other dementias.

OBJECTIVES: We compared the prevalence of subtypes of depression in patients with Alzheimer's disease (AD), vascular dementia (VaD), and unspecified dementia (UD). METHODS: Using the Integrated Healthcare Information Services database, we conducted an analysis of subtypes of depression (major depressive disorder, depressive disorder not otherwise specified, dysthymic disorder; depressive psychosis, and adjustment disorder depressive) among patients with AD, VaD, and UD. Six thousand four hundred and forty patients aged 60 years or older with dementia (2947 with AD, 725 with VaD, and 2768 with

The clinical spectrum of so-called "minor" depressions.

Reviewing the recent literature on the overlapping spectrum of neurotic depressive and dysthymic conditions--unofficially referred to as "minor," "atypical" or "characterologic" depressives--the authors conclude that significant symptomatologic admixtures with anxiety disorders do not represent the prototypical features of these disorders as defined in DSM-III-R. It is long-standing anhedonia on an intermittent or chronic basis which appears to characterize the trait illness of dysthymia. The emerging data on dysthymia suggest that it begins early in life, is often complicated by major depress

Quality-of-life impairment in depressive and anxiety disorders.

OBJECTIVE: Previous reports demonstrating quality-of-life impairment in anxiety and affective disorders have relied upon epidemiological samples or relatively small clinical studies. Administration of the same quality-of-life scale, the Quality of Life Enjoyment and Satisfaction Questionnaire, to subjects entering multiple large-scale trials for depression and anxiety disorders allowed us to compare the impact of these disorders on quality of life. METHOD: Baseline Quality of Life Enjoyment and Satisfaction Questionnaire, demographic, and clinical data from 11 treatment trials, including stud

[The concept of dysthymia. Its clinical usefulness and nosological status].

Dysthymia is currently conceived as an independent diagnostic category in the area of mood disorders, but the concept is controversial, both from a theoretical and clinical point of view. This article reviews the concept of dysthymia as well as its history. The clinical usefulness and validity are examined, taking into special consideration the influential classificatory notions of DSM-IV.

Discriminant validity of the General Behavior Inventory: an outpatient study.

Recent studies have provided strong support for the convergent validity of the General Behavior Inventory (GBI), a case identification inventory for chronic subsyndromal affective disorders (cyclothymia and dysthymia). Fewer data are available, however, on the ability of the GBI to distinguish chronic subsyndromal affective disorders from other forms of psychopathology. In order to address this issue, outpatients with cyclothymia (n = 9), dysthymia (n = 26), nonchronic major depression (n = 16), and nonaffective psychiatric disorders (n = 30) were compared on the GBI. Diagnoses were derived bl

Dysthymia and personality.

Data on dysthymia and personality presented here are derived from the Zurich cohort study, which followed 591 subjects from the ages of 20 to 35. From childhood on, dysthymics experienced themselves as far more anxious and less self-confident than did major depressives or controls. They manifested more behavioural problems and high neuroticism and introversion scores. Dysthymics scored low in terms of quality of life, self-esteem and mastery, whereas they scored high in avoiding/denying behaviours. The hypothesis is advanced that behavioural problems, anxiety, high levels of neuroticism and pe

Differential associations of specific depressive and anxiety disorders with somatic symptoms.

OBJECTIVE: Previous studies have shown that depressive and anxiety disorders are strongly related to somatic symptoms, but much is unclear about the specificity of this association. This study examines the associations of specific depressive and anxiety disorders with somatic symptoms, and whether these associations are independent of comorbid depressive and anxiety disorders. METHODS: Cross-sectional data were derived from The Netherlands Study of Depression and Anxiety (NESDA). A total of 2008 persons (mean age: 41.6 years, 64.9% women) were included, consisting of 1367 patients with a past

Systemic couple therapy for dysthymia.

We examined the effect of Systemic Couple Therapy on a patient diagnosed with dysthymic disorder and her partner. Marge and Peter, a middle-aged married couple, showed significant and meaningful changes in their pattern of interaction over the course of the therapy and, by the end of it, Marge no longer met the diagnostic criteria for dysthymic disorder. Her scores on the Structured Clinical Interview for DSM-IV Axis I Disorders (SCID-I) and Beck Depression Inventory, Second Edition (BDI-II) were in the clinical range before treatment and in the nonclinical one at the end of therapy. A family history study of binge-eating disorder.

Family studies of anorexia nervosa and bulimia nervosa have yielded important information about the etiologies of these eating disorders. By contrast, little is known about familial factors of etiologic importance for binge-eating disorder (BED). The purpose of the current family history study was to assess the prevalence of comorbid psychopathology in a non-treatment seeking female sample of 31 probands with BED, 32 control probands without BED, and their 283 first-degree relatives. In-person semistructured clinical interviews were conducted with the probands, who also served as informants fo

[Dexamethasone suppression test in psychiatric pathology].

The dexamethasone suppression test (DST) has been used in psychiatric pathology for about 10 years. Carroll et al. consider this test to be specific of endogenous depression. According to these, and many other authors, approximately 55% of patients with endogenous depression show a positive response to the test, whereas a positive response is observed in only 4% of normal subjects or patients with psychiatric disorders other than major depressive disorders. The DST was performed in 162 psychiatric inpatients (5 with organic disease, 28 with schizophrenic disorders, 17 with major affective diso

Clinical and EEG sleep changes in primary dysthymia and generalized anxiety: a comparison with normal controls.

All night EEG sleep recordings and clinical assessments of sleep quality were performed in normal controls, patients with generalized anxiety disorder and primary dysthymia. Patients were selected according to DSM-III R. Changes of sleep architecture, namely a reduction of slow wave sleep, are similar in generalized anxiety and dysthymia. Also the two groups do not exhibit the REM sleep disturbances usually observed in affective illness. Duration and continuity measures are unchanged in dysthymics, but anxious patients show some features of insomnia. The analysis of subjective aspects of sleep

Frequency of mental disorders among chronic pain patients with or without fibromyalgia in Japan.

AIM: To explore the characteristics of psychiatric morbidity in chronic pain patients who present with or without fibromyalgia. METHODS: Patients are referred to our chronic pain clinic from primary medical institutions, as we are a secondary medical institution. Although some patients have chronic pain, they have no clear organic disorder such as rheumatoid arthritis to account for the pain. Among the 367 new patients seen during the period from March 2009 to August 2012, 347 patients underwent psychiatric evaluation in face-to-face interviews with mental health specialists before a physical

Social adjustment in dysthymia, double depression and episodic major depression.

We contrasted the overall social functioning of pure dysthymics, double depressives, episodic major depressives and normal controls using both interview and self-report measures of social functioning and depression. In addition, we used hierarchical multiple regression to assess the differential impact of several variables (comorbid personality, anxiety and substance use disorders, life stress, duration of dysthymia and severity of depressive symptomatology) on social functioning in the dysthymics and double depressives. Participants included 41 outpatients with early-onset dysthymia alone, 56

Comparison of a community dysthymia sample at screening with a matched group of nondepressed community controls.

The primary goal of the study was to compare the psychosocial functioning of an untreated, community sample of DSM-III-R dysthymia subjects (N = 24) at screening to that of a matched sample of community nondepressed volunteers (N = 18) with no lifetime or current history of axis I disorders. Subjects were compared across a number of psychosocial indices. The dysthymics were found to be less sociable (introversion) and clinically high on neuroticism-instability, external in regard to their causal attributions, less stable for positive uncontrollable events and more stable and global for negativ
