Early intervention in eating disorders: introducing the chronopathogram.

Eating disorders (EDs) pose significant challenges to mental and physical health, particularly among adolescents and young adults, with the COVID-19 pandemic exacerbating risk factors. Despite advancements in psychosocial and pharmacological treatments, improvements remain limited. Early intervention in EDs, inspired by the model developed for psychosis, emphasizes the importance of timely identification and treatment initiation to improve prognosis. Challenges in identifying prodromal phases and measuring the duration of untreated illness highlight the complexity of early intervention efforts

Development of an early psychosis intervention system in Korea: focus on the continuing care system for first-episode psychosis treatment in Seoul.

Providing intensive psychosocial intervention within the 5-year critical period following the first psychotic episode is important for both symptomatic and functional recovery. Recently, community mental health centres in Korea have begun to shift their main roles from care of those with chronic schizophrenia to early detection of and interventions for those with first-episode psychosis. This pioneering approach was initiated by the Seoul Mental Health Center, which established a community network, formed a clinical consortium with hospitals and clinics, and developed guidelines for early psyc

Hunter Syndrome: Is It Time to Make It Part of Newborn Screening?

BACKGROUND: Hunter syndrome, or mucopolysaccharidosis type II (MPS II), is a lysosomal storage disease that affects the breakdown of sugar in the body. Research has made it possible to reveal the cause of the disease, thus helping diagnose and treating this rare disorder. Enzyme replacement therapy will help children live longer and healthier lives. PURPOSE: The purpose of this literature review is to explore the existing knowledge on MPS II and to inform the nursing community about the prevalence of MPS II and examine why it is important to offer screening to parents of a newborn or child.

[Tokyo Metropolitan Dementia-related Disease Medical Center and Current Problems of Outreach project for Early-stage Dementia].

There are twelve dementia-related disease medical centers including psychiatric hospitals in the Tokyo Metropolitan Area. Few psychiatric hospitals or long-term care hospitals exist in our area (Bunkyo, Chiyoda, Taito, Minato, and Chuo wards) and we receive requests for hospital transfer of demented patients. Since the Tokyo Metropolitan Government estimates that the numbers of aged persons will increase rapidly, a project for detecting and diagnosing early-stage dementias, the 'outreach project', has started. We visit people who show some cognitive symptoms and evaluate their cognitive functi

Fecal impaction: a systematic review of its medical complications.

BACKGROUND: Fecal impaction (FI) is a common problem in the elderly and other at-risk groups, such as patients with a neuro-psychiatric disease. It has been associated with medical problems and high morbi-mortality. A systematic review of this topic might be useful to improve the knowledge in this area and helpful to make an appropriate and early diagnosis. METHODS: A PubMed systematic search was performed using relevant keywords. Case reports published in English, Spanish or French till June 2014 were included if they had a diagnosis of FI and a medical complication secondary to it. Each cas

Sleep-disordered breathing and atrial fibrillation: review of the evidence.

Sleep-disordered breathing and atrial fibrillation are two common medical conditions. A strong association between them has been described and has prompted significant research in understanding the mechanism connecting the two conditions while explaining the synergic negative effects in terms of morbidity and mortality in affected patients. This review focuses first on the pathophysiologic mechanisms favoring the development of atrial fibrillation in patients with sleep-disordered breathing, considering specifically and separately obstructive sleep apnea and central sleep apnea. Then, the effe

[Microscopic hematuria : Reasonable and risk-adapted diagnostic evaluation].

BACKGROUND: Microscopic hematuria that is not explained by an obvious underlying condition is a frequent and often an incidental finding that commonly triggers urological or nephrological evaluation. Potential underlying conditions range from benign to severe malignant diseases of the kidneys and urinary tract. MATERIALS AND METHODS: A nonsystematic literature search was performed, focusing on potential urological and nephrological causes of hematuria. National and international guidelines were considered and diagnostic as well as follow-up strategies are discussed. We provide a recommendatio

Therapeutic Landscape of Early Symptomatic Alzheimer's Disease Translated into Everyday Practice for Geriatric Providers.

Alzheimer's disease (AD) accounts for 60-80% of all dementia cases. Recent advances in diagnostic biomarkers of early symptomatic AD (ie, mild cognitive impairment and mild dementia due to AD) and amyloid-targeting therapies (ATTs) have the potential to improve outcomes for patients with AD. Two ATTs (donanemab and lecanemab) are currently approved and available for use in the US. Both ATTs can slow disease progression as well as cognitive and functional decline in patients with early symptomatic AD. Treatment with ATTs is associated with specific safety concerns such as amyloid-related imagin

Early intervention for schizophrenia: a pathway to improved clinical outcomes.

Roger was a 60-year-old man living with both HIV and schizophrenia who was admitted to the hospital for treatment of a chronic obstructive pulmonary disease exacerbation. He was referred to the psychiatry consultation-liaison team due to persistent psychotic symptoms that had not responded to multiple antipsychotic trials. Roger's psychiatric history revealed a diagnosis of schizophrenia in early adulthood, marked by hallucinations and delusions of grandeur. Over the next 4 decades, he cycled through jails, prisons, shelters, and periods of homelessness. Though intermittently connected with ou

Secondary peritonitis: principles of diagnosis and intervention.

Secondary peritonitis accounts for 1% of urgent or emergent hospital admissions and is the second leading cause of sepsis in patients in intensive care units globally. Overall mortality is 6%, but mortality rises to 35% in patients who develop severe sepsis. Despite the dramatic growth in the availability and use of imaging and laboratory tests, the rapid diagnosis and early management of peritonitis remains a challenge for physicians in emergency medicine, surgery, and critical care. In this article, we review the pathophysiology of peritonitis and its potential progression to sepsis, discuss

Summary of Evidence on Early Carotid Intervention for Recently Symptomatic Stenosis Based on Meta-Analysis of Current Risks.

BACKGROUND AND PURPOSE: This study aimed to assess the evidence on the periprocedural (<30 days) risks of carotid intervention in relation to timing of procedure in patients with recently symptomatic carotid stenosis. METHODS: A systematic literature review of studies published in the past 8 years reporting periprocedural stroke/death after carotid endarterectomy (CEA) and carotid stenting (CAS) related to the time between qualifying neurological symptoms and intervention was performed. Pooled estimates of periprocedural risk for patients treated within 0 to 48 hours, 0 to 7 days, and 0 to 15

Early intervention in thyroidectomy scars: demographics, symptoms, and prevention.

OBJECTIVE: Although hypertrophic scars are cosmetically problematic for patients following thyroidectomy, the associated risk factors are not well defined. Our objective was to determine the factors associated with hypertrophic scar development following thyroidectomy. METHOD: A retrospective chart review was performed collecting data on sex, age, body mass index (BMI), operation site, skin characteristics (pigmentation, erythema, elasticity, and hydration), and clinical scar characteristics (itching, tightening, induration, adhesion, and oedema). It was also noted if the patient had early sc

[Fast treatment of septic shock saves lives. Although "early goal-directed therapy"-protocol does not show any effect in new studies].

Three recent randomised trials have shown a lack of effect of EGDT for the treatment of severe sepsis and septic shock. Notably, control group care was excellent in all trials, including early recognition, early and aggressive fluid resuscitation and prompt administration of antibiotics, and was reflected in a lower than expected mortality. The discussion emphasizes the need for continued vigilance and early aggressive management.

Management Issues in Intensive Care Units for Infants and Children with Heart Disease.

Admission of infants and children with cardiac disease to the neonatal (NICU) and pediatric ICU (PICU) is ever increasing in India (30-50 % of all admissions). The commonest indication for admission to the NICU or PICU is acute deterioration of cardiac disease. This includes: acute heart failure, hypercyanotic spells, arrhythmias, pericardial tamponade and sick cardiac neonates who need urgent intervention. Other increasingly frequent indications for ICU admission include heart failure with concomitant chest infection and impending respiratory failure and, severe cyanotic heart disease with va

Effect of early initiation of dialysis on cardiac structure and function: results from the echo substudy of the IDEAL trial.

BACKGROUND: Abnormalities of cardiac structure and function are common in patients undergoing dialysis, and cardiovascular disease is the major cause of mortality in this group. Heart failure is a common clinical manifestation of cardiovascular disease and is preceded by left ventricular hypertrophy (LVH). There are variable reports about the impact of dialysis on LVH, both deleterious and beneficial. Our study investigated whether the timing of the initiation of dialysis therapy had an impact on cardiac structure and function. STUDY DESIGN: Randomized controlled trial. SETTING & PARTICIPANT

Exploring the Feasibility of a Generic Early Intervention Programme for Emerging Personality Disorder in Adolescents: Guideline-Informed Treatment for Personality Disorders-Adolescent Version (GITPD-A).

The Guideline-Informed Treatment for Personality Disorders-Adolescent version (GITPD-A) is an accessible, general approach to treating personality disorders (PDs) in adolescents. This study assessed the feasibility of the intervention and of a multicentre research design, as a preliminary step towards larger, better-controlled trials. A one-group pretest-posttest design was used across five mental healthcare institutions. Eligible participants were adolescents aged 12-18 meeting threshold criteria for a PD under the DSM-5 alternative model. Participants (91.9% female; mean age 15.9 years) rece
