Total elbow arthroplasty for the treatment of distal humeral fractures.

OBJECTIVE: To report the clinical-functional outcomes of the treatment of humeral distal fractures with a total elbow prosthesis. MATERIAL AND METHODS: This retrospective study was performed in two surgical centres. A total of 23patients were included, with a mean age of 79years, and of which 21 were women. The inclusion criteria were: patients with humeral distal fractures, operated on using a Coonrad-Morrey prosthesis, and with a follow-up of more than one year. According to AO classification, 15fractures were type C3, 7 C2 and 1 A2. All patients were operated on without de-insertion of the

Aspiration of the elbow joint for treating radial head fractures.

BACKGROUND: Radial head fracture is the most common fracture of the elbow. It usually results from a fall onto an outstretched arm. In 1954, Mason classified these fractures into type 1 (undisplaced), type 2 (simple displaced), and type 3 (comminuted fractures). Aspiration of the elbow joint aims to relieve pressure in the elbow joint and has been used as an initial treatment option for radial head fractures. However, it is an invasive technique with the potential for complications such as infection and injury to nerves and vessels. OBJECTIVES: To assess the effects (benefits and harms) of el

Linked total elbow arthroplasty as treatment of distal humerus fractures.

Treatment of distal humerus fractures may be challenging, especially in the elderly patient. Total elbow replacement has been proposed as an option in selected patients. We present the results of a linked elbow replacement in 16 patients with a comminuted fracture of the distal humerus which was not considered amenable to reliable open reduction and internal fixation. At a mean follow-up of 57 months, average range of motion was from 28 degrees to 117 degrees of flexion-extension. Five patients with moderate to severe pain (31%) were not satisfied with the results of the operation. Three patie

Buried or unburied K-wires for lateral condyle elbow fractures.

INTRODUCTION: Lateral humeral condyle fractures typically require a longer period of internal fixation than other distal humeral fractures due to the increased risk of non-union. K-wires can be buried and left in situ until union or they can be left unburied and require removal after four weeks, with plaster immobilisation until union. There is no consensus as to whether wire burial is preferable or not. The aim of this study was to determine whether K-wire burial is associated with more complications than non-buried wires in treating lateral condyle fractures of the elbow. METHODS: All patie

Elbow arthroplasty for the treatment of bicolumn distal humeral fractures.

Distal humeral fractures are difficult to treat in elderly patients. Recent advances in precontoured and locked plates have made open reduction and internal fixation easier, but rigid and anatomic reconstruction is required for a successful result. In elderly patients with poor bone quality and an unreconstructable articular surface, total elbow arthroplasty can be a successful alternative to open reduction and internal fixation.

[Treatment of medial epicondyle fractures accompanying elbow dislocations in children].

OBJECTIVES: We evaluated the clinical and radiographic results of treatment for medial epicondyle fractures accompanying elbow dislocations in children. METHODS: The study included 10 children (9 boys, 1 girl; mean age 12 years; range 1.5 to 15 years) with medial epicondyle fractures accompanying elbow dislocations. Three patients had posteromedial and seven patients had posterolateral dislocations. Three patients were treated conservatively, while four patients and three patients underwent early and late surgical treatment before or after the seventh day of injury, respectively. Surgery was

The importance of elbow aspiration when treating radial head fractures.

A prospective study of elbow aspiration was performed to assess the effect of this procedure on pain relief and function following Type I and II fractures of the radial head. Aspiration of the haemarthrosis produced dramatic pain relief in 77% of elbows thus treated. Earlier return of function was also noted for these elbows compared to controls. We recommend elbow aspiration for patients with these fractures to both relieve pain and restore early function.

Below- vs above-elbow cast for distal radius fractures: is elbow immobilization really effective for reduction maintenance?

PURPOSE: The choice of the cast length in conservative management of distal radius fractures still represents a debated controversy. Historically, the elbow is immobilized to reduce the risk of secondary displacement; however, short-arm casts are currently felt to be equally effective with less complications and better patient comfort. This paper investigates whether immobilization of the elbow is actually effective in reducing the risk of loss of reduction in conservatively manipulated distal radius fractures. METHODS: We retrospectively studied 297 consecutive patients with distal radius fr

Elbow hemiarthroplasty for unreconstructible distal humerus fractures: a case series.

BACKGROUND: To evaluate the outcomes of distal humerus hemiarthroplasty (DHH) in a series of eight consecutive patients with unreconstructible distal humeral fractures or failed open reduction and internal fixation (ORIF). METHODS: Retrospective data including demographics, postoperative outcomes, and complications were extracted from electronic records at a level 1 trauma center. Surgeries were performed by a single fellowship-trained upper extremity surgeon. The indications for DHH were unreconstructible distal humerus fracture or failed ORIF. Eventually, as accepted by current literature,

Ulna Fractures After Elbow Arthrodesis: A Report of 3 Cases.

CASE: Two surgeons performed 3 elbow arthrodesis at 90° elbow flexion secured with a plate and screws. All 3 patients experienced fracture at the most distal aspect of the posterior plate, consistent with a stress riser in this location. CONCLUSION: The risk of fracture is likely related to vulnerability of the arm with no elbow flexion and may not be ameliorated by changes in operative technique or bone quality. Ulna fracture can be anticipated after elbow arthrodesis and might further limit enthusiasm for elbow arthrodesis.

Current Fixation Options for Elbow, Forearm, Wrist, and Hand Fractures.

The universal goals of upper limb fracture management are to restore anatomic alignment, establish stable fracture fixation (while preserving blood supply), and allow for early mobilization of the elbow, wrist, and digits. However, the indications for operative management and fixation constructs are specific to each fracture type. This article systematically reviews current classifications and treatment options for adult fractures of the distal humerus, radius, ulna, scaphoid, metacarpals, and phalanges. For each anatomic location, we discuss the salient imaging features to guide management de

Pediatric elbow fractures: a new angle on an old topic.

BACKGROUND: The three most common elbow fractures classically reported in pediatric orthopedic literature are supracondylar (50-70%), lateral condylar (17-34%), and medial epicondylar fractures (10%), with fractures of the proximal radius (including but not limited to fractures of the radial neck) being relatively uncommon (5-10%). Our experience at a large children's hospital suggests a different distribution. OBJECTIVE: Our goals were (1) to ascertain the frequency of different elbow fracture types in a large pediatric population, and (2) to determine which fracture types were occult on ini

Elbow arthroscopy for arthrofibrosis after type I radial head fractures.

The purpose of this study was to provide emphasis on recognizing arthrofibrosis as a potential complication of nondisplaced radial head fractures and the role of arthroscopy in its management. This series consists of five patients treated with arthroscopic debridement for arthrofibrosis after type I radial head fractures. Arthroscopy was considered only if the patient's range of motion had plateaued with significant residual loss of motion after an aggressive physical therapy program. Arthroscopy was performed an average of 6 months postinjury, and follow-up averaged 24 months. The primary com

[Elbow fractures in children; how do we minimize the risk of missing a fracture?].

Elbow fractures in children represent a frequent but diagnostically challenging presentation in both primary and secondary care. Due to multiple ossification centers and often subtle clinical signs, these injuries are at risk of being overlooked. Missed fractures may result in malunion, growth disturbances, chronic pain, or long-term functional impairment. This review highlights three commonly underdiagnosed fracture types: medial epicondyle fractures, lateral condyle fractures, and supracondylar humeral fractures. Using illustrative case material, we discuss key aspects of clinical evaluation

Retardation of fracture healing by cerclage wire near the elbow in radius fracture models.

Cerclage wire is widely used in the treatment of fracture internal fixation and is shown effective in clinic. But a report by S.L. has pointed that the wire loop delayed the growth of bone. We have established a radius fracture model to study the possible detrimental effects of cerclage wire on fracture healing and the potential mechanism. By high-resolution CT analysis cerclage wire is found to delay fracture healing, by histological assessment cerclage wire is found to extended the time of hematoma and the marrow cavity appearing, by confocal microscopy cerclage wire decreased the content of
