The Role for Imaging in the Investigation of Isolated Objective Vestibular Weakness.

OBJECTIVE: Unilateral vestibular weakness has considerable potential etiologies. One source is a vestibular schwannoma. This article evaluates, in the absence of other symptoms and signs, if unilateral vestibular weakness is an analogue to asymmetric sensorineural hearing loss and serves as an indication for lateral skull base imaging. STUDY DESIGN: Retrospective chart review. SETTING: Academic tertiary center. SUBJECTS AND METHODS: All patients undergoing caloric assessment between January 1, 2012, and June 30, 2018, were investigated. Patients with unilateral vestibular weakness (a left-r

The assessment of the balance system in cranial artery stenosis.

INTRODUCTION: Vertigo and balance disorders are a significant clinical problem, especially in elderly patients. The narrowing of cranial vessels may be asymptomatic or produce neurological symptoms. Very often nonspecific signs of ischemia occur, such as headache, vertigo, or dizziness. OBJECTIVE: The objective of the study was to assess the effect of carotid and vertebral arteries stenosis on the function of the equilibrium organ on the basis of electronystagmography and posturography. MATERIAL: The study was conducted in 63 patients, presenting with carotid and vertebral arteries stenosis.

Ménière's Disease: Molecular Analysis of Aquaporins 2, 3 and Potassium Channel KCNE1 Genes in Brazilian Patients.

OBJECTIVES: Ménière's disease (MD) is a complex disease of unknown etiology characterized by a symptomatic tetrad of vertigo, hearing loss, tinnitus, and aural fullness. In addition to factors related to homeostasis of the inner ear, genetic factors have been implicated in its pathophysiology, including genes related to the transport of water and ionic composition maintenance of the endolymph, such as the aquaporin genes AQP2 and AQP3, and the potassium channel gene KCNE1. The aim of this study was to identify polymorphisms of these genes and determine their association with clinical character

Refractory episodic vertigo: role of intratympanic gentamicin and vestibular evoked myogenic potentials.

INTRODUCTION: Even today, the treatment of intractable vertigo remains a challenge. Vestibular ablation with intratympanic gentamicin stands as a good alternative in the management of refractory vertigo patients. OBJECTIVE: To control intractable vertigo through complete saccular and horizontal canal vestibular ablation with intratympanic gentamicin treatment. METHODS: Patients with refractory episodic vertigo were included. The inclusion criteria were: unilateral ear disease, moderate to profound sensorineural hearing loss, and failure to other treatments. Included patients underwent 0.5-0.

[The effect of the upper cervical spine on hearing ability].

A chronic irritation in the upper cervical column of 10 rabbits by implantation of a wire string was performed. The vestibular and auditory functions were investigated by electronystagmography and by BERA before and several times after the operation. A distinct increase of the proprioceptive cervical nystagmus was found already in the first postoperative week. In contrary to the ENG findings the BERA did not show any alteration of the both threshold and latency values. These results indicate that the existence of the proprioceptive cervical hearing loss is very doubtful.

Vestibular involvement in Bell's palsy.

Vestibular involvement in the acute phase of idiopathic facial paralysis (Bell's palsy) was observed in 22 percent and 14 patients. These patients were evaluated by history, physical examination and caloric tests with electronystagmography (ENG) employing the Cawthorne-Hallpike technique. Bell's palsy has been considered a mononeuropathy limited to the facial nerve. Evidence of occasional involvement of other cranial nerves (Vth and/or VIIIth) suggests a polyneuropathy. Several theories have been advanced to explain the involvement of the vestibular nerve.

Vestibulospinal function in otoneurology.

The vestibulospinal level of vestibular function is commonly neglected in the evaluation of dizzy patients. Posturography provides a measuring technique for this function (static balance), and thus becomes a counterpart of electronystagmography, and proves to be an indispensable complement in the evaluation of a number of cases. It enables one to make a better assessment of the patient's condition and to obtain decisive indications for his treatment. Detailed analysis of the posturographic recording indicates several patterns of vestibulospinal dysfunction and allows one to evaluate the sensor

Dehydration testing and the diagnosis of Menière's disease: case report.

A 34-year-old female presented to our clinic with complaints of hearing loss, tinnitus, and severe rotational vertigo. A test battery including complete audiometric testing, auditory brainstem response (ABR) audiometry, electronystagmography (ENG), electrocochleography (ECoG), and dehydration testing was performed. Monitoring of hearing sensitivity, speech understanding, and ECoG responses following administration of a dehydrating agent were sensitive measures of cochlear function and confirmed the diagnosis of Meniere's disease. The induced changes also contributed to the therapeutic measures

Sudden deafness and anterior inferior cerebellar artery infarction.

BACKGROUND AND PURPOSE: Acute ischemic stroke in the distribution of the anterior inferior cerebellar artery (AICA) is known to be associated with vertigo, nystagmus, facial weakness, and gait ataxia. Few reports have carefully examined the deafness associated with the AICA infarction. Furthermore, previous neurological reports have not emphasized the inner ear as a localization of sudden deafness. The aim of this study was to investigate the incidence of deafness associated with the AICA infarction and the sites predominantly involved in deafness. METHODS: Over 2 years, we prospectively iden

[Research advances on mechanism and lesion location of vertical nystagmus].

Vertical nystagmus is a vertical nystagmus caused by structural abnormalities and/or dysfunction of the central vestibular system and observed in situ in the center of the eyeball. There are two kinds of nystagmus (UBN) and downbeat nystagmus (DBN) according to the direction of nystagmus. The diagnosis of UBN is mainly made by naked eye or electronystagmography/viewer. It is a common neuro-ophthalmologic sign in the field of vestibular medicine. In this paper, the mechanism of vertical nystagmus formation and the location of lesions were briefly introduced, in order to provide help for the dia

"How I do it"--otology and neurology. A specific issue and its solution. The diagnosis of a posterior fossa tumor.

In summary, the patient with a unilateral sensorineural type hearing loss, unilateral tinnitus, or dysequilibrium may have a tumor in the posterior fossa. This is diagnosed by the history, ear nose, throat and otoneurological examination, the audiological studies, electronystagmography, radiological tests using the Compere unit, the CT Scan, and when indicated the posterior fossa myelogram. The list of causes for these symptoms other than tumefaction is large and there are a significant number of cases that remain "cause undetermined." The latter requires diligent follow-up and even consultati

Effects of noise on the vestibular system of normal-hearing workers.

BACKGROUND: Studies in noise-exposed animals have shown changes in vestibular structures. Likewise, studies in humans have been suggesting that noise can damage the vestibular system, even with normal assessment results. OBJECTIVE: To assess the vestibular system of workers exposed to noise and to compare with individuals not exposed. METHODS: Twenty normal-hearing male adults were divided in the study group (SG), exposed to occupational noise, and control group (CG). We conducted the following procedures: medical history, Dizziness Handicap Inventory (DHI), Dix-Hallpike maneuver, and electr

Transtympanic gentamicin for Meniere's syndrome.

OBJECTIVES: Study the impact of transtympanic gentamicin on patients with unilateral Meniere's syndrome. Partial chemical labyrinthectomy is a relatively recent concept for the treatment of Meniere's syndrome. It uses the ototoxic effect of gentamicin to reduce the symptom of vertigo and maintain cochlear function. STUDY DESIGN: A prospective study using transtympanic gentamicin was begun in January 1994. Patients selected had failed medical therapy, but were not incapacitated. Patients had preinjection audiometric and electronystagmography data. Most had an imaging study. All had one injecti

Vestibular dysfunction in Parkinson disease.

Symptoms and signs of postural instability are often present in patients with Parkinson disease (PD). Because vestibular dysfunction might contribute to the postural instability, we carried out bithermal caloric tests and electronystagmography in 36 patients with PD and 316 controls. Significantly more PD patients had reduced and absent vestibular responses than controls. Decreased or absent vestibular responses in patients with PD were associated with postural instability and increasing severity of other symptoms. Vestibular dysfunction may contribute to the postural instability of PD.

A case of cerebello-pontine angle infarction found by vertebral angiography.

A patient was admitted complaining of sudden vertigo. Otoneurological examinations, electronystagmography (ENG), and vertebral angiography (VAG) showed constriction of vertebral artery and anterior inferior cerebellar artery, and basilar artery obstruction. Based on these findings, we diagnosed the case as posterior cranial fossa infarction. Computed tomography (CT) and magnetic resonance imaging (MRI) findings were negative. The authors conclude that VAG should be performed in certain cases of vertigo thought to be of central nervous origin when CT and MRI findings are both negative.

Head-shaking nystagmus: associations with canal paresis and hearing loss.

In 108 patients referred for caloric testing, head-shaking nystagmus (HSN) was noted in 37 electronystagmography (ENG) recordings. A canal paresis was found in 31 patients and a sensorineural or mixed hearing loss in one or both ears was found in 45 patients. Both HSN and canal paresis were insensitive predictors of either hearing loss or of each other. HSN was less sensitive as well as less specific than canal paresis for hearing loss. These data suggest that HSN is not as powerful a test as canal paresis in detecting lesions of the 8th nerve.

Glycerol dehydration tests in Ménière's disorder using extratympanic electrocochleography.

Eleven patients with classical Ménière's disorder were examined electrophysiologically using extratympanic electrocochleography. All had been fully investigated audiologically and by electronystagmography. In addition to the classical triad of symptoms, 7 complained also of fullness or blockage in the ear, this symptoms being associated with fluctuation of the deafness. All patients showed the characteristic enhancement of the SP component of the AP/SP waveform. Following glycerol the SP component was reduced in amplitude, producing a decrease in the width of the waveform. Maximum reduction oc

Vestibular impairment in a Dutch DFNA15 family with an L289F mutation in POU4F3.

Vestibular examination (electronystagmography with rotatory chair and caloric tests) was performed on 18 carriers and 1 phenocopy carrier in a Dutch family with autosomal dominant nonsyndromic DFNA15. This is the second DFNA15 family worldwide to have a novel L289F mutation in POU4F3. Vestibular involvement appeared to be present in 2 affected individuals according to their medical history. Vestibular examination results in an extended subset of L289F POU4F3 mutation carriers varied from normal to areflexia. DFNA15 is the third cochleovestibular disorder, after DFNA9 and DFNA11, in the autosom

Electronystagmographic investigation in X-linked ocular albinism.

Albinism represents the most frequent cause of nystagmus induced by visual sensory disorders. We studied five patients aged between three and five years with ocular albinism with the aim of identifying the characteristics of nystagmic oscillations. The patients underwent complete ophthalmological examination and electronystagmography (ENG). ENG revealed either pendular or jerk waveforms (or both) with the same complex effects of fixation seen in those with idiopathic congenital nystagmus. Three children presented face turn due to reduced severity of nystagmus in lateroversion. Latent nystagmus
