Clinical management where medicine meets management. All of a rush.

The time between a myocardial infarction patient arriving at hospital and receiving life-saving drugs is decreasing. Schemes to speed up care include paramedics administering thrombolytic drugs. Hospitals are also finding ways of speeding up admission patients.

Toxic trauma.

Hazardous materials (HAZMAT) carry many inherent dangers. Such materials are distributed widely in industrial and military sites. Toxic trauma (TT) denotes the complex of systemic and organ injury caused by toxic agents. Often, TT is associated with other injuries that also require the application of life-support techniques. Rapid onset of acute respiratory failure and consequent cardiovascular failure are of primary concern. Management of TT casualties is dependent upon the characteristics of the toxic agents involved and on the demographics surrounding the HAZMAT incident. Agents that can pr

Cricothyrotomy in Helicopter Emergency Medical Service Transport.

OBJECTIVE: Airway management is a requisite skill set for helicopter emergency medical service (HEMS) providers. Cricothyrotomy is a potentially lifesaving skill that is used when other airway maneuvers fail. The authors reviewed all transports by a helicopter program in which cricothyrotomy was performed to assess the frequency, success, and technique. METHODS: This was a retrospective chart review of air medical patient records from an electronic medical record system over a 112-month period. RESULTS: During the study period, 22,434 patients were transported, 13 (.057%) of whom underwent c

Diagnosing Testicular Torsion before Urological Consultation and Imaging: Validation of the TWIST Score.

PURPOSE: The TWIST (Testicular Workup for Ischemia and Suspected Torsion) score uses urological history and physical examination to assess risk of testis torsion. Parameters include testis swelling (2 points), hard testis (2), absent cremasteric reflex (1), nausea/vomiting (1) and high riding testis (1). While TWIST has been validated when scored by urologists, its diagnostic accuracy among nonurological providers is unknown. We assessed the usefulness of the TWIST score when determined by nonurological nonphysician providers, mirroring emergency room evaluation of acute scrotal pain. MATERIA

A Guided Comparative Analysis of Fatigue Frameworks in Australasian Ambulance Services.

OBJECTIVE: Paramedics work in a complex, unpredictable environment, subject to many external stressors including critically unwell patients, dangerous driving conditions, and prolonged shift work. Paramedic fatigue from these and other occupational demands is well documented. Ambulance services attempt to safeguard paramedics from fatigue using internal policies or procedures - a type of Fatigue Risk Management Systems (FRMSs). This study reviews ambulance service fatigue frameworks to understand the current situation in fatigue management in paramedicine, and to identify fatigue monitoring to

Workplace Violence Against Emergency Medical Services (EMS): A Prospective 12-Month Cohort Study Evaluating Prevalence and Risk Factors Within a Large, Multistate EMS Agency.

OBJECTIVES: To determine the prevalence and associated risk factors of workplace violence (WPV) experienced by emergency medical services (EMS) clinicians across a large, multistate ground/air EMS agency. METHODS: We used a prospective cohort study design from 1 December 2022 to 30 November 2023. A checkbox was added within the electronic medical record (EMR) asking staff to indicate whether WPV occurred. Patient characteristics, encounter (run), and crew factors were abstracted. Potential risk factors for WPV were assessed using logistic regression, with the occurrence of any form of violenc

Paramedic equipment bags: How their position during out-of-hospital cardiopulmonary resuscitation (CPR) affect paramedic ergonomics and performance.

This study investigates how the positions of paramedic equipment bags affect paramedic performance and biomechanical loads during out-of-hospital Cardiopulmonary Resuscitation (CPR). An experiment was conducted in which 12 paramedic teams (each including two paramedics) performed in-situ simulations of a cardiac-arrest scenario. CPR quality was evaluated using five standard resuscitation measures (i.e., pre- and post-shock pauses, and compression rate, depth and fraction). The spinal loads while lifting, pulling and pushing the equipment bags were assessed using digital human modeling software

Factors related to the decision to perform synchronized cardioversion for supraventricular tachyarrhythmias by prehospital emergency medical services workers.

BACKGROUND: While clinical indicators for synchronized cardioversion in regular supraventricular tachycardias are well-established, their application by prehospital emergency medical services (EMS) still needs to be explored. OBJECTIVE: The purpose of this study was to evaluate the factors influencing the decision-making process of prehospital EMS personnel to perform synchronized cardioversion in adults with narrow QRS regular tachyarrhythmias. METHODS: This descriptive study included 598 paramedics actively engaged in prehospital EMS. Data were collected using a form that included question

Community paramedicine-cost-benefit analysis and safety with paramedical emergency services in rural areas: scoping review protocol.

INTRODUCTION: Community paramedicine models have been developed around the world in response to demographic changes, healthcare system needs and reforms. The traditional role of the paramedic has primarily been to provide emergency medical response and transportation of patients to nearby medical facilities. As a response to healthcare service gaps in underserved communities and the growing professionalisation of the workforce, the role of community paramedicine has evolved as a new model of care. A community paramedicine model in one region might address other healthcare needs than a model in

Paramedic intubation: patient position might matter.

OBJECTIVE: Paramedics often intubate in challenging environments. We evaluated whether patient position might affect prehospital intubation success rates utilizing a cadaver model. METHODS: The study was conducted in two phases: a cross-sectional survey and an experimental model in which paramedics were asked to demonstrate intubation skills on cadavers in three positions. New York State certified paid and volunteer paramedics and critical care emergency medical technicians were recruited from multiple agencies. To assess past experience, participants self-reported the number of patients they

Prehospital medical care and the National Ski Patrol: how does outdoor emergency care compare to traditional EMS training?

OBJECTIVE: The purpose of this study was to identify the differences between the educational curricula, skill sets, and funds of knowledge required for certification as an Outdoor Emergency Care Technician (OEC-T), Emergency Medical Technician (EMT), and Emergency Medical Responder (EMR). METHODS: We directly and in detail compared topics and skills presented in the OEC-T curriculum with those presented in the EMT and EMR education and training curricula. RESULTS: The information and skills taught in the OEC-T curriculum are in general more extensive than those taught in EMR training but are

Tourniquet use in different emergency medicine professional groups, results from a regional cross-sectional online survey.

BACKGROUND: Traumatic haemorrhage is one of the most common causes of death in patients with injuries. Targeted pre-hospital haemostasis can reduce morbidity and mortality. The use of tourniquets is advisable for external bleeding in the extremities that cannot be treated with compression or wound packing. There are recommendations for the use of tourniquets. Little is currently known about the level of training and frequency of use of various professional groups of potential tourniquet users. METHODS: A regional cross-sectional online survey was conducted on the level of training, frequency

The impact of traumatic stressors in civilian occupational settings.

We use the literature reporting prevalence and aetiology of post-traumatic stress disorder (PTSD) in first responders as a catalyst to discuss for organisations the policy implications for prevention and intervention of psychiatric morbidity. We searched PubMed and Google to identify studies and reports of mental health and behavioural problems in occupations including police, fire, and emergency service workers. The prevalence of PTSD ranged from 6 per cent to 32 per cent. Biological markers of PTSD, such as neuroendocrine activity, appear less useful than psychological markers, such as level

Stress-Inducing Factors vs. the Risk of Occupational Burnout in the Work of Nurses and Paramedics.

INTRODUCTION: Contemporary healthcare faces new challenges and expectations from society. The profession of a nurse, as well as a paramedic, is essential for the efficient functioning of healthcare. It has its importance not only in promoting and preserving health but also in prevention. With the increasing importance of providing medical care at the highest level, it is expected of these two professional groups to have more knowledge and skills than a few years earlier. The daily contact with patients and their families, the low level of control of the environment, the hierarchical system of

We don't pay them like heroes.

To put this all in perspective: My 20-year-old daughter is a college student. She works in the Stockyards area of Fort Worth operating a mechanical bull for tourists who fancy themselves bull riders. Her job is fairly simple: Take their money, tell them how to get on the bull and then operate the single switch and two levers that control it. For this, she is paid $11 an hour. Contrast this to the paramedic in the MedStar ambulance who provides emergency care for the tourists in the Stockyards. The paramedic staffing that ambulance is earning approximately $13 per hour and receives no benefits

Cardiac arrest treated with a new automatic external defibrillator by out-of-hospital first responders.

Two hundred sixty patients in cardiac arrest were treated with an automatic external defibrillator by first-responding firefighters before arrival of paramedics. On average, first responders arrived 5 minutes before paramedics. Of 118 patients with ventricular fibrillation, 91 (77%) were administered shocks, 21 (23%) of whom had return of pulse and blood pressure by the time paramedics arrived. Fifty-six (62%) were admitted to the hospital and 30 (33%) survived. The survival rate for all 118 victims discovered with ventricular fibrillation was 27%. The device correctly classified the initial a

What do community paramedics in Germany do regarding the care of older people? A retrospective, descriptive analysis of low-acuity cases.

BACKGROUND: Non-life-threatening cases treated by emergency services have been increasing in recent years, especially in older people. In a region in Germany with approximately 600,000 inhabitants, the role of a specially trained community paramedic (Gemeindenotfallsanitäter, G-NFS) was introduced in 2019. The G-NFS is dispatched to low-acuity requests, attends the assignment alone and is allowed to treat patients at home. AIM: The aim of this study was to analyse the assignments attended by the G-NFS relating to the suspected diagnoses, with a focus on older people (≥ 65 years) according to
