An adjudicated hermeneutic single-case efficacy design study of experiential therapy for panic/phobia.

This article illustrates the application of an adjudicated form of hermeneutic single-case efficacy design, a critical-reflective method for inferring change and therapeutic influence in single therapy cases. The client was a 61-year-old European-American male diagnosed with panic and bridge phobia. He was seen for 23 sessions of individual process-experiential/emotion-focused therapy. In this study, affirmative and skeptic teams of researchers developed opposing arguments regarding whether the client changed over therapy and whether therapy was responsible for these changes. Biotypes of functional brain engagement during emotion processing differentiate heterogeneity in internalizing symptoms and interpersonal violence histories among adolescent girls.

Youth exposed to early life interpersonal violence (IPV) demonstrate heterogeneous clinical symptoms. Studies based on univariate methods suggest that neurocircuitry related to emotion processing explains heterogeneity in internalizing symptoms. Here, we use a multivariate, data-driven method of identifying distinct functional brain activation profiles (i.e., "biotypes") and test whether these biotypes differentiate internalizing symptoms among IPV-exposed youth. 114 adolescent girls (n = 38 with no IPV exposure or psychopathology; n = 76 with IPV exposure and heterogeneous internalizing sympt

The Cost of Bottling It Up: Emotion Suppression as a Mediator in the Relationship Between Anger and Depression Among Men with Prostate Cancer.

BACKGROUND: Prostate cancer is a risk factor for major depressive disorder. Recent psycho-oncology research suggests a potential role for male-specific mood-related symptoms in this relationship. Gender socialisation experiences may reinforce men's anger and emotion suppression responses in times of distress, and anger and emotion suppression may be implicated in pathways to, and maintenance of depression in prostate cancer. PATIENTS AND METHODS: Data were collected online from men with a self-reported diagnosis of prostate cancer (N=100; mean age 64.8 years). Respondents provided information

Dialectical behavior therapy for bulimia nervosa.

OBJECTIVE: The effects of dialectical behavior therapy adapted for the treatment of binge/purge behaviors were examined. METHOD: Thirty-one women (averaging at least one binge/purge episode per week) were randomly assigned to 20 weeks of dialectical behavior therapy or 20 weeks of a waiting-list comparison condition. The manual-based dialectical behavior therapy focused on training in emotion regulation skills. RESULTS: An intent-to-treat analysis showed highly significant decreases in binge/purge behavior with dialectical behavior therapy compared to the waiting-list condition. No significa

Effectiveness of the 'Hold me Tight' Relationship Enhancement Program in a Self-referred and a Clinician-referred Sample: An Emotionally Focused Couples Therapy-Based Approach.

While evidence-based couple therapies are available, only a minority of troubled couples seek help and they often do this too late. To reach more couples earlier, the couple relationship education (CRE) group program "Hold me Tight" (HmT) based on Emotionally Focused Couples Therapy (EFCT) was developed. This study is the first to examine the effectiveness of HmT. Using a three-wave (waiting period, treatment, and follow-up) within-subject design, HmT was delivered to 79 self-referred couples and 50 clinician-referred couples. We applied a comprehensive outcome measure battery. Our main findin

[Body-Focused Repetitive Behavior Disorders: Classification, Diagnosis and Treatment].

Body-focused repetitive behaviors (BFRBs) such as trichotillomania and skin picking are disorders at the interface of psychiatry/psychology, dermatology and dentistry. The disorders can be both either a consequence or a cause of severe somatic disorders. If BFRBs remain undetected and untreated, they tend to become chronic with at times serious somatic complications. There is currently no approved medication for BFRBs. Cognitive-behavioral therapy, especially habit reversal training, is the method of choice. The self-help technique decoupling is also effective for a subgroup of patients. In ad

Expressed Emotion, Family Functioning, and Treatment Outcome for Adolescents with Anorexia Nervosa.

The current study examined the relation between parental expressed emotion (EE) and treatment outcome among adolescents participating in a treatment study for adolescent anorexia nervosa, as well as its impact on family functioning. One hundred and twenty-one families were assigned to family-based treatment or adolescent-focused therapy. Paternal criticism predicted lesser improvement in eating disorder psychopathology at end of treatment. There was also a significant interaction between maternal hostility and treatment, indicating that adolescents whose mothers displayed hostility had greater

Pretreatment Dorsal Anterior Cingulate Cortex and Dorsolateral Prefrontal Cortex Activation Moderates Outcomes of Exposure-Focused Cognitive Behavioral Therapy in Pediatric Anxiety.

BACKGROUND: Cognitive behavioral therapy (CBT) for pediatric anxiety targets heightened emotional sensitivity and impaired cognitive control over emotion, core neurocognitive features of anxiety. Exposure is considered the most active component of CBT, but its efficacy varies. Pretreatment function of substrates for emotion processing and cognitive control may predict response to exposure relative to other CBT subcomponents, such as relaxation. METHODS: Youth with clinical anxiety (N = 118, 7-17 years) completed a functional magnetic resonance imaging task probing emotion processing and cogni

Defense-Oriented Psychoanalytic Psychotherapy as a Tailored Treatment for Boys: Neurobiological Underpinnings to Male-Specific Response Tested in Regulation-Focused Psychotherapy for Children.

This paper presents defense-oriented psychoanalytic psychotherapy as a tailored treatment for boys through a neurophysiological hypothesis. Male central nervous system development is reviewed, with a focus on the development of the emotion regulation system. The organizational effects of pre- and post-natal androgens delay central nervous system development in males relative to females, following a caudal to rostral phylogenetic framework. Ventromedial prefrontal structures mature at an earlier developmental age than dorsolateral prefrontal structures, creating less of a gender gap in the avai

Quantifying the dynamics of emotional expressions in family therapy of patients with anorexia nervosa.

Emotional interactions have been considered dynamical processes involved in the affective life of humans and their disturbances may induce mental disorders. Most studies of emotional interactions have focused on dyadic behaviors or self-reports of emotional states but neglected the dynamical processes involved in family therapy. The main objective of this study is to quantify the dynamics of emotional expressions and their changes using the family therapy of patients with anorexia nervosa as an example. Nonlinear methods characterize the variability of the dynamics at the level of the whole th

Working through the body in metacognitive interpersonal therapy to change relational patterns in dependent personality disorder: The case of Lia.

Persons with dependent personality disorder (DPD) have difficulties describing their inner world, and in realizing their negative ideas about the self, such as being weak, unworthy or powerless are just ideas. As a consequence, they tend to over-rely on others and may lose control over their emotions. Treating these persons can gain benefits from including body-focused techniques as they can promote a) awareness of internal states, b) better emotion regulation, c) the capacity to consider their negative ideas about themselves as not necessarily true, and d) gain power of and agency. We will de

Neural changes in youth at high risk for bipolar disorder undergoing family-focused therapy or psychoeducation.

BACKGROUND: Patients with mood disorders may benefit from psychosocial interventions through changes in brain networks underlying emotion processing. In this study, we used functional magnetic resonance imaging (fMRI) to investigate treatment-related changes in emotion processing networks in youth at familial high risk for bipolar disorder (BD). METHODS: Youth, ages 9-17, were randomly assigned to family-focused therapy for high-risk youth (FFT-HR) or an active comparison treatment, Enhanced Care (EC). Before and after these 4-month treatments, participants underwent fMRI while viewing happy,

Adapted emotion skills training group for young people with anorexia nervosa : A mixed-methods evaluation.

BACKGROUND: Brief interventions focused on emotion processing are needed to support young people (YP) in inpatient treatment programmes to manage emotions. Cognitive Remediation and Emotion Skills Training (CREST) has been found beneficial in adult anorexia nervosa (AN) groups. An initial case series found that CREST is a feasible intervention for YP with AN. Further age-appropriate adaptations were needed to improve YP's engagement. Following qualitative feedback on the original CREST manual, a second case series was conducted in an inpatient unit with age-appropriate adaptations to evaluate

Emotional Dysregulation as a Mediator Between Childhood Adversity and Negative Urgency in Borderline Personality Disorder.

BACKGROUND: The biosocial theory of borderline personality disorder (BPD) posits that emotional dysregulation develops from the transaction between a biological vulnerability and an invalidating environment (e.g., childhood adversity), and in turn leads to behavioral dyscontrol. This study aimed to empirically test this core mechanistic pathway in a non-Western clinical sample. METHODS: We hypothesized that the relationship between childhood adversity and the hallmark BPD trait of negative urgency (acting impulsively when distressed) would be mediated by difficulties in emotion regulation. A

Effectiveness of Emotionally Focused Couple Therapy in a Multidisciplinary Veterans Health Care Setting.

Emotionally Focused Couple Therapy (EFCT) is an empirically validated treatment for relationship distress with well-established efficacy. However, there is an increased need to understand its effectiveness and applicability in real-life clinical environments, including hospital and training clinics, and across varying therapist training backgrounds. Our clinical training program in a multidisciplinary US Veterans Affairs (VA) hospital follows a standardized protocol of Sue Johnson's externship in EFCT (ICEEFT 2023; Johnson 2019). Healthcare professional trainees complete a 6-month EFCT didacti

Psychological mechanism of acceptance and commitment therapy and rational emotive behavior therapy for treating hoarding: Evidence from randomized controlled trials.

Hoarding is a common problem behavior worldwide and is detrimental to the physical and mental health of individuals and groups. Currently, effective interventions for hoarding are cognitive-behavioral therapies, but their post-intervention efficacy is questionable, and the available research does not examine the mediating variables of the effects of interventions on clinical outcomes. Moreover, current research on hoarding has focused on Western countries. Therefore, there is a need to investigate the efficacy of other forms of cognitive behavioral therapy on hoarding as well as other psycholo

Emotion-centered problem solving therapy for generalized anxiety disorder: a single-arm pilot study with middle-aged and older adults.

OBJECTIVES: Older adults may be particularly vulnerable to the negative effect of distress on cognition, thus explaining cognitive behavioral therapy's (CBT's) diminished efficacy for generalized anxiety disorder (GAD) in later life. Emotion-centered Problem Solving Therapy (EC-PST) may address this and other barriers to skill acquisition. METHOD: This single-arm pilot aimed to determine the feasibility of 6-session EC-PST to achieve clinically significant improvements in anxiety and/or worry symptoms in adults aged 55+ with GAD. Secondary analyses also examined reductions in depressive sympt

Meta-analysis of psychological treatments for posttraumatic stress disorder in adult survivors of childhood abuse.

Posttraumatic stress disorder (PTSD) is highly prevalent in adult survivors of childhood sexual and/or physical abuse. However, intervention studies focusing on this group of patients are underrepresented in earlier meta-analyses on the efficacy of PTSD treatments. The current meta-analysis exclusively focused on studies evaluating the efficacy of psychological interventions for PTSD in adult survivors of childhood abuse. Sixteen randomized controlled trials meeting inclusion criteria could be identified that were subdivided into trauma-focused cognitive behavior therapy (CBT), non-trauma-focu

Metta-Based Therapy for Chronic Depression: a Wait List Control Trial.

OBJECTIVES: Current treatments for chronic depression have focused on reducing interpersonal problems and negative affect, but paid little attention to promoting prosocial motivation and positive affect. Following this treatment focus, the objective of the present study was to examine whether the combination of metta (Loving Kindness) group meditation and subsequent tailored individual therapy focusing on kindness towards oneself and others (metta-based therapy, MBT) shows greater improvements in depressive symptoms than a wait list control group in patients with chronic depression.
