The influence of transducer operating point on distortion generation in the cochlea.

Distortion generated by the cochlea can provide a valuable indicator of its functional state. In the present study, the dependence of distortion on the operating point of the cochlear transducer and its relevance to endolymph volume disturbances has been investigated. Calculations have suggested that as the operating point moves away from zero, second harmonic distortion would increase. Cochlear microphonic waveforms were analyzed to derive the cochlear transducer operating point and to quantify harmonic distortions. Changes in operating point and distortion were measured during endolymph mani

Evaluation of cochlear function in an acute endolymphatic hydrops model in the guinea pig by measuring low-level DPOAEs.

During and after microinjection of artificial endolymph into scala media of the guinea pig, the 2f1- f2 -DPOAE at 4.5 kHz generated by low-level primaries was recorded. Reproducible changes were measured when 1.1 microl of artificial endolymph was injected at a rate of 1.65 nl/s (1.53-1.83). This volume corresponds with an acute endolymphatic hydrops of 23%. After the onset of injection the inner ear pressure immediately increased to a mean higher level of 22 Pa, whereas the 2f1- f2 -amplitude and -phase did not change for about 1 min. Thereafter, the amplitude decreased 2.6 dB (+/- 0.7) on av

[Ways of increasing the effectiveness of the treatment of male patients with genital tuberculosis].

Endolymphic therapy proved to be a considerable resource for the rise in the treatment efficacy of specific inflammatory diseases of the male genital organs without adding to the antituberculous drug dosage. Indirect endolymphic chemotherapy appears to be more accessible and not labour-intensive, instead it can increase the efficacy of a conservative therapy of men with genital tuberculosis. Besides, it seems to be promising in relation to the preservation of fertility after tuberculous epididymitis.

Mechanism of the Ménière attack.

A survey of experimental evidence showing the influence of an increase in potassium concentration in the perilymph on cochlear and vestibular nerve branches is discussed. Informations have indicated that a concentration lower than 20--30 mM produced an increase in action potential frequency in nerve preparations and that a low potassium concentration in animal experiments produced an ipsilateral nystagmus. Increasing the concentration of potassium in the endolymph to values not over the normal endolymph concentration, blocked the conduction of action potentials in the vestibular nerve branches

Water channel proteins in the inner ear and their link to hearing impairment and deafness.

The inner ear is a fluid-filled sensory organ that transforms mechanical stimuli into the senses of hearing and balance. These neurosensory functions depend on the strict regulation of the volume of the two major extracellular fluid domains of the inner ear, the perilymph and the endolymph. Water channel proteins, or aquaporins (AQPs), are molecular candidates for the precise regulation of perilymph and endolymph volume. Eight AQP subtypes have been identified in the membranous labyrinth of the inner ear. Similar AQP subtypes are also expressed in the kidney, where they function in whole-body

Endolymphatic Hydrops is a Marker of Synaptopathy Following Traumatic Noise Exposure.

After acoustic trauma, there can be loss of synaptic connections between inner hair cells and auditory neurons in the cochlea, which may lead to hearing abnormalities including speech-in-noise difficulties, tinnitus, and hyperacusis. We have previously studied mice with blast-induced cochlear synaptopathy and found that they also developed a build-up of endolymph, termed endolymphatic hydrops. In this study, we used optical coherence tomography to measure endolymph volume in live CBA/CaJ mice exposed to various noise intensities. We quantified the number of synaptic ribbons and postsynaptic de

Meniere's syndrome and otitis media.

We here present a clinical study of 37 patients with Meniere's syndrome. Meniere's syndrome can occur subsequent to and in some cases simultaneously with chronic otitis media. When otitis media which has occurred many years earlier in childhood becomes inactive, leading to sequelae of Meniere's later in life, full-blown Meniere's symptom-complex with vertigo tends to occur; whereas when active chronic otitis media accompanies Meniere's, cochlear Meniere's syndrome tends to predominate. Endolymphatic hydrops is described in pathological cases of labyrinthitis and in 11 human temporal bone cases

Pathogenesis of experimental endolymphatic hydrops.

The protein content and the d.c. potential of the endolymph differs in the various parts of the endolymphatic space (cochlea, utricle, semicircular canals and endolymphatic sac) as also does the ion composition (chloride, potassium, sodium). 12 months after obliteration of the endolymphatic sac and duct in guinea pigs the d.c. potential falls, whereas, the sodium activity increases. The endolymphatic hydrops is not caused by an increased colloid osmotic pressure. The increased water-binding capacity of the cochlear endolymph is correlated with the increased Na+ activity.

Effects of exposure to noise on ion movement in guinea pig cochlea.

Healthy guinea pigs were exposed to broad band noise at levels between 95 and 115 dBA for 7 days. A significant decrease of the sound-induced cochlear responses, together with a substantial increase of the endocochlear potential, was observed in guinea pigs exposed to noise at 105 or 115 dBA. Microsamples of the endolymph obtained from these guinea pigs showed a significant increase of K+ and Cl- concentrations and a decrease of Na+ concentration, when compared with those from control animals. The K+, Na+ and Cl- concentrations in the perilymph were not markedly affected by noise exposure. Whe

Ephrin-B2 governs morphogenesis of endolymphatic sac and duct epithelia in the mouse inner ear.

Control over ionic composition and volume of the inner ear luminal fluid endolymph is essential for normal hearing and balance. Mice deficient in either the EphB2 receptor tyrosine kinase or the cognate transmembrane ligand ephrin-B2 (Efnb2) exhibit background strain-specific vestibular-behavioral dysfunction and signs of abnormal endolymph homeostasis. Using various loss-of-function mouse models, we found that Efnb2 is required for growth and morphogenesis of the embryonic endolymphatic epithelium, a precursor of the endolymphatic sac (ES) and duct (ED), which mediate endolymph homeostasis. Electron microscopic studies of the endolymphatic sac in Ménière's disease.

Five specimens of the endolymphatic sac taken from patients with Ménière's disease during epidural drainage surgery were examined under the electron microscope. The clinical history and histopathological findings are presented. The most common and prominent finding was perisaccular fibrosis of the endolymphatic sac which appears to disturb the free absorption of endolymph in the endolymph in the endolymphatic sac resulting in endolymphatic hydrops. Well-preserved epithelial cells of the endolymphatic sac could only be found in one of the specimens.

[Endolymphatic visualization in patients with Meniere's disease].

OBJECTIVE: To attempt to visualize the endolymph in patients with Meniere's disease by applying non-invasive intratympanic gadolinium through eustachian tube and three-dimensional fluid-attenuated inversion recovery magnetic resonance imaging (3D-FLAIR MRI). METHODS: With a 3 Tesla magnetic resonance imaging (MRI) unit, 3D-FLAIR imaging was performed 24 hours after intratympanic gadolinium through eustachian tube in two patients with medically active and intractable Meniere's disease. Pure tone test and tympanometry were performed 24 hours before and after the administration of gadolinium. R

Adenylate cyclase modulation of ion permeability in the guinea pig cochlea: a possible mechanism for the formation of endolymphatic hydrops.

The pathophysiological mechanisms leading to endolymphatic hydrops in Meniere's disease are unknown. Changes in ionic permeability of the cellular membranes between the endolymph and the perilymph, which alter the composition and osmolarity of the inner ear fluid, may be a major factor in the etiology of endolymphatic hydrops. To determine the possible involvement of adenylate cyclase in the formation of endolymphatic hydrops, we measured the endolymphatic K+, Na+, Cl- activities (AK, ANa, ACl) and the endocochlear potential (EP) by means of ion-selective microelectrodes while inner ear adenyl

Hormonal modulation of inner ear fluids.

In the cochlea, hormones such as antidiuretic hormone and adrenocorticosteroid hormones are supposed to modulate the endolymph osmolality acting on the labyrinthine water permeability, on the one hand, and on the Na+, K(+)-ATPase, on the other hand. To test the hypothesis that these hormones are involved in the inner ear fluids homeostasis, the electrochemical composition of cochlear fluids was studied in control Long Evans rats, Brattleboro rats that are genetically deprived of antidiuretic hormone, and in adrenalectomized Long Evans rats. The results demonstrated that: i) in Brattleboro rats

Noise-noise effect upon the spreading of the posttraumatic progressive necrosis in the organ of Corti.

Progressive necrosis of the organ of Corti caused by a disturbance of the ionic composition of the Cortilymph resulting from the infiltration of endolymph into the organi of Corti after traumatization of its barrier against the scala media, is greatly intensified by posttraumatic stimulation with noise of physiological intensities. This effect is attributed to oscillation of the cochlear partition, with resultant greater blending of the Cortilymph and the endolymph. Stimulation augments necrosis fourfold towards the base of the cochlea and twofold towards the helicotrema. Posttraumatic stimula

Regulation of the perilymphatic-endolymphatic water shunt in the cochlea by membrane translocation of aquaporin-5.

Volume homeostasis of the cochlear endolymph depends on radial and longitudinal endolymph movements (LEMs). LEMs measured in vivo have been exclusively recognized under physiologically challenging conditions, such as experimentally induced alterations of perilymph osmolarity or endolymph volume. The regulatory mechanisms that adjust LEMs to the physiological requirements of endolymph volume homeostasis remain unknown. Here, we describe the formation of an aquaporin (AQP)-based "water shunt" during the postnatal development of the mouse cochlea and its regulation by different triggers. The fina

Compartmentalization established by claudin-11-based tight junctions in stria vascularis is required for hearing through generation of endocochlear potential.

Claudins are cell adhesion molecules working at tight junctions (TJs) that are directly involved in compartmentalization in multicellular organisms. The cochlea includes a rather peculiar compartment filled with endolymph. This compartment is characterized by high K+ concentration (approximately 150 mM) and a positive endocochlear potential (approximately 90 mV; EP), both indispensable conditions for cochlear hair cells to transduce acoustic stimuli to electrical signals. These conditions are thought to be generated by the stria vascularis, which is adjacent to the endolymph compartment. The s

Responses of the endolymphatic sac to perilymphatic injections and withdrawals: evidence for the presence of a one-way valve.

Although the endolymphatic sac (ES) is thought to be a primary site for endolymph volume regulation, we have limited knowledge of how it responds to volume and pressure changes. In a prior publication, we demonstrated changes of K(+), Na(+) and endolymphatic sac potential (ESP) resulting from volume injections into, and withdrawals from, scala media of the cochlea. In the present study, we compared the influence of injections into and withdrawals from scala tympani of the cochlea on the endolymphatic sac. It is assumed that similar pressure changes are induced in endolymph and perilymph of bot

Imaging of Ménière's disease after intravenous administration of single-dose gadodiamide: utility of multiplication of MR cisternography and HYDROPS image.

A HYDROPS (HYbriD of Reversed image Of Positive endolymph signal and native image of positive perilymph Signal) image, created by subtracting a positive endolymph image from a positive perilymph image after intravenous injection of single-dose gadodiamide, has been reported to facilitate recognition of endolymphatic hydrops. To further increase the contrast-to-noise ratio (CNR) of HYDROPS images, we multiplied a T2-weighted MR cisternographic image onto the HYDROPS image. Ten patients with suspected Ménière's disease were included. The average CNR of generated images increased to more than 200

Pathogenesis of Menière's disease.

While endolymphatic hydrops is a characteristic pathologic feature of Menière's disease, there are exceptions to this rule. There is evidence that hydrops develops as a result of malabsorption of endolymph. This implies dysfunction of the endolymphatic sac and duct, which normally absorb endolymph. In approximately 20% of cases of Menière's disease, a specific pathologic condition in the temporal bone can be associated as a cause of endolymphatic hydrops. Syphilis, fractures of the temporal bone, otosclerosis, and preceding chronic otitis media are some of the more commonly encountered patholo
