[Unusual electrocochleography cases].

The electrocochleographic findings of 3 unusual cases from about 350 performed on patients with various hearing loss are presented; they demonstrate the value of electrocochleography as a clinical tool in otological investigation. The first case was an electrocochleographically detected acute impairment of the first neuron, the second a hereditary hearing loss with endolymphatic hydrops, and the last shows the effect of an endolymphatic shunt on a hydropic hearing loss.

A further critical assessment of the efficacy of endolymphatic sac surgery.

This paper further assesses the efficacy of two selected techniques of endolymphatic sac surgery (ELS) employing implants for the treatment of classic Meniere's disease and endolymphatic hydrops: the Arenberg inner ear valve implant and the newly developed Huang/Gibson inner ear shunt. Long-term (8-10 years) results for 57 patients given the Arenberg inner ear valve implant show that our demonstrated high success rate in achieving control of symptoms over the short term (1 year) is long-lasting and that although previously reported hearing gains have decreased in some cases, the overall long-t

Surgical treatment of chronic otitis media and Menière's syndrome.

Menière's symptom-complex in conjunction with chronic otitis media (COM) has occasionally been encountered in otologic practice. This study compares the effects of various surgical techniques on patients with COM and Menière's syndrome, primarily on incapacitating vertigo. One group of patients with incapacitating vertigo received endolymphatic system drainage procedures, either endolymphatic sac surgery (83 patients) or cochleosacculotomy (12 patients) simultaneously with surgery for COM. Another group (14 patients) had surgery for COM only. A comparison of the results for vertigo control bet

Surgery of the endolymphatic sac.

The wide variation in reported success rates in endolymphatic sac surgery has been the subject of much controversy in literature. This series is a follow-up of additional cases done since 1976 that were previously reported with 55% relief of vertigo on relatively long-term follow-ups of 24 months to 10 years. This present series is an additional 30 endolymphatic shunts that were done since that time. This group of patients as a rule had their disease for a shorter period of time. It is interesting that the long-term relief of vertigo in this series is 70%. This is considerably better than the

Menière's disease and endolymphatic hydrops: clinical-histopathological correlations.

The clinical-histopathological correlation between Menière's disease and endolymphatic hydrops was done to explain, as much as possible, causes of symptoms of Menière's disease. Twenty-three temporal bones with endolymphatic hydrops from 17 patients were reviewed and clinical and histopathological findings were correlated. Histopathological examination revealed frequent, severe deformities in the labyrinthine walls and permanent changes in Reissner's membranes. Evidence of rupture was difficult to assess. Of 21 ears of patients with the clinical diagnosis of Menière's disease, 93% had endolymp

[Long-term result of endolymphatic sac drainage for Ménière's disease].

Ten cases of Ménière's disease were operated with endolymphatic mastoid shunt. All patients were followed up 3 to 5 years postoperatively. The results showed that all patients obtained satisfactory relief of Vertigo, and in most of the cases the symptom of the tinnitus was diminished or vanished, only one patient demonstrated hearing loss. The advantages of this surgery are: shorter hospitalization, fast recovery and a low potential for serious intracranial complications. The relation between the type of endolymphatic sac and the result of operation was discussed.

Histopathology after endolymphatic sac surgery for Ménière's syndrome.

BACKGROUND: The putative goal of sac surgery in Ménière's syndrome is to promote the flow of endolymph from the labyrinth to the endolymphatic sac and thereby relieving hydrops. There are scant published histopathologic data whether sac surgery actually accomplishes this goal. OBJECTIVE: To determine whether sac surgery relieves hydrops by examining the histopathologic changes in temporal bones obtained from individuals who had undergone sac surgery during life for Ménière's syndrome. METHODS: Temporal bones were examined from 15 patients who had sac surgery. Data on the presence and severit

Use of polytomography in Meniere's disease.

In a long-term study of the treatment of Meniere's disease, 105 cases treated from 1965 through 1973 were divided into subgroups by virtue of the institution of routine polytomographic examination of the vestibular aqueduct (1968). A comparison of results of treatment in the two groups (empiric indication of endolymphatic shunt vs x-ray film evidence) yielded the following conclusions: (1) Success or failure of the endolymphatic shunt operation is significantly correlated with patency or obliteration of the vestibular aqueduct as seen on roentgenographic examination. (2) To be effective, medic

Hearing results from endolymphatic sac surgery.

This report examines the effect of the endolymphatic shunt on hearing. The study group was drawn from 101 shunt operations for intractable vertigo between 1983 and 1987. Thirty ears met criteria for diagnosis, level of preoperative hearing impairment, and length of follow-up. The control group consisted of 30 ears with symptoms severe enough to prompt recommendations for shunt surgery, but the patients either opted against surgery or improved. The results were analyzed using the 1985 AAO-HNS reporting criteria. When looking at worst postoperative (or worst scores after 2 years of follow-up in

Application of parasagittal surface coil MRI to otoneurological diagnosis.

Parasagittal surface coil magnetic resonance imaging (MRI) of the internal auditory canal and the inner ear was performed in patients with otoneurological diseases. T2-weighted fast spin-echo sequences, plain and enhanced T1-weighted sequences were used to examine the inner ear and the individual nerves in the internal auditory canal. Normal parasagittal images of the inner ear and the internal auditory canal and 4 patients with otoneurological disorders are presented. Precise location of the acoustic tumor was visualized and postoperative status of the internal auditory canal and the inner ea

Shunt surgery in Meniere's disease: a follow-up report.

Between December 1969 and May 1986, 140 patients were treated for Menière's disease using the endolymphatic sac subarachnoid shunt. The technique and results are contrasted with an earlier report. The surgical technique described includes modifications of the originally described procedure. A microcomputer was used for the storage and manipulation of data based upon 1972 Academy criteria. Control of vertigo was accomplished in 83% of patients; 51% of patients were graded class A or B, according to the 1972 Academy criteria. No serious complications occurred. Problems included the potential for

[Long-term results of surgical treatment of intractable Menière's disease for control of vertigo].

OBJECTIVE: To acquire the long-term (an average of over 10 years follow-up) results of surgical treatment of patients with Meniere's disease for control of vertigo. METHOD: Endolymphatic sac surgery was done in 6 cases (1 case of endolymphatic sac decompression and 5 cases of sac-mastoid shunting) and translabyrinth vestibular neurotomy was operated on 6 cases. RESULT: Complete relief from symptoms of vertigo occurred in the 6 cases through vestibular neurotomy and there were no recurrence in 11-13 years follow-up. Three of 6 cases with endolymphatic sac surgery had complete control of verti

The non-specific effect of endolymphatic sac surgery in treatment of Meniere's disease: a prospective, randomized controlled study comparing "classic" endolymphatic sac surgery with the insertion of a ventilating tube in the tympanic membrane.

A prospective, randomized study was carried out comparing the effect of two surgical modalities in the treatment of patients with Meniere's disease: insertion of an endolymphatic sac shunt and insertion of a ventilating tube in the tympanic membrane. A total of 29 patients, 12 males and 17 females, age 27-71 years, were operated on in two ear, nose and throat (ENT) departments. Of these patients, 15 had an endolymphatic shunt inserted and 14 had a ventilating tube inserted in the tympanic membrane. Postoperative follow-up was carried out in the department in which the patients had not been ope

[Historical evolution and frontier of endolymphatic sac surgery for treatment of Ménière's disease].

Surgical treatment could be necessary for patients with intractable Ménière's disease.Endolymphatic sac surgery is a favorable option as it could preserve patient's hearing and vestibular function. The overall vertigo control rate after the surgery was about 80%.Endolymphatic sac surgery includes several variations, namely sac shunting, sac drainage, and sac decompression.Although their establishment and development have experienced a long tortuous process, the theoretical basis and clinical efficacy of the surgery remain controversial.Meanwhile, new findings and surgical innovation keep emerging.Th

Three previous papers have detailed my experience with various methods of surgical treatment of Meniere's disease: 1) surgery for all patients; 2) surgery for only those patients who demonstrated patency of the vestibular aqueduct as seen on tomographic examination; and 3) prognostic tests to determine which patients were most suitable for endolymphatic sac surgery. This paper details the results of endolymphatic shunt surgery performed on 43 patients over a 3-year period, restudying my prior conclusions. All patients were operated using a new method of capillary endolymph dispersement regardl

Variations in surgical anatomy of the endolymphatic sac.

Twenty-nine specimens of the extraosseous portion of the human endolymphatic sac (ES) were serially sectioned longitudinally. The length and width of the extraosseous ES were measured and the surface area was calculated. As the specimens included the sigmoid sinus (SS), the relationship between the ES and the SS was analyzed. The extraosseous ES varied considerably in size. The lumen either consisted of a single tube or was subdivided into several tubules. The distal part of the ES overlapped the SS in one third of the specimens. The results indicate that a minute extraosseous ES could explain
