Parenteral vs enteral nutrition in tumor-bearing rats.

The development of cachexia may complicate cancer therapy, yet controversy exists concerning its nutritional management. For example, use of total parenteral nutrition (TPN) may not be appropriate because of gut atrophy, possible stimulation of tumor growth, and lack of total host protein repletion. In the present experiment, host and tumor responses were compared after identical parenteral or enteral nutritional supplementation (EN). Eighteen days after subcutaneous inoculation of adult male Fischer-344 rats with fresh methylcholanthrene-induced sarcoma (tumor-bearing [TB] rats), catheters we

Percutaneous endoscopic gastrostomy for long term enteral nutrition.

BACKGROUND: Percutaneous endoscopic gastrostomy for long term enteral nutrition is often indicated in patients with head, neck and oesophageal cancer but despite its growing popularity elsewhere, it is not widely used in India. METHODS: Between March 1990 and July 1991, we performed percutaneous endoscopic gastrostomy in 54 patients. The primary sites of tumour were the hypopharynx (11), oral cavity (7), tongue (7), cricopharynx (7), oesophagus (16) and other sites (6). The indications were difficulty in swallowing following treatment (22), preoperative nutritional support (7) and terminal ca

Mn-content of total parenteral and enteral nutrition.

Manganese has been proven to be an essential trace element in animals since 1931. Today, it is known that manganese deficiency results in a wide variety of structural and physiological defects in animals. For humans, the safe and adequate range of intakes for manganese by enteral nutrition is 2.0 to 5 mg per day; for parenteral nutrition a range of 0.15 to 0.8 mg per day is proposed. The manganese content of components of enteral nutrition (EN) and total parenteral nutrition (TPN) solutions is determined using electrothermal atomic absorption spectrometry. Comparison is made between calculated

[The treatment of intolerance for enteral nutrition of unknown origin with parenteral nutrition].

In recent years, there has been a notable rise in referrals to specialised chronic intestinal failure centres of patients with self-reported intolerance to enteral feeding. Despite incomplete or inconclusive diagnostic procedures and no convincing clinical evidence for the presence of intestinal failure, patients often urge physicians to start with total parenteral nutrition (TPN). The frequent use of opioids in these patients is a complicating factor concerning the diagnostic workup for motility disorders and the interpretation of clinical symptoms. Despite TPN administration, these patients

Early enteral nutrition vs parenteral nutrition following pancreaticoduodenectomy: Experience from a single center.

AIM: To analyze and compare postoperative morbidity between patients receiving total parenteral nutrition (TPN) and early enteral nutrition supplemented with parenteral nutrition (EEN + PN). METHODS: Three hundred and forty patients receiving pancreaticoduodenectomy (PD) from 2009 to 2013 at our center were enrolled retrospectively. Patients were divided into two groups depending on postoperative nutrition support scheme: an EEN + PN group (n = 87) and a TPN group (n = 253). Demographic characteristics, comorbidities, preoperative biochemical parameters, pathological diagnosis, intraoperative

[Total enteral nutrition vs. total parenteral nutrition in patients with severe acute pancreatitis].

OBJECTIVE: To compare the efficacy of early total enteral nutrition (TEN) vs. total parenteral nutrition (TPN) in patients with severe acute pancreatitis (SAP). METHODS: A total of 22 consecutive patients with SAP were randomized to receive TPN (group I) or TEN (group II). SAP was defined applying APACHE II score, C-reactive protein (CRP) measurements and/or Balthazar CT scan score. Acute inflammatory response (CRP, TNF-a, IL-6), visceral proteins (pre-albumin, albumin), complications (systemic inflammatory response syndrome, multiorgan failure, infections), surgical interventions, length of

Ischemic Gastroduodenitis: Navigating the Challenges of Enteral Nutrition Resumption.

Ischemic gastritis (IG) and ischemic duodenitis (ID) are rare due to the extensive anastomotic vascular supply arising from the celiac trunk. Profound splanchnic hypoperfusion is required to induce a clinically significant ischemic insult. Etiologies typically include systemic hypotension, vasculitis, or thromboembolism. Patients with IG/ID often face high morbidity and mortality given the severity of ischemia required to induce extensive mucosal damage, leaving clinicians to balance prolonged bowel rest and early enteral feeding. We present the case of an 18-year-old man with spastic quadripl

Therapeutic mechanisms of exclusive enteral nutrition in Crohn's disease.

Crohn's disease (CD) is a chronic, relapsing multifactorial inflammatory condition of the gastrointestinal tract, which is diagnosed under the age of 17 in 25% of patients, categorized as pediatric CD (pCD). Exclusive enteral nutrition (EEN) is a first-line therapy for inducing remission in pCD, yet its precise mechanisms remain poorly understood. This review summarizes the complex interplay of EEN-induced protective changes in the gut microbiota, epithelial barrier function and mucosal immune responses. EEN reshapes the gut microbiome by excluding potential pathobionts from the gut mucus laye

Effects of enteral nutrition in stroke: an updated review.

Stroke is a leading cause of death and functional decline that places a significant burden on healthcare systems. Malnutrition is a critical clinical concern that complicates the condition of stroke patients and contributes to unfavorable outcomes. Dysphagia is the primary cause of malnutrition in the acute stage after stroke. Enteral nutrition (EN) has been employed to manage the nutritional status of stroke patients to prevent and treat malnutrition. Early EN (EEN) has been shown to reduce mortality and the prevalence of malnutrition while enhancing functional outcomes. In the majority of ca

Enteral nutrition in the hemodynamically unstable critically ill patient.

The benefit of enteral nutrition in critically ill patients has been demonstrated by several studies, especially when it is started early, in the first 24-48h of stay in the Intensive Care Unit, and this practice is currently advised by the main clinical guidelines. The start of enteral nutrition is controversial in patients with hemodynamic failure, since it may trigger intestinal ischemia. However, there are data from experimental studies in animals, as well as from observational studies in humans that allow for hypotheses regarding its beneficial effect and safety. Interventional clinical t

Enteral refeeding syndrome after long-term total parenteral nutrition.

BACKGROUND: Early enteral feeding (EF) may result in fever, elevated white blood cell count, increased serum levels of liver enzymes, and diarrhea. We name the complications "enteral refeeding syndrome", as a subtype of refeeding syndrome, because they are likely to result from long-term lack of lumen nutrition. The aim of this study was to investigate the characteristics of enteral refeeding syndrome after long-term total parenteral nutrition (TPN), and the solution for the disease. METHODS: We collected the clinical data of 100 patients with gastrointestinal fistula, who were cured from Api

Enteral nutrition by a forward surgical team in Afghanistan.

OBJECTIVES: The modern practice of trauma surgery is a global physiologic approach to caring for the injured patient. Included in that approach is consideration of the traumatized patient's nutritional needs and implementing early enteral feeding. This is routine practice in the United States but logistically impractical when using commercial enteral feeding formulas in the austere environment of a Forward Operating Base in Afghanistan. METHODS: At a Forward Operating Base in southern Afghanistan, injured patients who were not expected to be taking a regular diet by 72 hours after injury are

Early enteral nutrition in gastrointestinal surgery: a pilot study.

There is still some concern about the safety of early enteral nutrition (EN) to patients with recent anastomoses. A pilot trial was carried out on a prospective basis to evaluate the tolerance and clinical outcome of 56 patients who received early EN following gastrointestinal (GI) surgery. A continuous infusion of an elemental, peptide-based diet was administered using a nasointestinal feeding tube placed beyond the pylorus by the operating surgeon. Tube feeds were started at 6.07 +/- 4.99 h after surgery and advanced as tolerated to a rate of 60 mL/h on the third postoperative day. Patients
