[Ultrasound diagnosis of necrotizing enterocolitis in neonates].

Necrotizing enterocolitis (NEC) is a common critical gastrointestinal disease in neonates. Severe cases have a poor prognosis and may be fatal. Early and accurate diagnosis is crucial for improving outcomes. Traditional abdominal X-ray has low sensitivity. As a noninvasive, radiation-free real-time imaging technique, abdominal ultrasound can assess intestinal wall structure, peristalsis, and perfusion at an early stage and has outstanding value for early diagnosis. Typical ultrasound findings include intestinal wall thickening or thinning, abnormal blood perfusion, pneumatosis intestinalis, po

Surgical considerations for neonates with necrotizing enterocolitis.

Necrotizing enterocolitis (NEC) is a potentially devastating condition that preferentially affects premature and low birth weight infants, with approximately half requiring acute surgical intervention. Surgical consult should be considered early on, and deterioration despite maximal medical therapy or the finding of pneumoperitoneum are the strongest indications for emergent surgical intervention. There is no clear consensus on the optimal surgical approach between peritoneal drainage and laparotomy; the best course of action likely depends on the infant's comorbidities, hemodynamic status, si

[Research advances in necrotizing enterocolitis in neonates].

Necrotizing enterocolitis (NEC) is a catastrophic disease caused by a variety of factors in neonates, especially preterm infants. Severe NEC has a high fatality rate, and most survivors may face short- and long-term adverse prognosis. Risk factors for NEC include preterm birth, non-breastfeeding, microbial abnormalities in the digestive tract, and ischemia-reperfusion injury. High-resolution abdominal ultrasound helps with the early diagnosis of NEC. The preventive measures for NEC include protecting the intestinal mucosa through nutritional intervention, interfering with intestinal injury sig

Biomarkers of necrotizing enterocolitis: a review of literature.

Necrotizing enterocolitis (NEC) is among the most serious gastrointestinal emergency in very low birth weight (VLBW), extremely low birth weight (ELBW) and extremely low gestational age neonates (ELGAN), affecting 7-14% of these neonates. Despite extensive research, the underlying aetiology of NEC still remains blurred. Due to high mortality, morbidity and its delayed presentation, early detection of NEC is considered to be lifesaving. A number of biomarkers have been studied for early detection and prediction of severity of NEC but till date, no ideal marker has been discovered. Molecular tec

Necrotizing Enterocolitis in the Full-Term Infant.

Necrotizing enterocolitis in full-term infants is relatively rare. When seen, it is usually associated with perinatal asphyxia, sepsis, or specific forms of congenital heart disease. It can also be associated with endocrinopathies. In this review, a full-term infant was found to have necrotizing enterocolitis and persistent hypoglycemia. Evaluation for hypoglycemia revealed pan-hypopituitarism, and magnetic resonance imaging confirmed this diagnosis. Timely evaluation and early initiation of hormone replacement therapy is essential to minimize long-term morbidities and mortality associated wit

International survey on the management of necrotizing enterocolitis.

AIM: The aim of this study is to define patterns in the management of necrotizing enterocolitis (NEC). METHODS: A total of 80 delegates (81% senior surgeons) from 29 (20 European) countries completed a survey at the European Pediatric Surgeons' Association 2013 annual meeting. RESULTS: Overall, 59% surgeons work in centers where>10 cases of NEC are treated per year. DIAGNOSIS: 76% surgeons request both anteroposterior and lateral abdominal X-rays, which are performed at regular intervals by 66%; 50% surgeons also request Doppler ultrasonography; most frequently used biochemical markers are

The role of surgery in management of necrotizing enterocolitis.

OBJECTIVE: Necrotizing enterocolitis (NEC) has become the most common gastrointestinal emergency among neonates and preterms admitted to the intensive care units. The aim of this study is to evaluate the surgical management of NEC as well as their outcome. PATIENTS AND METHODS: Thirty five cases were included in this prospective study (10 full terms, 15 preterm and 10 infants) with the diagnosis of NEC. The severity of NEC episodes were determined according to Bells classification into group A (stage I 10 cases), Group B (stage II 17 cases) and group C (stage III 8 cases). Full history and cl

Probiotics for necrotizing enterocolitis: a systematic review.

BACKGROUND: Necrotizing enterocolitis (NEC) is the most commonly acquired neonatal intraabdominal emergency and causes significant morbidity and mortality. A proposed strategy for the prevention of NEC is the administration of oral probiotics. Probiotics have been shown to reduce NEC in experimental rat models and have been used in clinical trials. The authors aimed to review the existing data on the use of oral probiotics for the prevention of NEC in preterm infants (age <33 weeks) and those with very low birth weight (VLBW). MATERIALS AND METHODS: Systematic review of randomized controlled

Gut motility and the pathogenesis of necrotizing enterocolitis.

Forward movement of intestinal nutrients is achieved by motor activity. Motor activity patterns are "abnormal" in adults as well as preterm infants who display intolerance to enteral feedings. Manometrics can be used diagnostically and prospectively to identify infants at risk for feeding intolerance.

Necrotizing enterocolitis in a neonatal piglet model.

The aim of this study was to develop an animal model for necrotizing enterocolitis (NEC). Twenty-five neonatal Hanford minipigs had carotid artery and external jugular vein catheters and rectal Clinical Tonomitors placed under anesthesia. Experimental animals were subjected to a hypoxic insult (50% reduction in baseline PaO2 for 30 minutes) and hypothermic stress (core temperature reduced to 35 degrees C for 30 minutes). Regular oral diet was resumed and the survivors were euthanized 3 to 4 days later. All animals underwent necropsy with gross and histopathological evaluation of the entire bow

Cocaine: a risk factor for necrotizing enterocolitis.

Necrotizing enterocolitis occurs most often in the small preterm infant, usually with birthweight less than 1.5 kg. Necrotizing enterocolitis has also been described in more mature infants, however, often the result of reduced gastrointestinal blood flow. We describe four relatively large infants from our nursery with varying severity of necrotizing enterocolitis. None had any of the known risk factors; all had a history of maternal cocaine use. Two were initially cared for in the term nursery. All four presented with abdominal distention, bloody stools, vomiting, or residuals. One infant pres

Necrotizing enterocolitis and volvulus in the premature neonate.

Premature infants presenting to the neonatal intensive care unit at the Hospital for Sick Children with a surgical abdomen over a 5-year period were studied retrospectively to determine the factors leading to the diagnosis of malrotation with volvulus and necrotizing enterocolitis (NEC). Fifteen preterm infants (less than 37 weeks) were diagnosed as having volvulus, and 54 had surgically treated NEC. Those with NEC were more likely to be systematically ill with grossly bloody stools, abdominal tenderness, and thrombocytopenia (P less than .005). Bilious vomiting and bilious gastric residuals w

[Ulcero-necrotic enterocolitis complicated by peritonitis].

Among 217 patients with acute peritonitis there were 12 patients with the peritonitis developed against the background of segmentary enteritis or colitis. The article describes this form of peritonitis characterized by great severity, rapid development and high death rate. On the detection of a necrotic segment of the small intestine, a one-step resection within the limits of healthy tissues is indicated. Two-steps operations are expedient for necrosis of the colon.

Resection with primary anastomosis for necrotizing enterocolitis.

Primary anastomosis following the resection of bowel for necrotizing enterocolitis (NEC) can be done with acceptable morbidity and mortality rates. Twenty-seven patients are reported with three deaths. Primary anastomosis will avoid problems seen with small-bowel stomas. The concomitant ligation of a significant patent ductus arteriosus (PDA) should be considered for patients undergoing abdominal procedures for NEC.

Surgical problems in necrotizing enterocolitis in childhood.

The problem of necrotizing enterocolitis is discussed based on the experience with 24 patients. It has been found that the time factor plays an important role in the decision whether to operate. The indications for operation are intestinal perforation, threatening perforation, and penetration of a covered perforation. Operation is carried out in order to treat the peritonitis caused by perforation. The various operative methods are described and evaluated. Intestinal resection, exclusion enterostomies, and suture of the perforation may all be justified, depending on the findings at operation.

Elevating Patient-Family Voices in Necrotizing Enterocolitis.

A diagnosis of necrotizing enterocolitis (NEC) often affects families for a lifetime. Given the profound burden of NEC, families are uniquely positioned to harness the power of their experience to advocate for and contribute to research. Clinicians and scientists can also collaborate with families affected by NEC to improve care as well as prevention and treatment strategies. Elevating the voices of patient-families affected by NEC can help advance the field by helping identify meaningful gaps in science, conveying the urgency of improving outcomes, and providing knowledge only known by famili

Do hematological biomarkers predict surgical necrotizing enterocolitis?

Necrotizing enterocolitis (NEC) is a devastating gastrointestinal emergency in preterm infants and the clinical presentation of NEC may vary with gestational age. We lack reliable biomarkers for early diagnosis of NEC limiting timely intervention. Hematological changes in NEC are actively researched for their potential role as biomarkers. The pattern and severity of hematological abnormalities have been correlated with rapid progression, the need for surgery, increased risk of mortality, and morbidity. In this issue of Pediatric Research, Chong et al. report GA-specific hematological biomarker

Enteric Nervous System in Neonatal Necrotizing Enterocolitis.

BACKGROUND: The pathophysiology of necrotizing enterocolitis (NEC) is not clear, but increasing information suggests that the risk and severity of NEC may be influenced by abnormalities in the enteric nervous system (ENS). OBJECTIVE: The purpose of this review was to scope and examine the research related to ENS-associated abnormalities that have either been identified in NEC or have been noted in other inflammatory bowel disorders (IBDs) with histopathological abnormalities similar to NEC. The aim was to summarize the research findings, identify research gaps in existing literature, and diss
