A reconsideration of psychological specificity in psychosomatic disorders.

In this paper, the author identifies the evolution of the specificity hypothesis in the psychogenesis of psychosomatic illness across the last half century. Specifically identified is the distinction made between the mechanism of conversion and that of vegetative neurosis. Considerable attention is addressed to the personality features thought to be common to all psychosomatic patients and the refinement of this theory over the past 40 years, including the present interest in alexithymia. These concepts are explored in the consideration of a man who sustained two different specific conflictual

Icy Hot: A Case of Unexpected Heat Illness.

Finish-line collapse is a unique phenomenon among athletes and includes a broad working differential. We explore the case of a young, healthy runner who collapsed at the end of a half marathon in the fall from exertional heat illness. This illness covers a wide range of severity from the mild symptoms of heat cramps to heat exhaustion and the extreme of life-threatening heat stroke. Development of these conditions is often associated with patient-specific acquired or congenital vulnerability factors such as extremes of age, fitness, obesity, medication use (eg, antipsychotics, anticholinergics

Reincarceration Risk Among Men with Mental Illnesses Leaving Prison: A Risk Environment Analysis.

Reentry interventions for persons with mental illness leaving prison have consisted primarily of linkage to mental health services and have produced mixed results on psychiatric and criminal recidivism. These interventions primarily focus on intra-individual risk factors. However, social and environmental factors may also increase risk of reincarceration by constraining choices and pro-social opportunities for community reintegration upon release from prison. In order to add to the knowledge base on understanding reincarceration risk for men with mental illnesses leaving prison, we examined in

Food-borne norovirus-outbreak at a military base, Germany, 2009.

BACKGROUND: Norovirus is often transmitted from person-to-person. Transmission may also be food-borne, but only few norovirus outbreak investigations have identified food items as likely vehicles of norovirus transmission through an analytical epidemiological study.During 7-9 January, 2009, 36 persons at a military base in Germany fell ill with acute gastroenteritis. Food from the military base's canteen was suspected as vehicle of infection, norovirus as the pathogen causing the illnesses. An investigation was initiated to describe the outbreak's extent, to verify the pathogen, and to identif

Exertional Heat Illness in Active Populations.

AbstractExertional heat illness (EHI) is a leading yet preventable cause of morbidity and mortality in active populations. EHI develops when the body's integrated thermoregulatory responses, including cardiovascular adjustments and evaporative cooling, are unable to dissipate the combined burden of metabolic heat production, environmental heat stress, clothing or protective equipment, and other contributing factors. This review examines the physiological mechanisms that underlie EHI and the effects of heat stress on the exercising heart. We also summarize best practices for the prevention, rec

ICU Delirium.

Delirium is not a harmless transient event during ICU hospitalization; rather, it is a severe complication of critical illness associated with increased mortality, morbidity, and persistent disability. Despite being recognized for decades, it remains underdiagnosed. Employing validated tools for detection helps reduce missed cases. Early detection facilitates prompt management. Sedatives, opioids, and antipsychotics should be avoided whenever possible. Optimizing environmental triggers, minimizing iatrogenicity, and treating underlying critical illness constitute the basis of the currently rec

Hysteria split asunder.

The authors present the proposed DSM-III classification of the traditional hysterical disorders, i.e., disorders that suggest physical illness but in which psychological factors are judged to be of importance. The use of the DSM-III inclusion and exclusion criteria--physical mechanism explains the symptoms, symptoms are linked to psychological factors, symptom initiation is under voluntary control, and there is an obvious recognizable environmental goal--are discussed in the differential diagnosis of somatoform disorder, factitious disorder, malingering, psychological factors affecting physica

An outbreak of Salmonella Saintpaul gastroenteritis after attending a school camp in the Northern Territory, Australia.

An outbreak of salmonellosis occurred following attendance at a school camp between 5 and 8 August 2014 in a remote area of the Northern Territory, Australia. We conducted a retrospective cohort study via telephone interviews, using a structured questionnaire that recorded symptoms and exposures to foods and activities during the camp. A case was anyone with laboratory confirmed Salmonella Saintpaul infection or a clinically compatible illness after attending the camp. Environmental health officers from the Environmental Health Branch undertook an investigation and collected water and environm

Listeria monocytogenes cross-contamination of cheese: risk throughout the food supply chain.

Listeria monocytogenes has been the most common microbial cause of cheese-related recalls in both the United States and Canada in recent years. Since L. monocytogenes is inactivated by pasteurization, the majority of these cases have been linked to environmental and cross-contamination of fresh-soft, soft-ripened, and semi-soft cheeses. Cross-contamination of foods with L. monocytogenes is a continuous risk throughout the food supply chain and presents unique challenges for subsequent illness and outbreak investigations. Reports on outbreaks of listeriosis attributed to cross-contamination dow

Outbreak of mass sociogenic illness in a school feeding program in northwest Bangladesh, 2010.

BACKGROUND: In 2010, an acute illness outbreak was reported in school students eating high-energy biscuits supplied by the school feeding programme in northwest Bangladesh. We investigated this outbreak to describe the illness in terms of person, place and time, develop the timeline of events, and determine the cause and community perceptions regarding the outbreak. METHODS: We defined case-patients as students from affected schools reporting any two symptoms including abdominal pain, heartburn, bitter taste, and headache after eating biscuits on the day of illness. We conducted in-depth inte

Can urban regeneration programmes assist coping and recovery for people with mental illness? Suggestions from a qualitative case study.

Researchers and policy-makers are increasingly recognizing that urban socio-environmental conditions can affect the development and course of numerous health problems. The aim of this paper is to investigate the impact an urban regeneration programme can have on everyday functioning, coping and recovery for people with a mental illness. We were also interested in discerning which component parts of the regeneration are the most important in positively affecting people with mental illness. These questions were explored through an in-depth qualitative case study of the Gospel Oak neighbourhood i

Norovirus contamination of a drinking water supply at a hotel resort.

AIM: To investigate a waterborne gastroenteritis outbreak and consider wider environmental contamination concerns. METHODS: An acute gastroenteritis outbreak was investigated through interviews, analysis of faecal samples, drinking water and environmental water samples. RESULTS: A total of 53 cases reported an illness of acute gastroenteritis following stays and/or dining at a hotel or neighbouring resort in southern New Zealand over a 1-month period in early spring 2012. The consumption of table or tap water was strongly associated with the illness. Faecal samples were positive for noroviru

Risk factors for delirium in intensive care patients: a prospective cohort study.

INTRODUCTION: Delirium is a common complication in the intensive care unit. The attention of researchers has shifted from the treatment to the prevention of the syndrome necessitating the study of associated risk factors. METHODS: In a multicenter study at one university hospital, two community hospitals and one private hospital, all consecutive newly admitted adult patients were screened and included when reaching a Glasgow Coma Scale greater than 10. Nurse researchers assessed the patients for delirium using the NEECHAM Confusion Scale. Risk factors covered four domains: patient characteris

Factors associated with perceived helpfulness and use of mental health websites by Australian young adults with a family history of mental illness.

Young adults with a family history of mental illness may be vulnerable to develop mental health conditions based on genetic and environmental features. This study aimed to explore factors associated with the use of mental health websites for young adults aged 18-30 years who reported a family history of mental illness. Young adults (n = 469) who reported a family history of mental illness completed an online survey regarding their use of mental health websites, demographic questionnaires and the Depression, Anxiety and Stress Scales. In total, 194 (41%) participants reported using mental healt

There have been negative tendencies for the formation of public health to be preserved in the past decade. The most important tendency is for the worse physical development and health of children and adolescents. The 2002 All-Russian medical examination of children has provided support for the negative tendencies in the health status of children - the proportion of healthy children during the above period had reduced from 45.5% to 33.9%. The incidence of endocrine, digestive, and locomotor diseases, congenital malformations, and mental disorders has particularly increased. It should be said th

Norovirus Illnesses in Children and Adolescents.

Norovirus is a leading cause of childhood vomiting and diarrhea in the United States and globally. Although most illnesses caused by norovirus are self-resolving, severe outcomes may occur from dehydration, including hospitalization and death. A vast majority of deaths from norovirus occur in developing countries. Immunocompromised children are at risk for more severe outcomes. Treatment of norovirus illness is focused on early correction of dehydration and maintenance of fluid status and nutrition. Hand hygiene, exclusion of ill individuals, and environmental cleaning are important for norovi

Parental smoking: asthma and wheezing illnesses in infants and children.

Parental smoking has an important impact on asthma and wheezing illnesses in infants and children. In utero exposure is associated with impaired lung growth and wheezing illnesses, particularly in preschool children. Exposure to environmental tobacco smoke is associated with increased wheezing illnesses and increased symptoms in asthmatics. There are no consistent data to confirm an effect of in utero or postnatal cigarette smoke exposure on the prevalence of asthma but there is evidence of increased severity of symptoms. The detrimental effects of parental smoking on lung growth will have an
