We studied 31 patients with symptomatic focal epilepsy (SFE). Mean age of patients was 31.3 years. Twelve patients were first diagnosed with epilepsy, three of them started treatment; 22 patients received antiepileptic therapy. Thresholds of motor cortex excitability (MCE) in patients with SFE were significantly lower than in healthy people. Thresholds of MCE were lower in patients without treatment compared to controls (p < 0.004). Significant differences between thresholds of MCE in patients with- and without treatment were found. The threshold of MCE was higher in treated patients (p < 0.01

Focal epilepsies of childhood with complex partial seizures, with or without secondarily generalized seizures are reviewed. Particular emphasis is placed on the semiology of seizures, which are subdivided into: aura, disturbance of consciousness, speech disturbances, automatims, and other motor and vegetative seizure symptoms. The significance of routine ictal and interictal EGGs and their importance for the localization is discussed. Recent neuropathological findings and etiology are presented and the courses epilepsies can take (especially those involving febrile convulsions), prognosis and

A significant proportion of the childhood epilepsies have a genetic component. Therefore, animal models that can be bred for seizure expression may provide important information regarding the mechanisms by which molecular defects result in the neuronal hyperexcitability states collectively termed "epilepsy." Because of the rate and ease of breeding, rodent models are the most commonly used. The genetically epilepsy-prone rat has motor seizures in response to auditory stimuli. It is likely that the seizures are generated in the inferior colliculus because of an abnormality in the noradrenergic

The epileptic semiology of 19 patients (from 15 families) with mitochondrial disease due to mutations in the POLG1 gene is presented. The patients were either homozygous for the 1399G > A (p.A467T) or 2243G > C (p.W748S) mutations or compound heterozygotes for these two mutations. While the clinical features have been reviewed, detailed analysis of their epilepsy is presented for the first time. Irrespective of genotype, patients developed an epileptic syndrome with initial features of occipital lobe epilepsy. Occipital seizure phenomena included flickering coloured light, sometimes persisting

Epileptic myoclonus can be defined as an elementary electroclinical manifestation of epilepsy involving descending neurons, whose spatial (spread) or temporal (self-sustained repetition) amplification can trigger overt epileptic activity and can be classified as cortical (positive and negative), secondarily generalized, thalamo-cortical, and reticular. Cortical epileptic myoclonus represents a fragment of partial or symptomatic generalized epilepsy; thalamo-cortical epileptic myoclonus is a fragment of idiopathic generalized epilepsy. Reflex reticular myoclonus represents the clinical counterp

Patients with parietal lobe epilepsy (PLE) exhibit an electroclinical epilepsy syndrome that is rarely seen even at large epilepsy centers. Clinically, most patients with PLE exhibit a somatosensory aura that may include painful dysesthesias, though vertigo, aphasia, disturbances of one's body image also occur, when ictal propagation occurs from the parietal lobe to the supplementary motor area, hypermotor manifestations are noted. When temporolimbic propagation occurs, complex visual or auditory hallucinations and automatisms may appear. Scalp electroencephalogram (EEG) is often nonlocalizing

With the wider availability of magnetic resonance imaging cavernous malformations are being recognised with increasing frequency in those patients presenting with intractable epilepsy. Surgical resection is the treatment of choice. However, because these lesions are usually small and may be located in eloquent areas stereotactic resection should be considered. Stereotactically-guided resection of pathologically verified cavernous angiograms was performed in 10 patients in this series presenting with epilepsy (8 males, 2 females, mean age 32 years). Eight patients presented with medically intra

Epilepsy with continuous spike-waves during slow-wave sleep (CSWS) is often characterized by a severe cognitive and behavioral impairment. Symptomatic cases also include patients with an early-onset hydrocephalus, but in literature detailed neuropsychological data on these subjects are not available. We describe the results of serial cognitive assessments in a girl with shunted early-onset hydrocephalus, followed by partial epilepsy complicated with CSWS at 4 years 10 months, in which a dramatic cognitive and behavioral deterioration occurred few months after CSWS onset. Adrenocorticotropic ho

INTRODUCTION: Non-ketonic hyperglycemia (NKH) is recognized both as a direct cause of a precipitating factor of many types of epileptic seizures-partial motor, including partial motor status, complex partial, as well as reflex motor seizures and hemichorea-hemibalism syndrome. Less known is its association with isolated visual alterations. We present a clinical case with visual episodes and diffusion restriction.
MATERIAL AND METHODS: Clinical description together with the results of complementary tests: magnetic resonance imaging (MRI) brain sequences: T1W, T2W, FLAIR, DP, diffusion; and mag

Chronic progressive focal encephalitis or Rasmussen encephalitis (RE) is chronic brain disease of viral etiology. It is manifested by either partial motor or myoclonic seizures combined with hemiparesis. RE was described in detail in a girl of 8.5 years. The disease debut was observed at the age of 5.5 and manifested in the form of simple partial motor fits with serial episodes of secondary generalization. Post-attack hemiparesis was observed from the age of 7.5 and became permanent after 8. From this time myoclonic fits became constant, changed for the worse, memory and attention deteriorated

OBJECTIVE: We studied the effects and tolerability of lamotrigine (LTG) on refractory epilepsies in persons with severe motor and intellectual disabilities (SMID).
METHODS: We introduced LTG to 40 subjects all who were severely retarded and suffered from symptomatic epilepsies consisting of six cases of generalized epilepsy (GE), 33 of partial epilepsy (PE), and one of unclassified epilepsy (according to the 1989 ILAE classification standards). We evaluated the efficacy of LTG in seizure reduction and its tolerability for persons with SMID.
RESULTS: Following treatment, 83% of GE and 42% of

Putative etiology was studied in 991 patients with symptomatic localization-related epilepsies seen in a university hospital in South India. They formed 39% of patients with various types of epilepsies and epileptic syndromes seen during the study period. Seizure occurred in close temporal association with an acute central nervous system (CNS) insult in 53% of patients. Infections of CNS including single CT enhancing lesion (SCTEL) accounted for 77% of patients with acute symptomatic epilepsy. Cerebrovascular diseases were the risk factors in 48% of patients with remote symptomatic epilepsy. N

INTRODUCTION: A wide range of conditions are due to alterations in neurone migration (ANM). Mental retardation, motor disorders and epilepsy are seen in all these disorders. Anomalies included with the ANM are those produced at the time of neuronal migration in the phase prior to neurone proliferation and during the time following cortical organization. All these have a common characteristic, namely an anomalous cerebral cortex (cerebral dysplasias).
DEVELOPMENT: There is a high incidence of epilepsy in ANM (60%), appearing early (10% neonatal, 42% before 1 year old). The frequency of crises

OBJECTIVE: Cenobamate (CNB) is an effective antiseizure medication, though its mechanisms of efficacy remain incompletely understood. We assessed changes in cortical responses to transcranial magnetic stimulation (TMS) following CNB treatment.
METHODS: We recruited people with drug-resistant focal epilepsy scheduled to start add-on CNB in a tertiary epilepsy center. We performed TMS coupled with electromyography (EMG) and electroencephalography (EEG) at baseline, at 100 mg/day and at the maximum tolerated or minimum effective dose. Participants were categorized as responders (≥50% seizure red

PURPOSE: Occipital lobe epilepsy is uncommon in epilepsy surgery series and often difficult to assess due to rapid seizure propagation, misleading seizure semiology and confounding interictal epileptiform activity. Ictal recordings with surface electrodes may not define properly the seizure onset zone in surgical evaluation for intractable occipital epilepsy. Specially in dysplastic lesions, the extension of the epileptogenic zone is not well defined by neuroimaging techniques, therefore, implantation of intracranial electrodes is often indicated. In this study we present our experience with i

Mutations in the ATP1A2 gene cause familial hemiplegic migraine type 2, alternating hemiplegia of childhood, and cerebellar function deficits, epilepsy, and mental retardation. These symptoms are likely related to glutamatergic hyperexcitability. Our patient is a 12-year-old boy with a history of complex partial seizures, attention-deficit/hyperactivity disorder, and fine motor difficulty. During early childhood, he had episodes of a self-resolving right-sided hemiparesis and focal epilepsy. His seizures did not respond to several antiepileptic medications but stopped after he received valproa

INTRODUCTION: In the chronic phase of Hemiconvulsion-Hemiplegia-Epilepsy (HHE) syndrome, developing epilepsy may be intractable. Herein, we report a case where adrenocorticotropic hormone (ACTH) ceased an intractable habitual partial seizure in a patient with HHE syndrome.
CASE REPORT: A developmentally normal one-year-old girl presented with left focal motor status epilepticus in the clinical course of rotavirus infection. She was diagnosed with HH syndrome. At 4 months after status epilepticus, she developed partial seizures that occurred daily, and which resulted in a stooped posture, head

Perinatal hypoxic-ischemic encephalopathy in term infants has been well-documented as a cause of selective brain damage over the parasagittal brain regions in cases of partial asphyxia. We describe 10 patients (7 boys, 3 girls) who developed normally after perinatal asphyxia, and who later experienced parieto-occipital epilepsy. We retrospectively reviewed medical records, electroencephalograms, and neuroimaging findings. All patients had a history of perinatal asphyxia requiring intensive care. Subsequent developmental milestones were apparently normal. Seizures started at ages ranging from 1
