BACKGROUND: Traumatic brain injury (TBI) is a major risk factor for neurological disorders, including post-traumatic epilepsy (PTE), a debilitating condition associated with significant long-term consequences. The prognosis of PTE occurrence remains challenging due to the complex pathophysiology of PTE and the impracticality of traditional blood biomarker- or imaging-based screening for large populations. This study proposes a graph-based deep learning approach that leverages electronic health records (EHR) to enhance the predictive assessment of PTE risk.
METHODS: We utilized Oracle Real-Wor

BACKGROUND AND OBJECTIVES: Traumatic brain injury (TBI) is a well-established epilepsy risk factor and is common among service members. Deployment-related TBI, where combat/blast may be more common, may have different outcomes than nondeployment-related TBI. This work examined associations of all TBI exposures (not just combat), and epilepsy, while adjusting for comorbidities associated with epilepsy, among veterans by deployment status.
METHODS: The cohort included post-9/11 veterans with ≥2 years of care in both Veterans Health Administration and Defense Health Agency systems. We identified

BACKGROUND: Traumatic brain injury (TBI) is one of the most important causes of acquired structural epilepsy, post-traumatic epilepsy (PTE), however, efficient preventative measures and treatment are still not available to patients. Preclinical studies indicated biperiden, an anticholinergic drug, as a potential drug to modify the epileptogenic process. The main objective of this clinical trial is to evaluate the efficacy of biperiden as an antiepileptogenic agent in patients that suffered TBI.
METHODS: This prospective multicenter (n = 10) interventional study will include 312 adult patients

BACKGROUND: A plethora of data show that the hippocampus and the amygdala are involved in post-traumatic stress disorder (PTSD). Neural dysfunctions leading to PTSD (e.g. how the amygdala and the hippocampus are altered) are only partially known. The unusual case of a patient presenting with refractory epilepsy and developing PTSD immediately after surgery is described. Such symptoms in epileptic patients may help to explore PTSD mechanisms.
CASE REPORT: A 41-year-old male suffering from partial refractory temporal lobe epilepsy was operated in May 2017. A right amygdala, hippocampus, and tem

A theory is proposed to explain the benefits of yoga practices in diverse, frequently comorbid medical conditions based on the concept that yoga practices reduce allostatic load in stress response systems such that optimal homeostasis is restored. It is hypothesized that stress induces (1) imbalance of the autonomic nervous system (ANS) with decreased parasympathetic nervous system (PNS) and increased sympathetic nervous system (SNS) activity, (2) underactivity of the gamma amino-butyric acid (GABA) system, the primary inhibitory neurotransmitter system, and (3) increased allostatic load. BACKGROUND AND OBJECTIVES: Epilepsy is defined by the occurrence of multiple unprovoked seizures, but quality of life (QOL) in people with epilepsy is determined by multiple factors, in which psychiatric comorbid conditions play a pivotal role. Therefore, understanding the interplay between comorbid conditions and QOL across epilepsy phenotypes is an important step toward improved outcomes. Here, we report the impact of QOL across distinct epilepsy phenotypes in a cohort of post-9/11 veterans with high rates of traumatic brain injury (TBI).

BACKGROUND: Traumatic brain injury (TBI) causes early seizures and is the leading cause of post-traumatic epilepsy. We prospectively assessed structural imaging biomarkers differentiating patients who develop seizures secondary to TBI from patients who do not.
DESIGN: Multicentre prospective cohort study starting in 2018. Imaging data are acquired around day 14 post-injury, detection of seizure events occurred early (within 1 week) and late (up to 90 days post-TBI).
RESULTS: From a sample of 96 patients surviving moderate-to-severe TBI, we performed shape analysis of local volume deficits in

The long-term pathological effects of chronic epilepsy on normal brain ageing are unknown. Previous clinical and epidemiological studies show progressive cognitive decline in subsets of patients and an increased prevalence of Alzheimer's disease in epilepsy. In a post-mortem series of 138 patients with long-term, mainly drug-resistant epilepsy, we carried out Braak staging for Alzheimer's disease neurofibrillary pathology using tau protein immunohistochemistry. The stages were compared with clinicopathological factors, including seizure history and presence of old traumatic brain injury. Overa

INTRODUCTION: Traumatic brain injury (TBI), caused by external force to the head, leads to anatomical or functional damage to cranial structures. It is a leading cause of morbidity and mortality in adults worldwide, with substantial economic burden. Post-traumatic epilepsy (PTE) is a significant complication of TBI, posing immense challenges to rehabilitation and exacerbating socioeconomic burdens. The incidence of PTE varies widely, underscoring the need for early detection and treatment.
OBJECTIVE: Through prospective electroencephalography (EEG) evaluations over a two-year period, our stud

This research examined how self-concept and experiential avoidance influence posttraumatic growth (PTG) by considering the mediating effect of ego strength in individuals with epilepsy who have complex post-traumatic stress disorder (C-PTSD). The clinical sample for this study consisted of 137 patients with epilepsy who also had a history of C-PTSD and were selected using a convenience sampling method as part of a cross-sectional descriptive study design from Eqlid City, Iran. In this study, the data was collected using the Post-traumatic Growth Inventory (PTGI-21), the Personal Self-Concept Q

PURPOSE: Research suggests that there are reciprocal relationships between mental health (MH) disorders and epilepsy risk. However, MH relationships to post-traumatic epilepsy (PTE) have not been explored. Thus, the objective of this study was to assess associations between PTE and frequency of depression and/or anxiety in a cohort of individuals with moderate-to-severe TBI who received acute inpatient rehabilitation.
METHODS: Multivariate regression models were developed using a recent (2010-2012) cohort (n=867 unique participants) from the TBI Model Systems (TBIMS) National Database, a time

Traumatic brain injury (TBI) is a leading cause of acquired epilepsy referred to as post-traumatic epilepsy (PTE), characterized by spontaneous recurrent seizures (SRS) that start in the months or years following TBI. There is a critical need to develop small animal models for advancing the neurotherapeutics of PTE, which accounts for 20% of all acquired epilepsy cases. Despite many previous attempts, there are few PTE models with demonstrated consistency or longitudinal incidence of SRS, a critical feature for creating models for investigation of novel therapeutics for preventing PTE. The close association of alexithymia with various somatic diseases has been studied for more than fifty years by different medical and related researchers. In the past decade, research on the connection between alexithymia and autoimmune diseases, as well as neurological and mental disorders, has gained significant interest. A literature review with a search depth of 10 years is presented. A literature search was conducted in the elibrary.ru, CyberLeninka, PubMed, Scopus, and Google Scholar databases using the key term «alexithymia.» Current evidence suggests a much more heterogeneous understa

BACKGROUND: Upper-extremity deep vein thrombosis (UEDVT) is not uncommon in neurocritical patients with indwelling peripherally inserted central catheters (PICCs). Pharmacological anticoagulation following UEDVT poses a clinical challenge for these patients. Here, we present a rare mechanism contributing to UEDVT: distortion and deformation of a PICC line. A stepwise anticoagulation strategy was implemented while the PICC remained in situ.
CASE REPORT: A 79-year-old man underwent an urgent craniotomy to remove a massive traumatic subdural hematoma. A 4 Fr single-lumen PICC was inserted after

The presented article deals with traumatic brain injury, which can cause convulsive seizures with the subsequent development of epilepsy, which is especially characteristic of young people. The problems of classification of posttraumatic epileptic seizures, development of their mechanisms, characteristics of diagnosis, as well as the stages of epileptogenesis leading to the development of traumatic epilepsy are considered. The variants of therapy are given in view of traumatic brain injury. It is noted that the use of electrical stimulation during individual phases of paroxysmal oscillations c

Traumatic brain injury (TBI) is one of the commonest presentations to emergency departments and is associated with seizures carrying different significance at different stages following injury. We describe the epidemiology of early and late seizures following TBI, the significance of intracranial haemorrhage of different types in the risk of later epilepsy and the gaps in current understanding of risk factors contributing to the risk of post-traumatic epilepsy (PTE). The delay from injury to epilepsy presents an opportunity to understand the mechanisms underlying changes in the brain and how t

A biomarker is a characteristic that can be objectively measured as an indicator of normal biologic processes, pathogenic processes, or responses to an exposure or intervention, including therapeutic interventions. Biomarker modalities include molecular, histologic, radiographic, or physiologic characteristics. To improve the understanding and use of biomarker terminology in biomedical research, clinical practice, and medical product development, the Food and Drug Administration (FDA)-National Institutes of Health (NIH) Joint Leadership Council developed the BEST Resource (Biomarkers, Endpoint

Traumatic brain injury (TBI) affects about 50 million people in the world every year. Posttraumatic epilepsy (PTE) is a significant complication of TBI of any severity. PTE occurs in 20% of patients with TBI. Treatment of patients with PTE is particularly difficult due to obvious tendency towards drug resistance. Currently, there are no validated predictive biomarkers for PTE. Development of a system of validated predictive markers would improve PTE prediction quality and therapeutic approach for these patients. This review is devoted to the current data on the most perspective predictive biom

PURPOSE: Traumatic brain injury (TBI) is a severe public health problem worldwide. Early post-traumatic seizures (EPTSs) are acute symptomatic events corresponding to the brain's response to the physical effects of TBI. Although there are ongoing findings regarding the association between EPTS and TBI, the correlation between EPTS occurrence and the severity of TBI remains unknown. The lack of knowledge regarding such mechanisms can also explain ineffective management. This systematic review and meta-analysis aimed to determine whether EPTS became a predictor of severity in patients with TBI.
