An investigation of a novel reusable radiochromic sheet for 2D dose measurement.

PURPOSE: Radiochromic film remains a useful and versatile clinical dosimetry tool. Current film options are single use. Here, we introduce a novel prototype two-dimensional (2D) radiochromic sheet, which optically clears naturally at room temperature after irradiation and can be reused. We evaluate the sheets for potential as a 2D dosimeter and as a radiochromic bolus with capability for dose measurement. METHODS: A novel derivative of reusable Presage® was manufactured into thin sheets of 5 mm thickness. The sheets contained 2% cumin-leucomalachitegreen-diethylamine (LMG-DEA) and plasticizer

Intermittent self-catheterisation: teaching the skill to patients.

Intermittent self-catheterisation (ISC) is not a new concept. However, it is a method that could be more widely used by patients to drain the bladder rather than having an indwelling catheter with a drainage bag and its associated problems. The author describes the procedure, patient assessment, and the training required by patients so that they can undertake ISC. Self-catheterisation promotes patient independence, improves quality of life and helps to avoid complications associated with indwelling urinary catheters.

Safely reprocessing duodenoscopes: current evidence and future directions.

Highly publicised outbreaks of various infections related to the use of duodenoscopes have increased public awareness of the once arcane and largely ignored subject of endoscope reprocessing. Since 2015, national professional societies, multiple federal agencies, and even the US Senate have written reports and guidelines on duodenoscope reprocessing. However, their recommendations are sometimes contradictory and difficult to implement, and new research on the topic is published regularly which further complicates the situation. With this flood of new and sometimes contradictory information, cl

Microbiological surveillance after automated reprocessing of gastrointestinal endoscopes in routine clinical practice: A prospective randomized microbiological surveillance study.

Microbiological surveillance is used to monitor the quality of gastrointestinal endoscope reprocessing, but prospective data comparing automated endoscope reprocessor systems under routine workflow conditions remain limited. We conducted an exploratory, single-center, prospective randomized surveillance study of 222 endoscope-use episodes involving patient-used forward-viewing upper and lower gastrointestinal endoscopes. Endoscope-use episodes were allocated 1:1:1 to strongly acidic electrolyzed water, ozonated water, or peracetic acid using an Excel-generated random-number procedure (n = 74 e

Reusable vs. single use cystoscope: economic & environmental assessment.

PURPOSE: To introduce a scalable methodology for assessing the economic and environmental impacts of reusable versus single-use devices, with a novel approach of processing reusable devices through low-temperature sterilization (LTS) (STERRAD®). METHODS: The economic analysis used micro-costing techniques while carbon footprint was calculated with two different methods: "UO Sté method" derived from Deschavannes et al. (2021) and an ISO 14040/14044 Life Cycle Assessment (LCA) of the sterilization of the reusable device performed with OPENLCA v2.4.0 software and ELCD v3.2 free database. The unc

Potential impacts to human health from climate change: A comparative life-cycle assessment of single-use versus reusable devices flexible ureteroscopes.

Climate change poses a significant global health challenge, with medical procedures contributing substantially to CO2 emissions. Urology, as part of the broader healthcare sector, has begun integrating Planetary Health concepts to address this issue. While earlier studies have focused on Life Cycle Assessments (LCA) of urological procedures, these evaluations remain data-dependent, and insights into intra-hospital emissions are limited. This study introduces a methodical approach for analyzing intra-institutional processes of LCA for single-use and reusable flexible ureterorenoscopes (fURS). T

High-level disinfection of re-usable neonatal resuscitation equipment through boiling and steaming.

Infection and asphyxia are two major causes of neonatal death globally. Where single-use resuscitation devices or sterilization of re-usable devices are unavailable, there is a need for effective, low-cost methods of high-level disinfection. Laboratory validation examined the efficacy of boiling and enclosed steaming (without pressure) as methods for attaining high-level disinfection of re-usable neonatal resuscitation equipment. The microbial load extracted and measured for each test article met internationally accepted standards for high-level disinfection. Boiling and steaming are low-cost,

Code Grey: Stained Surgical Instruments and Their Impact on One Canadian Health Authority.

In 2016, NL's largest RHA was faced with serious challenges stemming from the discovery of stained surgical instruments at its two largest hospitals. This discovery prompted a series of postponed surgeries, an extensive internal mobilization of labour and the purchase of millions of dollars of new equipment. In tackling these challenges, the organization not only acquired a better understanding of its surgical tools, but it also gained renewed appreciation for the resilience of its human resources. By describing this incident and the lessons learned, we hope to offer insight to providers in si

[Recycling and reuse of disposable products in radiology].

Legitimacy and appropriateness of recycling and reuse of single-use disposable medical devices is a common issue in many areas of medicine. Advocates of reprocessing claim economic savings and environmental benefits. From the manufacturers' point of view, the single use is a characteristic property of their products. Moreover, they warn that using of recycled instruments makes the procedure more difficult and increases the rate of adverse events. As comparative tests demonstrate, reused products are sometimes inferior to the genuine ones with respect to mechanical properties, but these differe

An investigation of the effect on 27-gauge needle tips following a single local anaesthetic injection.

One hundred and forty one, 27-gauge local anaesthetic needles were collected, each following a single injection of local anaesthetic using four commonly performed local anaesthetic injection techniques. The needles were examined for needle tip damage under the scanning electron microscope. Ten unused needles were used as controls. Bone was contacted in 75 (59%) instances and, of these, 73 (97.3%) showed needle tip deformity. Of the four techniques used, bone contact was commonest with the inferior alveolar nerve block, occurring in 88% of instances.

[Reprocessing of medical devices in clinical and practical routine].

Clinics and hospitals are facing increasing challenges due to changes and reforms regarding the hygienic perspective on surgical procedures and the reprocessing of medical devices. Differences regarding the implementation of provisions in the individual German federal states add to the difficulty of maintaining an overview of all provisions. The KRINKO Recommendations for the Prevention of Nosocomial Infections (2018) and the Requirements for the Reprocessing of Medical Devices (2012) are the central documents for surgical operations. Regarding infection hygiene, it is no longer reasonable and

Validity of the efficacy of the spray flushing cleaning method in flexible endoscope reprocessing.

The advancement in endoscopic technology and techniques has increased its use in the diagnosis and treatment of gastrointestinal diseases. Reprocessing of reusable endoscopes remains a challenge. Inadequate reprocessing leads to bacterial contamination of the endoscope, a significant cause of endoscope-related infections. To thoroughly address the complexities of cleaning a flexible endoscope, various cleaning methods have been devised and tested across different studies. This letter evaluates one such method, spray flushing for flexible gastroscopes, developed and tested in a randomized contr

Point of Use Treatment for Medical Devices: From Bedside to Battlefield.

INTRODUCTION: Point of use (POU) treatment is a critical first step of medical device reprocessing. Reusable instruments and flexible endoscopes require a minimum of terminal sterilization or high-level disinfection, neither of which can be guaranteed if POU is performed incorrectly. Compliance considerations for POU include hospital accreditation readiness, unique austere surgical mission requirements, and the transition of future conflict towards Large Scale Combat Operations. This integrative review aims to describe POU for reusable instruments and endoscopes, and extrapolate implications f

PMCA to demonstrate the efficacy of prion inactivation methods on reusable medical devices: a relevant alternative to animal bioassays.

Validation of prion inactivation processes for medical devices relies on in-vivo experimental protocols. However, bioassays are costly, long (1-2 years) and ethically disputable. Additionally, results obtained with one prion strain - for example, 263K (hamster-adapted strain originating from sheep scrapie) - cannot be easily extrapolated to relevant human prion strains, further questioning the utility of bioassays. Over the past two decades, cell-free prion amplification assays have emerged as potential alternatives to bioassays. Rather than measuring prion infectivity, they quantify prion see
