Characterization of Eosinophilic Esophagitis From the European Pediatric Eosinophilic Esophagitis Registry (pEEr) of ESPGHAN.

OBJECTIVES: Few pediatric data on phenotypic aspects of eosinophilic esophagitis (EoE) are available. The pEEr registry was developed to prospectively characterize children with EoE from Europe and Israel. METHODS: pEEr is an ongoing prospective registry enrolling children with esophageal eosinophilia (≥15 eos/HPF). Anonymized data were collected from 19 pediatric centers. Data regarding demographics, clinical manifestations, endoscopy, histology, and therapies were collected. RESULTS: A total of 582 subjects (61% male) were analyzed. The median age at diagnosis was 10.5 years [interquartile

Prevalence of asymptomatic erosive esophagitis and factors associated with symptom presentation of erosive esophagitis.

OBJECTIVE: Asymptomatic erosive esophagitis (EE) is incidentally found during endoscopy for health screening. The authors have aimed to investigate the prevalence of asymptomatic EE in subjects undergoing health checkups and factors associated with symptom presentation of EE. METHODS: Among the consecutive 13,342 adults who underwent esophagogastroduodenoscopy in health checkup programs of Ajou University Hospital from May 2010 to April 2011, 8840 subjects (5683 men and 3157 women; median age, 45 years) who met the enrollment criteria were included in the analysis. They completed a questionna

A case of phlegmonous esophagitis complicated by pharyngeal infection: so-called descending necrotizing esophagitis.

BACKGROUND: Descending necrotizing mediastinitis (DNM) is a fatal disease caused by odontogenic or pharyngeal infection extending into the mediastinum along the deep fascial surface, retropharyngeal space, or peritracheal space. Although the esophagus is generally excluded from the infectious spillover route, in extremely rare cases deep neck infections can cause phlegmonous esophagitis. Herein, we report a case of phlegmonous esophagitis that was difficult to differentiate from DNM. CASE PRESENTATION: A 61-year-old man was diagnosed with deep neck infection and DNM based on radiological find

Severe E-Cigarette-Induced Esophagitis in a Patient With Eosinophilic Esophagitis: A Case Report.

Chemical esophagitis, a rare and potentially life-threatening condition, was diagnosed in a 24-year-old man with food allergies, presumed IgA nephropathy, eosinophilic esophagitis (EoE), and electronic cigarette (e-cigarette) use. After a week of vaping, he developed severe dysphagia, and endoscopy revealed extensive esophageal ulcerations and necrosis. Treatment included total parenteral nutrition, esomeprazole, and sucralfate. Follow-up endoscopy showed diverticular formations and friable lesions. This case underscores the emerging link between vaping and ulcerative esophagitis, especially i

Clinical Characteristics, Endoscopic Findings, and Treatment Outcomes in Lymphocytic Esophagitis Compared With Eosinophilic Esophagitis.

INTRODUCTION: Lymphocytic esophagitis (LyE) and eosinophilic esophagitis (EoE) are immune-mediated esophageal diseases. Clinical characteristics, endoscopic findings, and treatment outcomes of LyE were compared with EoE. METHODS: This was an international retrospective study on adults enrolled at 3 centers in Europe. We recorded clinical characteristics and endoscopy findings at baseline and symptoms, histology, and endoscopy outcomes after treatment of patients with LyE and EoE. RESULTS: Demographics, clinical presentation, comorbidities, and endoscopy findings were largely different in 35

Precision medicine in eosinophilic esophagitis.

Eosinophilic esophagitis (EoE) is a common, chronic, immune-mediated disease of the esophagus, affecting approximately 1 of 700 individuals in the United States. Despite its rising prevalence, clinical management remains largely empiric and invasive: diagnosis and monitoring rely on endoscopy with biopsies, and treatment selection is guided by population-level response rates, patient preference, and access rather than individualized disease biology. Patients often experience diagnostic delays, repeated procedures, and cycles of trial-and-error therapy before achieving remission, whereas prolon

Tissue remodeling in eosinophilic esophagitis.

Eosinophilic esophagitis (EoE) is a chronic inflammatory disease characterized by esophageal dysfunction and tissue eosinophilia that is often associated with food impaction and dysphagia. The disease is triggered by allergic hypersensitivity to food involving a strong type 2 immune response accompanied by tissue remodeling and, if left untreated, there can be progression to strictures thought to be due, in part, to unabated lamina propria fibrosis. The composite finding of fibrosis and stenosis is referred to as "fibrostenosis." Current pharmacologic therapies include approved topical cortico

Eosinophilic esophagitis - more than eosinophils?

PURPOSE OF REVIEW: Eosinophilic esophagitis (EoE) is a chronic, type 2 inflammatory disease with a rapidly rising incidence and is a leading cause of solid-food dysphagia in young adults. While diagnosis has traditionally relied on esophageal eosinophilia, the central role of eosinophils in driving symptoms and disease activity has been increasingly questioned, as several non or low-eosinophilic EoE variants have recently been characterized. This review summarizes the current understanding of these emerging entities and their position within the EoE spectrum. RECENT FINDINGS: Current concepts

Dietary Approaches in Eosinophilic Esophagitis.

Eosinophilic esophagitis (EoE) is a chronic, immune-mediated, non-IgE-mediated food allergy and a leading cause of dysphagia and food impaction in adults, with rising prevalence worldwide. Diagnosis requires ≥15 eosinophils per high-power field on esophageal biopsy after exclusion of other causes of gastrointestinal eosinophilia. Food allergens, principally milk, wheat, eggs, legumes and seafood, drive the inflammatory process, and treatment options include proton pump inhibitors, topical corticosteroids, biologic agents and endoscopic dilation for stricturing disease. Among these, dietary the

Lymphocytic esophagitis and ringed esophagus.

Lymphocytic esophagitis is an uncommon condition characterized by the presence of intraepithelial lymphocytosis and lack of eosinophils in the esophageal biopsy samples. The main clinical manifestations include dysphagia, heartburn, and vomiting; endoscopic changes are absent in up to 30% of cases, and may mimic the common gross aspects of an eosinophilic esophagitis as rings, longitudinal furrows, and stenosis. The authors emphasize the role of this diagnosis confirmation as early as possible, with prompt and adequate management aiming to avoid eventual unfavorable outcomes. The objective of

Animal models of eosinophilic esophagitis.

Eosinophilic esophagitis is a chronic inflammatory disorder of the esophagus. Over the past 25 yr, great strides have been made toward understanding its pathogenesis, in part due to studies in several types of animal models. The vast majority of these models have been characterized in mice. In this review, we summarize the histopathological features of eosinophilic esophagitis recapitulated by these animal models, as well as discuss their strengths and weaknesses.

Endoscopic Features of Eosinophilic Esophagitis.

Edema, rings, exudates, furrows, and strictures (EREFS) represent the major endoscopic features of eosinophilic esophagitis (EoE). The Endoscopic Reference System (EREFS) grading system is easy to learn and apply during daily clinical practice in the diagnosis and follow-up of EoE patients. When endoscopy is performed by an EoE-experienced physician, the EREFS criteria will identify the majority of EoE patients. The EREFS score from the area of greatest involvement of the esophagus should be reported. The EREFS grading system was formally validated as an endoscopy score and several randomized

Eosinophilic esophagitis: Does age matter?

OBJECTIVES: Eosinophilic esophagitis (EoE) is often diagnosed in school-age children between 6- and 9-year-old. There is less known about those who are diagnosed with EoE that are younger than 6 years old. The objective of this study is to compare clinical presentation, comorbidities, and outcomes based on age at diagnosis of EoE. METHODS: Single-center retrospective chart review of children (<18 years) diagnosed with EoE between 2005 and 2020. We recorded demographics, clinical presentation, family history, past medical history, treatment, and endoscopic findings. Children in this cohort wer

Dietary Management of Eosinophilic Esophagitis.

Eosinophilic esophagitis (EoE) is a chronic immune-mediated food allergy-driven disease characterized by eosinophilic inflammation of the esophagus leading to symptoms of esophageal dysfunction. Prior studies have supported the key role of food allergen exposure as the main driver behind the etiopathogenesis showing that removal of food antigens can result in disease remission in both children and adults. These landmark studies serve as the basis for the rising interest and evolution of dietary therapy in EoE. This article will focus on the rationale for dietary therapy in EoE and provide help

Eosinophilic esophagitis and esophageal microbiota.

Eosinophilic esophagitis (EoE) is an antigen-mediated chronic inflammatory disease of the esophagus, the prevalence of which has steadily increased in recent years. The pathogenesis of EoE is not yet well-defined; however, recent studies have demonstrated that the esophageal microbiota is an essential regulator of physiological and pathological processes of EoE. Currently, research on EoE and microbiota is an emerging field of study that is receiving increasing attention. Here, we review existing EoE-related esophageal microbiota studies to explore the potential mechanisms underlying esophagea

Barrier Dysfunction in Eosinophilic Esophagitis.

PURPOSE OF REVIEW: Compelling evidence over the past decade supports the central role of epithelial barrier dysfunction in the pathophysiology of eosinophilic esophagitis (EoE). The purpose of this review is to summarize the genetic, environmental, and immunologic factors driving epithelial barrier dysfunction, and how this impaired barrier can further promote the inflammatory response in EoE. RECENT FINDINGS: Common environmental exposures, such as detergents, may have a direct impact on the esophageal epithelial barrier. In addition, the effects of IL-13 on barrier dysfunction may be reduce

Endoscopy in Pediatric Eosinophilic Esophagitis.

Eosinophilic esophagitis (EoE) is a chronic allergen-mediated clinicopathologic condition that currently requires esophagogastroduodenoscopy with biopsies and histologic evaluation to diagnose and monitor its progress. This state-of-the art review outlines the pathophysiology of EoE, reviews the application of endoscopy as a diagnostic and therapeutic tool, and discusses potential complications related to therapeutic endoscopic interventions. It also introduces recent innovations that can enhance the endoscopist's ability to diagnose and monitor EoE with minimally invasive procedures and perfo

Medical treatment of eosinophilic esophagitis.

BACKGROUND: Eosinophilic esophagitis (EoE) is a chronic antigen-mediated eosinophilic inflammatory disease isolated to the esophagus. As a clinicopathologic disorder, a diagnosis of EoE requires a constellation of clinical symptoms of esophageal dysfunction and histologic findings (at least 15 eosinophils/high-powered microscope field (eos/hpf)). Current guidelines no longer require the failure of response to proton pump inhibitor medications to establish a diagnosis of EoE, but continue to suggest the exclusion of other etiologies of esophageal eosinophilia. The treatment goals for EoE are im

Emerging therapies for eosinophilic esophagitis.

Eosinophilic esophagitis (EoE) is an inflammatory condition of the esophagus that is quickly becoming a more recognized cause of esophageal dysfunction and dysphagia. The cause of EoE is thought to be due to an inflammatory response triggered by exposure to dietary or environmental antigens. Symptoms may be progressive and include reflux, nausea, vomiting, dysphagia, and reduction in quality of life. Both children and adults may be affected. Initial treatments for EoE have focused on elimination diets to potentially identify and remove dietary triggers, acid suppression with proton pump inhibi
