Esophagitis secondary to ingestion of caustic material.

The records of 176 patients admitted to Grady Memorial Hospital over a 10-year period with a history of ingestion of corrosive material were reviewed. Esophagoscopy was performed in 168 patients an average of 15 hours after ingestion. In 94 patients, no esophageal burn was found. Seventy-four others had injury of the esophagus of varying degree; 3 of these had no evidence of oropharyngeal burn. The patients with esophageal injury were treated with steroids for two weeks and antibiotics for 5 days. No complications resulted from esophagoscopy or steroid therapy in any patient. Stricture did not

Pediatric caustic ingestion.

Accidental caustic ingestion in the pediatric age group continues to be a controversial and difficult problem. This retrospective study gives clinical support to treating this problem with early esophagoscopy, steroids, antibiotics and dilatation. Four hundred two patients were evaluated for possible caustic burn of the esophagus. Of the 70 patients who had esophageal burns demonstrated at esophagoscopy and who were treated with steroids, antibiotics and dilatation, none developed strictures. The two patients not treated in this fashion developed strictures.

Management of esophageal foreign bodies: a retrospective review of 400 cases.

OBJECTIVE: A retrospective study was conducted in 400 patients with esophageal foreign bodies (EFB) to assess characteristics of EFB and methods of treatment. METHODS: From 1962 through 1998, 400 patients with EFB were treated in our department on an emergency basis. There were 202 men (50%) and 198 women (49.6%) ranging in age from 1.5 to 95 years. The main symptoms patients complained of were difficulty in swallowing and pain. Detailed anamnesis, oropharynx and hypopharynx examination and finally radiological examination were the diagnostic tools. The location of the FB was in the cervical

The Difficult Secondary Tracheoesophageal Puncture: A Technique for Safe Insertion.

BACKGROUND/AIMS: The tracheoesophageal prothesis (TEP) has become the primary modality for laryngeal communication after total laryngectomy due to high success rates, minimal morbidity, and more natural pulmonary driven speech. Fibrosis, kyphosis, and post-radiation contracture may preclude TEP placement through rigid esophagoscopy, and certain patients may not tolerate an in-office awake procedure. For such patients, a technique for flexible esophageal stenting and TEP placement is necessary. METHODS: We performed a retrospective review of 3 patients who underwent TEP placement through endot

Transendoscopic laser ablation of an esophageal plasma cell tumor in a cat.

CASE SUMMARY: A 15-year-old male castrated domestic shorthair cat was presented for acute lethargy, vomiting and hyporexia. Abdominal and thoracic radiographs revealed a caudal thoracic/esophageal soft tissue opacity with concern for an esophageal mass or an esophageal foreign body. Esophagoscopy confirmed the presence of a large, irregular, mid-esophageal mass. Laser ablation using esophagoscopy was utilized to debulk the mass. Approximately 80% of the mass was removed without complication and an esophagostomy feeding tube was placed. The cat was discharged the same day of the procedure. Hist

Esophageal hamartoma as an unusual cause of neonatal apnea and bradycardia.

Esophageal hamartoma is a rare entity, as only 7 cases have been previously reported in the English-language literature. Common symptoms include dysphagia, weight loss, and vomiting. Life-threatening airway obstruction can also occur. Because of the nonspecific nature of the symptoms, patients with these dangerous polyps are often misdiagnosed as having more common entities such as gastroesophageal reflux, peptic ulcer disease, or achalasia. Most of these tumors are missed on esophagoscopy and radiologic studies, and they can go undiagnosed for years. We diagnosed an esophageal hamartoma in an

Exploring a predictive model for screening eosinophilic esophagitis in children with dysphagia.

Eosinophilic esophagitis (EoE) is frequently associated with dysphagia and requires esophagoscopy with biopsies for diagnosis, as no reliable diagnostic biomarkers currently exist. While inflammatory indices derived from complete blood counts have been investigated in esophageal dysfunction, their role in screening for EoE remains unexplored. This study evaluates the utility of different predictive models for screening EoE through a diagnostic study of patients <15 years old undergoing esophagoscopy for suspected EoE in a pediatric hospital between 2015 and 2022 (Reg. 3318-0000206).

Panendoscopy in screening for synchronous primary malignancies.

The entire upper aerodigestive tract must be evaluated at the time of initial tumor evaluation in patients with squamous cell carcinoma of the head and neck. The necessity of panendoscopy (laryngoscopy, bronchoscopy, esophagoscopy) in this evaluation has not been demonstrated convincingly. Between January 1, 1984 and December 31, 1985, 208 patients with previously untreated squamous cell carcinoma of the head and neck were analyzed prospectively. These patients underwent head and neck examination, chest radiograph, barium esophagram, and panendoscopy. Fifteen (7.2%) had synchronous malignancie

Swallowed denture stuck in the proximal esophagus.

A 72-year-old male with dementia and Parkinson's disease presented at the otorhinolaryngology outpatient clinic with acute dysphagia. A chest x‑ray showed a dental prosthesis in the upper esophagus, which was subsequently extracted via rigid esophagoscopy. Due to suspected esophageal perforation on postoperative CT, a cervical approach to the esophagus and flexible esophagoscopy were used, but no evidence of perforation could be identified. This case highlights challenges in managing high-risk esophageal foreign bodies in the upper esophagus, emphasizing the need for careful assessment and a m

Consequences of caustic ingestions in children.

A retrospective analysis of 98 patients, less than 15 years of age, treated for caustic ingestion during 1976-1990 was performed to evaluate the modern consequences of caustic ingestion in children and to set indications for esophagoscopies and radiographic and laboratory examinations. Dishwasher detergents were ingested by 56 children. There were no lye ingestions, since lye has not been freely available in Finland since 1969. Household acetic acid (vinegar) was the most commonly (12/23) ingested acid. Primary esophagoscopy was performed in 79 of the 98 cases (80.6%). Esophageal burns were fo

Exploring the predictive power of a combined model of cellular indices and food allergies to screen for eosinophilic esophagitis in children.

Eosinophilic esophagitis (EoE) typically presents with dysphagia and is frequently linked to food allergies (FA), although esophagoscopy remains essential for diagnosis as no dependable diagnostic biomarkers are currently available. While cellular indices obtained from the hemogram have been evaluated as diagnostic markers in EoE, their utility when combined with FA history for screening EoE remains uninvestigated. This research explores the predictive power of combined models of cellular indices and FA history for EoE screening through a diagnostic study of patients <15 years old undergoing e

Comparison of two combined models of cellular indices and food allergies for the screening of pediatric eosinophilic esophagitis.

Eosinophilic esophagitis (EoE) presents with dysphagia and is associated with food allergy (FA). Although esophagoscopy remains essential for diagnosis, predictive models based on cellular indices from the hemogram and FA history have been evaluated as screening tools for EoE; however, their theoretical comparison for EoE screening has not been analyzed. This study synthesizes the current literature and compares the predictive capacity of two combined models using cellular indices and FA history for EoE screening through a diagnostic study of patients <15 years old undergoing esophagoscopy for

Microscopic removal of an embedded foreign body from the hypopharynx: report of two cases.

Incidents of foreign bodies in the hypopharynx, especially fish bones, are very common. In most cases, these bones can be easily located and removed. However, in other cases, they become embedded in the pharyngeal wall and cannot be located, even by fiberoptic endoscopy and rigid esophagoscopy. Left in place, these foreign bodies can eventually cause serious complications. We treated two patients who had an embedded foreign body in the hypopharyngeal wall that we were unable to locate by fiberoptic endoscopy and rigid esophagoscopy. Ultimately, we performed direct laryngoscopy and were able to

The incidence and diagnosis of secondary esophageal carcinoma in the head and neck cancer patient.

The historical development of additional primary neoplasms of the esophagus is traced. To determine the incidence and time of appearance of these esophageal malignancies, a retrospective study of 790 head and neck cancer patients was conducted. In addition, a prospective study of 268 head and neck cancer patients was undertaken to determine the optimum diagnostic approach for esophageal neoplasms. All patients in the prospective study had a pharyngoesophagogram plus esophagoscopy as part of the initial diagnostic evaluation. The false negative rate of the pharyngoesophagogram was 72.2% in pati

A controlled trial of corticosteroids in children with corrosive injury of the esophagus.

BACKGROUND: It is controversial whether treatment with corticosteroids reduces stricture formation in the esophagus after the ingestion of caustic material. METHODS: We conducted a prospective study over an 18-year period in which 60 children (median age, 2 years) with esophageal injury from the ingestion of caustic material were assigned randomly to treatment either with or without corticosteroids. The corticosteroids were given initially as prednisolon (2 mg per kilogram of body weight per day intravenously) and then as prednisone orally to complete a three-week course. All patients were ev

Successful treatment of congenital esophageal web by endoscopic electrocauterization and balloon dilatation.

We report a 3-year-old boy who presented with a foreign body impacted in the esophagus and had a poor tolerance of solid food since he was 5 to 6 months old. In the last 2 years, he developed progressive dysphagia, anorexia, vomiting, and poor weight gain. An esophagoscopy and barium esophagogram revealed an esophageal web in the distal third of the esophagus. Three courses of endoscopic balloon dilatation resulted in transient improvement in his dysphagia and vomiting; a follow-up barium esophagogram and esophagoscopy showed limited improvement of the esophageal stenosis. Two courses of endos
