A novel pathophysiology of Barrett's esophagus and a new method of assessing biopsy specimens in patients with gastroesophageal reflux disease (GERD) are presented. This is based on the observation in autopsy studies of patients without GERD that the squamous epithelium of the esophagus transitions directly to fundic mucosa in many people and that the cardiac mucosa is of very short length in others. Available evidence suggests that what is termed gastric cardiac mucosa is in reality an abnormal mucosa resulting from metaplasia of the squamous epithelium of the esophagus as a result of GERD. T

A 14-year-old girl presented with dysphagia and weight loss. Barium swallow, computed tomography, and endoscopy showed a polypoid intraluminal mass in the cervical esophagus. The tumor was locally resected and postoperative chemotherapy and radiotherapy were administered. Histological examination of the surgical specimen established a diagnosis of synovial sarcoma. The patient remains free of discernible disease 30 months after surgery. Esophageal sarcomas are rare tumors. This appears to be the second reported case of esophageal synovial sarcoma in a child.

Esophageal replacement by a segment of isoperistaltic ileum with cecum or by transverse or left colon will allow near-normal swallowing for many years. The authors reviewed the course of 59 children who had bypass of their entire esophagus and of four whose distal esophagus was resected and replaced. The follow-up period ranges from 1 to 37 years; in 36 cases, it exceeds 5 years. Thirty children had caustic strictures and 25 had either isolated esophageal atresia or atresia with fistula. Two children with esophageal injury caused by foreign body ingestion and two with congenital strictures als

Duodenogastric reflux has been implicated in the pathogenesis of complicated Barrett's esophagus and gastric ulceration. A group of 123 Barrett's patients were followed for a mean of 41 months; 9 (7%) developed gastric ulceration (GU). Of the Barrett's patients treated by antireflux surgery, 14% developed GU. We reviewed the gastric histology on 54 Barrett's patients (34 men, 20 women; average age 64 years) at presentation and classified the gastritis according to the Sydney system. A normal histologic pattern was present in seven patients; 15 patients had chronic pangastritis, 9 chronic antra

Leiomyosarcoma of the esophagus is an uncommon disease of which only 97 cases including the present case have been reported in Japan. We report a case of the tumor which showed multiple hematogenous metastases after surgery. A 73-year-old male was admitted complaining of dysphagia and vomiting. Esophagography and endoscopy revealed a large protruding lesion in the lower esophagus. CT scanning revealed threefold-sized extramural mass. Boring biopsies failed to yield evidence of malignancy. However, we performed surgical treatment because of the uncommon size for a benign tumor. The excised tumo

A ten year retrospective survey of 346 cases of oesophageal foreign bodies is reported. Although coins were the most frequent foreign bodies in the pediatry group, meat was the most common offender in the material taken as a whole. Rigid oesophagoscopy in general anaesthesia was performed on 312 patients. A foreign body was removed in 281 cases (90%). Only one case of oesophageal cancer was discovered. The risk of iatrogenic perforation of the oesophagus was greatest in old patients who had a lump of meat stuck in the distal third of the oesophagus.

Fewer than 1% of all lymphomas involve the esophagus; however, lymphoma of the esophagus represents an important cause of dysphagia. This study reviewed all cases of biopsy-proven lymphoma involving the esophagus presenting at our institution between 1945 and 1992. Twenty-seven cases were identified. Three were primary esophageal lymphomas. Eleven percent of the cases presented Hodgkin's disease. Eighty-nine percent were non-Hodgkin's lymphoma. Eighty-nine percent of the patients experienced dysphagia. Eleven lymphomas (41%) were located at the gastroesophageal junction, while the other 17 wer

Resection of carcinoma of the cervical portion of the oesophagus, if it is to be radical surgery, calls as a rule for concurrent laryngectomy. Patients with severe dysphagia accept gladly even this variant of surgery. In the authors' opinion it is safest to replace the extirpated oesophagus by the colon on a vascular pedicle of the left colic arteries. In some instances in that case even the caecum remains vital. Perspectively the method of choice of a resected cervical oesophagus will be replacement by an isolated jejunal loop.

Endosonography of the oesophagus and stomach is important for staging of carcinomas, for judging the degree of thickening of the wall, and for assessment of submucosal and intramural lesions and of space-occupying processes in extra-oesophageal and extragastric organs.

The study was carried on 65 cadavers: 50 adults and 15 newborns. The esophagus was injected at the level to the neck 16 times, in the thorax at the superior level 18 times, middle level 30 times and inferior level 21 times. The lymphatic pathways were named short when joining the nodes at the same level, half long when joining the nodes of the neighbouring level, long when joining third level, and very long. At the level of the neck were injected 24 lymph nodes chains: in 12.5% of the cases the pathways were long, joining the nodes of the arch of the Azygos vein. At the superior thorax level w

In the upper aerodigestive tract, carcinogenesis in squamous cell mucosa is characterized by a tendency to field carcinogenesis leading to multicentricity of lesions and synchronous or metachronous multiple tumoral lesions, namely multifocality. During pretherapy broncho-esophagoscopy carried out on ENT-cancer patients, the rate of synchronous second primary cancer is 24%. In 85% of the cases, these second primaries are detected at an early stage (in situ, microinvasive or submucosal carcinoma) and do not give rise to symptoms. Early diagnosis of cancer of the upper aerodigestive tract is poss

Bioptates and resected material from patients treated for cancer of the esophagus at the Kazakh Research Institute of Oncology in 1980-1989 were analysed. Adenogenic tumors made up 6.0% of the 1671 cases of esophageal cancer studied. A gradual increase in the percentage of adenogenic cancers was established (P < 0.05). A group of 100 cases of adenogenic cancers was examined in detail. Adenocarcinoma made up 5.3%, adenosquamous cancer--0.5 and adenocystic cancer--0.2%. Adenogenic cancers most often occurred in the lower thoracic part of the organ. Poorly differentiated adenocarcinoma predominat

Oesophageal stenosis is a frequent complication of caustic burns, especially when deep injuries are present, due to the presence of scar tissue. After the treatment of initial complications and after an initial endoscopic evaluation, a few preventive measures can be decided to protect the upper part of the gastrointestinal tract. These measures include complete parenteral nutrition for some, and calibrating nasogastric tube for others; corticosteroid treatment is discussed. When stenosis is present, endoscopic dilatations can be performed. If they failed or instead of them, surgical replacemen

Spontaneous rupture of the esophagus is a rare emergency that requires immediate diagnosis and superior surgical treatment. Delay in diagnosis is common and is caused mostly by lack of consciousness of the disease and consequent tendency to diagnose a cases as perforated peptic ulcer, pancreatitis, acute coronary disease etc. Mortality and morbidity increase proportionally to the time between esophageal perforation and adequate treatment. In this paper the Authors present a case of Boerhaave's syndrome in a patient who underwent two surgical repairs followed by leakages of the esophageal sutur

One hundred and twenty-two patients with advanced mega-esophagus managed by esophagectomy without thoracotomy and cervical gastroplasty were evaluated. Sixty-nine patients were followed up for periods of 6 months to 16 years. Clinical assessment included X-ray studies and endoscopy of the cervical esophagus and mobilized stomach. The most common postoperative complications were pleural effusion (22.1%) and cervical fistula (8.2%). Mortality was 4.18%. Regurgitation was the most frequent complaint in the late follow-up, followed by heartburn. Both symptoms were related to esophagitis and diffus

Thirty four (75.5%) individuals sustained gunshot wounds, nine (20%) stab wounds, while two (4.4%) suffered blunt trauma. Thirty four patients (75%) displayed severance of the cervical portion of the esophagus, seven (15.5%) of the thoracic segment and only four (8.8%) had injuries on the abdominal portion. The cervical esophagus was surgically approached through oblique left side cervicotomy with primary suture and drainage with Penrose drain. A postero-lateral right side thoracotomy was employed for lesions of the upper and intermediate portion of the thoracic esophagus. When the lower porti

We describe a case of pemphigus vulgaris involving the lower esophagus, in a patient with gastroesophageal reflux. As near as we can tell, this is the first reported case of esophageal involvement by pemphigus in the absence of oropharyngeal disease. Gastroesophageal reflux may have precipitated the development of esophageal pemphigus.

Two hundred thirty-nine patients with carcinoma of the esophagus and cardia were seen between 1971 and 1985. Of 128 adenocarcinomas, 108 (84%) were resected. There were 111 squamous cell tumors, of which 104 (94%) were resected. Hospital mortality rate (3 months) was 19.4% in patients with adenocarcinoma and 14.4% in those with squamous cell carcinoma. It was significantly higher in the over 70 age group in both series. The 1 year survival rate in the adenocarcinoma group (52.3%) was lower than in the squamous group (59.1%), which reflects the higher operative mortality rate in the former. The
