Are Duane syndrome and infantile esotropia allelic?

PURPOSE: To evaluate the clinical overlap of families with Duane syndrome and infantile esotropia to determine whether the identification of genes for Duane syndrome may explain some cases of infantile esotropia. METHODS: Three separate groups of patients were evaluated. 1) Families with features of infantile esotropia were identified through the Strabismus Inheritance Study Tasmania (SIST). Clinical details of participants and their families were reviewed for any cases of Duane syndrome. 2) Cases of Duane syndrome were identified through the clinical diagnostic database at the Royal Children

Investigation of albinism genes in congenital esotropia.

PURPOSE: Esotropia is a feature of albinism. Amongst esotropic patients there may be mild unrecognised albinos. Oculocutaneous albinism shares several clinical features with congenital esotropia. It is well known that mammals with oculocutaneous albinism have misrouted retinal ganglion cell axons, most likely caused by the absence of melanin or DOPA during development. We investigated the hypothesis that mutations in the albinism genes Tyrosinase, the P Gene, and TYRP1 may also be responsible for congenital esotropia via a similar mechanism. METHODS: We screened these three genes in 21 famili

The role of anisometropia in the development of accommodative esotropia.

PURPOSE: To determine if anisometropia increases the risk for the development of accommodative esotropia in hypermetropia. METHODS: Records of all new patients with a refractive error of > or = +2.00 (mean spherical equivalent [SE] of both eyes) over a 42-month period were reviewed. Three hundred forty-five (345) patients were thus analyzed to determine the effect of anisometropia (> or = 1 diopter [D]) on the relative risk of developing esodeviation and of requiring surgical correction once esodeviation was present (uncontrolled deviation). RESULTS: Anisometropia (> or = 1 D) increased the

Surgical dose-responses of bilateral medial rectus muscle recession in acute acquired comitant esotropia compared to infantile esotropia.

Objective: To determine the surgical dose-responses of the bilateral medial rectus recession in acute acquired comitant esotropia (AACE) compared to infantile esotropia. Design: Retrospective study. Methods: The medical records of patients diagnosed with AACE and infantile esotropia who underwent bilateral medial rectus muscle recession (BMRR) were reviewed. Patient characteristics, ocular examinations and surgical data were collected. A motor success was defined as horizontal deviation within 10 prism diopters (PD) of ortho, and a sensory success was defined as no diplopia at 6 months after t

[Efficacy of botulinum toxin in infantile esotropia].

INTRODUCTION AND OBJECTIVE: Botulinum toxin (BT) is used in a variety of therapeutic applications, including the treatment of strabismus. Two injection techniques coexist - transconjunctival injection and open sky injection. The goal of this study was to evaluate the results of BT injections in esotropia in children under 10 years of age and to compare the two techniques. PATIENTS AND METHOD: This is a retrospective, monocentric study, including children who received BT injections to treat their strabismus by the aforementioned techniques between 2014 and 2017. The results of these injections

Stereopsis in patients with refractive accommodative esotropia.

PURPOSE: To investigate the factors associated with stereopsis in patients with refractive accommodative esotropia. METHODS: The medical records of patients with refractive accommodative esotropia from January 2008 to June 2016 were analyzed retrospectively. Age, sex, cycloplegic refractive error, presence of amblyopia, angle of deviation, and stereoacuity and fusional ability were evaluated. Patients were divided into good stereopsis (40-100 arcsec) and poor stereopsis (>100 arcsec) groups. RESULTS: A total of 85 patients were included, 31 (36.5%) in the good stereopsis group. The initial m

Control of intermittent esotropia by head shaking.

Head shaking is a rare phenomenon that refers to repetitive, low-amplitude rhythmic oscillations of the head around the cervical axis. In otherwise neurologically normal children with concomitant ophthalmic disease, head shaking (and/or nodding, ie, vertical oscillations) can be an associated finding or a compensatory mechanism to improve visual acuity.(1) Head shaking as a compensatory mechanism to maintain ocular alignment in intermittent esotropia is the subject of this case report.

[Treatment of esotropia with a high AC/A ratio].

We have treated 55 children with accommodative esotropia and high AC/A ratio with progressive addition lenses, measured by heterophoria method. All of them wear full hyperopic refraction for distance and addition of +1.00 to 3.5 spheres for near vision. The minimal add that allowed the maximal reduction in the near deviation was prescribed appointed to follow-up control three months after the use of the new lenses, and thereafter every six months. A progressive reduction of the abnormally high AC/A ratio (difference between distance and near deviation measurements) showed 45 children (81.81%)

[Consecutive exotropia as a result of esotropia surgery].

PURPOSE: To find out the probable reasons of the postoperative consecutive exotropy. MATERIAL AND METHODS: We studied 22 patients aged more than 14 years, operated due to esotropia during the first 7 years of life. Two categories were formed: the first contained the patients in the age between 14 and 18, and the second one the patients 19 years old and older. The period between the first and the second procedure was not shorter than seven years. The reoperations were performed due to exotropia in 15 cases. The following examinations were performed before the surgery: visual acuity, squint ang

Associations between siblings for esotropia and exotropia.

OBJECTIVE: To quantify familial aggregation of esotropia and exotropia in children examined in a large multicenter study. METHODS: Pregnant women and their children were examined in the Collaborative Perinatal Project of the National Institute of Neurological Disorders and Stroke, Bethesda, Md. Strabismus was evaluated in the children during follow-up examinations up to the age of 7 years. The second-order generalized estimating equations approach to logistic regression was used to estimate familial aggregation of esotropia and exotropia. RESULTS: For any pair of siblings, the odds for one s

Is the incidence of constant esotropia in childhood reducing?

Episodes of strabismus surgery in the under-14 year age group in West Berkshire have reduced by 42%, from 22.7 to 13.2 per 10,000 population, between 1968 and 1985. Clinical audit of patterns of referral shows that the incidence of constant esotropia has reduced by 55%, from 28.3 to 12.8 per 10,000 population, between 1971 and 1991.

Variability of strabismus surgery for acquired esotropia.

We developed a mathematical model to predict the probable success rate of the first surgery to correct acquired esotropia with normal accommodative convergence-accommodation ratio in children. A computer-averaged dose-response curve of the effect of 68 bilateral medial rectus recessions was combined with various assumptions regarding likely errors in surgical performance and likely errors in measurement of the preoperative deviation. Fifteen different sets of reasonable assumptions were tested, giving a range of theoretical success rates from 72% to 96%. The results of this study clearly indic

Neurological pathologies in acute acquired comitant esotropia.

BACKGROUND: Acute acquired comitant esotropia (AACE) is an uncommon subtype of esotropia characterized by sudden and usually late onset of a relatively large angle of comitant esotropia with diplopia in older children and adults. METHODS: A literature survey regarding neurological pathologies in AACE was conducted using databases (PubMed, MEDLINE, EMBASE, BioMed Central, the Cochrane Library, and Web of Science) in order to collect data for a narrative review of published reports and available literature. RESULTS: The results of the literature survey were analyzed to provide an overview of t

Long-Term Follow-Up of Cyclic Esotropia.

BACKGROUND AND PURPOSE: To describe the development and treatment of patients with cyclic esotropia. PATIENTS AND METHODS: The clinical course of seven patients with cyclic esotropia were retrospectively reviewed and described. All patients underwent bilateral medial rectus recession based on the deviation manifested on the day they were esotropic. Additionally, one patient had previously undergone surgery for intermittent exotropia. RESULTS: All patients developed excellent postoperative alignment. After strabismus surgery, normal eye alignment with some degree of binocularity was reached i

Esotropia greater at distance: children vs adults.

IMPORTANCE: Esotropia greater at distance than at near can be related to abducens palsy or to divergence insufficiency. Mild abduction weakness can be difficult to detect, blurring the distinction between these 2 conditions. OBJECTIVES: To examine the clinical and eye movement findings that distinguish abducens palsy from divergence insufficiency and to account for these findings based on current knowledge of vergence neurophysiology and saccadic adaptation. DESIGN: Retrospective medical record review. SETTING: A pediatric referral center. PARTICIPANTS: The medical records of 32 patients w

Incomitant esotropia secondary to lateral rectus injury.

Incomitant esotropia simulating a sixth nerve palsy from lateral rectus injury is an unusual diagnosis. Two patients are presented both with abduction defects thought secondary to either a restrictive orbitopathy or sixth nerve palsy. Force generation testing revealed a paretic process. Neuroimaging demonstrated transection of the lateral rectus muscle in both patients. One was secondary to a lateral wall fracture and the other iatrogenic during lateral wall decompression surgery for thyroid orbitopathy. Lateral rectus injury should be considered in patients with a history of trauma or orbital

Bilateral phakic hypermetropic epikeratoplasty for accommodative esotropia.

Optical correction of high hypermetropia with glasses is the primary treatment for accommodative esotropia. These glasses are often poorly accepted by adolescents and young adults for they are cosmetically undesirable, heavy, and esotropia occurs with their removal. We report an 11 1/2-year-old, physically mature female with high hypermetropia and accommodative esotropia corrected to orthophoria with glasses who was unable to be weaned from her glasses. She became intolerant to glasses wear, refused contact lenses, and was treated successfully with bilateral phakic hypermetropic epikeratoplast
