BACKGROUND: Unconjugated estriol (uE3) is an important biomarker in second trimester prenatal screening. Previous studies from our laboratory identified rare interference in the Beckman uE3 assay due to anti-ALP antibodies, which could be mitigated with a scavenger or heat-inactivated ALP (hALP). We report on a brother and sister with adrenal insufficiency due to isolated adrenocorticotropin hormone deficiency discovered in the neonatal period. The first-born, a male infant, died; pathological findings suggested bilateral adrenal hypoplasia transmitted as an autosomal recessive trait. AIMS: The aim of this study was to establish the pharmacokinetic profile of serum oestriol (E3 ) concentrations over 24 h following application of vaginal E3 in chronic users (>12 weeks of E3 use). The interindividual and intraindividual differences before and after E3 were examined.
METHODS: Ten women participated. Vaginal cream was omitted for ≥36 h prior to the study days. Blood sampling was performed for E3 , oestradiol and oestrone concentrations prior to cream application and at 1, 2, 3, 5, 8, 10, 12 and 24 h afterwards. In five women, all samples were repeated on a separate day.

A randomized double-blind trial with 15 postmenopausal women led to a remarkable result. Local treatment with vaginal tablets just containing 0.03 mg estriol proved to be equivalent to the widely used dosage of 0.5 mg. The test drug Gynoflor E induced significant proliferation of the vaginal epithelium and thus offers the basis for a complete restoration of the vaginal environment. In order to compare the predictive efficacies of serum estriol and urinary estrogen excretion in early diagnosis of fetal distress, 255 patients with normal (N = 128) or complicated (N = 127) pregnancies were monitored with simultaneous determinations of serum estriol (radioimmunoassay) and urinary estrogen excretion/24 h (colorimetric method) after the 32nd week of pregnancy.
