How generative AI shapes the last meal at life's end.

BackgroundThe selection of an end-of-life meal for a person nearing death is often framed as a deeply personal and meaningful act. Distinct from the well-documented practice of last-meal requests before execution, food and meal choices during the terminal phase of illness are shaped by medical constraints, family dynamics, cultural expectations, and ethical judgements. Palliative-care nursing literature increasingly recognises food and mealtimes as sources of both meaning and distress for patients and families. As generative artificial intelligence (AI) systems are increasingly introduced into

Psychiatrist experience of remote consultations by telephone in an outpatient psychiatric department during the COVID-19 pandemic.

OBJECTIVE: In response to the COVID-19 pandemic, there has been a shift globally from face-to-face consultations to remote consultations. In our department, remote consultations have taken in the form of telephone consultations. In this paper, we set out to study a group of Irish psychiatrists' experience of these consultations. METHODS: We identified recurrent themes in the existing literature on doctors' experience of telephone consultations with a view to determining the applicability of these themes to a group of Irish psychiatrists. A questionnaire was developed based on themes in the li

Microaggressions: Privileged Observers' Duty to Act and What They Can Do.

Racism and sexism that manifest as microaggressions are commonly experienced by members of minoritized groups. These actions and comments erode their subjects' vitality and sense of belonging. Individuals from minoritized groups are often left in a quandary, weighing the potential benefits and risks of addressing the comments. Placing the burden to interrupt bias on our marginalized colleagues is unjust. In part, it is inappropriate to expect them to dismantle a system that they did not create. It is essential for individuals with privilege who observe microaggressions to address the speaker a

What Should ChatGPT Mean for Bioethics?

In the last several months, several major disciplines have started their initial reckoning with what ChatGPT and other Large Language Models (LLMs) mean for them - law, medicine, business among other professions. With a heavy dose of humility, given how fast the technology is moving and how uncertain its social implications are, this article attempts to give some early tentative thoughts on what ChatGPT might mean for bioethics. I will first argue that many bioethics issues raised by ChatGPT are similar to those raised by current medical AI - built into devices, decision support tools, data an

Protocol for a scoping review to understand how interorganisational electronic health records affect hospital physician and pharmacist decisions.

INTRODUCTION: Patient records are often fragmented across organisations and departments in UK health and care services, often due to substandard information technology. However, although government policy in the UK and internationally is strongly pushing 'digital transformation', the evidence for the positive impact of electronic information systems on cost, quality and safety of healthcare is far from clear. In particular, the mechanisms by which information availability is translated into better decision-making are not well understood. We do not know when a full interorganisational record is

Using hormones to manage dairy cow fertility: the clinical and ethical beliefs of veterinary practitioners.

In the face of a steady decline in dairy cow fertility over several decades, using hormones to assist reproduction has become common. In the European Union, hormones are prescription-only medicines, giving veterinary practitioners a central role in their deployment. This study explored the clinical and ethical beliefs of practitioners, and provides data on their current prescribing practices. During 2011, 93 practitioners working in England completed a questionnaire (95% response rate). Of the 714 non-organic farms they attended, only 4 farms (0.6%) never used hormones to assist the inseminati

Understanding how a national clinical audit is established, maintained and sustained using the Irish Hip Fracture Database as an exemplar: a realist evaluation protocol.

There is an increasing focus on clinical audit within the Irish Health System to drive quality improvement and standardise care, with many established and emerging clinical audits underway. Since the burden of data collection typically falls to busy clinical staff, understanding the impact of audit is especially crucial. However, the literature rarely explores how audits generate impact, for whom and in what context, despite the considerable effort invested in collecting this valuable data. This study will explore how a national clinical audit is established, maintained and sustained in order

Evidence mapping of preference elicitation for non-pharmaceutical interventions targeting respiratory viral transmission: A scoping review protocol.

INTRODUCTION: Non-pharmaceutical interventions (NPIs) such as mask use, physical distancing, business or school closure, and isolation have been central to controlling respiratory viral infections, including influenza, COVID-19, and respiratory syncytial virus (RSV). Understanding the preferences of the public and stakeholders for these interventions is critical to ensure their acceptability, uptake, and effectiveness. Discrete choice experiments (DCEs) provide a robust method to quantify how individuals value different attributes of NPIs and the trade-offs they are willing to make. However, e

The ethics and regulation of overcharging: issues in the commerciality of the health practitioner-patient relationship.

Overcharging by health practitioners is a difficult issue with few guidelines available for practitioners or patients. For the most part it has not been the subject of disciplinary censure and has been dealt with by conciliation processes. However, during 2013 the Singapore High Court twice addressed the commerciality of the health-practitioner-patient relationship, acknowledging that this is a fundamental attribute of the contemporary dynamic between providers and recipients of health services. In Lim Mey Lee Susan v Singapore Medical Council [2013] SGHC 122, it concluded that the obligation

Optimizing the HIV/AIDS informed consent process in India.

BACKGROUND: While the basic ethical issues regarding consent may be universal to all countries, the consent procedures required by international review boards which include detailed scientific and legal information, may not be optimal when administered within certain populations. The time and the technicalities of the process itself intimidate individuals in societies where literacy and awareness about medical and legal rights is low. METHODS: In this study, we examined pregnant women's understanding of group education and counseling (GEC) about HIV/AIDS provided within an antenatal clinic in

COVID-19 legislative response and challenges in Republic of Korea.

This narrative review systematically analyzes the longitudinal evolution of South Korea's legal and institutional frameworks from the 2015 MERS outbreak through the COVID-19 pandemic. The study introduces the "Pandemic Response Pentad," an original conceptual model positioning Legislation as the foundational core, mediated by Governance, to drive three interconnected field operations. Based on this framework, Korea's infectious disease control system is evaluated across four operational domains: (1) governance reform and personnel structure enhancement, (2) epidemiological response capabilitie

The conect4children (c4c) Consortium: Potential for Improving European Clinical Research into Medicines for Children.

The need for information about new and existing drugs used in children was recognized in the European Union (EU) with the implementation of the Paediatric Regulation in 2007. In 2017, the 10-year review of the Paediatric Regulation identified barriers to the conduct of clinical trials, including delays in setting up and completing paediatric trials. Across Europe, the difficulties with clinical research are compounded by variation within countries and between countries. Ethics and regulatory review have national specificities. This paper describes the Collaborative Network for European Clinica

Neurobehavioral Effects of Organic Solvents Exposure Among Wood Furniture Makers in Ile-Ife, Osun State, Southwestern Nigeria.

BACKGROUND: Furniture making industries are small scale businesses that commonly use organic solvents. There has been minimal focus on the health effects of this chemical hazard on the nervous system among furniture makers in Nigeria. OBJECTIVES: The present study aimed to assess the association between organic solvents exposure and neurobehavioral status of furniture makers, using electronic technicians as a comparison group. METHODS: A comparative cross-sectional study design was employed. A sample size of 108 was calculated for each group. A semi-structured interviewer-administered questi

Integrating a brief mental health intervention into primary care services for patients with HIV and diabetes in South Africa: study protocol for a trial-based economic evaluation.

INTRODUCTION: Depression and alcohol use disorders are international public health priorities for which there is a substantial treatment gap. Brief mental health interventions delivered by lay health workers in primary care services may reduce this gap. There is limited economic evidence assessing the cost-effectiveness of such interventions in low-income and middle-income countries. This paper describes the proposed economic evaluation of a health systems intervention testing the effectiveness, cost-effectiveness and cost-utility of two task-sharing approaches to integrating services for comm

Perceptions of diabetes patients and their caregivers regarding access to medicine in a severely constrained health system: A qualitative study in Harare, Zimbabwe.

Nearly half of all sub-Saharan African countries lack operational Diabetes Mellitus policies. This represents an opportunity to build reliable evidence to underpin such policies when they are eventually developed. Representing the interests of those with the experience of living with the condition in national diabetes policies is important, particularly the interests regarding medicine access, a key pillar in diabetes management. One way to achieve this representation is to publish patient perceptions. Patient perspectives are especially valuable in the context of diabetes in Sub-Saharan Afric

Consequences and potential problems of operating room outbursts and temper tantrums by surgeons.

BACKGROUND: Anecdotal tales of colorful temper tantrums and outbursts by surgeons directed at operating room nurses and at times other health care providers, like residents and fellows, are part of the history of surgery and include not only verbal abuse but also instrument throwing and real harassment. Our Editor-in-Chief, Dr. Nancy Epstein, has made the literature review of "Are there truly any risks and consequences when spine surgeons mistreat their predominantly female OR nursing staff/colleagues, and what can we do about it?," an assigned topic for members of the editorial board as part

Protocol for an economic evaluation alongside a natural experiment to evaluate the impact of later trading hours for bars and clubs in the night-time economy in Scotland: The ELEPHANT study.

INTRODUCTION: The night-time economy comprises various sectors, including hospitality, transportation and entertainment, which generate substantial revenues and contribute to employment opportunities. Furthermore, the night-time economy provides spaces for leisure activities, cultural expression and social interaction. On-trade alcohol premises (places where consumers can buy and consume alcohol such as bars, pubs, clubs and restaurants) are a significant component of this night-time economy, functioning as focal points for socialising, entertainment and cultural events. However, when on-trade

AI at the Bedside: An Argument for a Training Wheels Approach.

Bedside use of artificial intelligence (AI) platforms is increasingly common. Busy clinicians may welcome these generative AI (Gen AI) tools, which have the potential to streamline many time-consuming tasks and aid in patient care. Trainees may find them useful to quickly evaluate complex medical information. Acceptance of bedside Gen AI tools by patients and their families, however, is less clear, and a definitive standard surrounding informed consent has yet to be established. Omission of certain types of information by Gen AI tools, including "small talk," which comprises an essential eleme

Comparative analysis of official controls and voluntary certifications in ensuring food safety compliance: a review.

Regulation (EU) 625/2017 emphasizes prevention over inspection, assigning primary responsibility for food safety to the food business operator. At the same time, official control systems ensure compliance with the European Union regulations. In recent years, voluntary third-party assurance programs have emerged as a complement to public regulations. This review explores their role, advantages, and challenges within this evolving regulatory landscape. Private standards have expanded rapidly in response to consumer demand for safer and more ethically produced food. While these certifications off
