A case of toxic hepatitis caused by combination therapy with methotrexate and etretinate in the treatment of severe psoriasis is presented in a 47-year-old woman. The patient had received methotrexate alone for more than 10 years without any signs of severe liver damage. The liver status had been controlled by consecutive liver biopsies. The treatment was discontinued and after 2 months the liver parameters became normal.

50 patients with actinic keratosis were studied over four months of treatment with either etretinate ('Tigason') or placebo. Each treatment was given for two months and the order of administration was randomised. The clinical response to treatment was assessed by direct measurement and photographs of the lesions at monthly intervals. The scalp hair of 15 patients, who were treated with etretinate for at least 6 months, was investigated, with the aims of confirming the previously described reduction in the duration of anagen and establishing the mechanism of etretinate alopecia. A combination therapy of mycosis fungoides (MF) with interferon-alpha (INF-alpha) and etretinate was reported. A 53-year-old patient suffering from MF was treated with intramuscular administration of INF-alpha at a dosage of 9 x 10(6) IU daily; skin lesions poorly responded to the treatment. After oral etretinate with a dosage of 50 mg daily was combined with INF-alpha, marked clinical improvement was seen in skin lesions with minor side effects. INF-alpha in combination with etretinate can have beneficial effects in the treatment of MF.

We cared for twin females with severe lamellar exfoliation of the newborn (collodion babies) and a family history of death of two brothers at age 3 months. We report on a 50-year-old woman having suffered from atypically located Kyrle's disease for 2 years. Especially the palmoplantar areas were affected by keratotic erythematous papules showing the typical histologic picture of Kyrle's disease. Treatment with acitretin (initial dose 30 mg/day) resulted in almost complete remission after 6 months. A concomitant lichen nitidus remained unchanged.

We have investigated the clinical responses of 21 patients with severe psoriasis to therapy with etretin, a metabolite of etretinate. In a preliminary double-blind dose-finding study, the optimum dose of etretin was determined to be 50 mg/d (mean, 0.66 mg/kg/d). To evaluate the clinical-effectiveness of etretinate in the treatment of papilloma virus infections, 20 children with extensive warts were given this oral retinoid for a period not exceeding three months at a dosage of 1 mg per kg per day. Sixteen patients showed complete regression of the disease without relapse, while in 4, lesions recurred after partial regression had been obtained. A follow-up of two years confirmed these findings. A 22-year-old woman suffering from dyskeratosis follicularis Darier became pregnant 4 months after the last dose of etretinate (Tigason). The pregnancy was terminated in the 10th week because of teratogenic risk. The pathological examination of the fetus revealed severe malformation of the left leg with aplasia of tibia and fibula, as well as hypoplasia of the left femur. This observation emphasises the necessity of effective contraception even after discontinuation of etretinate therapy.

Cutaneous manifestations in a case of Reiter's disease were successfully treated by oral etretinate administration. The clinical and histopathological similarities between the skin lesions of Reiter's disease and those of pustular psoriasis, which is known to respond to aromatic retinoids, suggested this treatment.

A total of 40 patients with palmoplantar pustulosis (PPP) were initially treated with oral etretinate (Tigason) in an open trail with a maximum treatment period of 16 weeks. Remission, with only slight residual changes in some cases, was achieved in 26 patients (65%) who were randomized to either a low dose of Tigason or placebo. Eleven patients suffering from cutaneous T-cell lymphoma (mycosis fungoides) were treated with recombinant interferon alpha-2A in combination (seven patients) or alone. Two patients, one in combined treatment, went into clinical complete remission, and five experienced partial remission. Two patients progressed during therapy, and two were nonevaluable because they stopped treatment early due to side effects. Hailey-Hailey disease (HHD; familial benign chronic pemphigus) is a hereditary blistering disorder characterized by episodic maceration and erosions mainly in intertriginous areas, and generalized eruptions are rarely seen. We report here a 51-year-old woman with generalized HHD who was successfully treated with oral etretinate. The present case suggests that oral etretinate is effective against the generalized eruptions even in cases in which bacterial infection has triggered the generalization of HHD.

The light and electron microscopic structure of pityriasis rubra pilaris (PRP) is described in five patients. A randomized open study was undertaken to compare the therapeutic effects between low-dose etretinate alone and low-dose etretinate combined with eicosapentaenoic acid in 40 patients with chronic, stable psoriasis vulgaris. Better and more rapid improvement was obtained with the combination therapy for 12 weeks than with low-dose etretinate monotherapy. Eicosapentaenoic acid was safe, and adverse reactions due to low-dose etretinate were mild or tolerable. The combination regimen, therefore, has a satisfactory effect on psoriasis without marked adverse reactions.

Three patients with epidermodysplasia verruciformis (EV) were treated with etretinate for 9-13 months. The patients had lesions characteristic of EV, including flat warts, common genital warts, pityriasis-versicolor-like lesions and malignant changes such as actinic keratosis and Bowenoid cancer in situ. Fingernail growth was studied in 28 patients who were receiving systemic treatment with the aromatic retinoid, Ro 10-9359 known as Tigason (etretinate; AR). A group of 15 patients with psoriasis was compared with a group of 13 patients with other dermatoses which are known to have a good response to AR therapy. In the second group, oral therapy with AR appeared to have no effect on the daily growth rate of fingernails (99 +/- 4 microns). With AR treatment, the nail-plate growth rate in the psoriatics increased to 132 +/- 5 microns.
