Evoked potentials as a biomarker of remyelination.

Multiple sclerosis (MS) is a common cause of neurologic disease in young adults that is primarily treated with disease-modifying therapies which target the immune and inflammatory responses. Promotion of remyelination has opened a new therapeutic avenue, but how best to determine efficacy of remyelinating drugs remains unresolved. Although prolongation and then shortening of visual evoked potential (VEP) latencies in optic neuritis in MS may identify demyelination and remyelination, this has not been directly confirmed. We recorded VEPs in a model in which there is complete demyelination of th

Neonatal somatosensory evoked potentials persist during hypothermia.

AIM: Treatment with therapeutic hypothermia has challenged the use of amplitude-integrated electroencephalography in predicting outcomes after perinatal asphyxia. In this study, we assessed the feasibility and gain of somatosensory evoked potentials (SEP) during hypothermia. METHODS: This retrospective study comprised neonates from 35 + 6 to 42 + 2 gestational weeks and treated for asphyxia or hypoxic-ischaemic encephalopathy at Helsinki University Hospital between 14 February 2007 and 23 December 2009. This period was partly before the introduction of routine therapeutic hypothermia, which e

[Clinical application of pain-related evoked potentials].

Pain-related evoked potentials (PREPs) represent a novel method for the evaluation of peripheral and central nociceptive pathways, e.g. in the diagnosis of small fiber neuropathy (SFN) or after therapeutic interventions for headache. Compared to contact heat-evoked and laser-evoked potentials, recording of PREPs is less stressful for the subjects and technically less demanding. The clinical usefulness of PREPs has been described for SFN associated with diabetes, HIV and hepatitis C infections as well as in headache and facial pain disorders. They have also been evaluated after interventional m

Vestibular evoked myogenic potentials in Behcet's disease.

The aim of this study was to investigate vestibular evoked myogenic potentials (VEMPs) and their clinical significance in Behcet's disease. Twenty-six patients with Behcet's disease and 25 healthy volunteers were evaluated for pure tone audiometry, caloric response, and VEMPs. Sensorineural hearing loss was found in 53.8% of patients with Behcet's disease, which was significantly higher than controls. Four patients had canal paresis, but no controls; this difference was not significant. Although VEMP recordings were elicited in all study subjects, mean p13 and n23 latencies were prolonged in B

Vestibular evoked myogenic potentials in preterm infants.

The goal of this study was to determine whether there was an association between perinatal risk factors of prematurity and vestibular evoked myogenic potentials (VEMPs). A prospective case-control trial was designed. Fifty preterm newborns (100 ears) with a gestational age <37 weeks were included. The control group consisted of 20 healthy term infants (40 ears). VEMP recordings were performed, and mean latencies of p13 were calculated in all study subjects. Multivariable logistic regression was used to investigate the influence of perinatal variables on abnormal VEMP responses. VEMPs were elic

Vestibular evoked myogenic potentials in neurofibromatosis 2.

Neurofibromatosis 2 (NF2) is characterized by bilateral vestibular neurofibromas. Although the facial nerve, the cochlear nerve, and the superior division of the vestibular nerve can be evaluated before surgery, whether the inferior division of the vestibular nerve is affected remains undetermined without an operation. A total of 7 patients with NF2 (2 men and 5 women) underwent pure tone audiometry, caloric testing, vestibular evoked myogenic potential (VEMP) testing, and magnetic resonance imaging. Audiometry revealed mean hearing levels of less than 26 dB for 1 ear, 26 to 70 dB for 8 ears,

Early visual evoked potentials in callosal agenesis.

Three participants with callosal agenesis and 12 neurologically normal participants were tested on a simple reaction time task, with visual evoked potentials collected using a high-density 128-channel system. Independent-components analyses were performed on the averaged visual evoked potentials to isolate the components of interest. Contrary to previous research with acallosals, evidence of ipsilateral activation was present in all 3 participants. Although ipsilateral visual components were present in all 4 unilateral conditions in the 2 related acallosal participants, in the 3rd, these were

Vestibular evoked myogenic potentials in brainstem stroke.

OBJECTIVES/HYPOTHESIS: Despite its widespread application in the posterior fossa tumor, the study of vestibular evoked myogenic potential (VEMP) in cases of posterior fossa stroke remains scarce. The purpose of the study was to establish the role of VEMP in patients with brainstem stroke. STUDY DESIGN: Retrospective study. METHODS: Patients with acute vertigo were admitted and underwent a battery of auditory vestibular tests including caloric and VEMP tests; then they were surveyed by magnetic resonance imaging scan. Seven patients (two men and five women) were demonstrated as having brainst

[Workshop on electroretinography and visual evoked potentials].

Studies of vision using electrophysiological techniques are tests which record functional alterations in very early phases of a disease. Full field flash ERG evaluates the integrity of the cones, rods and bipolar cells. It is obtained by photopsic stimulation with Ganzfeld and contact lens corneal electrodes or, alternatively, gold leaf or anchor electrodes. Five responses must be obtained: that of the rods, a maximum, oscillatory potentials, that of the cones and a flicker response. Pattern ERG is a response of the ganglionic cells, which is obtained by pattern reversal morphoscopic stimulati

Laser-evoked potentials in post-herpetic neuralgia.

OBJECTIVE: We evaluated the reliability of laser-evoked potentials (LEPs) as a diagnostic tool in patients with post-herpetic neuralgia (PHN), i.e. a chronic painful condition that causes small-diameter fibre dysfunction. Furthermore, we sought information on pathophysiology of PHN pain. METHODS: We recorded 'late' LEPs after stimulation of the supraorbital, upper cervical, lower cervical, upper thoracic, mid thoracic, and lower thoracic territories in 12 control subjects and 40 patients with PHN. We also determined the correlation of LEP data with age, duration of disease, and severity and q

Evoked potentials in children with Wilson's disease.

We assessed multimodal evoked potentials (EPs) in 13 children with newly diagnosed neurologically symptomatic Wilson's disease (WD) and in their first degree symptom-free relatives, consisting of seven presymptomatic and 15 asymptomatic siblings and 22 asymptomatic parents. EP abnormalities of at least one modality and one side stimulation were observed in 38.5% of patients, 42.9% of presymptomatic siblings, 21.4% of asymptomatic siblings and 18.2% of parents. Patients tended to have more prolonged central latencies of EPs. However, the left I-V interpeak brainstem auditory EP latency differen

[Visual evoked potentials in Creutzfeldt-Jakob disease].

Neuropathological studies show frequent and extensive effects on the visual system in Creutzfeldt-Jakob disease (CJD), but deterioration of vision is not reported by all patients. We examined the function of the visual system by means of visual evoked potentials (VEP). We recorded monocular pattern-reversal VEP in six patients with sporadic CJD 1-13 months after first symptoms occurred. Three patients had normal vision, and in a further three, vision was impaired. All patients had pathological VEP with a delayed P100 component (six eyes) or loss of cortical response (five eyes). The patients w

Somatosensory evoked potentials in progressive supranuclear palsy.

We report median and digital nerve somatosensory evoked potentials (SEPs) in 14 patients with probable progressive supranuclear palsy (PSP) along with transcortical long-loop reflexes, motor evoked potentials, and auditory startle responses. Enlarged cortical responses were found in 14 median and 13 digital nerve SEP studies, while long-loop reflexes were increased in only one patient. All motor evoked potential studies were normal. Auditory startle responses were either absent or reduced in 11 patients. The observed neurophysiological abnormalities may reflect cortical hyperexcitability but a

Multimodality evoked potentials in patients of heatstroke.

Brainstem auditory evoked potentials (BAEP) and somatosensory evoked potentials (SSEP) were recorded in 10 patients of heatstroke. Glasgow coma scale was used to assess level of consciousness. Values of either BAEP and/or SSEP were abnormal in all patients. Most consistent abnormalities of BAEP were delayed peak, latency of wave III & V and delayed IPL of III-V & I-V. The most consistent abnormality of SSEP were delayed absolute peak, latency of N20 and delayed IPL of N13-N20 (CCT). Distorted N20 was observed in 40 percent patients. The delay in conduction of electrical activities throughout t

The diagnostic utility of dermatomal evoked potentials.

A study was completed to evaluate the diagnostic utility of dermatomal evoked potentials (EPs) for identifying single and multiple sensory nerve root compromise. We examined the clinical records of 37 patients referred for neck and lower back pain with particular attention paid to presenting clinical symptoms, magnetic resonance (MR) imaging results, and cervical and/or lumbar dermatomal EP results. The primary object of this study was to determine whether or not dermatomal EPs provided useful diagnostic information about nerve root function that would supplement the structural information pro

Intercostal somatosensory-evoked potentials. A new technique.

Presently, there are few electrodiagnostic medicine techniques to evaluate lesions affecting the thoracic nerve roots or spinal cord. A new electrophysiologic technique to assess these structures, intercostal somatosensory-evoked potentials (SEPs), is described. Thirty neurologically normal subjects were used in this investigation to generate intercostal SEPs. Bilateral intercostal SEPs were easily elicited after stimulation of the third intercostal nerves just lateral to the sternum anteriorly. Intercostal SEPs were also easily elicited from the fifth, seventh and ninth intercostal nerves alo

Somatosensory evoked potentials in juvenile myoclonic epilepsy.

We analyzed the somatosensory evoked potentials (SEP) of 35 patients with juvenile myoclonic epilepsy (JME), 26 patients with idiopathic generalized epilepsy (IGE), and a control group consisting of 24 healthy people. The N19-P25 interval was significantly prolonged in the IGE group both as compared with the JME and control groups. This finding may be related to antiepileptic drug (AED) treatment, principally phenytoin (PHT). No differences were noted in N19 amplitude in any group. The P25 and N33 amplitudes were significantly higher in the JME group. In 5 patients of the JME group (14%) "gian

Intraoperative electrically evoked vestibular potentials in humans.

Electrically evoked short latency vestibular potentials were recorded in 9 patients during vestibular neurectomy. Patients were operated on because of intractable Meniere's disease. The VIIIth cranial nerve was exposed through a limited retrosigmoidal approach; the vestibular nerve was contacted in the cerebello-pontine angle with a bipolar platinum-iridium electrode and stimulated with biphasic current pulses (100 microseconds/phase, 0.75-1 mA p-p, 20/s). The responses were recorded over 12.8 ms between a forehead and an ipsilateral ear lobe electrode. Each recording consisted of 2 x 1,000 av

[Visual evoked potentials in benign intracranial hypertension].

In 20 patients with benign intracranial hypertension (BIH) a study of the P100 latency of visual evoked potentials (VEP) was carried out. At admission only 5 patients showed abnormal latencies. The mean of the P100 latencies in patients was significantly increased as compared with controls. Latencies persisted relatively prolonged once the funduscopy had become normal. In the last investigation, one year after BIH had disappeared, the latencies in patients were already similar to those in the control group. On the other hand, no patient with normal latencies had visual sequelae. Sequelae also
