Cutaneous Exophiala oligosperma Infection in a Patient with Bullous Pemphigoid with a Review of the Literature.

Phaeohyphomycosis is an infection caused by a heterogeneous group of melanized fungi. Human infections due to members of genus Exophiala are rare but may occur at any part of the body. We herein report a case of an 85-year-old male with a history of bullous pemphigoid who presented with a chronic, non-healing wound on his right dorsal hand for a month. Direct microscopy of a pus sample from the base of the ulcer revealed strands of pigmented, moniliform hyphae. The isolate was identified as E. oligosperma based on morphological characters and sequencing of the rDNA internal transcribed spacers

In vitro susceptibility of environmental isolates of Exophiala dermatitidis to five antifungal drugs.

Several dematiaceous fungi frequently isolated from nature are involved in cases of superficial lesions to lethal cerebral infections. Antifungal susceptibility data on environmental and clinical isolates are still sparse despite the advances in testing methods. The objective of this study was to examine the activities of 5-flucytosine, amphotericin B, itraconazole, voriconazole and terbinafine against environmental isolates of Exophiala strains by minimum inhibition concentration (MIC) determination. The strains were obtained from hydrocarbon-contaminated soil, ant cuticle and fungal pellets

Exophiala dermatitidis Revealing Cystic Fibrosis in Adult Patients with Chronic Pulmonary Disease.

Cystic fibrosis (CF) is a genetic inherited disease due to mutations in the gene cystic fibrosis transmembrane conductance regulator (CFTR). Because of the huge diversity of CFTR mutations, the CF phenotypes are highly heterogeneous, varying from typical to mild form of CF, also called atypical CF. These atypical features are more frequently diagnosed at adolescence or adulthood, and among clinical signs and symptoms leading to suspect a mild form of CF, colonization or infection of the respiratory tract due to well-known CF pathogens should be a warning signal. Exophiala dermatitidis is a mel

Bronchobiliary fistula secondary to hepatic malignancy with Exophiala dermatitidis infection: a case report.

BACKGROUND: Bronchobiliary fistula (BBF) is a rare but severe complication following the treatment of hepatic malignancies. Exophiala Dermatitidis(E. Dermatitidis), a dematiaceous fungus, is an opportunistic pathogen that predominantly affects immunocompromised individuals. However, its role in BBF-associated infections has not been previously documented. CASE PRESENTATION: A 79-year-old man who was diagnosed with metastatic hepatic malignancy three years prior presented with a fever that persisted for seven days and dyspnoea for two days; a comprehensive evaluation was initiated on admission.

Madura leg due to Exophiala jeanselmei successfully treated with surgery and itraconazole therapy.

Exophiala jeanselmei, also known as Phialophora jeanselmei, is a dematiaceous fungus widely distributed in nature. The organism can cause cutaneous, subcutaneous or systemic infections. An uncommon clinical presentation is eumycetoma, i.e., a subcutaneous infection associated with draining sinuses releasing clumps or granules of the fungus. We describe a case of a long standing mycetoma with several draining sinuses involving the left lower leg of a 47-year-old Saudi Arab male. The disease required extensive surgical excision coupled with intense antifungal chemotherapy to achieve cure.

Diverse antifungal potency of terbinafine as a therapeutic agent against Exophiala dermatitidis in vitro.

Exophiala dermatitidis (E. dermatitidis), which causes skin or respiratory disease, is occasionally fatal in immunocompromised patients. Here, we report the unique antifungal potency of terbinafine (TRB), which targets squalene epoxidase, against E. dermatitidis (SQLEED) using various in vitro approaches. The versatile antifungal activities, including fungicidal activity, biofilm formation inhibition, biofilm eradication activity, and the combination effect of TRB, posaconazole (PSC), and amphotericin B (AmB) with great antifungal potency against E. dermatitidis were evaluated using crystal vi

Exploring a Rare Pulmonary Coinfection: Cryptococcal Pneumonia and Exophiala dermatitidis in an Immunocompetent Host.

Pulmonary cryptococcosis is becoming increasingly common in immunocompetent hosts, manifesting with variable clinical presentations ranging from asymptomatic colonization to severe pneumonia. Radiological findings are non-specific, such as nodular infiltrates, mass-like lesions, and mediastinal lymphadenopathy. We present a case of a 61-year-old woman with Cryptococcus neoformans pneumonia coinfected with Exophiala dermatitidis, an unusual occurrence in an immunocompetent host and the first of its kind. This coinfection posed significant diagnostic challenges due to the rare occurrence of each

The convoluted process of diagnosing pulmonary mycosis caused by Exophiala dermatitidis: a case report.

BACKGROUND: Etiological diagnosis is a key step in the treatment of patients with rare pulmonary mycosis, and the lack of understanding of this disease and lack of specific markers for the detection of rare species, such as Exophiala dermatitidis, add to the difficulty in diagnosing the condition. Therefore, improving the diagnostic strategies for this disease is very important. CASE PRESENTATION: A 52-year-old man presented with cough, sputum production and hemoptysis; chest computed tomography (CT) revealed multiple bilateral lesions. The pathogen was unable to be identified after three bio

[Phaeomycotic cyst caused by Exophiala xenobiotica in a patient with rheumatoid arthritis and lung cancer].

In black fungal infections, Exophiala species are frequently encountered as causative agents of human mycosis, particularly in immunocompromised patients. Among them, Exophiala jenselmei was previously reported as the most common etiological agent. Advances in molecular taxonomy proved this taxon to be heterogeneous, and led to newly introduced or redefined species. Exophiala xenobiotica is one of the novel species differentiated from E. jenselmei on the basis of molecular phylogeny. Here, we report a case of pheomycotic cyst caused by E. xenobiotica, which was well controlled via drainage and

The clinical spectrum of Exophiala jeanselmei, with a case report and in vitro antifungal susceptibility of the species.

Exophiala jeanselmei is clinically redefined as a rare agent of subcutaneous lesions of traumatic origin, eventually causing eumycetoma. Mycetoma is a localized, chronic, suppurative subcutaneous infection of tissue and contiguous bone after a traumatic inoculation of the causative organism. In advanced stages of the infection, one finds tumefaction, abscess formation and draining sinuses. The species has been described as being common in the environment, but molecular methods have only confirmed its occurrence in clinical samples. Current diagnostics of E. jeanselmei is based on sequence data

Molecular typing and antifungal susceptibility of Exophiala isolates from patients with cystic fibrosis.

The black yeast Exophiala dermatitidis is a frequent agent of colonization of the lungs of patients with cystic fibrosis (CF). A total of 71 clinical isolates of Exophiala from 13 patients were identified at the species level by sequencing the internal transcribed spacer (ITS) regions 1 and 2 of the rDNA genes and typed by random amplification of polymorphic DNA (RAPD), using two different primers, BG-2 and ERIC-1. In vitro susceptibility of these isolates to some systemic antifungal drugs was investigated using the CLSI method. Almost all the isolates were identified as E. dermatitidis, but l

Pulmonary Chromomycosis Caused by Exophiala phaeomuriformis in a Patient with a Typical Pulmonary Carcinoid Tumor.

Exophiala spp. cause dematiaceous mycoses, but rarely infect human lungs. A 61-year-old man presented with bloody black sputum. Chest computed tomography showed consolidation in the truncus intermedius and peripheral bronchus, and a mass shadow in the S7 region of the right lung. Bronchoscopy revealed a protruding black lesion in the bronchial orifice of the truncus intermedius, and Exophiala phaeomuriformis was isolated. The consolidation resolved spontaneously, and the residual tumor was diagnosed as a carcinoid tumor. This case highlights the fact that pulmonary chromomycosis may occur in i

Subcutaneous pheomycotic cyst of the finger caused by Exophiala jeanselmei: association with a wooden splinter.

A 58-year-old man with mild glucose intolerance noted a subcutaneous cystic nodule on the ventral aspect of his ring finger. He had experienced trauma to the finger while gardening five weeks before the lesion occurred. Exophiala jeanselmei was isolated and identified on culturing of aspirate from the cystic nodule. The nodule was then removed surgically, and a wooden splinter was found close to the subcutaneous fibrous cyst, suggesting that the causal organism had entered the subcutaneous tissue with the wooden splinter.

Late-onset Exophiala dermatitidis keratitis after cataract surgery: A case report and literature review.

We report Exophiala dermatitidis keratitis developing one year following cataract surgery in a 73-year-old immunocompetent man. Before the correct diagnosis was established, the patient was initially treated for suspected epithelial ingrowth with corticosteroids for ten months. Therapeutic penetrating keratoplasty was required, despite intensive antifungal therapy including topical and systemic voriconazole and amphotericin B. This case underscores the criticality of considering fungal infection in atypical postoperative presentations with prolonged corticosteroid exposure.
