Using low-dose atropine in control of intermittent exotropia.

CLINICAL RELEVANCE: There are a variety of non-surgical methods to control and correct intermittent exotropia. Identifying the effectiveness of an alternative method that carries minimal risk and ocular symptoms can guide clinicians in optimising patient care. BACKGROUND: The aim of this work is to evaluate the efficacy of 0.05% atropine eye drops on exotropia control in children with intermittent exotropia. METHODS: This parallel-group, controlled, randomised clinical trial was conducted on children aged 2-8 years with intermittent exotropia who had been admitted at the Paediatric Ophthalmo

Delayed Sensory Exotropia With Retained Metallic Intraorbital Foreign Body.

This report describes the longest case of a retained metallic intraorbital foreign body with no complications and development of delayed sensory exotropia following traumatic sclopetaria in childhood. A 9-year-old girl suffered a BB gun injury to the left eye, leading to chorioretinitis sclopetaria and loss of vision. The visual acuity was 20/800 with a relative afferent pupillary defect and choroidal rupture with subretinal hemorrhage that evolved to sclopetaria over time. Computed tomography of the orbit demonstrated the BB foreign body. A sensory exotropia of 30 prism diopters developed ove

[Application of modified fusion convergence index in intermittent exotropia].

Objective: To investigate a modified fusional convergence parameter-total convergence amplitude/distance angle and its relationship with exotropia control, stereoacuity, and other visual functions in intermittent exotropia. Methods: The cross-sectional study included children diagnosed with intermittent exotropia at the First Affiliated Hospital of Nanjing Medical University from August 2020 to June 2021. A modification was made by combining total convergence amplitude using synoptophore and distance angle at distance using prism bars to calculate total convergence amplitude/distance angle. Ex

Nonsurgical Consecutive Exotropia Following Childhood Esotropia: A Multicentered Study.

PURPOSE: Nonsurgical consecutive exotropia (NCX) occurs when an esotropia (ET) spontaneously converts to exotropia (XT) without surgical intervention. Although NCX is considered to occur in early-onset accommodative ET with high hyperopia, consensus on causation is lacking. We report the clinical characteristics of NCX and assess the response to conservative management. DESIGN: Retrospective, multicenter, observational case series. METHODS: Patients aged 6 months and older with an initial diagnosis of ET who converted to XT without surgical intervention. Sensory strabismus was excluded. Age,

Surgical management of superior oblique palsy with coexisting exotropia.

PURPOSE: To analyze the surgical results of patients treated for superior oblique palsy with coexisting exotropia. METHODS: The medical records of patients with superior oblique palsy and exotropia who underwent inferior oblique weakening and simultaneous lateral rectus recession by a single surgeon from 1996 to 2022 were reviewed retrospectively. Demographics, pre- and postoperative vertical and horizontal deviation, and presence of diplopia were recorded. Surgical success was defined as horizontal deviation <10Δ and vertical deviation ≤4Δ without overcorrection or diplopia. The decision to

Three-muscle surgery for very large-angle constant exotropia.

PURPOSE: To report the outcomes of surgery on three horizontal muscles for very large-angle constant exotropia. METHODS: In this prospective case series, consecutive adult patients with primary constant exotropia with angles ≥120(Δ) underwent strabismus surgery on three horizontal muscles. Surgery consisted of lateral rectus recession of 10-15 mm on the fixing eye in addition to lateral rectus recession of 9-13 mm and medial rectus resection of 6.5-9 mm on the nonfixing eye. The mean follow-up period was >6 months. Success was defined as horizontal eso- or exodeviations of ≤10(Δ). RESULTS: A

Supermaximal recession and resection in large-angle sensory exotropia.

In cases of extropia with an exodeviation angle over 50 prism diopter (PD), a 3- or 4-muscle surgery is a rational option. But, in patients with sensory exotropia, there is usually a strong preference for a monocular procedure to avoid surgery on the single seeing eye. Thus, we confined surgery to visually poor eyes, and performed a medial rectus muscle resection with a mean of 10.3 mm (range, 9-11 mm) and a lateral rectus muscle recession with a mean of 12.8 mm (range, 10-14 mm) in 4 adult sensory exotropia patients who had a mean deviation of 82.3 PD (range, 75-90 PD). The mean postoperative

Parry-Romberg syndrome presenting with recurrent exotropia and torticollis.

Parry-Romberg syndrome, or progressive facial hemiatrophy, is a unilateral, slowly progressive atrophy that affects skin, subcutaneous tissues, muscles, and bones. The authors describe an 8-year-old girl with Parry-Romberg syndrome, amblyopia, recurrent exotropia, and torticollis, and stress that the association between this syndrome and amblyopia, recurrent exotropia, and torticollis should be recognized.

Divergence excess and basic exotropia types of intermittent exotropia: a major review. Part 1: prevalence, classification, risk factors, natural history and clinical characteristics.

INTRODUCTION: Intermittent exotropia (IXT) is a common form of strabismus. It is an outward deviation of one eye typically when viewing at distance. Symptoms include, but are not limited to double vision, eyes feeling tired, excessive blinking, and reduced quality of life. Its clinical characteristics are distinctive from other types of strabismus. This paper provides a comprehensive review of prevalence, classification, risk factors, natural history and clinical characteristics of the divergence excess and basic exotropia types of IXT. METHODS: Search strategies involving combination of keyw

Gender-based analysis of visual and refractive characteristics in exotropia.

PURPOSE: To analyze gender-based differences in visual acuity, refractive error, angle of deviation, and amblyopia in patients with alternate exotropia (Alt XT) and intermittent exotropia (IXT). METHOD: A retrospective study of 4,417 surgical exotropia patients (2,970 Alt XT; 1,447 IXT) compared sex-based differences in CDVA, refraction, deviation angles, and amblyopia within each subtype. Alongside unadjusted tests, subtype-specific multivariable models were used: linear regression for right-eye logMAR CDVA (adjusting for age, near/distance deviation, spherical equivalent, and astigmatism),

Surgical Outcomes of Intermittent Exotropia in Adults: A Systematic Review.

Adult intermittent exotropia (IXT) is less well studied than paediatric IXT, yet counselling is challenging due to postoperative exodrift, diplopia risk, and differing functional expectations. This systematic review synthesised surgical outcomes for adult (≥18 years) IXT. A Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020-guided systematic review of PubMed/MEDLINE was performed using two searches ("surgical" AND "outcomes" AND "exotropia"; "surgery" AND "intermittent exotropia"), limited to English-language human studies published within the last 10 years. Nomogram for predicting secondary surgery in patients with concomitant exotropia.

Concomitant exotropia (CX), a common form of strabismus, often requires surgical correction, yet up to 60% of patients undergo secondary surgery (SS) due to recurrence or residual deviation. Current risk prediction tools remain limited by inconsistent variables and short-term follow-up. This study aimed to develop and validate a nomogram integrating long-term follow-up data to predict individualized SS risk in CX patients. This is a retrospective cohort study of patients with CX who underwent surgery at the Peking University First Hospital between January 1, 2008, and December 31, 2010. Of the

Management and outcomes of Exotropia in hyperopic patients: systematic Review.

BACKGROUND: The management of exotropia (XT) in the presence of hyperopia is challenging, as these patients are at an increased risk of developing amblyopia and compromised binocular vision. This study aims to evaluate the management approaches and clinical outcomes in XT associated with hyperopia. METHODS: A systematic review was conducted with a comprehensive search across PubMed, Web of Science, and Google Scholar, following PRISMA 2020 guidelines. The search strategy utilized a combination of free-text keywords and MeSH terms, including "Exodeviation," "Exotropia," "Intermittent Exotropia

Brain abnormalities in infantile esotropia as predictor for consecutive exotropia.

Cerebral palsy, neurological abnormalities, prematurity or periventricular lesions may affect motor and sensory fusion mechanisms that favorably control eye alignment. White matter damage of immaturity (WMDI) is a form of white matter brain injury characterized by the necrosis of white matter near the lateral ventricles. In these cases, it is difficult to establish fusion after strabismus surgery and consecutive deviations may be seen more frequently especially in association with WMDI. The aim of this study is to evaluate and compare the cerebral magnetic resonance imaging (MRI) findings in o
