IMPORTANCE: Growing literature recognizes the importance of contextual influences on outcomes across the cancer continuum, beyond those of individual biological and lifestyle factors. These contextual influences represent a neighborhood exposome consisting of socioeconomic, demographic, and built environmental factors.
OBJECTIVE: To identify which neighborhood-level factors are most relevant for cancer outcomes and determine the geographic scale of their operation.
DESIGN, SETTING, AND PARTICIPANTS: This cohort study of women with a primary diagnosis of breast cancer developed a geographical

Prenatal exposures influence infant health, yet the microbiota related mechanisms remain unclear. We profiled 776 maternal exposome variables and meconium microbiota in 1,689 mother-infant pairs, establishing a network linking maternal exposures, neonatal microbiota, and 10 infant outcomes. Using multiomics, pathway enrichment, and robust regression, we identified maternal anthropometrics and clinical indicators as key predictors of infant development, and specific metabolic and chemical exposures as major factors associated with microbiota composition. These microbial changes were associated

Following EU restrictions on dental amalgam from January 1, 2025, and the Minamata Convention's COP-6 decision to phase out dental amalgam globally by 2034, the era of amalgam restorations is drawing to a close. However, populations with historical mercury exposure may retain persistent, species-dependent tissue mercury burdens, particularly within the kidney and brain. This persistent body burden may prolong internal mercury exposure after cessation or substantial reduction of the external source. We propose "Mercury Debt" as a conceptual construct describing residual, species-specific intern

Background: Dermatological conditions represent a leading cause of global nonfatal disease burden, accounting for approximately 42.9 million disability-adjusted life years annually. Their complex pathogenesis is increasingly understood through the skin-brain-exposome axis, a bidirectional neuroimmunological and environmental communication network. The study aims to synthesize the neurobiological mechanisms of the skin-brain-exposome axis with macroscopic sociodemographic modifiers, clinical manifestations, and evidence-based psychodermatological interventions. Methods: A narrative review was c

Pesticides play an essential role in modern agricultural systems by enhancing crop yields and controlling pest-related losses and are applied extensively across the globe. However, their pervasive use has led to widespread environmental contamination, including accumulation in household and environmental dust, a significant yet understudied pathway of human exposure. Despite growing evidence that dust ingestion serves as a critical route for pesticide uptake, the associated health hazards remain poorly quantified and understood. We developed a Reverse Dosimetry Evaluation System (RDES) to esti

BACKGROUND: Evidence indicates that cognitive function is influenced by potential environmental factors. We aimed to determine the variables influencing cognitive function.
METHODS: Our study included 164,463 non-demented adults (89,644 [54.51%] female; mean [SD] age, 56.69 [8.14] years) from the UK Biobank who completed four cognitive assessments at baseline. 364 variables were finally extracted for analysis through a rigorous screening process. We performed univariate analyses to identify variables significantly associated with each cognitive function in two equal-sized split discovery and

Relatively little is known about the relationship between socio-demographic factors and the chemical exposome in adolescent populations. This knowledge gap hampers global efforts to meet certain UN sustainability goals. The present work addresses this problem in Swedish adolescents by discerning patterns within the chemical exposome and identify demographic groups susceptible to heightened exposures. Enlisting the Riksmaten Adolescents 2016-17 (RMA) study population (N = 1082) in human-biomonitoring, and using proportional odds ordinal logistic regression models, we examined the associations b

Neurodegenerative diseases (NDDs) remain a global health challenge. Previous studies have reported potential links between environmental factors and NDDs, however, findings remain controversial across studies and elusive to be interpreted as evidence of robust causal associations. In this study, we comprehensively explored the causal associations of the common environmental factors with major NDDs including Alzheimer's disease (AD), Parkinson's disease (PD), amyotrophic lateral sclerosis (ALS), and multiple sclerosis (MS), based on updated large-scale genome-wide association study data through

BACKGROUND: Exposome research aims to describe and understand the extent to which all the exposures in human environments may affect our health over the lifetime. However, the way in which humans interact with their environment is socially patterned. Failing to account for social factors in research exploring the exposome may underestimate the magnitude of the effect of exposures or mask inequalities in the distribution of both exposures and outcomes.
OBJECTIVES: We aimed to describe the extent to which social factors appear in the exposome literature, the manner in which they are used in emp

The current study within the frame of the HEALS project aims at the development of a lifelong physiologically based biokinetic (PBBK) model for exposome studies. The aim was to deliver a comprehensive modelling framework for addressing a large chemical space. Towards this aim, the delivered model can easily adapt parameters from existing ad-hoc models or complete the missing compound specific parameters using advanced quantitative structure activity relationship (QSAR). All major human organs are included, as well as arterial, venous, and portal blood compartments. Xenobiotics and their metabo

BACKGROUND: Harmonized data describing simultaneous exposure to a large number of environmental contaminants in-utero and during childhood is currently very limited.
OBJECTIVES: To characterize concentrations of a large number of environmental contaminants in pregnant women from Europe and their children, based on chemical analysis of biological samples from mother-child pairs.
METHODS: We relied on the Early-Life Exposome project, HELIX, a collaborative project across six established population-based birth cohort studies in Europe. In 1301 subjects, biomarkers of exposure to 45 contaminants

IgE-mediated food allergy is a global health issue with rising prevalence that cannot be explained by genetic change alone, strongly suggesting that environmental exposures are a key driver. The external exposome encompasses several exposures that influence the developing immune system and modify food allergy risk. Environmental exposure to food allergens in household dust is increasingly recognized as a risk factor for sensitization, whereas early oral introduction promotes tolerance. Early-life microbial exposures, especially those associated with rural and farming environments, may protect

Microplastic pollution is a major environmental problem affecting ecosystems, wildlife and people worldwide. Microplastics are solid synthetic polymer particles > 5 mm, insoluble in water and potentially containing functional additives and adsorbed chemical contaminants. They originate from bulk industrial products and from the degradation of large plastic debris (EFSA CONTAM Panel, in EFSA Journal 14, 2016). The extent of the contamination is critical, with global plastic production projected to exceed 1.2 billion metric tons per year by 2060 and an estimated 170 trillion plastic particles in

Systemic inflammation during pregnancy is associated with adverse health outcomes for both mother and child. C-reactive protein (CRP) is a widely used biomarker of inflammation, and its levels may be influenced by environmental factors. This study examined the association between 26 components of the external urban exposome-including air pollutants, land use, and access to green and blue spaces-and CRP concentrations in 1547 pregnant women from the INMA birth cohort in Spain (Gipuzkoa, Sabadell, and Valencia). Environmental exposures during the first trimester were assessed using GIS-based ind

BACKGROUND AND AIMS: Previous hypothesis-driven studies focused on limited risk factors for ischemic stroke (IS) with inconsistent findings. We aimed to systematically identify modifiable factors, assess causality, quantify joint impact, and evaluate public health implications of conservative versus radical multi-domain IS intervention strategies.
METHODS: We conducted a large prospective cohort study using UK Biobank and a bidirectional two-sample Mendelian randomization study. Exposures included 323 modifiable factors in seven domains: early life, health and medical history, lifestyle, loca

The exposome, defined as the totality of external exposures individuals experience throughout the lifespan and the internal biological responses they trigger, is increasingly recognized as a major, modifiable determinant of health. The eye is uniquely vulnerable to exposome risk factors because it comprises an external interface directly exposed to the environment, a transparent lens highly sensitive to cumulative oxidative injury, and metabolically active retina prone to inflammation and vascular dysregulation. Research linking ocular diseases to air pollution, climate and weather factors, he

Exposome studies are advancing in high-income countries to understand how multiple environmental exposures impact health. However, there is a significant research gap in low- and middle-income and tropical countries. We aimed to describe the spatiotemporal variation of the external exposome, its correlation structure between and within exposure groups, and its dimensionality. A one-year follow-up cohort study of 506 children under 5 in two cities in Colombia was conducted to evaluate asthma, acute respiratory infections, and DNA damage. We examined 48 environmental exposures during pregnancy a

The aging population worldwide is facing a significant increase in age-related non-communicable diseases, including cardiovascular and brain pathologies. This comprehensive review paper delves into the impact of the exposome, which encompasses the totality of environmental exposures, on unhealthy aging. It explores how environmental factors contribute to the acceleration of aging processes, increase biological age, and facilitate the development and progression of a wide range of age-associated diseases. The impact of environmental factors on cognitive health and the development of chronic age

Metabolic alterations are increasingly implicated in neurological disorders, including Alzheimer's disease (AD), highlighting the relevance of the peripheral metabolome, shaped by genetic and environmental exposures, for brain health. We examined the relation of 991 blood metabolites with cognition and magnetic resonance imaging (MRI) measures cross-sectionally in 1,082 dementia-free middle-aged participants of the population-based Rotterdam Study and quantified contributions of genetic variation, lifestyle, comorbidities, medication and gut microbiota to metabolite variance.
