Annexin A1 Bioactive Peptide Promotes Resolution of Neuroinflammation in a Rat Model of Exsanguinating Cardiac Arrest Treated by Emergency Preservation and Resuscitation.

Neuroinflammation initiated by damage-associated molecular patterns, including high mobility group box 1 protein (HMGB1), has been implicated in adverse neurological outcomes following lethal hemorrhagic shock and polytrauma. Emergency preservation and resuscitation (EPR) is a novel method of resuscitation for victims of exsanguinating cardiac arrest, shown in preclinical studies to improve survival with acceptable neurological recovery. Sirtuin 3 (SIRT3), the primary mitochondrial deacetylase, has emerged as a key regulator of metabolic and energy stress response pathways in the brain and a p

Blood collection from dairy calves at exsanguination post-slaughter yields similar biochemical and packed cell volume measurements compared with in vivo collection during lairage.

Blood collection at exsanguination is a method of collecting samples at abattoirs which itself has no effect on animal welfare, compared with collection prior to stunning. However, there is the potential for artefact to affect measurements. It was hypothesised that, for most blood analytes measured, the differences between lairage and exsanguination measurements would be minimal, except for creatine kinase, which was expected be higher in exsanguination samples. Fifty-nine male dairy calves approximately 5-10 days old were sampled prior to slaughter, and again during exsanguination after stunn

Transient Radial-Dominant Neuropathy Following the Use of Sterile Exsanguination Tourniquet During the Posterior Plating of a Humeral Shaft Fracture: A Case Report.

Tourniquet-related nerve injury is an uncommon but clinically important complication of extremity surgery. Postoperative radial nerve palsy after humeral shaft fixation is usually attributed to fracture-related injury or surgical manipulation, particularly when a posterior approach is used. We present a case of transient radial-dominant neuropathy following the use of a sterile exsanguination tourniquet during the posterior plating of a distal humeral shaft fracture. A 27-year-old male construction worker with an AO/OTA (Arbeitsgemeinschaft für Osteosynthesefragen/Orthopaedic Trauma Associatio

Dilemmas and challenges in the management of a neonate with Adams-Oliver syndrome with infected giant aplasia cutis lesion and exsanguination: a case-based update.

BACKGROUND: Aplasia cutis is a rare developmental anomaly usually involving the calvarium, associated with a variable extent of defective formation of the scalp. Adams-Oliver syndrome is a condition mainly characterized by the congenital absence of skin, known as "aplasia cutis" which is usually limited to the vertex scalp and transverse limb defects. CASE REPORT: A 17-day-old term female neonate was referred to us with an infected scalp lesion of the vertex. The lesion which is about 10 × 9 cm had signs of infection with necrotic eschar. We started the neonate on systemic parenteral antibiot

A comparison study between the Hemorrhage-Arresting Lever-Operated (HALO) tourniquet and the Combat Action Tourniquet (CAT) for the management of exsanguinating extremity hemorrhage.

INTRODUCTION: Tourniquets have been demonstrated to reduce preventable deaths due to exsanguination. However, studies have suggested that the Combat Action Tourniquet (CAT®), a popular prehospital device, may have a 19-30 percent failure rate, thus prompting the creation of a new, lever-operated device for exsanguinating extremity hemorrhage. However, the efficacy of this device compared to the CAT has not been reported. OBJECTIVE: To determine efficacy and ease of use of the Hemorrhage Arresting Lever Operated (HALO®) tourniquet compared to the CAT. METHODS: This was a prospective crossover

Feasibility study for implementation of resuscitative balloon occlusion of the aorta in peri-arrest, exsanguinating trauma at an adult level 1 Australian trauma centre.

OBJECTIVE: This prospective, observational, interventional study sought to determine if the introduction of resuscitative balloon occlusion of the aorta (REBOA) at an Australian adult major trauma centre would improve survival for major trauma patients. METHODS: Patients aged 18-60 years, transported directly from scene with exsanguinating, sub-diaphragmatic haemorrhage and hypovolaemic shock (systolic BP <70 mmHg or hypovolaemic cardiac arrest) were eligible for recruitment and followed up until hospital discharge (ACTRN12618000550202). RESULTS: During the 14-month study period (17 January

Prioritizing circulation over airway to improve survival in trauma patients with exsanguinating injuries: a world society of emergency surgery-panamerican trauma consensus statement.

INTRODUCTION: Hemorrhage is one of the leading causes of preventable death in trauma patients. For decades, the Airway-Breathing-Circulation (ABC) approach has been the cornerstone of trauma care. However, emerging evidence suggests that prioritizing airway management in exsanguinating patients may worsen hypotension and increase mortality. This systematic review and meta-analysis aim to evaluate the effectiveness of the Circulation-Airway-Breathing (CAB) approach compared to the traditional ABC sequence in improving survival in trauma patients with severe hemorrhage. METHODS: A systematic re

Prehospital External Aortic Compression for Temporizing Exsanguinating Sub-Diaphragmatic Hemorrhage - A Promising Technique, but with Challenges: Four Illustrative Cases, Including Two Survivors.

External aortic compression (EAC) has long been used to control exsanguinating post-partum hemorrhage, but it has only recently been described in the prehospital trauma setting. This paper reports four cases where manual EAC was used during transport to manage life-threatening bleeding, twice from stab wounds, once from ruptured ectopic pregnancy, and once from severe lower-limb trauma. It showed that EAC has life-saving potential in the prehospital setting, but that safety and efficacy during transport requires the use of a hands-free compression device, such as an aortic tourniquet.

Midthoracic Pain, Sentinel Arterial Haemorrhage and Exsanguination after a Symptom-Free Interval (Chiari's Triad) is Diagnostic of Arterio-Oesophageal Fistula: A Life-Threatening Cause of Gastrointestinal Bleeding.

INTRODUCTION: Arterio-oesophageal fistulae are a very uncommon cause of severe gastrointestinal bleeding, and mostly result from an aberrant right subclavian artery and mediastinal surgery or prolonged endotracheal/nasogastric intubation. MATERIAL AND METHODS: We present the case of a patient with an oesophageal adenocarcinoma and haematemesis due to a subclavian arterio-oesophageal fistula after mediastinal radiotherapy. CONCLUSION: We discuss the rare, life-threatening condition of acute erosion of the left subclavian artery caused by an oesophageal tumour and presenting with Chiari's tria

Early Resuscitation of Patients With Non-exsanguinating Trauma Using Packed Red Blood Cells Versus Low-Volume Crystalloids: Have We Gone Too Far?

The early resuscitation of patients with mild to moderate non-exsanguinating trauma has shifted from the conventional use of one to two liters of crystalloids to the use of one to two units of PRBC. This evolution assumes that the transfusion of PRBC is superior to the administration of any volume of crystalloids because of the propensity of crystalloids to migrate from the intravascular to the interstitial space leading to organ dysfunction, organ failure, and worse outcomes. However, the premise of the fluid migration relies on Starling original model of fluid exchange between the hydrostati

Comparing outcomes in patients with exsanguinating injuries: an Eastern Association for the Surgery of Trauma (EAST), multicenter, international trial evaluating prioritization of circulation over intubation (CAB over ABC).

INTRODUCTION: Hemorrhage is a major cause of preventable trauma deaths, and the ABC approach is widely used during the primary survey. We hypothesize that prioritizing circulation over intubation (CAB) can improve outcomes in patients with exsanguinating injuries. METHODS: A prospective observational study involving international trauma centers was conducted. Patients with systolic blood pressure below 90 who were intubated within 30 min of arrival were included. Prioritizing circulation (CAB) was defined as delaying intubation until blood products were started, and/or bleeding control was pe

Hemorrhagic Shock: Blood Marker Sequencing and Pulmonary Gas Exchange.

BACKGROUND: The early identification of internal hemorrhage in critically ill patients may be difficult. Besides circulatory parameters, hemoglobin and lactate concentration, metabolic acidosis and hyperglycemia serve as laboratory markers for bleeding. In this experiment, we examined pulmonary gas exchange in a porcine model of hemorrhagic shock. Moreover, we sought to investigate if a chronological order of appearance regarding hemoglobin, lactatemia, standard base excess/deficit (SBED) and hyperglycemia exists in early severe hemorrhage. METHODS: In this prospective, laboratory study, twel

Double fatal outcome after ruptured vasa previa in monochorionic twins: case report and review of the literature.

Vasa previa is a condition in which one or more fetal blood vessels run through the amniotic membranes and cross or run near the external orifice of the uterus. Rupture of membranes can lead to tearing of these vessels and cause acute fetal exsanguination. In monochorionic twin (MC) pregnancies, acute exsanguination in one twin can lead to severe complications in the co-twin due to the presence of inter-twin placental vascular connections. We report a MC pair with severe perinatal asphyxia due to acute exsanguination after prenatally undetected ruptured vasa previa. This resulted in severe hem

Pelvic crush injuries with occlusion of the iliac artery.

During a 1-year period, three patients presented with acute traumatic thrombosis of the common or external iliac artery concomitant with a massive crush injury to the pelvis. All had vascular compromise of the involved extremity on initial physical examination, but in two patients with open pelvic trauma, exsanguination, major visceral injury, hypothermia, and a coagulopathy precluded emergency vascular reconstruction. Both required hindquarter amputation for adequate debridement. The third patient presented without exsanguination or visceral trauma. Angiography and vascular reconstruction wer

Timing of antibiotic prophylaxis in tourniquet surgery.

It is customary to administer prophylactic antibiotics before exsanguination of the limb and inflation of a tourniquet in extremity surgery. To compare the clinical outcome in lower limb operations when prophylactic antibiotics were administered before versus after limb exsanguination and tourniquet inflation, we randomized patients to the administration of prophylactic antibiotics 5 minutes before exsanguination and inflation of the tourniquet (ABT) and administration of prophylactic antibiotics 1 minute after inflation of the tourniquet (AAT). A total of 106 patients completed the study, inc

Massive epistaxis from aneurysm of the carotid artery.

The syndrome of massive and sometimes fatal epistaxis from an aneurysm of the internal carotid artery is reviewed. Two cases are reported in which exsanguination occurred with anterior and posterior gauze packing in place. The use of standard methods for controlling severe epistaxis in this syndrome was a futile exercise. This distressing and frustrating experience prompted the development of an emergency method to prevent exsanguination pending definitive surgical treatment. A description of this technique is presented and a proposal made for its use.

Mild hypothermia disturbs regional cerebrovascular autoregulation in awake rats.

The effects of mild hypothermia on regional CBF (rCBF) and autoregulation were investigated in 60 awake and spontaneously breathing Wistar rats. They were divided into normothermic (rectal and brain temperatures: 37.0 +/- 0.5 degrees C) and mildly hypothermic (33.0 +/- 0.5 degrees C) groups the temperature of the latter group was controlled by cooling a lead cast around each rat with ice-cold water. rCBF was measured by means of an autoradiographic technique with 14C-iodoantipyrine. In normothermia, rCBF in most of the supratentorial cortical regions was maintained down to a mean arterial bloo

Classification of renal injuries as a guide to therapy.

Renal injuries require immediate or early surgical intervention for threatened exsanguination, damage to the pedicle vessels and disruption of the renal pelvis. Elective delayed operation prevents late complications in those patients in whom there is transparenchymal connection between the pelvocaliceal system and the perirenal space following trauma. There appears to be an increased incidence of nephrectomy if the latter are explored early. Expectant treatment is appropriate for renal lacerations without extravasation, if there is no threat of exsanguination and if the pedicle is intact.
