Are axillary recurrence and overall survival affected by axillary extranodal tumor extension in breast cancer? Implications for radiation therapy.

PURPOSE: To determine the overall survival and local recurrence significance of axillary lymph node extranodal tumor extension (ETE) and whether axillary/chest-wall irradiation influenced any of these outcomes. MATERIALS AND METHODS: The records of 81 breast cancer patients treated with radical or modified radical mastectomy at a single surgical practice were eligible for study. Thirty-four patients had ETE: 17 with focal ETE (< 10 x high-power field) and 17 with extensive ETE (> 10 x high-power field). RESULTS: With a median follow-up duration of 92 months, only two patients had an axillary

Efficacy of PD-1 inhibitor plus induction chemotherapy for nasopharyngeal carcinoma with high-grade extranodal extension: a multicenter retrospective analysis.

BACKGROUNDS: High-grade radiologic extranodal extension (G2/3-rENE) is a well-established adverse prognostic factor in nasopharyngeal carcinoma (NPC), as demonstrated in our previous research. This study aims to evaluate the efficacy and safety of combining a PD-1 inhibitor with induction chemotherapy (IC) in this high-risk patient population. METHODS: We retrospectively analyzed 217 histologically confirmed non-metastatic NPC patients from three centers. Group A (n = 104) received IC plus anti-PD-1 therapy followed by concurrent chemoradiotherapy (CCRT), while Group B (n = 113) received IC al

Oncological outcomes of pathological extranodal extension (pENE) in oral squamous cell carcinoma (OSCC): A proposal to refine the current staging.

BACKGROUND: Extranodal extension (ENE) is an important prognostic factor in oral squamous cell carcinoma (OSCC), often associated with poor survival outcomes. However, the current nodal (N) staging system may need to be revised to reflect the prognosis. Emerging evidence suggests that the number of positive lymph nodes may offer improved prognostic accuracy. This study evaluates the outcomes of OSCC patients with pathological ENE (pENE) and explores the prognostic significance of absolute positive lymph node count. MATERIALS AND METHODS: This retrospective cohort study analyzed 640 OSCC patie

Prognostic implications of extranodal extension in node-positive squamous cell carcinoma of the vulva: A systematic review and meta-analysis.

Squamous cell carcinoma (SCC) of the vulva is the fourth most common gynecological cancer, usually staged with the TNM or FIGO systems. Since 2009, FIGO staging has taken the extranodal extension (ENE) of lymph node metastases into account. ENE is defined as the spread of a lymph node metastasis into surrounding soft tissue. Although the TNM and FIGO systems acknowledge the importance of ENE in SCC, no comprehensive studies have analyzed the prognostic impact of this parameter. We therefore queried the PubMed and SCOPUS databases from their inception up until 04/01/2015, adopting no language r

Extranodal extension of lymph node metastasis is a marker of poor prognosis in oesophageal cancer: a systematic review with meta-analysis.

The extranodal extension (ENE) of nodal metastasis is the extension of neoplastic cells through the nodal capsule into the perinodal adipose tissue. This histological feature has recently been indicated as an important prognostic factor in different types of malignancies; in this manuscript, we aim at defining its role in the prognosis of oesophageal cancer with the tool of meta-analysis. Two independent authors searched SCOPUS and PubMed until 31 August 2015 without language restrictions. The studies with available data about prognostic parameters in subjects with oesophageal cancer, comparin

Extranodal extension of axillary metastasis of invasive breast carcinoma as a possible predictor for the total number of positive lymph nodes.

INTRODUCTION: By the implementation of the sentinel node procedure in the treatment of breast carcinoma routine axillary lymph node, dissection can be abandoned in patients with a tumour-negative sentinel node. When the sentinel node is positive there are two options; an axillary dissection or radiotherapy of the axilla. In the latter case one is not informed about the total number of positive lymph nodes which can be of importance for the choice of adjuvant chemotherapy. In this paper we analyse whether it is possible to use histological parameters of a lymph-node metastasis to predict the nu

Malignant lymph nodes of the head and neck: imaging pearls, pitfalls, and practical approach to reporting in the era of extranodal extension.

Accurate characterization and reporting of cervical lymph nodes is critical when evaluating head and neck malignancies, as findings directly impact staging, treatment planning, and prognosis. Knowledge of lymphatic drainage patterns predicts nodal spread of disease and conversely, may help in identifying primary malignancy. Variability in size criteria for an abnormal lymph node across literature makes reporting difficult when used in isolation; however, careful attention to morphologic changes improves diagnostic accuracy. FDG-PET imaging serves as a valuable complementary tool when available

Imaging-detected extranodal extension in head and neck cancer: clinical implications, diagnostic criteria, and the potential of photon-counting detector CT.

Imaging-detected extranodal extension (iENE) is increasingly recognized as an objective adverse prognostic factor in head and neck cancer. The newly published 9th edition of the UICC/AJCC TNM Classification now incorporates iENE into the nodal (N) categories for nasopharyngeal carcinoma, HPV-associated oropharyngeal carcinoma, and salivary gland carcinoma, typically upstaging patients to higher N categories, and designates imaging findings as an accepted standard for the clinical ENE (cENE) as the N3b category in all other head and neck subsites. To establish diagnostic criteria for iENE, the

Depth of invasion and extranodal extension: the influential factors to predict survival rate of patients with oral tongue squamous cell carcinoma.

BACKGROUND: Cancer staging is essential in determining patients' prognoses and designing the appropriate treatment strategy. American Joint Committee on Cancer has released the latest version of the staging system for tongue SCC. However, it is interesting to know whether this change in staging and the addition of depth of invasion (DOI) and the extra-nodal extension (ENE) have any influence on patients' prognosis. METHODS: In this retrospective cohort study, the pathology records of patients with tongue SCC who underwent surgery at the Pathology Department of Cancer Institute Hospital, 2017-
