Early temporalis muscle transposition for the management of facial paralysis.

Temporalis muscle transposition is a reliable surgical technique for the reanimation of patients with long-standing facial paralysis. It is often employed when facial nerve reinnervation via crossover or cable grafting is not possible. Temporalis muscle transposition can also be used for the immediate treatment of complete facial paralysis due to insults leaving the facial nerve anatomically intact but requiring a prolonged recovery time (more than 1 year). Because temporalis muscle transposition does not interfere with neuronal regeneration, it may be employed early in the management of compl

The role of facial muscle resection in reconstruction of the paralyzed face.

The technique of resecting the levator labii superioris alaeque nasi muscle can help improve nasal symmetry and is a useful ancillary procedure in reconstruction of the paralyzed face. In 3 patients with facial paralysis, this technique markedly changed the nasal deviation seen at rest and also affected the nasolabial angle and medial nasolabial fold. A patient report is presented.

[Effects of facial nerve denervation on masseter muscle of the rats].

The effects of unilateral facial nerve denervation on the facial growth were studied by physiological and by histochemical examinations, using four- week- old Wistar rats. Two hundred twenty rats were divided into three groups: a control group, a group of subjects which resection of the left facial nerve, and a sham operation group. The rats were killed 1, 5, 10, 30, 60, 90, 120 days later. The superficial and deep masseter muscle fibers on both sides were classified into type I and type IIA or IIB on the basis of myosin ATPase activity and the diameter of these fibers was measured. The masset

The Facial Muscle Chain Concept in Forehead Line Treatment.

Current treatment of horizontal forehead lines typically involves botulinum toxin injections targeting the frontalis muscle, though this approach may occasionally result in complications such as eyebrow ptosis. This letter introduces the facial muscle chain concept, which describes the three-dimensional anatomical relationships between facial muscles that may influence dynamic wrinkle formation. Unlike conventional muscle pairing descriptions found in existing literature, this framework explores how targeting the corrugator supercilii muscle could potentially affect frontalis muscle activity t

A novel muscle for electroneurography in peripheral facial palsy: occipitalis.

Electroneurography (ENoG) is one of the most objective tests in grading the damage and prediction of prognosis in peripheral facial palsy (PFP). We aimed to determine temporal changes of ENoG recorded over occipitalis muscle in acute idiopathic PFP. Consecutive 21 patients with unilateral acute idiopathic PFP and age- and sex-matched 15 healthy volunteers were included in the study. Nasal and occipital ENoG values were recorded once in the control group and the same procedure was repeated daily between the second and eight days of the disorder in the PFP group. Occipital ENoG value began to in

Reference values for dynamic facial muscle ultrasonography in adults.

INTRODUCTION: Ultrasonography can detect structural muscle changes caused by neuromuscular disease, but it has not yet been applied to facial nerve diseases. Quantitative analysis would be the preferred method to determine whether ultrasound findings are within normal limits, but there are no normative data for mimic muscles. The purpose of this study was to provide gender-specific normative ultrasonography data for muscle area, diameter, and contractility for mimic and masticatory muscles in adults. METHODS: Bilateral scans of multiple mimic and masticatory muscles were performed at rest and

Facial reanimation by staged, split masseter muscle transfer.

BACKGROUND: Facial paralysis of the lower face presents severe functional and aesthetic disturbance to patients. The gamut of facial paralysis correction is diverse and must be tailored to the patient. When nerve repair or free functional muscle transfer is unavailable, regional muscle transfer has become a staple in surgical management of facial paralysis. Previous masseter transfers relied on orbicularis oris attachment, which may be atrophic, adhered, or lengthened. Using fascia lata grafts, we describe the senior author's method of staged, split masseter transfer as a reliable method for r

Angular measurement for determining muscle tonus in facial paralysis.

BACKGROUND: the decrease of facial movements in peripheral facial paralysis and the resulting aesthetical sequels may have important emotional repercussions as a consequence to the functional deficit, and depending on the intensity of the clinical condition. Orofacial rehabilitation has as a purpose to favor the recovery of orofacial movements and to adequate and/or adapt orofacial functions and facial mimic. However, quantifying therapeutic results in an attempt to measure the muscle tonus is a challenge. Generally, the used forms of measurement are general and subjective. AIM: to propose th

The role of extraocular and facial muscle trigger points in cephalalgia.

Trigger points play a part in the development and maintenance of chronic headache states. This article reviews the physiology of trigger points, with a focus on the -latest understanding of their biochemistry. The importance of facial muscle and extraocular muscle trigger points is only beginning to be understood; the data exploring their role in headache are reviewed. The concept of central sensitization and the way in which it relates to trigger points and headache is explained, along with treatment strategies for helping patients with their pain.

[Facial asymmetry, asymmetry of muscle tonus and unilateral amblyopia].

The authors noted the high prevalence of facial asymmetry and of unilateral amblyopia in some hundred children suffering from chronic neurological conditions. Unilateral changes in muscle tone are responsible for such asymmetry. Amblyopia in itself may cause a facial asymmetry. The dental arcades are little affected even by severe deformities of their underlying bony supports. Dento-maxillary abnormalities are no more severe in these children than in a normal population.

Dynamic muscle transfer in facial nerve palsy: the use of contralateral orbicularis oculi muscle.

The aim of the study is to describe the results of dynamic muscle transfer with an orbicularis oculi muscle flap from the contralateral side to the paralyzed side in patients with House-Brackmann grade 6 facial nerve palsy. This case series included six patients who underwent dynamic muscle transfer with a flap of healthy orbicularis oculi muscle fibers from the contralateral side into the paralyzed orbicularis oculi muscle. All patients had a House-Brackmann grade 6 facial nerve palsy. They all had previous multiple surgical procedures to improve the eyelid function. In spite of this, they we

Facial Nerve Repair by Muscle-Vein Conduit in Rats: Functional Recovery and Muscle Reinnervation.

The facial nerve is the most frequently damaged nerve in head and neck traumata. Repair of interrupted nerves is generally reinforced by fine microsurgical techniques; nevertheless, regaining all functions is the exception rather than the rule. The so-called "postparalytic syndrome," which includes synkinesia and altered blink reflexes, follows nerve injury. The purpose of this study was to examine if nerve-gap repair using an autologous vein filled with skeletal muscle would improve axonal regeneration, reduce neuromuscular junction polyinnervation, and improve the recovery of whisking in rat

Effects of the Number of Muscle-Nerve-Muscle Grafts on Rat Facial Nerve Functional Recovery.

INTRODUCTION: Facial nerve denervation can be devastating for patients. Primary neurorrhaphy and interposition (IP) nerve grafting are common reinnervation techniques. Muscle-nerve-muscle (MNM) grafting is a lesser known alternative. After previously demonstrating significant return of function using MNM grafting in a rat facial nerve model, the authors compare the use of multiple MNM nerve grafts with that of single MNM and IP nerve grafts. METHODS: Thirty-six male rats were randomized into 4 groups: (1) repair with IP grafts, (2) 1 MNM graft, (3) 3 MNM grafts, and (4) control with no interv

Cross-facial nerve graft: a report of chronically paralyzed facial muscle neurotization by a nerve graft.

A 21-year-old housewife with long-standing hemifacial paralysis on the left side decided to undergo the classic two-stage cross-facial nerve graft with muscle transfer. The first stage of the operation was performed successfully with the use of a sural nerve graft. After the appearance of Tinel's sign 8 months later, gain of function in the muscles of the paralyzed side was observed and negated the need for the second-stage muscle transfer.

Facial animation with free-muscle transfer innervated by the masseter motor nerve in unilateral facial paralysis.

PURPOSE: Facial paralysis is a congenital or acquired disorder of varying severity leading to an asymmetric or absent facial expression. It represents an important debilitation from both esthetic and functional points of view. In this article we report our experience with patients treated with gracilis muscle transplantation innervated by the motor nerve to the masseter muscle. We discuss the surgical technique and the functional and esthetic results and evaluate the effectiveness of this donor nerve in providing adequate innervation to the muscle transfer for lower facial reanimation. Intraoperative evoked facial muscle responses and recovery process of the facial nerve in acoustic neuroma surgery.

The prognostic value of intraoperative evoked facial muscle responses (EFMR) was studied and correlated with the recovery process of the facial nerve during a follow-up period of 18 months. The patients were classified into four groups according to EFMR amplitudes, group A (150 microV or greater, 190.8, SD28.9 microV, n = 24), group B (100-149 microV, 125.7, SD14.3 microV, n = 14), group C (50-99 microV, 79.0, SD17.0 microV, n = 13) and group D (less than 50 microV, 22.1, SD13.3 microV, n = 15). Significant improved facial function appeared at 3 months after the operation in group A, at 6 mont

Recording nasal muscle F waves and electromyographic activity of the facial muscles: a comparison of two methods used for intraoperative monitoring of facial nerve function.

OBJECT: A comparison of two electrophysiological methods used to assess facial nerve function intraoperatively was conducted in 33 patients with tumors of the cerebellopontine angle. METHODS: All 33 patients had presented with normal facial nerve function preoperatively. After general anesthesia had been induced by a mixture of midazolam and fentanyl, continual online EMG recordings from the orbicularis oculi and oris muscles were alternated with nasal muscle F-wave recordings. Facial nerve outcomes, assessed using a modified House-Brackmann scale, varied among good (48%), moderate (18%), and

Morphofunctional Improvement of the Facial Nerve and Muscles with Repair Using Heterologous Fibrin Biopolymer and Photobiomodulation.

Peripheral nerve injuries impair the patient's functional capacity, including those occurring in the facial nerve, which require effective medical treatment. Thus, we investigated the use of heterologous fibrin biopolymer (HFB) in the repair of the buccal branch of the facial nerve (BBFN) associated with photobiomodulation (PBM), using a low-level laser (LLLT), analyzing the effects on axons, muscles facials, and functional recovery. This experimental study used twenty-one rats randomly divided into three groups of seven animals, using the BBFN bilaterally (the left nerve was used for LLLT): C
