Fish oil-based lipid emulsion alleviates parenteral nutrition-associated liver diseases and intestinal injury in piglets.

BACKGROUND: Thisstudy aimed to investigate the impact of fish oil-based lipid emulsion (FO) on enterohepatic injuries and intestinal microbiota in piglets of parenteral nutrition (PN). METHODS: Newborn piglets were divided into three groups, including enteral diet (the controls), PN with 100% FO and PN with medium-chain triglyceride/long-chain triglyceride-based lipid emulsion (MCT/LCT) for 14 days. Serum biochemical indicators, hepatic and intestinal histology, and expression of genes associated with inflammation, oxidative stress, and lipid metabolism were measured. The bile acid (BA) profi

Dosage of Lipid Emulsions as an Antidote to Lipid-Soluble Substances.

INTRODUCTION: Lipid emulsions are increasingly used as an antidote to lipophilic drug intoxications. The dose recommended by the American Society of Regional Anesthesia is used primarily for the treatment of local anesthetic systemic toxicity. There is insufficient information about what the dose of lipid emulsions (LE) should be in other intoxications depending on their severity. AIM: To determine the LE dose in a shock or haemodynamic instability in patients with acute exogenous intoxications treated with LE. MATERIALS AND METHODS: Forty-nine patients with acute lipophilic drug intoxicatio

Effect on placental transfer of exogenous lipid administered to the pregnant rabbit.

The transfer of lipids across the placenta was measured after infusion of an emulsion of triacylglycerol and phospholipid (intralipid) into 10 anesthetized rabbits. Maternal and umbilical venous and arterial samples were collected at 10-min intervals. All samples were analyzed for concentration and fatty acid composition of FFA, triacylglycerol, and phospholipid fractions. At the end of the infusion period of intralipid, there was a significant increase in the maternal concentrations of total triacylglycerol (p < 0.01) and of total phospholipid (p = 0.01) but not of total FFA (p > 0.05). [Practical aspects of lipid emulsion stability and peroxidation processes in parenteral nutrition].

The stability of lipid emulsions used in parenteral nutrition (PN) is a key determinant of both the safety and effectiveness of intravenous nutritional therapy. This review summarizes the main physicochemical factors influencing the stability of ternary PN admixtures, emphasizing the role of electrolytes, pH, osmolarity, temperature, light exposure, and handling during storage and administration. Mechanisms leading to emulsion destabilization-such as lipid globule coalescence or phase separation-are described, highlighting the importance of maintaining a mean particle diameter below 5 µm to pr

Response to "Lipid emulsion resuscitation for intractable calcium channel blocker toxicity in pediatric patients".

Due to their role in oxygen transport and the presence of redox active haemoglobin molecules, red blood cells (RBC) generate relatively high levels of reactive oxygen species (ROS). To counteract the potential deleterious effects of ROS, RBCs have a well-integrated network of anti-oxidant mechanisms to combat this oxidative stress. ROS formation is increased in sickle-cell disease (SCD) and our studies in a murine SCD model showed a significant increase in the generation of ROS when compared with normal mice. Our data also indicated that murine sickle RBCs exhibit a significantly increased ATP

Efficacy of lipid emulsion therapy in treating cardiotoxicity from diphenhydramine ingestion: a review and analysis of case reports.

INTRODUCTION: Lipid emulsion therapy (LET) has been most thoroughly studied to reverse local anesthetic systemic toxicity (LAST). Case reports suggest that LET can successfully rescue cardiovascular collapse from bupropion, amitriptyline, and propranolol. The efficacy of LET against refractory hypotension and dysrhythmias from diphenhydramine, a commonly ingested lipophilic cardiotoxic agent, is less well described. OBJECTIVE: Summarize the evidence that LET rescues cardiac ion channel blockade (QRS, QTc widening) or hypotension attributable to diphenhydramine overdose. METHODS: We searched

[No support for lipid therapy as an effective antidote in acute intoxication. A systematic literature review and analysis of 114 case reports].

Lipid rescue is increasingly used as treatment of various types of poisoning despite weak scientific evidence. Some experimental studies have confirmed a positive effect, but others have failed. Clinical studies are lacking, wherefore we decided to do a systematic review of all published human cases. Case reports were searched for in PubMed and Web of Science and thereafter examined by two experts according to an assessment form grading the probability for a causal connection between lipid rescue and improved symptoms. A total of 114 case reports were identified. No correlation was found betwe

Intralipid causing adult respiratory distress syndrome.

Adult respiratory distress syndrome developed in a patient after he was administered intralipid. Intralipid was implicated as the causal agent after careful assessment of the complete clinical picture. To the authors' knowledge this is the first documented case of intralipid-induced adult respiratory distress syndrome. Clinicians should be aware of the possibility of its occurrence.

Priapism in a patient treated with total parenteral nutrition.

Venous thrombosis is a common complication of total parenteral nutrition. We report a case of priapism in a 40-year-old man after administration of total parenteral nutrition for chronic idiopathic intestinal pseudo-obstruction. The patient received glucose, amino acids, and 20% fat emulsion; 12 hours after administration, the patient complained of a persistent, painful penile erection lasting 5 hours. Bilateral corpora cavernosa spongiosum shunts achieved immediate and sustained detumescence, but the patient remained impotent. There was no history of penile or pelvic trauma, hemoglobinopathy,

Effect of intravenous lipid and glutathione infusions on metabolism, inflammation, and immune functions of dairy cows.

During the transition period, dairy cows often present metabolic and immunological disturbances. For several years, negative energy balance and the associated increase in blood nonesterified fatty acids (NEFA) have been considered the most likely causes of these disturbances. However, the transition period is also associated with a state of systemic inflammation. Some recent studies suggest that this inflammation is the result of a leakage of LPS from the intestines, uterus, and mammary gland into the blood compartment and that metabolic and immune disturbances are the consequences. As it is d

Methods of preventing vinorelbine-induced phlebitis: an experimental study in rabbits.

PURPOSE: In order to identify methods for preventing phlebitis caused by intravenous administration of vinorelbine (VNR), we established a procedure for estimating the severity of phlebitis in an animal model. METHODS: Four different factors (administration rate, dilution, flushing, and infusion of fat emulsion) were evaluated for alleviation of phlebitis caused by VNR infusion. VNR was diluted with normal saline to prepare test solutions with concentrations of 0.6 mg/mL or 0.3 mg/mL for infusion into the auricular veins of rabbits. Two days after VNR infusion, the veins were subjected to his

Experimental study on the drainage of so-called intravenous fat pigment from the liver. Long-term observation with histologic, enzyme-histochemical and electron microscopic analysis.

The drainage and decrease of so-called intravenous fat pigment (i.v. fat pigment), which was a lipoid-pigment complex and deposited in the reticuloendothelial system after repeated infusions of Intrafat (intravenous fat emulsion), was observed in the liver of the rabbit by histological, histochemical and electron microscopic methods. As a result, the i.v. fat pigment was mainly found in Kupffer cells immediately after the repeated injections of Intrafat. After then, depending on the period when no injection were given, it decreased gradually and disappeared finally from Kupffer cells. On the o

Lipofundin mediates major inhibition of intravenous propofol on phorbol myristate acetate and Escherichia coli-induced neutrophil extracellular traps.

BACKGROUND: Neutrophil extracellular traps (NETs) consist of chromatin DNA networks that are studded with cytosolic and granular antimicrobial proteins to trap or kill an infected microorganism. A lipid emulsion, the solvent of pure propofol for intravenous application, is given to clinical patients who require intravenous feeding of fatty acids and fat for energy. Intravenous propofol is widely used to sedate critically ill patients. Both intravenous propofol and its lipid emulsion have immunomodulatory activity. However, the role of lipid emulsion of intravenous propofol on NET induction rem

Development of vitamin-E-status of premature infants after intravenous application of all-rac-alpha-tocopheryl acetate.

A low vitamin-E-status was found in premature infants with birth weight less than or equal to 1,500 g. In this study the plasma vitamin-E-concentration of premature infants was analysed 2-3 h postpartal. During a total parenteral nutrition including fat emulsions the children received 4.5 mg all-rac-alpha-tocopherol acetate/kg/d (approximately 3 mg rrr alpha-tocopherol equivalents/kg/d) intravenously for 5 days. On the same days control-infants received the same fat emulsions without supplemented vitamin E. Serum alpha-tocopherol-levels were analysed on day 1, 3, 7 and 14 by HPLC. Initially th

Risk factors and impact of hypertriglyceridemia in preterm infants under 32 weeks of gestation: optimizing intravenous lipid emulsion infusion rates-a single center retrospective study.

INTRODUCTION: Intravenous lipid emulsions are essential for parenteral nutrition in preterm infants. However, hypertriglyceridemia (HTG) is a common complication that can lead to severe complications if left untreated. This study analyzed factors influencing HTG and the effects of reduced intravenous lipid emulsion dosages. METHODS: A retrospective cohort study involving premature infants born at less than 32 weeks of gestation and admitted to Ajou University Hospital between 2017 and 2022 was conducted. Infants with documented triglyceride levels within the first 10 days of life were include

The passage of fat emulsion across the human placenta.

Six patients near term were given an intravenous infusion of a fat emulsion (Intralipid) a few hours before normal delivery or Caesarean section. Six other non-infused patients were studied as controls. Maternal venous and umbilical venous and arterial blood samples were taken at delivery and analyzed for individual fatty acid concentrations in triglyceride, free fatty acid (FFA) and phospholipid fractions. The emulsion, being rich in oleic and linoleic acids, affected the composition of the maternal triglycerides. The fetal lipids were also altered and the infusions resulted in large positive

Use of Fish Oil Intravenous Lipid Emulsions as Monotherapy in the Pediatric Intestinal Failure Patient: Beyond the Package Insert.

In July 2018, an intravenous lipid emulsion (ILE) composed of 100% fish oil (Omegaven, Fresenius Kabi, Bad Homburg, Germany) received Food and Drug Administration (FDA) approval as a source of fatty acids and calories for infants and children with parenteral nutrition-associated cholestasis. This soy-free fat source is rich in ω-3 fatty acids and α-tocopherol and contains few phytosterols. In comparison to conventional soybean oil ILE, this emulsion appears to be less hepatotoxic. The purpose of this paper is to guide the practitioner on the use of this alternative fat source in clinical pract

Physical and Chemical Compatibility of Etomidate and Propofol Injectable Emulsions.

INTRODUCTION: The mixture of etomidate and propofol is widely used in clinical practice to improve efficacy of general anesthesia and to minimize side effects. As a thermodynamically unstable system, emulsion is prone to destabilization through mechanisms including coalescence, flocculation, and creaming. Such unwanted phenomenon can induce fat embolism after intravenous administration. This study was aimed to investigate the physical and chemical stability of the mixture of etomidate and propofol in the dosage form of emulsion. METHODS: This compatibility study focused on the critical qualit
