BACKGROUND: Predictive models to identify low-risk febrile neutropenia (FN) have been developed with heterogeneous samples, which included stable and unstable patients, solid tumours, acute leukaemia and bone marrow transplantation. These models fail to recognise 5-15% of cases with unexpected complications, and literature specifically addressing apparently stable patients (ASPs) is scarce.
METHODS: We reviewed 861 episodes of FN in outpatients with solid tumours, including 692 (80%) episodes with apparent clinical stability. We aimed to investigate the prognosis of this latter

Owing to concern about aminoglycoside-related acute kidney injury (AKI) in therapy for febrile neutropenia, the aim of this study was to ascertain the incidence, severity and persistence of AKI secondary to aminoglycoside use for febrile neutropenia at an adult tertiary referral hospital. All admitted adults with neutropenia in a 27-month period were reviewed. Cases of febrile neutropenia due to chemotherapy who received an aminoglycoside were identified and renal function was assessed up to Day 30 after aminoglycoside administration. Transient renal impairment (TRI) was defined as any tempora

OBJECTIVES: Febrile neutropenia is a life-threatening complication of chemotherapy, requiring prompt risk stratification to guide initial management decisions. While the Multinational Association for Supportive Care in Cancer (MASCC) score has long been used to identify low-risk patients, the Clinical Index of Stable Febrile Neutropenia (CISNE) was developed more recently to refine prognostication among apparently stable individuals. However, there remains no consensus on which score performs better in real-world emergency settings.
METHODS: This retrospective, two-center study included adult

PURPOSE: The incidence of febrile neutropenia (FN) in adults with castrate-resistant metastatic prostate cancer (mCRPC) receiving docetaxel in real-world settings has not been well studied since the expanded role of hormonal treatments. The study objective was to determine the incidence of FN and neutropenia among adults with mCRPC receiving docetaxel. Secondary objectives were to quantify outcomes of patients who develop FN and to identify predictors for FN in this population.
METHODS: A single-center retrospective cohort study was conducted which included adults with mCRPC receiving docetax

Febrile neutropenia is associated with a high risk of infection-related mortality if appropriate antimicrobial therapy is not initiated promptly. In recent years, the diagnostic and therapeutic concepts for febrile neutropenia have substantially advanced, along with new epidemiological data on pathogens, particularly drug-resistant bacteria. According to evidence-based medical evidence from both domestic and international sources, the Chinese Society of Hematology, Chinese Medical Association and Chinese Medical Doctor Association, Hematology Branch organized experts in related fields to updat

BACKGROUND: Febrile neutropenia (FN) is a serious and potentially life-threatening complication of chemotherapy, frequently leading to hospitalization, treatment delays, and increased healthcare utilization. Identifying clinical factors associated with FN-related hospital admission is essential for improving risk stratification and supportive care strategies.
METHODS: This observational study evaluated cancer patients 2207 receiving chemotherapy who developed FN. Clinical, laboratory, and treatment-related variables-including body mass index (BMI), hemoglobin level, absolute neutrophil count,

Background: For pediatric patients with suspected neutropenic fever in the emergency department, the gold standard antibiotic administration time is 60 min upon presentation. Unfortunately, this is difficult to achieve given operational delays and the lack of multidisciplinary collaboration. Objective: To implement quality improvement strategies to improve the time to antibiotic administration for pediatric patients with suspected neutropenic fever. Methods: Eligible participants included children with suspected febrile neutropenia. A chart review was conducted to determine if intravenous anti

Histoplasmosis is an infection caused by a dimorphic fungus. Disseminated disease in children is described mainly in infants under two years of age, but can be particularly aggressive in immunocompromised patients. We report the case of a six-year-old girl with B-cell acute lymphoblastic leukemia (ALL) who presented with fever of unknown origin, respiratory symptoms, hepatosplenomegaly, and pancytopenia. She was admitted to the hospital with febrile neutropenia and exhibited a progressively worsening clinical course. Initial work-up, including laboratory and imaging studies, failed to identify

BACKGROUND: Pediatric oncology patients with prolonged (≥96 hours) febrile neutropenia (absolute neutrophil count < 500/μL) often undergo an evaluation for invasive fungal disease (IFD) and other infections. Current literature suggests that beta-D-glucan (BDG), galactomannan, bronchoalveolar lavage (BAL), and computed tomography (CT) scans (sinus, chest, and abdomen/pelvis) may help determine a diagnosis in this population.
METHODS: In a retrospective cohort study of all cancer/stem cell transplant patients (diagnosed 2005-2019) from one pediatric hospital, all episodes with prolonged febrile

Hyperthyreoidism is a clinical manifestation of excessive production of thyroid hormones. In most cases pacient ´s condition allows ambulant treatment. Rarely, it can develop into an acute, life- threatening thyrotoxic crisis which has to be treated in the intensive care unit. Main therapy includes antithyroid medication, corticosteroids, beta- blockers and rehydratation, mostly parenteral. If initial treatment fails, plasmapheresis provides effective strategy. Antithyroid medication may come with side effects as rash, digestive issues, joint pain.Agranulocystosis or acute liver lesion which l

Febrile neutropenia is the most frequent complication in children treated with chemotherapy. Nevertheless, neutropenic children are a very heterogeneous group and invasive bacterial infections concern a minority of patients. Reducing antibiotics would bring many benefits. Yet, we can only explore this strategy if the safety of children is preserved. The main aims of this review were to study the safety and effectiveness of reducing antibiotic use in children with febrile neutropenia in terms of duration, route of administration (oral versus intravenous) and narrowing of antimicrobial spectrum.

Infectious diseases are associated with a high morbidity and mortality rate among pediatric cancer patients undergoing treatment or receiving a transplant. Neutropenia represents a potentially fatal complication of cancer treatment and is associated with a high risk of developing bacterial infections. Although febrile neutropenia (FN) can affect both adults and children, the latter has a higher chance of infections with an unknown origin. Prompt empiric broad-spectrum antibiotic administration is collectively considered the best therapeutic approach. This review aims to analyze the latest work

BACKGROUND: Acute leukemia is mainly treated with chemotherapy leading to febrile neutropenia (FN). There is limited data on clinical factors predictive of mortality in adults with acute leukemia and FN.
METHODS: This was a retrospective cohort study and enrolled adult patients, diagnosed as acute leukemia, and developed FN. The eligible patients were admitted and followed up with mortality as the primary outcome. A stepwise, multivariate logistic regression analysis was used to find predictors for mortality.
RESULTS: There were 203 patients met the study criteria. Of those, 14 patients died

PURPOSE: Febrile neutropenia (FN) is an important cause of morbidity and mortality in children with acute lymphoblastic leukemia (ALL). We aimed to look at complications in febrile neutropenia and to derive a risk model for developing complications from the variables predicting complications.
METHODS: Children on treatment for ALL, presenting with FN, were prospectively enrolled over a period of 1 year. Their clinical presentation, course during hospital stay, and outcomes were recorded. Complications recorded included septic shock, pneumonia requiring invasive or non-invasive ventilation, re

Fever during chemotherapy-induced neutropenia continues to be a major cause of morbidity and mortality in cancer patients. Mortality depends on the duration and degree of neutropenia, bacteremia, sepsis, performance status, comorbidities and other parameters. The highest mortality rates in cancer patients hospitalized with febrile neutropenia (FN) are observed in those with documented infection. The objectives of the study were to present available tools for risk assessment, to review pathogens causing infections in adult FN patients and to assess outcomes. Methods: This cross sectional study

Infection is the main problem among the patients receiving cancer therapy. The mortality rate can be reduced by the appropriate treatment in the right time. Although blood culture is the gold standard for the diagnoses of sepsis, many factors influence the results of blood culture in children. For this reason, real time polymerase chain reaction (Rt-PCR) has gained importance for the diagnoses of microbiological agents as it is faster than the conventional methods. In this study, we aimed to compare the efficacy of SeptiFast (SF) test with blood culture among children with neutropenic fever. B
