The effect of exercise time and frequency on the development of cam morphology.

OBJECTIVE: Femoroacetabular impingement syndrome (FAIS) with cam morphology is a prevalent cause of intra-articular hip pain in young adults and is widely associated with sporting activity during skeletal immaturity. The exact association between activity levels and cam morphology is not fully understood. This scoping reviews aims to (1) systematically map the available evidence regarding the association between physical activity levels and cam morphology and (2) elucidate the nature of this association, examining whether a threshold effect exists at specific training intensities or durations

Editorial Commentary: Hip Arthroscopy in Patients With Spine Pain: Outcomes Vary Compared to Patients With Isolated Hip Pathology, but Many Hip-Spine Patients Do Just as Well.

Low back pain present concurrently with hip impingement or labral pathology is frustrating for both patient and surgeon. It is difficult to distinguish true hip pathology from secondary low back symptoms, even with the use of diagnostic injections, cross-sectional imaging, electromyography, and/or nerve conduction studies. In addition, even properly indicated hip arthroscopy can exacerbate sciatic nerve dysfunction, a known complication from traction, and altered gait in the early postoperative period can aggravate sacroiliac and lumbar pathology. Moreover, difficult results vary in these pati

Cartilage Lesions of the Hip.

Cartilage lesions of the hip are frequently encountered in the setting of multiple concomitant hip pathologies. For patients with symptoms refractory to comprehensive nonoperative management, numerous surgical treatments are available. To date, the ideal management strategy for hip chondral defects remains controversial, with patients best suited by an individualized approach accounting for the lesion being treated as well as any potentially modifiable underlying factors such as femoroacetabular impingement or dysplasia. Studies on the treatment of hip chondral defects continue to be published

The Principles of Hip Joint Preservation.

The 3 primary factors involved with preservation of the hip joint are femoroacetabular impingement (FAI), hip dysplasia, and femoral torsion abnormalities. Each of these factors affects the health of the acetabular labrum and femoroacetabular cartilage. The appropriate surgical treatments for each of these factors include arthroscopic or open femoroplasty or acetabuloplasty for FAI, periacetabular osteotomy (PAO) for acetabular dysplasia, and de-rotational femoral osteotomy for femoral torsion abnormalities. When evaluating patients with prearthritic hip conditions, orthopaedic surgeons should

Editorial Commentary: Hip Central Acetabular Osteophyte Treatment Improves Outcome: Flip Your Arthroscope 90° and Look Down!

Three goals are at the top of our minds when achieving optimal outcomes for the arthroscopic treatment of femoroacetabular impingement syndrome in the primary setting: (1) accurate bony correction, (2) labral function restoration-typically with labral repair-and (3) comprehensive capsular management-which involves capsular preservation and closure. Notwithstanding, additional intra-articular concomitant conditions require our attention as well. Central acetabular impingement is characterized by the presence of central acetabular osteophytes at the cotyloid fossa. It has been proposed that cent

Editorial Commentary: Symptomatic Femoroacetabular Impingement Resulting in Severe Hip Osteoarthritis: Predicting the "Perfect Storm".

In the last 20 years, femoroacetabular impingement (FAI) has transitioned from a disputed disease to a well-established cause of hip pain and osteoarthritis (OA). Cam-type FAI, specifically, is supported by several studies as a risk factor for osteoarthritis. Elevated α-angle is also a mild-to-moderate risk factor for OA in patients with FAI. Other risk factors include age, sex, body mass index, activity level, range of motion, 3D acetabular and femoral morphology, and femoral version. To further complicate the picture, when we look at the contralateral hip (where many of these factors are hel

Controlled repositioning and primary osteoplasty as a treatment option for the management of unstable slipped capital femoral epiphysis.

The ideal management of an unstable slipped capital femoral epiphysis (SCFE) is still controversial. The currently available options are in-situ screw fixation with delayed corrective osteotomy, closed reduction and screw fixation and anatomic reduction by modified Dunn's procedure (MDP). We present the results of our technique of controlled re-positioning (CRP) with primary osteoplasty in which the epiphysis is repositioned to the preacute slip stage and a mini-open osteoplasty is done in the same sitting to avoid later femoro-acetabular impingement. We had 27 unstable slips which presented t

Successful Arthroscopic Fixation of a Posterior Wall Acetabular Fracture Nonunion on a Patient With Femoroacetabular Impingement: A Case Report.

CASE: This report presents the results of a hip arthroscopic technique that was used to treat an unusual posterior wall acetabular fracture nonunion. A 30-year-old man presented with 1 year of persistent left-sided buttock and groin pain with associated feeling of instability when climbing stairs. Magnetic resonance imaging demonstrated a small left acetabular posterior wall fracture nonunion. We describe outpatient treatment with reduction and cannulated screw fixation using arthroscopic techniques that permitted immediate partial weight bearing and achieved excellent outcomes at 1 year. Editorial Commentary: Spine Pathology May Compromise the Results of Hip Arthroscopy: Will Hip Arthroscopy Improve Low Back Pain?

Pathology of the lumbar spine and hip commonly occur concurrently. The hip-spine connection has been well documented in the hip arthroplasty literature but until recently has been largely ignored in the setting of hip arthroscopy. Physical examination and diagnostic workup of the lumbosacral junction are warranted to further our understanding of the effects of lumbosacral motion and pathology in patients with concomitant femoroacetabular impingement syndrome. An understanding of this relationship will better allow surgeons to counsel and preoperatively optimize patients undergoing evaluation a

Acetabular labral reconstruction with medial meniscal allograft: preliminary results of a new surgical technique.

PURPOSE: Reconstruction of the acetabular labrum during femoroacetabular impingement (FAI) surgery is accepted when the labrum is deficient and irreparable. Here we describe a novel technique using fresh-frozen medial meniscal allograft for labral reconstruction during surgical hip dislocation for correction of pincer FAI due to acetabular overcoverage. METHODS: The results from seven hips (six patients) that underwent this procedure with 1 year minimum follow-up, and radiographs are presented. RESULTS: Six of the seven hips had improvements in pain, hip flexion, hip abduction, and Merle d'A

Clinical presentation, imaging findings, and arthroscopic features in skeletally immature and mature adolescent hip patients: a comparative double-cohort retrospective study.

The purpose of this study was to evaluate differences in clinical presentation and extent of surgery required based on skeletal maturity between two cohorts of adolescent hip arthroscopy patients. We hypothesized that skeletal immaturity would be associated with a lower frequency of pincer impingement and a decreased need for surgical acetabuloplasty. A database of 1481 hip arthroscopies performed by a single orthopaedic surgeon between 2008 and 2016 was queried. Patients ≤18 years of age with femoroacetabular impingement were divided into two groups based on Risser score: Risser 1-4 (skeletal

The Role of Comprehensive Capsular Management in Hip Arthroscopy for the Treatment of Femoroacetabular Impingement Syndrome.

Advances in the understanding of femoroacetabular impingement syndrome and advancements in hip arthroscopic techniques, including chondrolabral preservation and labral repair, have led to improvements in success rates, functional outcomes, and return to sports over the past several years. This improvement in outcomes also is attributed to the increased awareness of performing capsular closure after addressing intra-articular hip pathology, to preserve the biomechanical properties of the hip. A number of biomechanical studies have demonstrated that the iliofemoral ligament is a critical compone

Editorial Commentary: The Competitive Dancer's Hip-An Acquired Flexible Attitude to a High-Performance Hip.

Competitive dancers have a high prevalence of hip injuries. The dancer's hip is a unique scenario in which hip impingement is associated with compensatory soft-tissue laxity. Particularly in these athletes, normal osseous hip morphology with symptomatic femoroacetabular impingement may be observed. The repetitive training, including extreme daily hip range of motion, results in compensatory soft-tissue laxity and secondary impingement-induced subluxation. Peculiarly, hip impingement in dancers occurs in the posterosuperior aspect of the hip and extra-articularly between the anterior inferior i

A New Method for Cartilage Evaluation in Femoroacetabular Impingement Using Quantitative T2 Magnetic Resonance Imaging: Preliminary Validation against Arthroscopic Findings.

OBJECTIVE: The outcome of arthroscopic treatment for femoroacetabular impingement (FAI) depends on the preoperative status of the hip cartilage. Quantitative T2 can detect early biochemical cartilage changes, but its routine implementation is challenging. Furthermore, intrinsic T2 variability between patients makes it difficult to define a threshold to identify cartilage lesions. To address this, we propose a normalized T2-index as a new method to evaluate cartilage in FAI. DESIGN: We retrospectively analyzed magnetic resonance imaging (MRI) data of 18 FAI patients with arthroscopically confi

Treatment of Femoroacetabular Impingement: Labrum, Cartilage, Osseous Deformity, and Capsule.

Femoroacetabular impingement (FAI) surgery aims to restore the native anatomical relationships between bones and the soft tissues comprising the hip joint. The goal of this approach is to mimic the natural biomechanical function of the hip joint and translate it into a perfect clinical outcome. In this article, we describe the indications and contraindications for our preferred hip arthroscopic techniques for correcting FAI in both primary and revision cases and discuss the role of postoperative rehabilitation and use of biologics in further improving patient outcomes.

Arthroscopic Versus Open Treatment of Femoroacetabular Impingement: A Systematic Review of Medium- to Long-Term Outcomes.

BACKGROUND: Surgical treatment of symptomatic femoroacetabular impingement (FAI) aims to improve symptoms and potentially delay initiation of hip osteoarthritis and prevent progression to end-stage hip osteoarthritis and possible total hip arthroplasty (THA). Hip arthroscopy and open surgical hip dislocations are the 2 most common surgical approaches used for this condition. PURPOSE: To perform a comparative systematic review to determine whether there is a significant difference in clinical outcomes and progression to THA between hip arthroscopy and open surgical hip dislocation treatment fo
