Intracapsular Femoral Neck Fractures-A Surgical Management Algorithm.

Background and Objectives: Femoral neck fractures are common and constitute one of the largest healthcare burdens of the modern age. Fractures within the joint capsule (intracapsular) provide a specific surgical challenge due to the difficulty in predicting rates of bony union and whether the blood supply to the femoral head has been disrupted in a way that would lead to avascular necrosis. Most femoral neck fractures are treated surgically, aiming to maintain mobility, whilst reducing pain and complications associated with prolonged bedrest. Materials and Methods: We performed a narrative rev

Treatment of femoral neck fractures in young adults.

Femoral neck fractures in physiologically young adults, which often result from high-energy trauma, are less common than intracapsular femoral neck fractures in elderly patients. They are associated with higher incidences of femoral head osteonecrosis and nonunion. Understanding the multiple factors that play a significant role in preventing these complications will contribute to a good outcome. Although achieving an anatomic reduction and stable internal fixation are imperative, other treatment variables, such as time to surgery, the role of capsulotomy, and the method of fixation remain deba

[Management of ipsilateral femoral neck and shaft fractures].

PURPOSE: The combination of ipsilateral femoral neck and shaft fractures remains a treatment challenge in orthopedic surgery because both fracture types constitute separate entities and require specific treatment concepts. MATERIAL AND METHODS: In a case control study, incidence, treatment strategies, and outcomes of this injury were analyzed. All patients with femoral fractures treated between 1 January 2001 and 31 July 2007 at a level I trauma center were included in the study. RESULTS: Twenty-one out of 1,935 patients (1.1%) sustained 22 combined fractures of the femoral neck and shaft. A

Displaced femoral neck fatigue fractures in military recruits.

BACKGROUND: Displaced fatigue fractures of the femoral neck are uncommon, but they can lead to substantial patient morbidity. This study was performed to examine the incidence, long-term consequences, radiographic findings, risk factors, and complications associated with this fracture. METHODS: Between 1975 and 1994, twenty-one military recruits sustained a displaced fatigue fracture of the femoral neck. Nineteen patients were followed for an average of eighteen years. Data regarding the population at risk, hospital records, initial and follow-up radiographs, and physical findings were analyz

Femoral neck fractures complicating Gaucher disease in children.

In normal children, fractures of the femoral neck are uncommon and accompany severe trauma and multiple injuries elsewhere in the skeleton [16, 17]. In children with Gaucher disease, a rare hereditary disorder of lipid metabolism, midcervical or basicervical fractures can occur with minor or no trauma and without other injury to the skeleton. Three children with Gaucher disease who developed pathologic fractures of the femoral neck are described. In all three, the fractures occurred between five and nine years of age, and the fracture lines passed through areas of abnormal bone characterized b

Classification of femoral neck fractures in the elderly as pathologic fractures.

The following distinctions can be made between fractures of the femoral neck in elderly persons and similar fractures in younger patients: They are much more common in the elderly. They are more frequent in females than in males. There is a much higher incidence in the white than in the black population. Fractures of the opposite hip are more common in the elderly. Associated medical conditions in the elderly may make the patient more susceptible to falls and may increase the degree of osteopenia. There is a high rate of mortality in elderly patients within 1 year of the fracture. Minor trauma

Femoral neck shaft angle in relation to the location of femoral stress fracture in young military recruits: femoral head versus femoral neck stress fracture.

OBJECTIVE: To evaluate the influences of the femoral neck shaft angle (FNSA) on the location of the femoral stress fracture and to assess the potential differences in FNSA between fractured and normal femurs. MATERIALS AND METHODS: Thirty-seven patients with femoral stress fractures who underwent both plain hip radiographs and MRI, from January 2016 to September 2019, were retrospectively included. Patients were classified as having either femoral head stress fracture (group A, n = 26) or femoral neck stress fracture (group B, n = 11). The FNSA was measured in anteroposterior (AP) hip radiogr

Fixation failure following femoral neck system fixation for intracapsular femoral neck fractures: Association with fracture orientation.

PURPOSE: To evaluate fixation failure following femoral neck system fixation (FNS) for intracapsular femoral neck fractures (FNF) and to determine whether fracture characteristics, particularly fracture orientation, were associated with fixation failure. METHODS: A retrospective cohort study was performed at a Level I trauma center. 82 patients with intracapsular femoral neck fractures treated with FNS fixation were included. Fixation failure was defined as implant cut-out, loss of fixation, nonunion, or revision surgery. Associations between fracture characteristics and fixation failure were

Ballistic femoral neck fractures: Associated injuries and outcomes.

BACKGROUND: Ballistic fractures of the femoral neck, rare injuries that overwhelmingly affect younger adults, pose significant challenges to the treating surgeon. However, there is limited literature that the treating surgeon can leverage to guide their treatment decisions. The goal of this study is to describe the demographics, associated injuries, outcomes, and complications associated with ballistic femoral neck fractures. PATIENTS AND METHODS: This retrospective case series, performed at a single, academic, urban, level-one trauma center, evaluated patients with a ballistic fracture of th

Use of Cemented Hemiarthroplasty for Femoral Neck Fractures.

Geriatric femoral neck fractures are common orthopaedic injuries, which are associated with a high morbidity and mortality. Arthroplasty is the optimum treatment for many of these injuries, but debate exists regarding optimal surgical strategy. Multiple recent investigations have demonstrated strong superiority for cemented stems as compared with noncemented fixation with a decreased risk of periprosthetic fracture, shorter length of stay, lower cost, and decreased rate for revision surgery. The main purpose of this article is to refamiliarize the resident or practicing surgeon with cemented a

Lateral femoral neck stress fractures: A case report.

BACKGROUND: Sport-induced injuries particularly affect young, healthy, and active individuals. Running, a popular and accessible sport, can cause a wide range of injuries, including stress fractures. Stress fractures can occur during high-intensity training or competitions, especially among well-trained amateurs and professional athletes. Adequate diagnosis can be complicated by the typically young age, unremarkable medical history, and vital condition of the patient. Stress fractures present insidiously, and this is specifically the case with stress fractures of the femoral neck. Timely inter

Plating of femoral neck fractures: when and how?

Intracapsular femoral neck fractures are common in the elderly population after a simple fall. However, femoral neck fractures in the physiologically young are less common. Regardless, femoral neck fractures can be a source of significant morbidity and mortality if not treated correctly. Multiple management strategies have been described. The decision for the ideal treatment involves careful consideration of the patient's age, activity, and medical comorbidities. Plating techniques of femoral neck fractures continue to evolve. The authors describe the current indications and plating options fo

Percutaneous pinning in undisplaced subcapital femoral neck fractures.

Three hundred and five undisplaced subcapital femoral neck fractures managed by pinning in situ with Knowles' pins were evaluated to elucidate the role and effect of such treatment. The protocol of management and follow up, and evaluation, both radiographically and functionally, were set up prospectively. The duration from injury to management was 3.5 (1-14) days, the operation time was 22 (9-48) min and most of the patients were discharged without hospitalization. The follow-up period was 75 (28-136) months. The final results showed 282 (92.5 per cent) fractures united without complications (

Scintimetric evaluation of complications after femoral neck fractures.

The primary surgical approach to the femoral neck fracture should be one with the lowest mortality and morbidity. Internal fixation appears to be the safe and logical approach for most of the reducible intracapsular fractures. Careful follow-up of these patients then is important. 85Sr scintimetry is a simple and accurate method in the early detection and evaluation of secondary complications in these patients. The 85Sr scintimetric pattern in the normal healing process was defined in relation to time interval after the fracture. At any time after 4 months following fracture, those hips destin
