[Bone marrow edema--an early form of femur head necrosis].

We examined 15 patients (16 hips) with painful hips whose radiographs were either normal (n = 9) or showed a minimal decrease in radiodensity (n = 7). The available bone scintigrams of 9 cases were positive. T1-weighted images visualised a diffuse signal loss of the bone marrow in all hips, with various extensions in the head, neck, and intertrochanteric area. These regions were hyperintensive on T2-weighted images. Focal anomalies were not seen in any of the cases. All patients underwent core decompression treatment. Histology of 13 hips confirmed not only the presence of bone marrow oedema b

[Femur head necrosis and bone marrow edema syndrome in pregnancy].

MR examinations were performed in 9 patients suffering from severe pain of the hip during the third trimenon without any relief after birth. Pathologic signal changes could be observed in 11 hips (oedema in the region of the femoral head and neck (n = 8); avascular necrosis of the femoral head surrounded by bone marrow oedema (n = 3)). In 7 hips a relatively rapid decrease of the oedema following core decompression was demonstrated. Focal necrosis, however, did not show any changes. In two patients, treated conservatively, markedly delayed healing was evident. MR imaging is the modality of cho

[Transient osteoporosis or femur head necrosis? Early diagnosis using MRT].

Transient osteoporosis of the hip is a syndrome that does not seem to be widely known; this is also true for its radiological appearance. It is often mistaken for avascular necrosis of the femoral head, metastatic or inflammatory disease. These differential diagnoses lead to more or less invasive procedures, although transient osteoporosis does not require more than immobilisation for complete remission. MRI was done in 38 patients with acute hip pain, 13 had femoral head necrosis and 8 transient osteoporosis. Follow-up studies via MRI in patients with transient osteoporosis revealed 3 stages

[Is remodeling still possible after collapse in femur head necrosis?].

A case of a femoral head necrosis, with collapsed joint surface after fracture of the femoral neck will be presented. Following varisation osteotomy, the necrotic parts of the bone was removed and filled with cancellous bone. A standardized bone block of the iliac crest was implanted into the hole. After the operation the patient was painfree. The X-rays showed the improvement of the bony structure and later a spherical remodeling of the femoral head. This result is in opposition with the earlier opinion of the disability of femoral head remodeling in adults. It is technically very important t

[Juvenile hip pain. 2. Femur head epiphysiolysis, hip dysplasia, tumors].

The early symptom in hip joint diseases in children is pain. Pain is localized in the groin and thight, but mostly in the knee. Other important signs are limping and reduced internal rotation. If a hip disease is suspected it is necessary to take X-rays in two planes. If diagnosis is early and special therapy started immediately, the results are usually excellent without deformation of the hip. Otherwise early osteoarthritis can develop. This is important because osteoarthritis in the hip joint is in 75% of the cases due to hip joint diseases in childhood. The problems of diagnosis and treatme

Arsenic content in the femur head of the residents of southern and central Poland.

Arsenic content was assayed in the samples of the femur head of the people living in southern and central Poland (Kraków, n=13; Silesian region, n=13; Lódz, n=12). The average age being 68.7+/-8.7 yr. Arsenic content in the femur head was determined applying the hydride generation atomic absorption spectrometry (HG-AAS) method after microwave mineralization. The average arsenic contents in the femur head of the residents of the Lódz, Kraków, and Silesian regions were 0.41 microg/g, 0.37 microg/g, and 0.18 microg/g, respectively. No correlation has been found between arsenic content in the femu

[MRI in the diagnosis and treatment of Perthes disease and epiphysiolysis of the head of the femur].

In a 10-year prospective study, we were able to examine 93 children with 109 hips suffering from Perthes' disease and 6 hips with Meyer's dysplasia epiphysealis by magnetic resonance imaging (MRI) follow-up studies. Routine imaging was done by coronary T1- and T2-enhanced sequences and sagittal T1-enhanced sequences. In total of 400 MR images, the following criteria were evaluated: staging of the disease according to MRI signal changes, extent of the necrosis, prognostic criteria, differential diagnosis of Meyer's dysplasia epiphysealis, and proposals for therapy. Typical signal changes in MRI

["Transient osteoporosis" as a special reversible form of femur head necrosis].

There is still controversy whether transient osteoporosis of the hip joint represents a distinct self-limiting disease, or reflects only an early, reversible subtype of non-traumatic osteonecrosis (ON). Transient osteoporosis has several synonyms: algodystrophy of the hip; transient marrow oedema; or bone marrow oedema syndrome--BMOES. Clinical presentation of BMOES shows mechanical hip joint pain, ON risk factors, and a diffuse bone marrow oedema in MR imaging. Histomorphological changes resemble early ON, but with diffuse sufficient repair in BMOES and focal and insufficient repair only at t

[Femoral head necrosis after intramedullary nailing of the femur in adults].

An avascular necrosis of the femoral head occurred after intra-medullary nailing of a femoral diaphyseal fracture. This complication may be related to the posterior situation of the proximal insertion hole just posterior to the trochanteric notch and piriformis fossa as demonstrated by the CT scan. This situation of the nail may cause injury to posterior branch of the circonflex artery at the time of rod insertion. Even if it appears exceptional in adult patients with intra-medullary nailing, this potential complication and the anatomy of the superior extremity of the femur should be known.

[Hemodynamics changes in proximal femur of patients with femoral head necrosis].

Intraosseous pressure of the proximal femur was measured with type II physical pressure detector in 46 patients with femoral head necrosis (Legg-perthes' disease 25, idiopathic necrosis 21) and in 49 normal persons (25 children and 24 adults). Fourteen of those with head necrosis and 9 of the normals were further surveyed by stress test, a test to see the pressure change following intraosseous injection of 5 ml heparinized normal saline solution. Again, intramedullary venography was done in 16 of those with diseased head and 12 of the normals to investigate their status of venous flow. The int

[Idiopathic femur head necrosis in the adult--results of surgical therapy].

In this paper a review is given first on the various techniques in the operative treatment of the idiopathic necrosis of the femoral head in the adult. Because of the high rate of pseudarthrosis (50%) arthrodesis is rarely done today. Decompression of the medullary space is highly successful in 80-90%, but only in stage I and II. For stage III of the disease different osteotomies of the proximal femur are recommended. Artificial joint replacement is usually done in stage IV. Our case material consists of 46 operations in 37 patients (27 endoprostheses, 16 osteotomies of the proximal femur, 3 f

Arthroscopic Fixation of Osteochondral Fracture of Head of Femur. Case Study.

This article describes the case of an 18-year-old male after hip dislocation caused by snowboard injury, which resulted in osteochondral fracture of the head of femur with displacement of a loose body. CT as well as MRI confirmed the presence of a loose osteochondral fragment (20x14x4mm) fractured from the weight-bear-ing superoanterior part of head of femur (Pipkin type II). The surgical management undertaken was only arthroscopic, this representing a very rare case when com-par-ed to the literature. The article describes in detail the technical aspects of the procedure. At 4 months post-surg
