Maternally Perceived Fetal Movement and Computerized Cardiotocography Parameters: What's the Link? A Multicenter Pilot Study on Fetal Health.

Background: Maternal perception of fetal movement (FM) is an indirect indicator of fetal well-being. The aim was to investigate the relationship between computerized cardiotocography (cCTG) parameters and maternal perception of FM. Methods: We conducted a prospective, multicentric observational study at two tertiary referral hospitals. After cCTG, all women completed a questionnaire assessing maternal perception of FM and fetal hiccups. The primary outcome was the relation between multiple cCTG parameters and maternal perceptions of FM. Results: We included 419 patients. Women who reported inc

Fetal movement counting improved identification of fetal growth restriction and perinatal outcomes--a multi-centre, randomized, controlled trial.

BACKGROUND: Fetal movement counting is a method used by the mother to quantify her baby's movements, and may prevent adverse pregnancy outcome by a timely evaluation of fetal health when the woman reports decreased fetal movements. We aimed to assess effects of fetal movement counting on identification of fetal pathology and pregnancy outcome. METHODOLOGY: In a multicentre, randomized, controlled trial, 1076 pregnant women with singleton pregnancies from an unselected population were assigned to either perform fetal movement counting from gestational week 28, or to receive standard antenatal

Small estimated placental volume (EPV) in the setting of decreased fetal movement.

Although 60.5 % of clinically unexplained stillbirths can be attributed to placental insufficiency and/or fetal growth restriction, clinicians rarely calculate estimated placental volume (EPV). We present a scenario in which EPV was used to inform patient care of a 28-year-old with decreased fetal movement at 32 weeks and 1 day gestation. Although estimated fetal weight (EFW), amniotic fluid index (AFI), and nonstress test (NST) were normal, EPV was low (<0.1st percentile), which prompted more frequent patient follow-up. Oligohydramnios was detected at 38 weeks and 1 day, and labor was induced

Breaking through the silence in antenatal care: Fetal movement and stillbirth education.

BACKGROUND: Fetal movements are a key indicator of fetal health. Research has established significant correlations between altered fetal activity and stillbirth. However, women are generally unaware of this relationship. Providing pregnant women with information about the importance of fetal movements could improve stillbirth rates. However, there are no consistent fetal movements awareness messages globally for pregnant women. AIMS: This study aimed to explore the antenatal care experiences of Australian mothers who had recently had a live birth to determine their knowledge of fetal movement

Metastatic neuroblastoma diagnosed on prenatal sonographic examination performed for decreased fetal movement.

We report a case of fetal neuroblastoma presenting with massive liver metastasis diagnosed during the biophysical profile sonographic examination performed for decreased fetal movement. The patient presented at 37 weeks' gestation with limited fetal movement over 24 hours. Biophysical profile showed marked polyhydramnios and an enlarged abdomen filled with a homogeneous mass lesion suspicious for liver metastasis. Primary urgent cesarean section was performed revealing a cachectic neonate with a rigid and grossly distended abdomen. Neonatal evaluation confirmed the etiology of the abdominal ma

Catheter Displacement Into the Amniotic Cavity by Fetal Movement After Thoracoamniotic Shunting.

Thoracoamniotic shunting (TAS) is an effective treatment for fetal pleural effusion. We report a case of bilateral fetal pleural effusion in which catheter displacement into the amniotic cavity occurred due to fetal movement after TAS. Initially, left TAS was performed twice using a 60-mm catheter, resulting in displacement and the catheter being pinched between the fetal fingers. Subsequently, a 50-mm catheter was employed for left TAS, which successfully prevented further displacement. Labor was induced at 35 weeks and three days of gestation due to an increase in right fetal pleural effusio

Fetal movement frequency and the effect of associated perinatal factors: Multicenter study.

BACKGROUND: Decreased fetal movements are associated with adverse perinatal outcomes, including stillbirth. Delayed maternal visits to a health care provider after perceiving decreased fetal movements are frequently observed in stillbirths. Informing pregnant women of the normal range of fetal movement frequency is essential in their earlier visits in order to prevent stillbirth. AIM: To investigate the fetal movement frequency in late pregnancy and the effects of associated perinatal factors. METHODS: This prospective multicenter study was conducted in 20 obstetric facilities in our region

Deficits in eye movement control in children with fetal alcohol spectrum disorders.

BACKGROUND: Prenatal exposure to alcohol can result in a spectrum of adverse developmental outcomes in offspring, collectively termed fetal alcohol spectrum disorders (FASD). Deficits in executive function--the psychological processes involved in controlling voluntary goal-oriented behavior--are prevalent in FASD. Oculomotor tasks have been designed as highly sensitive tools to evaluate components of executive function. Because of the extensive overlap in the brain areas controlling eye movements and those affected in FASD, we hypothesized that individuals with FASD display specific neurobehav

[Human fetal breathing movements: basic analysis and clinical relationship to fetal well-being (author's transl)].

By means of real time B-scan, human fetal breathing movements (FBM) was basically and clinically investigated on 244 observations in 215 pregnancies. The results obtained were as follows. (1) FBM was frequently observed during active sleep in which FHR variability and fetal movement increased. (2) The incidence of FBM increased rapidly from 12.5% at 23-27 weeks to 77-100% at more than 35 weeks gestation, but reduced to 44% at delivery. (3) FBM became more regular and deeper with advancing weeks of pregnancy, especially after 36 weeks of gestation. The frequency of FBM in normal pregnancy was s

Low dose indomethacin via fetal vein or cerebral ventricle stimulates breathing movements in fetal sheep.

Indomethacin, a prostaglandin synthesis inhibitor, transforms normally intermittent fetal breathing into nearly continuous breathing. If indomethacin acts in brain, we hypothesized that indomethacin would stimulate breathing at a lower dose when given via the lateral cerebral ventricle (i.c.v) than via the fetal jugular vein (i.v.). Chronically catheterized fetal sheep were studied at 0.88 of gestation (128 days). A 2-h control period was followed by an indomethacin infusion at 7 micrograms.kg-1 fetal body weight over the first 10 min of each hour for 3 h then at 28 micrograms.kg-1 over the fi

Association between polyhydramnios and decreased fetal movements: A retrospective cohort study.

OBJECTIVE: To evaluate the association between polyhydramnios and maternal perception of decreased fetal movements (DFM) and to explore whether a volume-dependent threshold exists for this effect. METHODS: This retrospective cohort study included 7215 singleton pregnancies (2006-2025). The exposed group comprised 2066 women with polyhydramnios (amniotic fluid index [AFI] >25 cm or maximal vertical pocket >8 cm), and the unexposed group included 5149 women with normal amniotic fluid volume. DFM rates were compared across gestational age groups and polyhydramnios severity. Adjusted odds ratios

Maternal experience of fetal movements from a child with AMC: MECA survey.

OBJECTIVE: The prevailing assumption is that fetal movements are always absent or reduced in pregnancies affected by arthrogryposis multiplex congenita (AMC), leading to the belief that mothers do not perceive or perceive less fetal movements during affected pregnancies. This study aims to investigate the maternal perception of fetal movements in pregnancies with a child diagnosed with AMC and to challenge this assumption. Additionally, it seeks to expand current knowledge on the perception by comparing with pregnancies with children not affected by AMC. METHODS: A survey-based study was cond

The effects of lockdown during the COVID-19 pandemic on fetal movement profiles.

PURPOSE: The current study investigated the direct impact of the COVID-19 lockdown on fetal movements, addressing a critical research gap. While previous research has predominantly examined the effects of lockdown on maternal health and postnatal outcomes, little attention has been paid to the direct consequences on fetal well-being as indicated by their movement profile. METHODS: We conducted analysis of movement profiles in 20 healthy fetuses during the COVID-19 pandemic lockdown (third national UK lockdown period between January and March 2021) and compared them with 20 healthy fetuses fro

Is it possible to monitor fetal movements with a wearable device? A review of novel technologies.

Stillbirth is often preceded by reduced fetal movements and obstetric intervention is necessitated for stillbirth prevention. Yet, despite fetal movements being one of the few tangible ways a pregnant woman and the clinical team can assess the wellbeing of her baby, there are few validated, objective means for pregnant women to quantify the frequency and nature of an unborn baby's movements. Traditional methods of assessing fetal wellbeing such as cardiotocography and fetal movement charts have a lack of diagnostic accuracy, and often lead to false positive intervention. The need for fetal mov

Risk factors for poor neonatal outcome in pregnancies with decreased fetal movements.

INTRODUCTION: The incidence of Swedish stillbirths has varied little in the past 40 years, with a reported frequency of 400-450 stillbirths/y (approximately 4‰), despite increased information about fetal movement in the media and awareness among healthcare providers. The objectives of this project were to describe the outcome of pregnancies with reduced fetal movement in a Swedish context and to investigate factors associated with poor neonatal outcome in this group. MATERIAL AND METHODS: A retrospective cohort study was performed at Soder Hospital, Stockholm, Sweden. All single pregnancies a

Reduced fetal movements is twin pregnancies and the association with adverse neonatal outcomes.

OBJECTIVE: Reduced fetal movements (RFM) is an obstetric complaint known to be associated with adverse neonatal outcomes and should serve as an alarming sign in obstetric triage. Whether this assumption holds for twin pregnancies, is still an obstetric enigma, and this complaint is sometimes overlooked in twins. We, therefore, aimed to study neonatal outcomes in twin pregnancies complicated by RFM. We hypothesised that in twin pregnancy, maternal ability to perceive RFM will be limited, and therefore, will not be associated with adverse neonatal outcome. STUDY DESIGN: Included were all dichor

Midwives' and obstetricians' knowledge and management of women presenting with decreased fetal movements.

BACKGROUND: Maternal perception of decreased fetal movements (DFM) affects 5-15% of pregnancies. DFM is associated with intra-uterine fetal death (IUFD) and intra-uterine growth restriction (IUGR). It has been proposed that maternal perception of DFM may be used as a screening tool for IUFD or IUGR. However, this proposal is complicated by variations in definitions and management of DFM. HYPOTHESIS: We hypothesised that uncertainties in the definition and management of women presenting with DFM leads to variation in clinical practice. METHODS: A postal questionnaire was sent to midwives and
