Predicting adverse neonatal outcome in severe fetal compromise: a pivotal role for estimated fetal weight Z scores.

OBJECTIVE: To identify significant predictors for adverse neonatal outcome in severe by compromised fetuses. METHODS: Consecutive premature fetuses at between 25 and 32 weeks' with severe placental insufficiency were examined prospectively. Inclusion criteria were: singletons; normal anatomy; abnormal umbilical artery Doppler pulsatility index; abnormal cerebroplacental ratio; middle cerebral artery pulsatility index (PI) <or=2 standard deviations. We considered as independent potential predicting variables: absent or reversed end diastolic flow in umbilical artery, abnormal ductus venosus S/

Fetal heart rate patterns in term labor vary with sex, gestational age, epidural analgesia, and fetal weight.

OBJECTIVES: Our purpose was to analyze the previously unreported effect of fetal sex on the fetal heart rate in labor and to measure its magnitude in relation to the effects of other independent clinical variables. STUDY DESIGN: The last hour of the intrapartum heart rates of 1884 term singleton fetuses collected during routine clinical monitoring over 19 months in Oxford, United Kingdom, was analyzed by computerized techniques. The records were selected for completeness and continuity until within at least 30 minutes of delivery. A subset of records from earlier in labor and a separate archi

Does fetal or maternal leap-2 level affect infant birth weight?

INTRODUCTION: The etiology of maternal and infant weight gain during pregnancy is multifactorial. Many different biomarkers have been studied to elucidate this complex issue. Recent studies have led to the idea that Leap-2, which is thought to have an active role in energy metabolism, may also have an influence on infant birth weight. Therefore, we aimed to evaluate the relationship between Leap-2 levels and infant birth weight. METHODS: In this prospective study, we evaluated 102 patients with pregnancies of 34 weeks or more. Among those group, Leap-2 levels collected from maternal venous blo

The role of estimated fetal weight discordance in dichorionic twin pregnancies.

Evaluation of relative fetal growth in the form of estimated fetal weight discordance (EFWd) is a necessary element of any ultrasound examination in twin pregnancies. It is one of the criteria for the diagnosis of selective fetal growth restriction (sFGR) according to the most established worldwide guidelines. Apart from the effectiveness of this parameter for the diagnosis of sFGR, it may also be used as an independent factor for risk stratification of neonatal and maternal complications. Furthermore, numerous studies have proven the greater prognostic value of EFWd in dichorionic pregnancies

Fetal Growth Pattern and Prediction of Low Birth Weight in Gastroschisis.

OBJECTIVES: To examine growth patterns and predictions of low birth weight in gastroschisis fetuses. METHODS: This is a retrospective study of isolated fetal gastroschisis before week 24. Ultrasound fetal biometric parameters - head circumference (HC), abdominal circumference (AC), femur length, HC/AC ratio and estimated fetal weight (EFW)--were plotted against normal growth charts. The percentage difference in mean values between normal and gastroschisis fetuses was determined. The growth deficit for each ultrasound parameter was calculated for the fetuses with 1 examination in each designat

Declines in birth weight and fetal growth independent of gestational length.

OBJECTIVE: To estimate whether the decrease in birth weight of term singletons in the United States and elsewhere over the past decade, despite trends in common maternal characteristics expected to contribute toward an increase, is attributable to the simultaneous decrease in gestational length. METHODS: Using data from Intermountain Healthcare, where a successful initiative reduced the number of early-term (37-38 weeks) elective deliveries, we examined trends in birth weight, being small for gestational age (SGA), and being large for gestational age (LGA) among 219,694 singleton neonates bor

Limited usefulness of fetal weight in predicting neonatal brachial plexus injury.

OBJECTIVE: The objectives were to determine the neonatal morbidity rate from vaginal birth and examine fetal weight-based injury-prevention strategies. STUDY DESIGN: Selected neonatal morbidities were categorized by birth weight for all vertex vaginal deliveries occurring during a 12-year period. Sensitivity, specificity, and predictive values for brachial palsy were calculated at increasing birth weight cutoff levels. A policy of cesarean delivery for macrosomic infants was evaluated. RESULTS: There were 80 cases of brachial palsy among 63,761 infants (0.13%). In mothers without diabetes, r

Pregnancy and delivery outcomes in fetal macrosomia (birth weight exceeding 5,000 g).

OBJECTIVES: This study examines the association between fetal macrosomia (>5,000 g) and pregnancy and birth outcomes. METHODS: This was a retrospective cohort study using data from the perinatal survey of Lower Saxony, Germany (2001-2009). RESULTS: Pregnancies complicated by advanced maternal age (>35 years), elevated BMI (>25.0), and excessive gestational weight gain were significantly more likely to result in macrosomic births. Additionally, pregnancies involving macrosomic fetuses were characterized by prolonged gestation and an increased likelihood of cesarean delivery. Neonates with mac

Good accuracy of ultrasound estimations of fetal weight performed by midwives.

BACKGROUND: This study was undertaken to evaluate the quality of ultrasound estimation of fetal weight when performed by midwives experienced in ultrasound examinations. We also examined whether the accuracy was affected by fetal presentation, twin pregnancy or birth weight category. The results of 5 different formulas were compared to determine which was most accurate in our study population. METHODS: The study population consisted of 620 fetuses in 607 pregnancies, on whom fetal weight estimations had been performed within 3 days prior to delivery. The group of twins (n=27) was analysed sep

Maternal-fetal histocompatibility in intrauterine growth retarded and normal weight babies.

PROBLEM: To determine whether risk for intrauterine growth retardation (IUGR) is increased in HLA-DQA1 compatible pregnancies. METHOD: Paired maternal and cord blood samples were obtained from 30 IUGR and 31 non-IUGR pregnancies delivered at a university hospital. Samples were typed for eight HLA-DQA1 alleles using 10 sequence-specific oligonucleotides probes. Associations between IUGR and HLA-DQ compatibility status, and other risk factors were examined using logistic regression analysis. RESULTS: HLA-DQ compatibility and history of spontaneous abortion were not individually significant ris

Gestational duration and birth weight for twins related to fetal sex.

It has been postulated that male twin pregnancies, in contrast to male singleton pregnancies, differ in some distinctive biologic sense, leading to a shorter gestational duration and a lower individual birth weight than is the case in female twin pregnancies. To test this hypothesis in a relatively large dataset, information on gestational duration and birth weight for nearly all twins born in Sweden during a 4-year period (n = 3,472 twin pairs) was collected from the Medical Birth Registry, National Board of Health and Welfare. Included in the Medical Birth Registry are all pregnancies with a

Fetal cardiac responding: a correlate of birth weight and neonatal behavior.

Certain intrapartum fetal heart-rate (FHR) patterns are associated with low Apgar scores and other neonatal risks. Intrapartum FHR has been the subject of extensive clinical investigation, but only occasional reference is made to the possible behavioral correlates of this phenomenon. The current investigation found that neonates classified as FHR decelerators had lower birth weights and shorter gestational ages than FHR accelerators. Decelerators exhibited higher baseline heart rate and experienced more frequent uterine contractions than accelerators. Neurobehavioral assessment of these neonat

Effect of maternal dietary restriction during pregnancy on maternal weight gain and fetal birth weight in the rat.

The effects of the following dietary regimes on maternal body weight and weights of the products of conception were investigated in the laboratory rat: 50% dietary restriction during the catabolic phase (days 15--21) of gestation; 50% dietary restriction during the anabolic phase (days 1--14) but adequate nutrition during the catabolic phase, and 50% dietary restriction throughout gestation. Total body weight and net maternal weight at term were significantly reduced in all three test groups. Restricting dietary intake during the anabolic phase resulted in decreased maternal weight gain. The Rate of Intrapartum Cesarean Delivery Based on Estimated Fetal Weight in Comparison to Previous Child's Birth Weight.

OBJECTIVE: The aim of the study is to examine whether an estimated fetal weight of the current pregnancy greater than previous birth weight is associated with increased odds of intrapartum cesarean delivery. STUDY DESIGN: We conducted a retrospective cohort study of all women who had more than one singleton pregnancy at 23 weeks' gestation or greater at a single labor and delivery unit. We only analyzed the second pregnancy in the dataset. We excluded women who had preterm birth in the second pregnancy. Women were categorized according to the difference between estimated fetal weight and prev

Genetic effects on the timing of parturition and links to fetal birth weight.

The timing of parturition is crucial for neonatal survival and infant health. Yet, its genetic basis remains largely unresolved. We present a maternal genome-wide meta-analysis of gestational duration (n = 195,555), identifying 22 associated loci (24 independent variants) and an enrichment in genes differentially expressed during labor. A meta-analysis of preterm delivery (18,797 cases, 260,246 controls) revealed six associated loci and large genetic similarities with gestational duration. Analysis of the parental transmitted and nontransmitted alleles (n = 136,833) shows that 15 of the gestat

Accuracy and modifying factors for ultrasonographic determination of fetal weight at term.

OBJECTIVE: To determine the accuracy of various sonographic formulas for estimating fetal weight at labor. METHODS: We evaluated prospectively four published equations by Hadlock et al, Shepard et al, Rose and McCallum, and Sabbagha et al, as well as clinical examination for accuracy in determining fetal weight during labor. Two hundred twenty-three patients at 35-42 weeks' gestation underwent sonographic examination. Amniotic fluid index (AFI), placental location, maternal weight, and fetal station were recorded for each study. RESULTS: The Hadlock and Shepard equations both had a lower per

Fetal maturity and morbidity as related to placental weight and secondary ossification.

The problem of objectivity in histopathology is illustrated by an attempt to determine the normal range and significance of two observations, placental weight and secondary ossification, made in perinatal pathology. Neither offered a satisfactory estimation of fetal maturity. The placental weight provided little evidence of any primary placental dysfunction: the placenta was very small in association with only 50% of the very small infants and a very small placental fetal weight ratio was an infrequent finding of doubtful significance. A method of classifying relatively retarded and advanced s
