'Possible shunt malfunction' pathway for paediatric hydrocephalus-a study of clinical outcomes and cost implications.

BACKGROUND: Shunt insertion for hydrocephalus is a common paediatric neurosurgery procedure. Shunt complications are frequent with an estimated 20-40% failure rate within the first year, and 4.5% per year subsequently. We have an open-door 'possible shunt malfunction' pathway for children treated with a shunt or endoscopic third ventriculostomy, providing direct ward access to ensure rapid assessment and timely management of children. OBJECTIVE: To audit the 'possible shunt malfunction' pathway in terms of clinical outcomes (percentage-confirmed shunt dysfunction and number of re-attendances)

Pharmacological prophylaxis for venous thromboembolism in hospitalized surgical patients at a secondary teaching hospital: a best practice implementation project.

INTRODUCTION: Thromboprophylaxis reduces venous thromboembolism (VTE) in hospitalized patients. However, despite robust evidence, financial constraints, structural barriers-including limited access, socioeconomic inequalities-alongside patient complexity and diagnostic challenges, contribute to persistently high thromboembolism-related mortality. AIM: This project aimed to improve VTE prophylaxis in surgical patients through the implementation of evidence-based practices in a secondary teaching hospital in Brazil. METHODS: This evidence implementation project was guided by the JBI Evidence I

Compression Therapy Is Cost-Saving in the Prevention of Lower Limb Recurrent Cellulitis in Patients with Chronic Edema.

Background: Cellulitis is a common and often recurrent infection that causes substantial financial burden and morbidity. Compression therapy reduces the risk of recurrent cellulitis episodes for adults with chronic edema; however, little is known about the cost-effectiveness of the intervention. Methods and Results: A cost analysis was undertaken during a randomized controlled trial (RCT) involving 84 participants with lower limb chronic edema and a history of recurrent cellulitis. The intervention group received compression therapy and education, while the control group received education onl

Specialist practice for minor oral surgery: a comparative audit of third molar surgery.

Cost efficiency of removing third molars in a practice specialising in minor oral surgery was compared to that of a hospital oral surgery department. A total of 100 patients treated in each locality were prospectively audited during the financial year 1989-90. Surgical complexity, waiting time, complications and patient satisfaction were compared. The hospital cases were costed individually and compared to fees received for the patients treated in the practice. The practice fees were also compared to average expenses as assessed by the Dental Rates Study Group Inland Revenue Enquiry. Patients

Currently performed continence surgery: how useful are hospital episode statistics?

The aim of this study was to examine current trends in selecting continence operations for female patients in the UK. An online data search was carried out using the Hospital Episode Statistics website (www.hesonline.nhs.uk). Only operations performed on women were included. A request for more detailed information was submitted but no information was provided. Data were available for 4 years, from 2002-2003 until 2005-2006. It was difficult to establish trends regarding mid-urethral slings, including tension-free vaginal tape and transobturator tape, both inside out and outside in, as well as

Ponseti treatment in the management of clubfoot deformity - a continuing role for paediatric orthopaedic services in secondary care centres.

INTRODUCTION: The Ponseti technique is a well-proven way of managing paediatric clubfoot deformity. We describe a management set-up which spreads the care between secondary and tertiary care with no loss of quality. PATIENTS AND METHODS: In our audit of the first 2 years of Ponseti casting in the treatment of idiopathic congenital talipes equinovarus (CTEV, clubfoot) deformity, we identified 77 feet having been treated in 50 patients. Forty-nine feet were treated primarily in Oswestry, a tertiary referral centre for paediatric orthopaedic conditions, and 13 feet were treated in conjunction wi

The case for a national service for primary immune deficiency disorders in New Zealand.

Primary immune deficiency disorders (PIDs) are rare conditions for which effective treatment is available. It is critical these patients are identified at an early stage to prevent unnecessary morbidity and mortality. Treatment of these disorders is expensive and expert evaluation and ongoing management by a clinical immunologist is essential. Until recently there has been a major shortage of clinical immunologists in New Zealand. While the numbers of trained immunologists have increased in recent years, most are located in Auckland. The majority of symptomatic PID patients require life-long i

Measuring the success of transbronchial needle aspiration in everyday clinical practice.

BACKGROUND: Trans-bronchial needle aspiration (TBNA) is a simple, safe technique that can be performed without additional resources in any centre with a bronchoscopy service. It provides rapid diagnostic information in malignant and benign conditions and staging information in non-small cell lung cancer (NSCLC) and may avoid the delays, risks, in-patient stay and financial implications associated with surgical exploration of the mediastinum. Despite this, centres have been slow to adopt the technique. This may be due to the lack of clarity about the clinical success of TBNA in everyday clinica

Fiscal strain and access to opiate substitution therapy at Department of Veterans Affairs Medical Centers.

This study examines the relationship between institutional fiscal strain and the availability of opiate substitution therapy (eg, methadone maintenance), an effective but relatively expensive treatment for heroin addiction. An observational design was used to examine the association of changes in funding and changes in provision for treating opiate addiction at 29 VA Medical Centers (VAMCs). We hypothesized that VAMCs experiencing greater fiscal strain would show reduced availability of opiate substitution treatment. Administrative records from each of 29 VAMCs that provided opiate substitutio

Prolonged Postoperative Ileus Significantly Increases the Cost of Inpatient Stay for Patients Undergoing Elective Colorectal Surgery: Results of a Multivariate Analysis of Prospective Data at a Single Institution.

BACKGROUND: Prolonged postoperative ileus is a common major complication after abdominal surgery. Retrospective data suggest that ileus doubles the cost of inpatient stay. However, current economic impact data are based on retrospective studies that rely on clinical coding to diagnose ileus. OBJECTIVE: The aim of this study was to determine the economic burden of ileus for patients undergoing elective colorectal surgery. DESIGN: Economic data were audited from a prospective database of patients who underwent surgery at Auckland City Hospital between September 2012 and June 2014. SETTINGS: A

Finding diabetics--a method of screening in general practice.

BACKGROUND: It is well known that many diabetic patients go undiagnosed until complications have started to develop. Screening can be expensive in time and money, and ineffective, and is therefore unpopular with general practitioners (GPs). AIM: This study aimed to develop a screening method that was cost-effective and practical within the setting of an ordinary general practice. METHOD: Urine-testing sticks for glucose were sent with an explanatory letter to all (1736) non-diabetic subjects over 50 years old in a general practice. At the same time, diabetic recall and care were audited and

How does the COVID-19 affect earnings management: Empirical evidence from China.

Using China's A-share listed companies from 2018 to 2020, this paper examines the impact of COVID-19 on earnings management. The results reveal that: (1) The COVID-19 shock intensifies earnings management, which is reflected in the increasing accrual-based earnings management and real earnings management. Moreover, when enterprises face a higher degree of financial constraints, this shock effect is more evident. (2) Enterprises in industries and regions where COVID-19 is more severe are more affected by the suspension of work and production caused by the epidemic prevention policies, so these

POSTOPED: improving surveillance of postoperative delirium in a Scottish tertiary hospital.

Delirium is the most common postoperative complication among patients over the age of 65 years. It is associated with increased morbidity and is a significant financial cost to healthcare systems. We aimed to improve the detection of delirium on the surgical wards of a tertiary surgical centre. This would take the form of completion of 4AT assessments (the 4 AT test for delirium, on admission and 1 day postoperatively). Prior to this project, the 4AT was in use in the surgical admission clerking paperwork for over 65 s, however, 4AT assessments were not routinely performed as part of day 1 post

The role of surgical audit in improving patient management; nasal haemorrhage: an audit study.

BACKGROUND: Nasal bleeding remains one of the most common Head & Neck Surgical (Ear Nose and Throat [ENT]/Oral & Maxillofacial Surgery [OMFS]) emergencies resulting in hospital admission. In the majority of cases, no other intervention is required other than nasal packing, and it was felt many cases could ideally be managed at home, without further medical interference. A limited but national telephone survey of accident and emergency departments revealed that early discharge practice was identified in some rural areas and urban departments (where adverse socio-demographic factors resulted in

The use of a micropore particle technology in the treatment of acute wounds.

OBJECTIVE: A clinical audit was performed to evaluate whether Acapsil micropore particle technology (MPPT) powder could improve the management of acute wounds to heal by secondary intention. METHOD: Wounds, which could be characterised as sloughy, wet and probably infected, normally managed by debridement followed by negative pressure wound therapy (NPWT), were included in the evaluation. The MPPT powder was applied topically to the wound surface either once daily or on alternate days, with each wound receiving a total of two to five applications. Most patients had NPWT after the MPPT powder

Beyond unit-level certification: An operational agenda for biodiversity-positive tropical production landscapes.

Rising global demand for tropical agro-commodities such as palm oil, cocoa, timber, and soy, intensifies pressure on the forests that hold most terrestrial biodiversity. Unit-level sustainability certification has largely failed to halt biodiversity loss, constrained by three interacting barriers: translating species' persistence requirements into practice, an annual biodiversity financing shortfall near US$711 billion, and fragmented governance. In this perspective, I argue for a shift from certified management units to biodiversity-positive production landscapes, structured around three inte

Compression therapy for NHS inpatients with leg ulcers: a literature review.

OBJECTIVE: Leg ulcers are common, distressing and painful for patients, and are a significant financial burden to healthcare providers. Compression therapy is the mainstay of treatment for venous leg ulceration. Several studies have evaluated leg ulcer management and compression therapy in the community. However, little is known about the prevalence of leg ulceration and use of compression therapy in acute hospitals. The aim of this study was to explore the published literature on the use of compression therapy for inpatients with leg ulcers in UK National Health Service hospital settings. ME
