Ulnar Parametacarpal Flap for Small Finger Avulsion Injury.

In difficult cases of replantation following small finger avulsion injury, in which amputation occurs at the proximal interphalangeal joint, the ulnar parametacarpal island flap, rotated 180° (propeller flap), can be used as an alternative method for covering a skin defect of the proximal phalanx. This flap can prevent metacarpophalangeal joint dysfunction and additional finger shortening. We propose the use of an ulnar parametacarpal flap for this purpose and report the outcomes of two successful cases treated with this method and followed up for 12 months.

Mallet Finger in a Toddler: A Rare But Easily Missed Injury.

A mallet finger is a flexion deformity of a finger at the distal interphalangeal joint due to an injury of the extensor mechanism at the base of the distal phalanx. Most common in middle-aged men, injuries in the pediatric population are less common and rare in toddlers. We describe a case of missed mallet finger and its subsequent treatment in a female toddler.

[Transposition of the Ring Finger after Severe Hand Injury].

We report a severe hand injury with a fracture of the third metacarpal bone, destruction of the metacarpophalangeal joint of the fourth finger, amputation of the little finger of the right hand and several tendon injuries, in an active musician. The fourth metacarpal bone was offset close to the base, the hand narrowed, and the ring finger transferred to the base of the little finger. The outcome was very favourable.

Replantation of fingers and hands after avulsion and crush injuries.

INTRODUCTION: Avulsion and crush injuries constitute a particularly difficult problem due to extensive damage of vessels and nerves. In cases where a crush is the dominating injury factor causing complex fractures of forearm and carpal bones, shortening of the extremity is necessary for primary vessel and nerve reconstruction. Surgical experience in vessels dissection and optimal sequence of reconstruction procedures using vessels and nerve grafts are of paramount importance. MATERIAL AND METHODS: In the years 1986-2006 the author carried out 18 replantations and 4 revascularizations at vario

Investigation of an alleged mechanism of finger injury in an automobile crash.

This investigation centers on the case of an adult male whose finger was allegedly amputated by the steering wheel of his car during a crash. The subject claimed to have been driving with his left index finger inserted through a hole in the spoke of his steering wheel and was subsequently involved in an offset frontal collision with a tree. The finger was found to be cleanly severed at the mid-shaft of the proximal phalanx after the crash. This injury was alleged to have been caused by inertial loading from the rotation of the steering wheel during the crash. To determine whether this injury m

[A rare complication of conduction anesthesia in the treatment of finger injury].

At the present time novocain still remains the anesthetic of choice in ambulatory surgery and traumatology. In practical work the 2% and 0.5% solutions are used. The 2% solution of novocain can in some cases (in patients prone to angiodystonic reactions) cause a persistent spasm of digital arteries that results in irreversible alterations in finger tissues down to the development of gangrene. This fact was confirmed in experiments with 2% solutions of novocain. It is proposed to follow the recommendations on using the 0.5% and 1% solutions of novocain for regional anesthesia in fingers. Afferent and efferent nerve injury in vibration white fingers.

To study afferent pain and efferent sympathetic nerve function in vibration-induced Raynaud's phenomenon, 10 patients and 11 healthy controls were examined. Thresholds for mechanically and thermally induced pain were determined on the left side of the tip of the second finger, on the skin fold between thumb and second finger and on the ear lobe. Superficial skin blood flow was simultaneously measured by laser Doppler technique on the right third finger. The patients had higher pain thresholds than controls on the vibration-exposed index finger but not on other stimulation areas. Thus receptors

Point of care ultrasound in acute injuries of fingers and thumb.

INTRODUCTION: Acute closed trauma to the long fingers and thumb presents a therapeutic challenge. Ultrasound refines clinical examination and enables optimal management. This study aims to assess the role of ultrasound in the management of acute closed trauma of the fingers and the thumb during post-emergency consultations. METHOD: Patients were re-evaluated within 15 days of their initial emergency visit. The evaluation combined a clinical examination and a standardized ultrasound assessment. Based on ultrasound findings, initial management was either confirmed or modified. The primary endpo

New Insights into Boxer's Knuckle Injury of the Little Finger.

BACKGROUND: The original description of boxer's knuckle injury of the fifth ray mentions that the injury occurs between the extensor digitorum communis (EDC) and the extensor digiti minimi (EDM). Subsequent reports claim similar findings. Anatomical studies show that the EDC of the fifth ray is absent in most patients, while the EDM is generally composed of two slips. We present a modification of the current description of boxer's knuckle injury of the little finger based on the correlation between advanced preoperative 3D imaging and intraoperative findings. METHODS: Five patients were inves

Usefulness of ectopic implantation in multiple finger amputation injury.

Replantation combined with simultaneous ectopic implantation can be considered in patients with multiple finger amputation injury. This technique has the advantages of reducing the operating time and optimizing hemodynamic stability. However, it is only possible when multiple hand and microsurgery team can be organized at short notice.

Optic Nerve Avulsion After Finger-Poke Injury.

We present a case of optic nerve avulsion as a result of finger-poke injury to the eye. Spectral domain optical coherence tomography demonstrated a plunging cup indicative of the avulsion, a finding not previously described. Optic nerve avulsion is a form of anterior indirect traumatic optic neuropathy evoked by a sudden severe rotation at the junction of the optic nerve and globe induced, in this case, by penetration of the finger into the nasal orbit.

Current concepts in the evaluation and treatment of mallet finger injury.

The mallet finger is a frequently encountered fingertip injury that leads to extensor lag of the distal phalanx. Classification systems stratify these injuries as ranging from soft-tissue disruption of the extensor mechanism alone to those that have articular involvement and volar subluxation. The management of mallet finger injuries varies based on injury pattern and surgeon preference. These treatment options include splinting regimens, closed reduction and percutaneous pinning, and open reduction and internal fixation. Although the final goal of treatment is to establish a congruent joint,

Mechanisms of injury and prognosis in finger pulling--a retrospective study.

Eighteen patients were treated for finger pulling injuries between 1977 and 1986. There were two main types of injury, subcutaneous rupture of the flexor profundus tendon (n = 15) and fracture of the proximal phalanx of the finger (n = 3). Their mechanisms of injury were different. The prognosis in tendon ruptures is comparable to that in other open flexor tendon injuries. Age and distal localization were associated with a poorer prognosis in younger patients. Fractures healed well.

Retardation of finger growth after injury to the flexor tendons.

The absence of any part of the total mechanical force (i.e., flexor tendons) may be responsible for retardation of bone growth during a child's growth phase. The cause of retardation may be dedifferentiation of cartilage cells at the epiphyseal plate. Four cases in which unrepaired flexor tendons in digits caused retardation of growth illustrate this possible phenomenon. We feel that this is not a well recognized problem and thus deserves mention to stimulate further study. We believe that meticulous primary repair in children is desirable not only to preserve function, but also to prevent gro

Qualitative features of finger movement during the Halstead finger oscillation test following traumatic brain injury.

Qualitative and quantitative performance on the Halstead Finger Tapping test may help differentiate brain dysfunctional patients from normal controls. "Normal" and "abnormal" finger tapping patterns during this task have been characterized and illustrated pictorially. Data from 65 patients with traumatic brain injury (TBI) and 15 normal controls support the dual proposition that (1) abnormal finger tapping patterns are more commonly observed in TBI patients than in controls and (2) the frequency of abnormal finger movements may relate to the severity of TBI during the acute stages after trauma
