Race Impacts Outcomes of Patients With Firearm Injuries.

BACKGROUND: To study the relationship between race and outcomes of patients with firearm injuries hospitalized in the United States. METHODS: The 2016 National Inpatient Sample was used. Patients were included if they had a principal diagnosis of firearm injury. Exclusion criteria were age <16 years and elective admissions. The primary outcome was in-hospital mortality. Secondary outcomes were morbidity (traumatic shock, prolonged mechanical ventilation, acute respiratory distress syndrome [ADRS], and ventilator-associated pneumonia [VAP]), and resource utilization (length of stay and total h

Multiple variations of firearm injuries - a case report.

Variation from common findings in death due to firearm injury is not commonly encountered. When there is an atypical finding, the forensic pathologist may feel it difficult for differentiating firearm entry wound from firearm exit wound, tracing the wound track, estimating approximate range of firing and differentiating suicidal injury from homicidal injury. Failure to differentiate entry wound from exit wound can cause error in calculating number of projectiles entered into the body, lodged inside the body or exited through the body, besides posing problem in ascertaining direction of firing

Universal primary colonic repair in the firearm era.

All patients with colonic trauma treated at King Edward VIII Hospital, Durban, from August 1993 to May 1994 underwent primary repair of the colonic wound. They were evaluated prospectively to assess the mode of injury and outcome variables. Colonic injuries were sustained by 102 patients. These were inflicted by gunshots (62), stabs (22), shotguns (14), and blunt trauma (4). The transverse colon was injured most frequently (53). All shotgun injuries were multiple. Average time from admission to theatre was similar for shocked and non-shocked patients. Eighty-seven patients had simple closure (

Morbidity and injury recurrence in victims of firearm injuries.

Civilian violence has become an increasing problem in the industrial world. Gunshot wounds, fatal or non-fatal, are often considered as acute trauma episodes. However, our previous study, based on 820 firearm injuries, showed that this group of patients was characterized by a high mortality rate and a pronounced involvement in criminality when compared to a control group. The aim of this study was to determine the general morbidity in the same group of firearm victims. Our hypotheses were that these patients consume a considerable amount of hospital care due to recurring trauma episodes and th

Firearm-related injuries in Canada: issues for prevention.

We reviewed the available data on firearm-related injuries in Canada to suggest strategies for prevention in the context of the proposed amendments to the Criminal Code (Bill C-17) currently before Parliament. The risk of death from a firearm in Canada is equivalent to the risk of death from a motor vehicle crash. We discuss the risks associated with firearms with regard to suicides, homicides and "accidents." We also discuss the accessibility of firearms. This article builds upon a recently published update on the epidemiologic basis of the public health approach for the prevention of firearm

[Non-fatal firearm injuries of the head and neck].

There is depicted the evolutive course of 6 men with gunshot wounds of the neck and head, fortunately not threatening their lives. All victims were men between 18 and 67 years of age. Three times the gun fired a single projectile, whereas the other 3 were multiple projectiles. The latter cases resulted in heavier damages. Etiologically the events were classified as: 3 cases as suspected murder, 2 as attempted suicide and in the last case, an accident. The management has been both medical and surgical under general anesthesia, in order to repair the disorders and/or remove the bullets in 5 occa

Multishot firearm suicide. Examination of 58 cases.

Suicide by firearm is a frequent mode of death and the most common mode of suicide in the United States. So typically is there but one bullet entrance would in the suicide victim, and so often are there multiple wounds with homicide that some investigators and the public are unaware or forget that the person intending suicide may discharge his gun into himself more than once. The frequency, incidence, and other characteristics of the phenomenon deserve more recognition. The data presented are from medical examiner reports and related material from 7,895 gunshot deaths, including 3,522 suicides

Cardiorespiratory demands of firearms training instruction and 15m shuttle tests in British law enforcement.

OBJECTIVES: Law enforcement agencies require minimum fitness standards to safeguard their officers and training staff. Firearms instructors (FI) are expected to maintain the same standards as their operational counterparts. This study aimed to quantify the daily physiological demands placed on FI. METHODS: 19 FI (45 ± 5 years) completed occupational tasks whilst wearing heart rate (HR) monitors for a minimum 10 days. Maximal oxygen consumption (VO2max) testing was conducted on FI during a treadmill test (TT) and a multistage shuttle test (ST). Linear regression models were used to model the r

Familial firearm culture among the families of youth who died by firearm suicide in Maryland: a qualitative analysis of psychological autopsy interviews.

BACKGROUND: Suicide is a leading cause of death among adolescents in the USA, with firearm-related suicides comprising approximately half of these deaths. This study explores culturally rooted firearm perspectives among the families of youth who died by firearm suicide, including families' perspectives on suicide prevention interventions such as extreme risk protective orders (ERPOs). METHODS: We performed psychological autopsy interviews with the relatives of youth aged 15-21 who died from firearm-related suicide in Maryland between 2013 and 2021. Using an iterative, inductive, analytical ap

Lower extremity vascular injuries caused by firearms have a higher risk of amputation and death compared with non-firearm penetrating trauma.

OBJECTIVE: Firearm injuries have traditionally been associated with worse outcomes compared with other types of penetrating trauma. Lower extremity trauma with vascular injury is a common presentation at many centers. Our goal was to compare firearm and non-firearm lower extremity penetrating injuries requiring vascular repair. METHODS: We analyzed the National Inpatient Sample from 2010 to 2014 for all penetrating lower extremity injuries requiring vascular repair based on International Classification of Diseases, Ninth Revision codes. Our primary outcomes were in-hospital lower extremity am

Firearms, alcohol, suicide, and intimate partner violence perpetration among service members: Military partner experiences.

U.S. Armed Forces members experience specific demands and stressors that lead to several risk factors of self-harm or harm against others. Military spouses face unique challenges as partners of service members and are often placed in a position to intervene in potentially harmful situations. This study aimed to use qualitative methods to explore military spouses' exposure to their active-duty partner's risk of violence perpetration, firearm suicide, and associated circumstances such as alcohol use and access to personally owned firearms. Participants were adult spouses/partners of active-duty

Weaker gun state laws are associated with higher rates of suicide secondary to firearms.

BACKGROUND: Firearm-related suicides comprise over two-thirds of gun-related violence in the United States, and gun laws and policies remain under scrutiny, with many advocating for revision of the regulatory map for lawful gun ownership, aiming at restricting access and distribution of these weapons. However, the quantitative relationship between how strict gun laws are and the incidence of firearm violence with their associated mortality is largely unknown. We therefore, sought to explore the impact of firearm law patterns among states on the incidence and outcomes of firearm-related suicide

Histopathological detection of entry and exit holes in human skin wounds caused by firearms.

The judiciary needs forensic medicine to determine the difference between an entry hole and an exit hole in human skin caused by firearms for civilian use. This important information would be most useful if a practical and accurate method could be done with low-cost and minimal technological resources. Both macroscopic and microscopic analyses were performed on skin lesions caused by firearm projectiles, to establish histological features of 14 entry holes and 14 exit holes. Microscopically, in the abrasion area macroscopically observed, there were signs of burns (sub-epidermal cracks and kera

Lead contaminated moonshine: a report of Bureau of Alcohol, Tobacco and Firearms analyzed samples.

The CDC's Healthy People 2010 has set a US population lead level goal of < 25 microg/dL. A recent study of Emergency Department patients in Atlanta, GA, revealed a significant association between reported moonshine consumption and elevated blood lead. However, beyond anecdotal reports and isolated case histories, laboratory analyses confirming the presence and extent of lead contamination among moonshine samples are absent from modern scientific literature. One hundred and fifteen suspected moonshine samples seized by local law enforcement between 1995 and 2001 were voluntarily submitted to th

Firearms law raises issue of MDs' duty to predict and report potentially violent patients.

The possibility that an amendment to new firearms legislation would require physicians to report potentially violent patients raises the controversial issues of physicians' legal duty to warn and the ability of physicians and other health care professionals to predict violent behaviour. For these reasons, it will be necessary to follow carefully any proposed amendments to the legislation.
