Foot deformities in children with hereditary motor and sensory neuropathy.

The authors reviewed 104 feet from 52 consecutive children with hereditary motor and sensory neuropathy (HMSN) seen for the first time in clinics in two pediatric institutions between 1996 and 2003. Sixty-nine feet had a cavovarus deformity, 23 feet had a planovalgus deformity, and 12 feet had no significant deformity. All cases with deformity had bilateral involvement, and of those with deformity, only 45% had symmetric involvement. In HMSN I, III, IV, V, and X-linked HMSN, cavovarus was the most common deformity. However, in HMSN II, 55% of feet had a planovalgus deformity, 36% had a cavovar

Management of the neuromuscular foot deformities with the Ilizarov method.

Nine feet of eight patients with neurologic foot deformities caused from poliomyelitis, Charcot-Marie-Tooth disease, and traumatic nerve injury were treated with V-osteotomy and the Ilizarov method. A painless and plantigrade foot was obtained in all but one patient. A residual deformity occurred in this case because of the soft-tissue problems during the correction period. No major complication had been encountered by the latest follow-up at a mean of 42 months. The results of the current study indicate that the V-osteotomy and the Ilizarov method, while technically difficult, is an effective

Foot deformities in rheumatoid arthritis and relevance of disease severity.

OBJECTIVE: To investigate foot deformities in rheumatoid arthritis (RA) in relation to the disease severity. METHODS: Radiographs of 100 weight bearing feet of 50 patients who had had RA for >10 years (mean 13.5 years) were studied. The patients were classified into 2 study groups according to the severity of disease. We measured hallux valgus angle (HVA), intermetatarsal angle between first and 2nd (M1/2), and intermetatarsal angle between first and 5th (M1/5) on anteroposterior (AP) radiographs, as well as calcaneal pitch (CP) and first metatarsal pitch (MP) on lateral radiographs. The diff

Longitudinal epiphyseal bracket: associated foot deformities with implications for treatment.

The longitudinal epiphyseal bracket is a rare ossification defect of bones of the hands and feet and results in an abnormal epiphysis that is C-shaped, longitudinally oriented, and vertically extends into the diaphysis. The longitudinal epiphyseal bracket may exist as an isolated deformity, but is commonly associated with digital deformities such as polydactyly, duplicated digits, congenital hallux varus, and clinodactyly. Five case histories of children with the longitudinal epiphyseal bracket in different bones of the foot are described with three distinct clinical presentations. The develop

Free gracilis muscle transfer for coverage of severe foot deformities.

The purpose of this study was to describe the gracilis free-tissue transfer for soft-tissue coverage after surgical correction of severe clubfoot and other complex foot deformities in children older than 4 years. Requirements for success include complete correction of deformity, temporary Kirschner-wire fixation to maintain correction, experience in microvascular surgery, and close postoperative monitoring of the tissue transfer. Atrophy of the muscle transfer over the first year precludes the need for debulking procedures. This procedure was used in three children aged 4.5, 7, and 7.3 years w

[Treatment of foot deformities in diastrophic dysplasia--a case report].

We report the follow-up of a neonate with Pes equinovarus et adductus due to diastrophic dysplasia. The feet have been treated operatively by lengthening of the calcaneal tendon after initial redressement and casting. The normalized position is maintained by means of an removable plastic cast and later an instep raiser. Instructions for the maintenance are given.

[Correction of severe deformities of the foot, using Ilizarov's equipment].

MATERIAL AND METHODS: 38 feet in 36 patients who underwent a progressive foot correction without osteotomies, with an Ilizarov frame have been reviewed. There were 23 equinovarus, 2 equinovalgus, 13 equinus deformities. In 20 patients, the deformity had relapsed after one to four previous operations. The mean follow-up was 22 (12-35) months. The mean age was 17 years (6-68). There were 15 females and 21 males. RESULTS: All but 4 patients were satisfied with the result, although 15 still had pain during daily activities. The shape of the feet was distinctly improved in all patients, although r

The correction of complex foot deformities using Ilizarov's distraction osteotomies.

Twenty-five very complex foot deformities were treated by Ilizarov distraction osteotomies. The osteotomy types included supramalleolar, U, V, posterior calcaneal, talocalcaneal neck, midfoot, and metatarsal osteotomies. In addition, the leg was lengthened and widened in most cases. The mean treatment time was 6.4 months. There were 20 minor or major complications related to the foot osteotomies in 18 feet, including deep pin-tract infection in three, failure of osteotomy separation in nine, acute postoperative tarsal tunnel syndrome in two, toe contractures in three, wire breakage or cutout i

Rotational deformities of the tibia and foot in preterm infants.

Metatarsus adductus and medial tibial torsion are common in term infants. Our study shows that metatarsus adductus is rare, whereas medial tibial torsion does not occur in preterm infants with gestational ages of less than or equal to 30 weeks. These preterm infants have lateral rotation of the legs, external tibial rotation, and everted or normal feet at birth. This posture may explain the rarity of metatarsus adductus and medial tibial torsion in preterm infants.

The management of spastic deformities of the foot and ankle.

The most common ankle and foot deformities in cerebral palsy are equinus, equinovalgus, equinovarus, calcaneus and hallux valgus. It makes little difference how the length of the triceps surae is re-established and how its stretch reflex is weakened as long as the patient is carefully chosen, the procedure done well and the postoperative regimen prolonged and detailed. The most common cause of failure is inadequate use of night support during growth to prevent recurrence. Significant equinovalgus has been successfully managed by heel cord lengthening and subtalar arthrodesis. The arthrodesis s

[Treatment of a Progressive Collapsing Foot Deformity].

Treatment of a Progressive Collapsing Foot Deformity Abstract. The so-called "flat foot" can be more accurately described in German as "Knick-Senkfuss" or "kinking-flatfoot". The "kink" refers to the hindfoot axis as such, which can be identified by the intersection of the longitudinal axes of the Achilles tendon and the tuber calcanei. The designation "flat foot" marks the appearance of the longitudinal axis, that is, the medial longitudinal arch, which is easy to determine clinically. Nowadays, a new terminology has been added: Progressive collapsing foot deformity (PCFD). The forms of a PCF

Managing the Complex Cavus Foot Deformity.

This article describes approaches to and the management of complex cavus foot deformities. Correction of rigid multiplanar deformities can be very challenging, given the presence of skeletal deformities in multiple planes and combined with a varying degree of muscle imbalance. The complexity of these cases always requires a case-by-case approach. Some of the cases presented here occur in patients who have previously undergone surgical management for their deformity, several of which are complicated by additional deformities. With a firm understanding and application of the principles of deform

Reliability of pediobarographs for paediatric foot deformity.

PURPOSE: Dynamic pediobarograph measures foot-floor contact pressure during walking and provides a quantitative functional assessment. The goal of this study was to assess the reliability of pediobarograph measurements in normal children and in those with cerebral palsy (CP). MATERIALS AND METHODS: During the first investigation, five non-disabled children and four with CP had three pediobarograph measurements taken of each foot, repeated five times. The pediobarographs were analysed by dividing the foot into five segments; the heel, the lateral midfoot, the medial midfoot, the lateral forefo

[Linear scleroderma and foot deformity in children].

Linear scleroderma is a subtype of localized scleroderma generally observed in children, and may produce secondary bone and joint deformities. Its localization at the foot or ankle is rarely reported. A complete review of the literature reveals 21 cases of foot or ankle deformity due to linear scleroderma to which we add one case. The clinical and radiological aspects of the deformity, its pathogenesis, histological findings as well as orthopaedic management are presented and discussed.

Planovalgus foot deformity. Current status.

Flatfoot may be classified as pathologic or physiologic. Pathologic flatfoot is often characterized by stiffness of the foot, causes disability, and requires treatment. Physiologic flatfoot is a normal variation; it causes no disability and tends to improve with time. Physiologic flatfoot is most common in individuals who are loose-jointed, are obese, or usually wore shoes during childhood. Treatment of children with physiologic flatfoot with orthoses or shoe modifications not only is ineffective but is uncomfortable and embarrassing for the child and is associated with lowered self-esteem in
