Functional impact of custom-made foot orthoses in patients with haemophilic ankle arthropathy.

Although foot orthoses are often prescribed to patients with haemophilia (PWH) and ankle arthropathy, the efficacy and biomechanical effects of such devices are not fully understood. We experimentally investigated the effects of orthopedic insoles (OI) and shoes (OS) in PWH presenting ankle arthropathy, with specific attention being paid to pain, spatiotemporal parameters, kinematics and kinetics of lower limb joints, as well mechanical and energetic variables. Using three-dimensional gait analysis (3DGA), synchronous kinematics, kinetics, spatiotemporal, mechanics, and metabolic gait paramete

Ankle-foot orthoses: effect on energy expenditure of gait in spastic diplegic children.

This study examined the effect of conventional plastic bilateral ankle-foot orthoses (AFOs) on the energy expenditure of ambulation in spastic diplegic cerebral palsy patients. Eighteen subjects (aged 8.3 +/- 2.83 years) ambulated five minutes at freely chosen velocities with and without their AFOs. Heart rate (beats per minute) and distance (meters) ambulated were recorded for each minute. A physiologic cost index was calculated by determining the ratio of walking heart rate to velocity [(HRamb-HRrest) divided by meters per minute]. The mean physiologic cost indexes for the last three minutes

The effects of foot orthoses and sensory facilitation on lower limb electromyography: A scoping review.

Foot orthoses (FO) are used as a treatment for biomechanical abnormalities, overuse injuries, and neuropathologies, but study of their mechanism remains inconclusive. The neuromotor paradigm has proposed that FOs may manipulate sensory input from foot sole skin to reduce muscle activity for movement optimization. This review argues that a FO likely alters the incoming mechanical stimuli transmitted via cutaneous mechanoreceptors and nociceptors as the foot sole interfaces with the surface of the orthotic. Thus, all FOs with or without intentional sensory facilitation, likely changes sensory in

Effect of functional foot orthoses on first metatarsophalangeal joint dorsiflexion in stance and gait.

Reduction in first metatarsophalangeal joint maximum degree of dorsiflexion with dorsiflexion of the first ray has been proposed to be the predominant cause of hallux abducto valgus and hallux rigidus. We sought to determine whether orthoses made from a cast with the first ray plantarflexed and a 4-mm medial skive could increase the maximum degree of dorsiflexion in patients with functional hallux limitus in stance and gait. Forty-eight feet of 27 subjects were casted for orthoses with the first ray plantarflexed and in the customary neutral rearfoot position with locked midtarsal joint. First

The importance of foot posture when recording lower leg electromyography when walking in non-textured and textured foot orthoses.

Foot posture describes the anatomical variance in an individual's overall foot shape, an important consideration in the provision of foot orthoses. Current orthoses designs could be optimized by considering the topographical organization of cutaneous mechanoreceptors. Currently, the effect of foot orthoses designs to enhance skin stimulation across different anatomical foot posture remains unknown. Thus, the purpose of this study was to investigate how foot posture variance modulates lower leg muscle activity when walking in non-textured orthoses and in textured orthoses which facilitates cuta

Do Ankle-Foot Orthoses Reduce Cavovarus Deformity in Individuals With Charcot-Marie-Tooth Disease?

BACKGROUND: The use of ankle-foot orthoses (AFOs) is a mainstay in the treatment of cavovarus deformity associated with Charcot-Marie-Tooth (CMT) disease, but their ability to reduce cavovarus deformity is poorly characterized. Similarly, associations between AFO design features and degree of deformity reduction are unknown. Weightbearing CT has been used to describe CMT disease-related cavovarus deformity through both two-dimensional (2D) radiographic measures and three-dimensional statistical shape modeling (SSM). However, neither method has been used to evaluate differences in foot posture

Can Foot Orthoses Benefit Symptomatic Runners? Mechanistic and Clinical Insights Through a Scoping Review.

BACKGROUND: Running is a widely practiced sport worldwide associated with a host of benefits on cardiovascular, metabolic, musculoskeletal, and mental health, but often leads to musculoskeletal overuse injuries. The prescription of a foot orthosis (FO) is common to manage musculoskeletal impairments during physical activity or functional tasks. Although FOs are frequently prescribed by clinicians for symptomatic populations of runners, the existing literature supporting the prescription of FOs in runners has predominantly focused on either uninjured individuals or a mix of uninjured and sympto

Energy expenditure of paraplegic patients standing and walking with two knee-ankle-foot orthoses.

To determine which kind of knee-ankle-foot orthosis (KAFO) is more efficient, we measured energy expenditure in standing, walking with a walker, and walking with crutches by eight subjects using the Scott-Craig KAFO and a single-stopped long-leg KAFO. All subjects had complete motor paralysis below the level of their lesion and had been fully trained to use KAFOs. Every subject used both types of KAFO, and energy expenditure--per minute and per meter traveled--was measured by oxygen consumption. No significant difference in energy expenditure appeared during standing. During ambulation, howeve

Foot orthoses alter kinematics in runners with patellofemoral pain: a statistical parametric mapping analysis.

Clinical guidelines recommend foot orthoses for patients with patellofemoral pain (PFP) and excessive subtalar pronation. Although Statistical Parametric Mapping (SPM) is established for analysing continuous kinematic waveforms in biomechanics, foot orthoses research in PFP frequently relies on discrete variables (e.g. peak angles). Applying SPM allows detailed assessment of movement adaptations throughout the stance phase that isolated data points may overlook. This study evaluated the immediate effects of medially wedged orthoses on lower limb kinematics, pain, and self-perceived improvement

Ankle-foot orthoses improve walking but do not reduce dual-task costs after stroke.

BACKGROUND: Cognitive-motor interference, as measured by dual-task walking (performing a mental task while walking), affects many clinical populations. Ankle-foot orthoses (AFOs) are lower-leg splints prescribed to provide stability to the foot and ankle, as well as prevent foot drop, a gait deficit common after stroke. AFO use has been shown to improve gait parameters such as speed and step time, which are often negatively impacted by dual-task walking. OBJECTIVES: Our objective was to establish whether AFOs could protect against cognitive-motor interference, as measured by dual-task walking
