The incidence of neurologic injury in paediatric forearm fractures requiring manipulation.

This study sought to evaluate the incidence of neurologic injury in children referred for manipulation in our hospital. A retrospective chart analysis of the first 100 children to be referred with a fracture of any segment of the radius and/or ulna was performed. The incidence of neurologic injury was found to be 15.6%. Neurological injury occurs most frequently with distal physeal fractures with an incidence of 37%. Clinicians require a high index of suspicion when evaluating forearm fractures. If neurologic injury is missed at initial assessment, the child may not receive a prompt reduction,

Ulnar nerve injury after closed forearm fractures in children.

We treated two children with the unusual complication of ulnar nerve palsy after closed both-bone forearm fractures. Both patients developed an ulnar claw-hand deformity within 7 weeks of injury that resolved spontaneously by 20 weeks postinjury with nonoperative treatment. No patient showed any signs or symptoms of an ischemic compartment syndrome. Both nerve injuries were identified immediately at the time of fracture by a careful neurologic examination. This avoids confusion with a postreduction nerve entrapment injury or ischemic injury after a localized compartment syndrome, which may hav

Ulnar Nerve Injuries Post Closed Forearm Fractures in Paediatric Population: A Review of the Literature.

Ulnar nerve injury as a result of closed forearm fractures in children is a rare but disastrous complication, affecting significantly the function of the upper extremity. The management of these injuries is still controversial in the literature. This is a review of small case series and case reports, in which an algorithm of treatment is proposed. A brief description of a relative case, which was treated successfully in our department, is presented as well. This case motivated the authors to perform this study.

Functional reconstruction of bilateral injuries to the dorsum of hand using radial forearm tendocutaneous flaps.

We treated a patient with skin and tendon defects of both hands as a result of injury by a heat press. There have been no reports of bilateral hand injuries being treated using simultaneous bilateral tendocutaneous flaps. In this case, we reconstructed the injured tissue using simultaneous bilateral radial forearm tendocutaneous flaps, with satisfactory results.

The role of fabricated chimeric free flaps in reconstruction of devastating hand and forearm injuries.

Devastating hand and forearm injuries almost exclusively need free flap transfer if reconstruction is attempted. Early active and passive motion is only possible with aggressive, early, and comprehensive reconstruction. Despite recent advances in compound flaps, in selected cases it might be wise to harvest several smaller flaps and microsurgically combine them to one "chain-linked" flap "system." Four microsurgically fabricated chimeric free flaps were used in four patients for complex hand and forearm injuries. The combinations were sensate anterolateral thigh (ALT) flap plus sensate extende

Outcome of surgical treatment for forearm pronation deformities in children with obstetric brachial plexus injuries.

Seven children were operated on for pronation contractures of the forearm due to obstetric brachial plexus injuries. All underwent extensive preoperative evaluations to determine the extent of injury, secondary deformities, and capacity to perform a few basic tasks. Sequential video studies were used to document these findings. Operative procedures performed included various combinations of tendon/muscle lengthenings and/or transfers. Postoperative evaluations focused on function rather than gains in active range of motion and the patient/parental assessment of the benefit of the procedure by

Virtual goniometric measurement in patients with hand and forearm injuries: a double-blind psychometric study.

PURPOSE: Accurate assessment of joint range of motion (ROM) is essential for hand and wrist disorders, and virtual tools need validation as telehealth use rises. This study evaluated the validity and reliability of virtual goniometry versus manual goniometry for wrist and finger ROM in 42 adults with hand or wrist injuries. METHODS: In a double-blind, cross-sectional study, 42 patients (32.2 ± 14.2 years) were recruited. ROM of the wrist and fingers was measured using a standard manual goniometer and a web-based virtual goniometer (Protractor v3.1.2). Criterion validity was assessed with Spea

A Useful Technique for Templating the Reverse Radial Forearm Flap (RFF) for Thumb Degloving Injuries.

Degloving injuries of the thumb pose a significant challenge as replantation is usually unsuccessful. A 60-year-old man was transferred to "Queen Victoria Hospital NHS Trust" four hours after having his left thumb completely avulsed at metacarpophalangeal level. The anastomosis repeatedly failed during urgent replantation and a distally based pedicled RFF was utilized. To achieve accurate flap design, we used the amputated part as a template for the missing tissue. After a longitudinal cut exposing the inner surface, the thumb was placed flat on top of a clean gauze package which was used as a

Assessment of the Forearm Tendon Transfer with Irreparable Radial Nerve Injuries Caused by War Projectiles.

INTRODUCTION: Injuries to the radial nerve can occur at any point along its anatomical route, and the etiology quite varies. A particular entity are war injuries of the extremities, which have high morbidity but low mortality. After irreparable radial nerve injury, the only treatment is tendon transfer (if we neglect arthrodesis) with over then 40 methods. Four tendon transfers are considered as better than the other and two of them are the subject of our article flexor carpi radialis (FCR) and flexor carpi ulnaris (FCU). AIM: To evaluate the ultimate functional results of forearm tendon tran

Efficacy of Pull-In Suture for Tendon Avulsion Injury Associated With Severe Forearm Open Fracture, Forearm Amputation, and Hand Amputation.

BACKGROUND: The pull-in suture technique is a method of primary tendon repair that involves pulling the distal stump of the ruptured tendon into the proximal muscle belly and suturing it to the intact muscle fascia for avulsion injuries at the musculotendinous junctions associated with severe open forearm trauma. The aim of this study is to report the clinical outcomes of the pull-in suture technique for musculotendinous avulsion injuries associated with severe open forearm trauma and to evaluate its effectiveness. METHODS: This study included patients who underwent pull-in suture repair for

Brachialis muscle transfer for reconstructing digital flexion after brachial plexus injury or forearm injury.

UNLABELLED: Restoration of digital flexion after brachial plexus injury or forearm injury has been a great challenge for hand surgeons. Nerve transfer and forearm donor muscle transfer surgeries are not always feasible. The present study aimed at evaluating the effectiveness of restoring digital flexion by brachialis muscle transfer. Ten lower brachial plexus- or forearm-injured patients were enrolled. After at least 12 months following surgery, the middle-finger-to-palm distance was less than 2.5 cm in six patients. In the other four patients with less satisfactory results, secondary tenolysi

A new fracture model for "terrible triad" injuries of the elbow: influence of forearm rotation on injury patterns.

OBJECTIVE: The purpose of this study was to evaluate the influence of forearm rotation on failure patterns of the elbow under axial loads. METHODS: Fourteen upper extremities were resected mid-humerus and mounted on a custom apparatus, which allowed rotation of the ulna, radius, and humerus about a fixed wrist while loading in axial compression. Seven specimens were loaded to failure with the forearm in pronation and 7 in supination. RESULTS: Six of the 7 elbows axially loaded in pronation resulted in fractures of the radial head and coronoid with posterior dislocation (terrible triad). Six

Hold that K Wire! Fixing Nondisplaced Distal Forearm Fractures in Pediatric Floating Elbow Injuries is Unnecessary.

INTRODUCTION: Floating elbow injuries are uncommon, combined injuries in children involving a supracondylar humerus fracture and a distal fracture of the forearm. Orthopaedic dogma has suggested that a more aggressive approach to the distal fracture is necessary due to the potential risk of acute compartment syndrome. Our group recently showed that the rate of compartment syndrome is less than one percent. As such, we hypothesized that concomitant nondisplaced distal forearm fractures managed with operative fixation would result in similar outcomes compared to nonsurgical treatment. METHODS:

Predictive value of disability scoring systems for return-to-work outcomes in hand and forearm injuries.

BACKGROUND: Predicting return-to-work (RTW) outcomes following upper extremity trauma is crucial for optimizing patient care, guiding rehabilitation, and reducing the socioeconomic burden. Although several disability scoring systems have been proposed to assess injury severity, their ability to estimate RTW status and duration remains underexplored. This study aimed to evaluate the predictive value of the Modified Hand Injury Severity Score (MHISS), Upper Extremity Disability Rate (UEDR), and Total Body Disability Rate (TBDR) for RTW outcomes in patients with hand, wrist, and forearm injuries.

Combined Radiocarpal Dislocation and Forearm Joint Injuries: A Proposed Modification of the Locker-Based Classification System.

The comprehensive locker-based classification system brought revolutionary insight into the treatment of often misdiagnosed forearm joint injuries. The authors of the classification scheme subsequently described a combination of simple elbow dislocations and forearm joint injuries (two- and three-locker injuries), but to date, no review of the literature on combined radiocarpal dislocation and forearm joint injuries has been undertaken. The combination of radiocarpal dislocation and forearm joint injury is a rare traumatic pattern, usually related to high-energy trauma. The aim of this study w

Transposed replantation of fingers at forearm bones in severe segmental injuries across the hand and wrist.

There are situations in which amputated hands or fingers cannot be replanted directly back to their original positions. When there is severe segmental injury across the hand and wrist but one or several fingers are still healthy, the fingers can be selected to be replanted at the forearm bones to restore pinch function. This is different from the toe to antebrachial stump transplantation presented by Dr. Vilkki. From the results in four patients, the following conclusions are drawn: (1) When the fingers are replanted at the forearm bone(s), only pinch function can be obtained. There is no oppo
