Use of a reverse-flow plantar marginal septum cutaneous island flap for repair of a forefoot defect.

Soft-tissue reconstruction of the foot frequently requires flap coverage to preserve exposed tissues such as bones, joints, tendons, and weight bearing areas. Recently, reconstruction of skin defects in the heel has been greatly improved by use of the medial plantar sensory flap. However, forefoot coverage remains a challenge, since the alternatives for flap coverage are very limited. A case report is presented of a 52-year-old man with a chronic forefoot ulcer, successfully covered with a plantar marginal septum cutaneous island flap. This recently reported technique, which utilizes a reverse

Lesser metatarsal osteotomies. A biomechanical approach to metatarsalgia.

As with most other orthopedic conditions, a firm understanding of the normal and pathologic biomechanics of the lesser metatarsals is essential when contemplating treatment of metatarsalgia. Despite its prevalence, metatarsalgia remains a technically demanding surgical challenge. Some of the difficulty with treatment of this problem arises because of some of the controversies discussed previously. Many lesser metatarsal osteotomies have been described, and their success depends on many factors. Alleviating the correct amount of pressure underneath the metatarsal head without adversely affectin

Surgery of the rheumatoid forefoot with special reference to the plantar approach.

The key to the understanding of rheumatoid forefoot deformities is the predominant joint involvement of the tibial side of the hindfoot, of the fibular tarsometatarsal joints, and the destruction especially of the metatarsal heads. Independent of these patterns, the predominant sources of pain and impaired function, and thus of surgical remedies, remains the metatarsophalangeal joints. The authors prefer forefoot arthroplasty consisting of a judicious resection of the metatarsal heads, plantar capsulorraphy, tenolysis and rerouting of the tendons, through a dorsomedial approach to the great to

A case of pseudo-Kaposi sarcoma with chronic limb-threatening ischemia.

BACKGROUND: Pseudo-Kaposi sarcoma (PKS) is a rare vascular proliferative disease, caused by arteriovenous malformation (AVM) and chronic venous insufficiency. The lesions are characterized by purple or reddish-brownish papules, plaques, and nodules. Although benign, it is clinically similar to Kaposi's sarcoma (KS), a malignant disease, and must be differentiated by histopathological examination. We report a rare case of PKS with chronic limb-threatening ischemia (CLTI). CASE PRESENTATION: An 83-year-old man with diabetes mellitus (DM) presented to a local dermatology department with a compla

Foot orthoses for forefoot pressure reduction and the hypothesized role in calf muscle stretching: A systematic review highlighting an evidence gap.

BACKGROUND: Excessive forefoot plantar pressures are associated with pain, ulceration risk, and functional limitations in both healthy individuals and those with pathologies such as rheumatoid arthritis (RA). Limited ankle dorsiflexion, often related to gastrocnemius-soleus tightness, may contribute to forefoot overload. Although foot orthoses (FOs) are widely used to redistribute plantar loads, it remains unclear whether the existing evidence includes orthotic interventions designed to promote posterior chain flexibility. OBJECTIVE: To systematically evaluate the effectiveness of foot orthos

Dynamic correction for forefoot varus in stage II-A adult flatfoot: Technique tip.

Posterior tibial tendon dysfunction (PTTD) is a progressive disorder and a common cause of adult acquired flatfoot deformity, and forefoot varus is a frequent component in advanced cases. The author proposes peroneus brevis-to-longus transfer as an additional step to correct the forefoot varus component of stage II-A posterior tibial tendon dysfunction. We have performed this dynamic correction of forefoot varus in 12 patients at our institution, and observed promising clinical and radiographic improvement. It is a soft tissue procedure that avoids additional incisions and represents a favorab

Forefoot tendon transfers.

Flexible forefoot deformities, such as hallux varus, clawed hallux, hammer toes, and angular lesser toe deformities, can be treated effectively with tendon transfers. Based on the presentation of the flexible forefoot deformities, tendon transfers can be used as the primary treatment or as adjuncts to bony procedures when there are components of fixed deformities.

Forefoot problems in athletes.

Athletes who participate in high-impact sports involving running, jumping, or contact are at risk for forefoot injury. These injuries occur as a result of acute trauma or chronic overuse. Some athletes may be predisposed to injury because of preexisting foot deformity, such as cavus, hallux valgus, or Achilles contracture. This article reviews the common causes of forefoot pain in the athlete. The most common causes of forefoot pain in the athlete are metatarsal stress fracture, interdigital neuroma, sesamoid pathology, metatarsalgia, hallux rigidus, hallux valgus, and turf toe. The pathophysi

Percutaneous Forefoot Decompression in a Foot Compartment Syndrome Model.

BACKGROUND: Acute compartment syndrome of the foot is a controversial topic. Release of the foot has been seen as complicated because of large incisions and postoperative morbidity, and there has been debate over whether this procedure is actually effective for releasing all areas of increased pressure. New sensor technology affords the opportunity to advance our understanding of acute compartment syndrome of the foot and its treatment. The purpose of the present study was to determine whether percutaneous decompression could be performed for the treatment of compartment syndrome in a forefoot

Forefoot morphology change following subtalar arthroereisis for symptomatic flexible flatfoot in children.

BACKGROUND: When symptomatic pediatric flexible flatfoot treated with subtalar arthroereisis to improve hindfoot alignment, the postoperative forefoot supinatus can be a concern. METHODS: We retrospectively reviewed 110 patients (220 feet), ages 5-12, who underwent subtalar arthroereisis from January 2017 to December 2019, with at least two years of follow-up. Of these, 22 patients (44 feet) developed forefoot supinatus with metatarsus primus elevatus (MPE). Radiographic evaluations assessed postoperative forefoot supinatus, calcaneus and talus height. Functional outcomes were measured using

Conservative surgery for forefoot osteomyelitis may increase reulceration-free survival compared to minor amputation in diabetes-related foot disease.

This study suggests that conservative surgery may improve reulceration-free survival in patients with diabetes-related forefoot osteomyelitis, compared to minor amputation. By excluding patients with prior surgeries, the analysis more accurately reflects the benefits of conservative surgery. These findings highlight the importance of preserving foot structure to maintain biomechanics and reduce reulceration risk.

Complex Forefoot Reconstruction with Percutaneous Techniques.

Complex forefoot deformities are challenging to treat and are labor intensive for the surgeon and the patient. New minimally invasive surgery (MIS) shows great potential and in some instances outperforms traditional open surgery. Another advantage MIS has is that it is technically easier to perform (once proficient) and produces less pain and recovery for the patient. This article takes the reader through MIS preoperative planning, soft tissue considerations, the different osteotomy configuration options, rationale, transverse plane correction, sagittal plane correction, revision MIS surgery o

Treatment Options for Turf Toe: A Systematic Review.

Turf toe is hyperextension injury of the plantar plate at the first metatarsophalangeal joint. Etiologies have often included sports/activities with excessive forefoot axial loading and/or violent pivotal movements. The purpose of the systematic review was to systematically review and present an overview for the current evidence-based treatment options of turf toe. Both authors systematically reviewed the PubMed and EMBASE databases from inception to April 2016 based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. The level of evidence and quality of evide

Correction of a Forefoot Deformity Caused by a Large, Solitary Metatarsal Osteochondroma in an Adolescent: A Case Report.

Solitary osteochondroma is the most common bone tumor, accounting for 20% to 50% of all benign bony tumors. Osteochondromas are usually found on the metaphysis of the long bones near the physis; the bones of the foot are less commonly involved. We describe a 13-year-old female with a large osteochondroma arising from the fourth metatarsal. Pressure from the tumor on the adjacent metatarsals had deformed her forefoot, creating cosmetic and functional problems. The second and third toes were deviated laterally at the metatarsophalangeal joint, and the fourth and fifth toes were deviated medially

Case series of volar juvenile xanthogranuloma: clinical observation of a peripheral rim of hyperkeratosis.

Juvenile xanthogranuloma is a benign histiocytic tumor predominantly occurring in children as yellowish papules on the head and trunk. Presentations on the volar surfaces are rare and may cause diagnostic confusion with pyogenic granuloma, eccrine poroma and digital fibrokeratoma. We report two patients with unusual presentations of solitary juvenile xanthogranuloma on the palm or sole. Both had lesions lacking the classic yellowish color and demonstrating a well-defined, peripheral hyperkeratotic rim. Histopathological evaluation revealed prominent orthokeratosis corresponding to the rim. Add

Reconstruction of a child's forefoot defect using a distally based pedicled medial plantar flap.

This report describes a case of a 10-year-old boy who received a distally based, pedicled medial plantar artery flap to cover a defect on the distal lateral side of his right foot. The defect resulted from amniotic constriction. The flap served as defect coverage and was kept viable solely by the distal medial plantar vessels. Use of this particular kind of flap proves advantageous in that it provides good protection in the weight-bearing area of the foot, while causing only a minor donor site defect.

Hallucal sesamoid osteonecrosis: an overlooked cause of forefoot pain.

Four cases of osteonecrosis of hallucal sesamoids are reported here. Surgical excision of necrotic sesamoid tissue yielded satisfactory results, with the patients reporting no residual pain. Although it has not been frequently addressed in the literature, avascular necrosis of the sesamoid bones should be considered in the differential diagnosis of persistent forefoot pain.

Plantar pressure and radiographic changes to the forefoot after the Austin bunionectomy.

We describe the effects of the Austin bunionectomy on plantar pressure distribution and radiographic measurements in the forefoot in 31 subjects (44 feet) with mild-to-moderate hallux valgus deformity and 36 control subjects (36 feet). Plantar pressure measurements before and 24 months after surgery showed peak pressure beneath the hallux reduced to normal values. Peak pressure measurements beneath the first, second, and third metatarsal heads in hallux valgus feet were relatively unchanged after surgery and remained higher than normal values. The operation produced significant decreases in me

The effects of cycling shoe stiffness on forefoot pressure.

Plantar pressure data were recorded in two different shoe types to determine the effect of cycling shoe stiffness on peak plantar forefoot pressure in cyclists. Two pairs of shoes of the same size and manufacturer, identical except for outsole material and stiffness, were tested. Shoe stiffness measurements were collected under controlled conditions and in two different configurations using a dynamic hydraulic tensile testing machine. Measurements of plantar pressure were done using Pedar capacitive-based sensor insoles while subjects pedaled in a seated position at a controlled power output.
