Techniques and criteria in pathologic and forensic-medical diagnostics in sudden unexpected infant and perinatal death.

For each case of sudden infant and perinatal death, a full review of clinical and epidemiologic data and a complete necropsy study were performed according to the necropsy protocol devised by the Institute of Pathology, University of Milan, Milan, Italy (available at: http://users.unimi.it/~pathol/sids_e.html). Histopathologic examination of unexpected late fetal and neonatal death and SIDS cases allowed us to identify frequent alterations, mainly congenital, of the autonomic nervous system, modulating respiratory, cardiovascular, arousal, and upper digestive activities. The data and arguments

[Sudden, unexplained infant death--sudden infant death syndrome. Forensic pathological aspects].

Delimitation of the sudden unexplained infant death syndrome (SIDS) is difficult as the diagnosis is made by exclusion. The difficulties in the differential diagnosis are concentrated on interpretation of the significance of positive viral and bacterial findings, inflammatory changes in the respiratory organs, heart and central nervous system together with malformations. Classification of SIDS appears, therefore, to vary according to time and place. New techniques, e.g. DNA analysis, have explained the etiology in a few per cent of the cases but have not yet solved the riddle of SIDS. The arti

Microscopic cardiac pathology in forensic autopsies: a comparative study of anabolic androgenic steroid users and non-users.

PURPOSE: Anabolic androgenic steroid (AAS) abuse is a known risk factor for left ventricular (LV) hypertrophy and cardiac fibrosis. However, these changes are not exclusive to AAS abuse, and similar pathology is often observed in other types of cardiomyopathies. In this study, postmortem cardiac tissue specimens of AAS screened cases were re-examined in detail to determine, whether specific histopathological features could further support the detection of AAS abuse in forensic cause-of-death investigations. METHODS: The sample comprised of 46 Finnish forensic autopsy cases, including 16 AAS p

Spina bifida, the normal, the pathological and the in-between: first evidence from a forensic osteological collection.

Spina bifida (SB), a rare congenital disorder, is often mentioned as an individualizing factor in Forensic Anthropology. A lack of empirical data regarding SB is noticed in the scientific literature. Moreover, within the scope of anthropological research on SB disparities in terminology, classification systems, and methodological approaches result in incomparable results. The wide range (1,2%-50%) of "spina bifida occulta" reported prevalences is a good example. This research aims to analyze and debate the standard diagnostic criteria of SB on human skeletal remains, and attempts to elaborate

Traumatic extra-axial hemorrhage: correlation of postmortem MSCT, MRI, and forensic-pathological findings.

PURPOSE: To evaluate the diagnostic accuracy of in situ postmortem multislice computed tomography (MSCT) and magnetic resonance imaging (MRI) in the detection of primary traumatic extra-axial hemorrhage. MATERIALS AND METHODS: Thirty forensic neurotrauma cases and 10 nontraumatic controls who underwent both in situ postmortem cranial MSCT and MR imaging before autopsy were retrospectively reviewed. Both imaging modalities were analyzed in view of their accuracy, sensitivity, and specificity concerning the detection of extra-axial hemorrhage. Statistical significance was calculated using the M

Pathological gambling and criminal culpability: An analysis of forensic evaluations presented to German penal courts.

In Germany penal courts and expert witnesses are being confronted with an increasing number of offenders claiming that their criminal behavior had been caused by an irresistible urge to gamble. In 48 of 56 evaluated cases the defendants were diagnosed as pathological gamblers. However, diminished culpability (§ 21 German Criminal Code) was granted in only 32 cases. In these cases the illegal activity to get money could be classified as primarily gambling related.

Molecular pathology of pulmonary edema in forensic autopsy cases with special regard to fatal hyperthermia and hypothermia.

Fatalities due to an extreme ambient temperature might present with poor or nonspecific pathologies; thus, the diagnosis of the cause of death in such cases is one of the most difficult tasks in forensic pathology. The present study investigated the molecular pathology of alveolar damage involving pulmonary edema with special regard to hyperthermia (heatstroke) and hypothermia (cold exposure) in forensic autopsy cases (total, n=122; within 48 h postmortem). Intrapulmonary mRNA and immunohistochemical expressions of matrix metalloproteinases (MMPs), intercellular adhesion molecule-1 (ICAM-1), c

Ear-ly Warnings: The diagnostic accuracy of Frank's sign in fatal coronary artery disease - A forensic pathology study.

BACKGROUND: Globally, Coronary Artery Disease (CAD) is the leading cause of death. The mortality burden is rising in South Africa, imposing significant socioeconomic strain. In 1973, it was postulated that Frank's sign could serve as a cutaneous marker for CAD, enabling early, cost-effective, and non-invasive diagnosis with the timeous implementation of secondary preventative measures. However, its reliability as a diagnostic marker remains uncertain, and clinicians have not incorporated it into routine examinations. Given this uncertainty in the clinical context, post-mortem findings can be c

Co-occurrence of rabies and organophosphorus insecticide poisoning: A rare case of a diagnostic dilemma in forensic pathology.

Rabies is a vaccine-preventable zoonotic viral infection, but organophosphorus insecticide poisoning continues to be a toxicological emergency in India. An unusual co-occurrence is described in a 31-year old man who, following a puppy bite, ingested insecticide due to fear of rabies. Autopsy confirmed both poisoning and rabies presenting a rare diagnostic dilemma in forensic pathology.

Four cases of sodium nitrite suicidal ingestion: A new trend and a relevant Forensic Pathology and Toxicology challenge.

Sodium nitrite (NaNO2) is an inorganic compound commonly used as a food additive, antifreeze admixture, and fertilizer. Its toxicity mechanism is mainly represented by the oxidation of ferrous iron to ferric iron of one of the four heme structures in haemoglobin with the onset of methaemoglobin. The mechanism of death by sodium nitrite toxicity is severe hypoxia. We present four cases of suicidal sodium nitrite ingestion that closely occurred within a two months-period. Self-poisoning with sodium nitrite actually represents an increasing trend in nitrates' related deaths. In order to reach a p

Late-Onset Vitamin K Deficiency Bleeding in Four Infants: A Forensic Pathology Case Series With Clinical-Autopsy Correlation.

Late-onset vitamin K deficiency bleeding (VKDB) remains a preventable cause of catastrophic intracranial hemorrhage in early infancy, yet continues to present complex medicolegal challenges where parental refusal of prophylactic vitamin K persists. We present four infant deaths evaluated within the Turkish medico-legal system, each lacking prophylactic vitamin K at birth, in which autopsy played a critical role in excluding alternative causes and supporting findings most consistent with VKDB. Comprehensive review of clinical records and full forensic autopsies, including neuropathologic examin

Evaluating the Reliability of Dipstick Drug Screens on Vitreous and Postmortem Blood as a Triage Modality in Forensic Pathology.

Dipstick drug screens are cheap, easy to use, and quick presumptive tests to detect common drugs of abuse. Dipsticks are designed for drug detection in urine. There is no literature regarding their potential use on fluids different from urine. The study aimed to determine the performance of dipstick screening tests on postmortem vitreous and blood specimens compared to urine dipsticks and final confirmatory toxicology analyses on blood. The study population included cases in which a complete toxicology analysis was performed. Each subject was screened for three substances: cocaine, fentanyl, a

Forensic pathological study of methadone-related deaths in the Genoa (Italy) district: A six-year study.

Methadone is a synthetic opioid, a pure agonist of the μ receptor. It is used for opioid maintenance therapy in heroin addiction. In recent years, Italian studies of incidence and prevalence have indicated an increase in the illegal sales of methadone and, consequently, an increase in deaths due to acute methadone intoxication as well. The present review is a prospective-observational study regarding epidemiological and toxicological analyses of methadone-related deaths recorded in the district of Genoa (Italy) from 2013 to 2018. The study includes a list of twenty-six people that have died fr

Forensic pathological study of complex and complicated suicides: A twelve-year case series in Genoa (Italy).

Generally, the term complex suicide refers to a form of suicide in which more than one traumatic method is applied, simultaneously or consecutively, through multiple self-injurious actions using different methods and involving various bodily sites. A distinctive feature of complicated suicides is the failure of the initial attempt of the self-suppressive method that, nonetheless, results in death due to a subsequent accidental occurrence. In such cases, forensic pathologists and coroners are faced with the challenging differential diagnosis that generally includes homicide as well as suicide.
