Semi-Synthetic Cannabinoids in Forensic Toxicology and Public Health: Analytical Challenges, Emerging Detection Strategies, and Regulatory Implications.

Semi-synthetic cannabinoids (SSCs) are chemically modified derivatives of naturally occurring phytocannabinoids that have rapidly emerged in commercial cannabis and hemp-derived products, including vape cartridges, edibles, infused oils, and concentrated extracts. Increasing availability of compounds such as hexahydrocannabinol (HHC), HHC analogues, and Δ8-tetrahydrocannabinol (Δ8-THC) has created significant challenges for forensic toxicology, analytical detection, public health surveillance, and regulatory control. This structured narrative review evaluated current evidence on the forensic,

Forensic toxicology perspectives and trends on abused drugs in Adana, Turkiye: A five-year overview (2019-2023).

Monitoring toxicological findings in forensic cases offers valuable data on substance use patterns. This study aims to characterise long-term trends in illicit drugs and new psychoactive substances (NPS) in a strategically important transit region of Turkiye and to evaluate their implications for forensic intelligence and public health surveillance. In this retrospective study, we analyzed toxicological results from than 100,000 forensic cases examined between 2019 and 2023 at the Council of Forensic Medicine, Adana. Biological samples such as blood, urine, and hair etc. collected from both an

Prevalence of N,N-Dimethylpentylone in forensic toxicology casework: A retrospective study and trends in the state of Florida.

Substituted cathinones (SCs) have been more prominent in the state of Florida than in many other jurisdictions across the United States. Since mid-2021, N,N-Dimethylpentylone (DMP) has been the most encountered cathinone in forensic casework, though its prevalence has varied across the state. Given this prevalence and variability, developing a more comprehensive understanding of DMP became increasingly important. The aim of this study was to assess the prevalence of DMP in Florida from late 2021 through 2024 and to characterize its pharmacologic and toxicologic effects observed in authentic ca

Forensic Application of Genetic and Toxicological Analyses for the Identification and Characterization of the Opium Poppy (Papaver somniferum L.).

BACKGROUND: A reliable and science-based taxonomic determination of Papaver somniferum L. (opium poppy), the illegal species of the genus Papaver, may have practical and legal implications for law enforcement. P. somniferum is a controlled plant because of its narcotic substances, such as morphine and codeine. As poppy plants have rather similar morphological features, both chemical and genetic analysis are required in order to achieve an accurate characterization of such species. The chemical structures of alkaloids are extremely variable even within the same species, which is why the genetic

When herbal supplements cause heart problems: Clinical case report and forensic toxicological analysis.

The global expansion of travel and online supplement markets has eliminated traditional geographic barriers to toxic plant exposures. We report a case of a 42-year-old female presenting with digoxin-like intoxication symptoms, including nausea, vomiting, and hypotension. Electrocardiography revealed sinus bradycardia (51/min), first-degree AV block, and downsloping ST-segment depressions with reverse tick morphology. Upon further comprehensive anamnesis, the patient revealed the consumption of a weight-loss supplement labeled as "Tejocote root" (Crataegus mexicana), purchased during a travel t

Toxicological interpretation of N-ethylpentedrone and its metabolites in 12 fatal and non-fatal forensic cases.

N-Ethylpentedrone (NEP) is a synthetic cathinone that remained legally available in the Netherlands until its inclusion under the Opium Act as of 1 July 2025. Although NEP emerged in the mid-2000s, toxicological data, especially regarding its metabolism and concentration in real-world cases, are scarce. The aim of this study is to demonstrate analytical methods for the detection and quantification of NEP and its metabolites in antemortem and postmortem biological matrices and to provide reference values and guidance for interpretation. Cases where NEP was detected between May 2024 and Septembe

Simultaneous multi-targeted forensic toxicological screening in biological matrices by MRM-IDA-EPI mode.

The toxicologist ascertains drug assumptions in case of paediatric intoxications and death for overdose. The analytical approach consists of initially screening and consequently confirming drug positivity. We developed a toxicological screening method and validated its use comparing the results with a LC-MS/MS analysis. The method identifies 751 drugs and metabolites (704 in positive and 47 in negative mode). Chromatographic separation was achieved eluting mobile phase A (10 mM ammonium formate) and B (0.05% formic acid in methanol) in gradient on Kinetex Phenyl-Hexyl (50 × 4.6 mm, 2.6 μm) wit

Forensic Toxicological and Medico-Legal Evaluation in a Case of Incongruous Drug Administration in Terminal Cancer Patients.

BACKGROUND: In most cases, palliative care is prescribed to adults diagnosed with cancer. The definition of the most suitable therapy for an effective sedation in terminal cancer patients still represents one of the most challenging goals in medical practice. Due to their poor health, the correct dosing of drugs used for deep palliative sedation in terminal cancer patients, often already on polypharmacological therapy, can be extremely complicated, also considering possible drug-to-drug interactions that could lead to an increased risk of overdose and/or incongruous administration with fatal o

Pre-offense alcohol intake in homicide offenders and victims: A forensic-toxicological case-control study.

BACKGROUND AND AIM: Alcohol is associated with violent behavior, although little is known regarding to what extent alcohol increases homicide risk. We aimed to estimate risks of homicide offending and victimization conferred by the presence of ethanol in blood by using toxicological data from homicide victims and offenders and from controls who had died in vehicle-related accidents. METHODS: From nationwide governmental registries and databases, forensic-toxicological results were retrieved for victims (n = 200) and offenders (n = 105) of homicides committed during the years 2007-2009 and ind

Forensic toxicological implication of an autopsy case of mixed drug overdose involving clomipramine, chlorpromazine and flunitrazepam.

A case of fatal poisoning involving clomipramine, chlorpromazine and flunitrazepam is presented. Quantitative toxicological analysis showed that the concentrations of clomipramine, chlorpromazine and 7-aminoflunitrazepam (a metabolite of flunitrazepam) in the femoral blood were 3.24 microg/ml, 0.36 Kg/ml and 0.61 microg/ml, respectively, and large amounts of drugs were also detected from the stomach contents. We concluded that the cause of death was due to the combined use of clomipramine, chlorpromazine and flunitrazepam.

Four cases of sodium nitrite suicidal ingestion: A new trend and a relevant Forensic Pathology and Toxicology challenge.

Sodium nitrite (NaNO2) is an inorganic compound commonly used as a food additive, antifreeze admixture, and fertilizer. Its toxicity mechanism is mainly represented by the oxidation of ferrous iron to ferric iron of one of the four heme structures in haemoglobin with the onset of methaemoglobin. The mechanism of death by sodium nitrite toxicity is severe hypoxia. We present four cases of suicidal sodium nitrite ingestion that closely occurred within a two months-period. Self-poisoning with sodium nitrite actually represents an increasing trend in nitrates' related deaths. In order to reach a p

Pharmacometabolomic signatures as individualised biochemical response evidence: towards a precision jurisprudence framework for forensic toxicology and criminal responsibility.

Precision medicine is transforming drug response from a population level assumption into an individualised biochemical event. Pharmacometabolomics, which profiles metabolic signatures associated with drug exposure and response, offers a mechanistic basis for explaining interindividual variability in therapeutic efficacy, adverse drug reactions, neurocognitive impairment and toxicological ambiguity. Legal systems, however, continue to evaluate drug related conduct largely through conventional toxicology, generalised impairment thresholds and binary assumptions about intent, voluntariness and ne

Application of a molecular networking approach using LC-HRMS combined with the MetWork webserver for clinical and forensic toxicology.

BACKGROUNDS AND AIMS: In toxicology, LC-HRMS for untargeted screening yields a great deal of high quality spectral data. However, there we lack tools to visualize/organize the MS data. We applied molecular networking (MN) to untargeted screening interpretation. Our aims were to compare theoretical MS libraries obtained in silico with our experimental dataset in patients to broaden its application, and to use the MetWork web application for metabolite identification. METHODS: Samples were analyzed using an LC-HRMS system. For MN, data was generated using MZmine, and analyzed and visualized usi

Identification and quantification of 35 psychotropic drugs and metabolites in hair by LC-MS/MS: application in forensic toxicology.

Despite a non-invasive sampling, hair samples are generally collected in limited amounts for an obvious esthetic reason. In order to reduce the required quantity of samples, a multianalytes method allowing simultaneous identification and quantification of 35 psychoactive drugs was developed. After incubation of 50 mg of hair in a phosphate buffer pH 5 for one night at room temperature, the substances of interest were extracted by a simple liquid-liquid extraction step, with a dichloromethane/ether mixture (70:30, v/v). After evaporation under a gentle stream of nitrogen and reconstitution in f

[Opium alcaloids in toxicological medico-legal practice of Department of Forensic Medicine, Jagiellonian University Medical College].

Most likely, opium was the first narcotic substance discovered at the dawn of mankind. Contemporary drug abuse predominantly poses a social and clinical problem and encompasses among other aspects emergency procedures in cases of intoxication and treatment of addictions. On the other hand, this is also a problem of the judicial system, which implements the rule of apt punishment in criminal cases (rapes, robberies, drivers, production and trade in narcotic substances) and of the necessity of monitoring drug-associates deaths. In all drug-associated cases, investigative capabilities have increa

Postmortem nasal swab screening for cocaine: Preliminary evidence for early toxicological orientation in forensic autopsy practice.

Early toxicological information may be useful during death scene investigation and early autopsy, when decisions regarding sampling, ancillary analyses and investigative priorities must be made before conventional laboratory results are available. This preliminary study evaluated the analytical feasibility of nasal swab testing for postmortem cocaine screening and early toxicological orientation. Nasal swabs were collected during 104 autopsies, extracted in phosphate buffer and analyzed using a rapid immunochemical screening test for cocaine metabolites. Each extract was tested in triplicate.

Studies on the distribution of lidocaine in tissue after spinal and intravenous application in rabbits: clinical and forensic toxicological aspects.

Distribution of lidocaine in tissue was investigated after spinal anesthesia and intravenous application in rabbits to examine aspects of forensic and clinical toxicology. Respiratory arrest occurred at 5-15 min after injection of 5 mg/kg hyper-baric lidocaine solution into the subarachnoid space followed by standing in the head down position. The minimum concentration of lidocaine in the middle part of the cervical spinal cord of the dead rabbits was 197 micrograms/g. On the other hand, maximum concentration in the same site at the critical state of almost complete respiratory arrest was 175
