Fifteen children with segmental forearm fractures were classified into two groups based on their proximal injury pattern. Proximal injury in group A was either Monteggia fractures or its variant and in group B it was diaphyseal fractures. The distal lesions in both these groups were either a metaphyseal fracture or an epiphyseal separation, or a combination of these two. Eleven children of group A were managed conservatively, whereas three of four children in group B required open reduction. After 2.67 years of follow-up in 13 children, clinical outcome based on modified Boyd and Boal's criter

The treatment of complex elbow dislocation fractures is a challenge to both the treating surgeon as well as to the patient due to the complex bony and soft tissue anatomy of the joint. In order to establish an expedient treatment algorithm, all osseous and ligamentous injuries need to be thoroughly assessed. Furthermore, a detailed knowledge of the joint-stabilizing structures, practicable surgical approaches as well as the possible techniques for fracture fixation and/or arthroplasty are essential to facilitate early rehabilitation of the elbow and avoid injury-related complications. Fractures and dislocations of the midfoot and Chopart complex are among the most difficult foot injuries to manage. The treating surgeon is faced with a wide array of treatment challenges. Plain radiographs often grossly underestimate the extent of injury. The anatomy in this region of the foot is quite intricate with numerous articulations. Fractures can occur in isolation or as part of a more complex injury pattern. Misdiagnosis and under treatment can lead to severe alterations of both normal anatomy and function. This article discusses the rationales and techniques for treating these diffi

Radiocarpal fracture-dislocations most often are caused by high-energy trauma. These difficult, uncommon injuries involve significant soft-tissue and osseous trauma, requiring meticulous reduction and fixation. The mechanism of injury is generally a severe shear or rotational insult. Anatomically, the dislocation results in disruption of the radiocarpal ligaments and, usually, both the radial and the ulnar styloid. Understanding the anatomy of the radiocarpal joint is central to understanding the osseous and soft-tissue constraints that are disrupted with a radiocarpal dislocation. Diagnosis c

In 1934, fractures of the middle and distal third of the radius associated with instability of the distal radial ulnar joint (DRUJ) were described by Galeazzi. This type of lesion is characterized by its unstable nature and the need for open reduction and internal fixation to achieve a satisfactory functional outcome. A high index of suspicion should be maintained by the surgeon, and a thorough examination for instability of the DRUJ must be conducted. The marked instability of this fracture-dislocation complex is further enhanced by the disruption of the triangular fibrocartilage complex, eit

The incidence and severity of fractures and dislocations vary depending on their location and on the direction of the forces responsible for the injury. The radiologist's role in evaluation of the injured foot is to recognize the types of injuries produced by particular forces. Because there is considerable overlap of fractures and dislocations of the foot and ankle, evaluation of one area to the neglect of the other can lead to significant oversights and failure to recognize additional injury patterns. For example, fractures of the talus, calcaneus, base of the fifth metatarsal, and to a less

Wrist injuries are and will continue to be one of the most difficult and interesting orthopedic problems encountered by emergency physicians. With a knowledge of basic wrist anatomy, the gathering of a detailed history of the mechanism of injury, thorough physical examination, and appropriate radiography, the physician will be able to make an accurate early diagnosis. This will prevent the development of serious functional problems such as osteonecrosis or malunion with all their attendant pain and disability, which is especially serious for the young active worker or athlete. A thorough radio

Metacarpal fractures are common injuries. Most can be treated successfully by closed reduction and cast or splint immobilization. Unstable fractures, however, require internal fixation. Many such techniques are discussed. Metacarpal dislocations are more difficult to diagnose and treat than are metacarpal fractures. Careful attention to detail will help ensure a successful result.

Although one-eighth to one-tenth of all fractures of the wrist are carpal fractures, they may be overlooked, unless attention is directed particularly to them in diagnostic examination. Symptoms may be slight or lacking, but diagnosis is important because such injuries may give rise to pain or disability later. There are certain guides and procedures in physical and roentgenographic examination which make diagnosis easier and more certain. Treatment depends upon the nature of the lesion. It must be based on all the knowledge obtainable by examination, mental review of the anatomy and pathologi

A variety of injuries can occur to the medial four CMC joints. Chronic problems are best avoided by a combination of attentive diagnostic evaluation, appreciation of unstable injury patterns, and prompt fixation either by percutaneous pinning or open reduction and internal fixation.

A fracture-dislocation of the ankle with fixed anterior displacement of the proximal fibular fragment is reported. The mechanism of this injury was likely eversion of the foot which produced rupture of the deltoid ligament and distal tibiofibular diastasis, and lateral dislocation of the talus. The proximal fibular fragment was probably displaced laterally and returned to a position anterior to the fibular notch of the tibia. This injury was extremely unstable and required open reduction with internal fixation, not only to reduce the anterior displacement of the fibula but also to repair the a

Most carpal fractures are based on the same injury mechanism. In most cases the injury is caused by a fall on the dorsiflexed hand, which is raised as a protective measure, and less frequently by a fall on the clenched fist. The latter leads to passive palmar flexion, pronation and radial inclination of the hand; in the former, the hand is passively superextended by the impact, supinated, and brought into ulnar inclination. The instability caused by injury to various articular structures has four developmental stages. The bones or syndesmoses that are involved depend on the stage and quantitat

BACKGROUND: Our understanding of transolecranon fracture dislocation (TOFD) has evolved to include associated ligament, coronoid, and radial head lesions impacting stability, rather than solely bone stability. Similarly, the understanding of coronoid fracture patterns has progressed. O'Driscoll et al state that basal subtype 2 coronoid fractures correspond to a TOFD, with a fracture passing through the base of the coronoid. In the literature, there is no clear differentiation between basal coronoid TOFD (BC-TOFD) and pure-TOFD outcomes. The main objective of this study is to evaluate the funct

The acetabular depression fracture is defined as a rotated, impacted, osteocartilaginous fragment of the posteromedial acetabulum that occurs in conjunction with a posterior fracture dislocation of the hip. Displacement of this fracture fragment creates incongruity of the posterior acetabular articular surface and the potential for hip joint instability. A retrospective review of hip dislocations over a 3-year period disclosed 75 posterior fracture dislocations of the hip. A total of 71 hips had computerized tomography (CT) scanning after successful closed reduction of the dislocation. Of the

PURPOSE OF THE STUDY: Classicaly Lisfranc fractures dislocations are unusual. Our study is based on 69 observations from January 1974 to January 1992.
MATERIAL AND METHODS: Fractures occured during tremendous impacts with a sex ratio of two men for one woman. The authors insist on the diagnostic value of good quality standard x-rays. Using Trillat's classification, we had 19 homolateral spatular fractures dislocations, 22 homolateral columno-spatular, 8 divergent columnar and 19 divergent columno spatular. A dislocation was reduced in emergency before radiography. The treatment consisted in o

Fractures and fracture dislocations of the forearm are common injuries that are being seen more frequently today in our active population. These injuries can occur in any sport, by direct blows to the arm or by falls on the outstretched hand. Many of these injuries are dramatic in their presentation and easily diagnosed, but only careful physical examination and radiographic evaluation avoids the pitfalls in diagnosis associated with the Monteggia's, Galeazzi's, and Essex-Lopresti's fracture dislocations. Most of these injuries require anatomic reduction to preserve the delicate interaction be

Atlantooccipital dislocation (AOD) is a rare and usually fatal injury. In the current study, the authors reported an extremely rare case of posterior AOD with Jefferson fracture and fracture-dislocation of C6-C7. The patient survived the injury and had only incomplete quadriplegia below the C7 segment with anterior cord syndrome. He was successfully managed with in situ occipitocervical fusion using the Cotrel-Dubousset rod system, corpectomy of C6, and anterior interbody fusion of C5-C7 with plating. To our knowledge, this is the first report of posterior AOD with two other non-contiguous cer
