Outcomes of open reduction and internal fixation of proximal humerus fracture dislocations.

INTRODUCTION: Proximal humeral fracture with associated glenohumeral dislocation (PHFD) is a challenging clinical problem. Outcomes of open reduction and internal fixation (ORIF) of these injuries have not been widely reported. The purpose of this analysis is to report our experience with ORIF of PHFD. METHODS: A retrospective review of our 2 institutions' shoulder surgery databases was conducted to identify all PHFDs that were treated with ORIF from 2008 through 2017. Radiographs were reviewed for fracture healing by 12 weeks postoperatively. All reoperations were recorded. Patient-reported

[Research progress on finite element analysis of internal fixation for clavicular fracture].

Clavicular fracture is a common upper limb fracture. Because of its special anatomical structure, it directly affects the function of the shoulder joint. The different injury mechanisms of clavicular fracture affect the mechanical effect of internal fixation, so the selection of internal fixation method has been puzzling orthopedists. As a modern computer based mechanical analysis, finite element analysis application in the internal fixation of clavicular fracture can not only clarify the pathogenesis of fractures, biomechanical properties of internal fixators and complications of fractures, b

Open Reduction Internal Fixation of a Bimalleolar Ankle Fracture With Syndesmotic Injury.

Rotational ankle fractures are incredibly common, resulting in a wide spectrum of bony and ligamentous injury patterns. After open reduction of an ankle fracture, the treating surgeon must always evaluate syndesmotic stability. If the syndesmosis is determined to be unstable, a reduction of the distal tibiofibular joint should be performed. Failure to adequately identify and treat injuries to the syndesmosis may result in continued ankle instability and poor patient outcomes. Lateral fluoroscopic images are necessary to assess a closed reduction of the syndesmosis before stabilization, althoug

Biomechanical analysis of four types of internal fixation in subtrochanteric fracture models.

OBJECTIVE: To compare the biomechanical properties of four types of internal fixation (proximal femoral nail [PFN], dynamic hip screw [DHS], dynamic condylar screw [DCS], and proximal femoral locking plate [PFLP]) for different types of subtrochanteric fractures. METHODS: Thirty-two antiseptic femurs were randomly divided into four groups. After internal fixation had been implanted, different types of subtrochanteric fracture models were produced and each tested under vertical, torsional and vertical damage loads. RESULTS: The stiffness ratio of PFN in each fracture model and failure load we

Delayed osteonecrosis following reduction and internal fixation of a femoral neck fracture.

Osteonecrosis (ON) is a common complication following femoral neck fracture and most studies have reported the incidence of ON after less than 3 years follow-up. Here we report a case of delayed osteonecrosis 7 years after initial treatment for a femoral neck fracture. This example supports the need for continued clinical and radiographic follow-up over an extended period after femoral neck fracture and suggests that 3 years is too short a time to reliably rule out ON as a postoperative complication.

Complex orbital fracture repair using rigid fixation of the internal orbital skeleton.

Large orbital fractures involving more than one orbital wall are frequently associated with enophthalmos and vertical globe dystopia. The authors repaired 22 orbits in 20 patients using the technique of rigid fixation of the internal orbital skeleton. Eleven orbits were reconstructed with titanium or vitallium implants that were custom-shaped to span the bony defect and fixed to the orbital rim. Seven orbits were reconstructed with bone grafts rigidly supported by a miniplate, and, in four cases, direct lag screw support of bone grafts was used. The intraoperative goal was to restore the nativ

Femoral head vitality in femoral neck fracture after hook-pin internal fixation.

Femoral head vitality was evaluated by tetracycline labeling and 99mTc-MDP scintimetry in 317 femoral neck fractures before and after internal fixation with two hook-pins. The preoperative vitality was about the same as that observed by others using comparable methods. The postoperative vitality was reduced in five of 81 undisplaced fractures and in 76 of 236 displaced fractures. These data would predict a rate of avascular healing complications of 7% in undisplaced and 32% in displaced fractures treated with hook-pins.

Typical nerve damage after fractures due to the topographical proximity of the nerves to the bones, are shown and therapeutic principles are discussed. Primary traumatic lesions are differentiated from so-called "creeping paralysis". The methods of choice are neurolyses, nerve suture or transplantation, each at the appropriate time. Evaluation of 83 of the 128 Patients undergoing microsurgical operations on the nerves in recent years is presented after re-exmination by independent neurologists. One-third showed definite signs of regeneration, more than one-third had good functional results, an

Rigid internal fixation of displaced distal radius fractures.

Distal radius fractures, the most common long bone fracture, are treated in several ways, including closed reduction, percutaneous pinning, external fixation, and open reduction and internal fixation. This article presents a surgical technique and a series of patients treated with a novel minimally invasive intramedullary fixation technique. The implant is a partially flexible intramedullary rod that can be locked in a rigid position once it is implanted in the bone. An awl and reamer are passed through a starter hole in the radial styloid using a 2-cm incision between the 1st and 2nd dorsal c

Supracondylar fractures of the elbow: open reduction, internal fixation.

This principle-based approach for fixing distal humerus fractures has many advantages. Complex fractures are able to be fixed with sufficient stability to permit immediate intensive rehabilitation. Some fractures believed to be unfixable have been fixed satisfactorily by applying the principles outlined in this article. More straightforward fractures are fixed easily using the same techniques. In the author's experience, the stability achieved with this approach is so much greater than that with traditional methods of fixing distal humerus fractures that bone graft has been required only rarel

Salvage of failed internal fixation of intertrochanteric hip fractures.

Most intertrochanteric hip fractures treated with internal fixation heal. If nonunion or early loss of fracture fixation occurs, treatment options include prosthetic replacement and revision internal fixation. The purpose of the current study was to evaluate the results of revision internal fixation and bone grafting for salvage of failed internal fixation of intertrochanteric hip fractures. Between 1981 and 2000, 20 patients with 20 intertrochanteric fractures who had initial internal fixation that failed were treated with revision open reduction and internal fixation and bone grafting.
