Knee surgery, sports traumatology, arthroscopy : official journal of the ESSKA2011eng
PURPOSE: The study was aimed to share the experience of managing posterior cruciate ligament (PCL) avulsion fractures of tibia with open reduction and internal fixation (ORIF). The study also evaluated the effect of delay in treatment and presence of occult PCL injury on the final outcome after surgery.
METHODS: Forty-two patients (30 males and 12 females) with a median age of 26 years (range: 14-53 years) who underwent ORIF through a modified posterior approach for PCL fossa avulsion fractures were assessed after a median follow up of 18 months (range 10-42 months). In 30 patients surgery wa

Three cases of acute dorsal avulsion fracture of the scaphoid are presented. Anatomical and radiological studies indicate that these fractures arise from a ridge on the dorsum of the scaphoid at the distal end of the radial facet. This is not the site suggested by other authors. The clinical significance of these fractures is that they are only visible on the 45 degrees anteroposterior oblique X-ray view of the scaphoid. Without this view they are indistinguishable from X-ray negative scaphoid fractures, since both have similar clinical signs and isotope bone scans.

A 17-year-old male tennis player presented to physical therapy via direct access with complaints of intermittent left lateral foot pain. Due to positive fracture testing and application of the Ottawa foot/ankle rules, an orthopaedist was consulted and radiographic imaging was performed. Radiographs revealed a nondisplaced avulsion fracture of the fifth metatarsal. J Orthop Sports Phys Ther 2019;49(8):620. doi:10.2519/jospt.2019.8534.

The typical injury pattern of a posterior hip dislocation includes posterior wall fracture, capsular injury, and muscular strain. This report presents an example of a posterior wall acetabulum fracture-dislocation associated with complete hip abductor tendon and capsular avulsion from the femur. The patient was found to have an unstable hip despite a small posterior wall fragment due to the extensive soft tissue injury. This was repaired. The patient had improvement in function initially, but ultimately required repair of the hip abductor tendons. To the best of the authors' knowledge, this is

An isolated avulsion fracture involving the femoral origin of the medial head of the gastrocnemius muscle without an associated muscular, meniscal, or ligamentous injury is extremely rare. We report a case of a 14-year-old male wrestler who presented with a radiographically occult avulsion fracture of the medial gastrocnemius tendon sustained during competition. To our knowledge, this is the first case to describe a mechanism of injury as well as to report a return to competition after non-operative management.

Antiphospholipid syndrome and systemic erythematosus have been associated with metatarsal stress fractures. Stress fractures of the Lisfranc joint complex are uncommon injuries but have been reported to occur most frequently in ballet dancers. We present a case of an avulsion fracture of the Lisfranc joint complex that occurred spontaneously. We have reviewed the association between systemic conditions and metatarsal fractures and proposed a series of hypothetical pathological events that may have contributed to this unusual injury.

We report two cases of avulsion fractures at the fovea of the ulna. This injury is caused by a strong distraction force which avulses the insertion of the triangular ligament at the fovea of the ulna. This injury can be classed as an injury to the triangular fibrocartilage complex.

Previously reported avulsion fractures of the calcaneus have been confined to the attachment of the tendo achillis, bifurcate ligament, extensor digitorum brevis muscle, or the plantar fascia. A case report of a previously undescribed avulsion fracture of the calcaneus due to the abductor hallucis muscle is presented. The mechanism of injury, forced dorsiflexion of the forefoot, is described. A diagnostic and treatment regimen is provided. With appropriate identification of the injury, this unusual calcaneal fracture can be adequately managed.

Avulsion fractures of the tibial tuberosity occur mainly during sport activities and are closely related to the strains exerted on the anterior tibial tuberosity by the extension complex of the thigh. A knowledge of the mechanical aspects of these avulsions may improve understanding of the mechanisms of such injuries. In such avulsion fractures, tensile forces due to the contraction of the quadriceps complex overcome the cohesive forces within the apophyseal cartilage. A 1-month cast immobilisation on an extended knee gives good results in the management of nondisplaced fractures. In minor dis

This paper attempts to familiarize the podiatrist with the pathologic entity of avulsion fracture at the extensor digitorum brevis muscle origin and its mechanism of injury, clinical presentation, and treatment. It can be seen on routine radiograms and the problem it presents is one of diagnosis rather than treatment.

Avulsion fractures of the lesser trochanter of the femur are extremely rare injuries in the pediatric population, most commonly resulting from physical activity. The typical mechanism involves a sudden contraction of the iliopsoas muscle, which inserts on the lesser trochanter. We present a rare case of such an injury in a pediatric patient, emphasizing the importance of appropriate imaging and the effectiveness of conservative management in achieving full recovery. An 11-year-old boy presented to the emergency department with perineal trauma sustained after losing balance while riding a bicyc

Tears of the posterior cruciate ligament (PCL) are rare, and avulsion fracture of the tibial attachment of the posterior cruciate ligament is even rarer. These injuries usually occur in accidents such as car crashes, causing acute pain, swelling as well as total functional impotence of the knee. Studies on the incidence of these injuries show variable results, but there appears to be an upward trend. The surgical management of PCL avulsion fracture is not clearly established, although arthroscopic techniques are becoming more popular due to their potential benefits. However, some medical cente

The present study reports a rare case of avulsion fracture of the tibial tuberosity in an adolescent. A 14-year-old male patient sprained his left knee during a soccer match. At the first emergency-room visit, he presented pain in his left knee, 2 +/4+ edema, and inability to walk and flex the affected knee, but no neurovascular changes or signs of compartment syndrome. Radiographs revealed a physeal fracture at the left proximal tibia, classified by Ogden, Tross and Murphy, and modified by Ryu and Debenham, as type IV, and complemented by Aerts et al. as type IV-B. Immobilization was performe

Avulsion fractures compromise function and movement at the affected joint. If left untreated, it can lead to deformity, nonunion, malunion, pain, and disability. The purpose of this review was to identify and describe the epidemiology and available treatment options for common avulsion fractures of the upper and lower extremities. Current evidence suggests that optimal treatment is dependent on the severity of the fracture. Conservative efforts generally include casting or splinting with a period of immobilization. Surgery is typically indicated for more severe cases or if nonoperative treatme

Avulsion fracture of the anterior tibial eminence is an uncommon injury. If bone union does not occur, knee extension will be limited by impingement of the avulsed fragment and knee instability will be induced by dysfunction of the anterior cruciate ligament (ACL). This report describes a 55-year-old woman who experienced an avulsion fracture of the right anterior tibial eminence during recreational skiing. Sixteen months later, she presented at our hospital with limitation of right knee extension. Plain radiography showed nonunion of the avulsion fracture region, and arthroscopy showed that t

OBJECTIVE: To assess a causal relationship between physical activity or boney surgical intervention and the occurrence of avulsion fracture in the pelvis. Secondarily to assess the average age at which avulsion fracture occurs in cases associated with physical activity or boney surgery.
METHOD: A literature search was performed on a variety of databases using text words and MeSH terms. Results were limited to English language. Cases involving trauma or pathological disease were excluded. Causation Criteria scores were calculated for each paper to establish a link between the suspected mechani

PURPOSE OF THE STUDY: Injury to the posterior cruciate ligament (PCL) is relatively rare and, if combined with avulsion fracture of the PCL insertion site, it accounts for only a small number of knee injuries. This is why PCL avulsion fracture is an easily missed diagnosis resulting in knee instability and arthritis development. The aim of this study was to show the necessity of surgical treatment of these injuries.
MATERIAL: Between January 2004 and September 2005, four patients with PCL injury underwent surgery. Three had avulsion fracture at the PCL insertion site, and in one the avulsion
