Orbital floor reconstruction is the most challenging component in the midfacial trauma management. Most often owing to the complexity of the fractures, the floor reconstruction requires grafts or other substitutes. Literature reveals several sources of autogenous sources of such grafts. Though most of the grafts are well taken and gives an ideal result, at certain instances, owing to the complex nature of the graft, its biochemical nature, reaction to the grafting, biochemical response, a reactionary change may result at late stages. The aim of this manuscript is to present a rare instance of

Crystal distribution in articular cartilage of femoral heads resected at hip arthroplasty for fracture of the femoral neck was examined in 10 patients (77-91 years) by computerised image analysis of electron micrographs. Crystal content of the zenith did not differ from that of the infrafoveal region of the femoral head in the superficial 0.5 mm of the cartilage. Crystal area density (percentage area of the section occupied by crystal profiles) was higher in a zone 0-50 microns than in the subjacent zone 50-500 microns from the articular surface, particularly in the zenith of the femoral head.

In vivo rabbit models that deliver mechanical impacts to knee cartilage are important for studying injury-induced osteoarthritis, but variability in impact characteristics limit the repeatability of the models. In this study, the mechanical impact load delivered to a cadaveric rabbit posterior medial condyle was characterized, with particular emphasis on the influence of placement of the k-wire that stabilized the joint during impact. K-wire position was not prescribed and varied across specimens (7.85 ± 2.28 mm anterior and 2.61 ± 3.47 mm distal to the impact site) and was treated as a source

CASE: An 18-year-old adolescent boy presented with knee pain and stiffness secondary to tibial plateau valgus malunion and osteochondral defect, 8 months after initial injury/fixation. We opted for a novel technique that reconstructs the convex lateral tibial plateau by using osteotomy and an osteochondral autograft harvested from the lateral aspect of the ipsilateral femoral condyle.
CONCLUSION: The reported novel reconstruction technique is inexpensive, achievable with routine techniques, and demonstrated a favorable short-term outcome. At 3 years of follow-up, the patient had excellent, as

OBJECTIVE: To study clinical effects of multi-point fixation with bioabsorbable cartilage screws for the treatment of osteochondral fracture caused by patellar instability.
METHODS: During the period from August 2014 to Steptember 2016, there were 21 patients with osteochondral fracture caused by patellar dislocation, including 7 males and 14 females, ranging in age from 12 to 42 years old, with an average of 27 years old. During the operation, free fractures fragments of cartilage were found with a open surgery, and the patients were treated with fixation by absorbable cartilage screws combi

Bilateral zygomatic arch fractures are uncommon but may result in significant midfacial deformity and functional impairment, particularly when fractures are comminuted and structural continuity is compromised. This report describes the surgical management of a 49-year-old male who sustained severe facial trauma following a mountain-climbing accident, resulting in comminuted bilateral zygomatic arch fractures and marked midfacial disfigurement. Due to the complexity of the injury and loss of structural support, autogenous costal cartilage grafting combined with titanium mesh fixation was select

PURPOSE: Retrograde intramedullary screw fixation for metacarpal fractures has become increasingly popular; however, the technique requires violation of the extensor mechanism at the metacarpophalangeal (MCP) joint and penetration of the metacarpal articular surface. Although prior studies have quantified the proportion of metacarpal articular surface involved in screw placement, none have evaluated how the resulting cartilage defect articulates with the proximal phalanx at the MCP joint as a potential source of altered wear patterns.
METHODS: In a cadaveric study, six specimens (24 metacarpa

As a treatment for humeral transcondylar fractures, conservative therapy is prone to false joints. Although various surgeries are available, plate fixation is highly invasive to soft tissues and prone to contracture of the elbow joint, and percutaneous pinning results in weak fixation and tends to damage the ulnar nerve. In this report of two cases, we describe a new transarticular cartilage retrograde intramedullary pinning surgical method that is less invasive to soft tissues, has an accurate reduction and strong fixation, and does not damage the ulnar nerve. This surgery was performed on tw

The most commonly used techniques for frontal sinus obliteration involve the implantation of an autogenous tissue graft: either fat, muscle or bone. Lyophilized allogenic cartilage due to its unique properties, such as the tendency to ossification and resistance to volume reduction, can be used as the material of choice for sinus obliteration. A clinical and radiological study of 66 patients operated on for frontal sinus fractures, between January 1 1988 through December 31 1995 was undertaken. Variables recorded included the aetiological factors, the clinical and radiological fracture feature

A 9-month-old male entire flat-coated retriever was referred for investigation of chronic right pelvic limb lameness following trauma. Radiographs and contrast-enhanced CT confirmed a right femoral head Salter-Harris type III fracture with monostotic metaphyseal mixed-type lysis and joint effusion. An aggressive process due to underlying neoplasia or infection with secondary pathological fracture could not be excluded. The dog underwent surgical bone biopsy and, consequently, right femoral head and neck ostectomy (FHO). Histopathology described retained cartilage cores within the trabecular bo

Palatal bone fractures are often associated with vertical maxillary fractures, but direct reduction of the palatal bone itself is rarely performed. We report a case in which a high-arched narrow palate deformity following palatal bone fracture associated with multiple facial fractures was successfully corrected by grafting costal cartilage chips to the hard palate. The patient was a 25-year-old man. At the age of 19, he sustained a cervical spinal cord injury at the C4 level and multiple facial fractures-including maxillary fracture with zygomatic and palatal bone fractures, as well as mandibu

BACKGROUND: To repair orbital medial wall fractures, otorhinolaryngologists often use the silicone sheet technique by inserting an inverted U-shaped silicone sheet into the middle meatus after the removal of fractured bones, and packing gauze inside the silicone sheet for several days to fix the shape of the medial wall. However, this method does not sufficiently reduce the orbital content.
OBJECTIVE: To describe the surgical procedure to repair medial wall fractures using nasal septal cartilage as the reconstructive material.
METHODS: First, endoscopic septoplasty is performed. Although the

Posttraumatic arthritis commonly develops following articular fracture. The objective of this study was to develop a closed joint model of transarticular impact with and without creation of an articular fracture that maintains the physiologic environment during loading. Fresh intact porcine knees were preloaded and impacted at 294 J via a drop track. Osteochondral cores were obtained from the medial and lateral aspects of the femoral condyles and tibial plateau. Chondrocyte viability was assessed at days 0, 3, and 5 postimpact in sham, impacted nonfractured, and impacted fractured joints. Tota

OBJECTIVE: To study the clinical effect of Treatment of blow-out fracture of medial orbital wall with nasoseptal cartilage under nasal endoscope.
METHOD: Under a nasal endoscope, the fracture and the prolapsed orbital contents were reduced to the orbit, and then an autogenous nasoseptal cartilage was grafted into the orbital defect. The variations in the visual acuity, diplopia, enophthalmos degree and eyeball position were detected preoperatively and postoperatively.
RESULT: During the follow up of three months to four years after operation, all the 28 cases showed neither loss nor distinct

A new surgical technique for the treatment of recurrent temporomandibular joint subluxation or dislocation is described. Following a horizontal osteotomy and down-fracture of the articular eminence an inter-positional bovine cartilage xenograft is inserted in order to augment the vertical height of the eminence. The procedure combines simplicity with minimal post-operative morbidity. The increase in eminence height is both predictable and stable.

Attempts to analyze clinical unexpected cases of laryngeal fractures based on mild symptoms and physical findings. Even though a laryngeal fracture is rare in a living patient and when happens it is related to severe airways emergency, this case reported seems important to offer a possible differential diagnosis in patients who present with the same symptoms, if no other causes have been detected, like another one benign laryngeal disease.

We describe using an osteochondral autograft from the lateral femoral condyle of the knee to treat a symptomatic die-punch lesion of the lunate facet of the distal radius. An 18-year-old woman who sustained a distal radius fracture remained symptomatic after nonoperative treatment and diagnostic wrist arthroscopy with microfracture. We used a commercial harvesting system to transfer an osteochondral plug into a cartilage defect involving the lunate facet of the distal radius. At final follow-up, 34 months after surgery, the patient was assessed with a visual analog scale (VAS) and Disabilities

BACKGROUND: Cardiac rupture is rare but has a high mortality rate. Cardiac rupture due to blunt trauma is generally caused by the heart being pinched into the bony thorax.
CASE PRESENTATION: A 90-year-old man in shock was transported to the hospital after his precordium was stepped on by a cow. He developed right atrial rupture from penetration of the heart by fractured and calcified costal cartilage, with massive hemothorax, hemorrhagic shock, and respiratory failure. We immediately performed cardiac injury repair. The patient was discharged without sequelae.
CONCLUSION: Emergency physician

GαS, the stimulatory G protein α-subunit that raises intracellular cAMP levels by activating adenylyl cyclase, plays a vital role in bone development, maintenance, and remodeling. Previously, using transgenic mice overexpressing GαS in osteoblasts (GS-Tg), we demonstrated the influence of osteoblast GαS level on osteogenesis, bone turnover, and skeletal responses to hyperparathyroidism. To further investigate whether alterations in GαS levels affect endochondral bone repair, a postnatal bone regenerative process that recapitulates embryonic bone development, we performed stabilized tibial oste
