Closed reduction methods for treating distal radial fractures in adults.

BACKGROUND: Fracture of the distal radius is a common clinical problem, particularly in older white women with osteoporosis. Displaced fractures are usually reduced using closed reduction methods, which are non-surgical and generally comprise traction and manipulation, and the resulting position stabilised by external means, typically plaster cast immobilisation. OBJECTIVES: To examine the evidence for the relative effectiveness of different methods of closed reduction for displaced fractures of the distal radius in adults. SEARCH STRATEGY: We searched the Cochrane Musculoskeletal Injuries G

Old principles revisited--traction splinting for closed proximal phalangeal fractures.

Proximal phalangeal fractures are commonly caused by a blunt injury to the dorsum of the hand. This typically results in a closed transverse fracture with volar angulation, which is an inherently unstable fracture pattern. K-wiring and internal fixation have the disadvantage of damage to the soft tissue envelope, risk of infection and wire loosening. We describe 15 patients treated with an alternative method of skin traction and splintage in our regional trauma centre. Fractures were reduced under X-ray and reduction was maintained using a combination of a malleable metal splint and tape to pr

Segmental fractures of the femur treated by closed intramedullary nailing.

Twenty acute segmental fractures of the femoral shaft were treated with closed intramedullary nailing. The severity of the soft-tissue damage and associated injuries necessitated careful preoperative care. The surgical technique and means of reduction are outlined. Follow-up of at least one year on all patients showed a 100 per cent rate of union with no infections. Major complications included splitting of the greater trochanter during nailing (one case) and an 11-degree valgus deformity (one case). Maximum shortening was 1.5 centimeters. Knee flexion averaged 135 degrees in these acute fract

Closed Reconstruction of Depressed Frontal Sinus Fractures: An Innovative Technique.

This article describes a novel technique for reconstructing a frontal sinus fracture that minimizes the risks associated with conventional approaches. In this study, we report a patient presented with a depressed frontal sinus fracture following a car accident. Through a small (5 mm) sub-brow incision, a freer elevator was inserted beneath the fractured fragments to elevate them. A 16-gauge venous catheter was then introduced horizontally from the medial aspect of the bone, exiting at the lateral side. Following this, a 24-gauge wire was threaded through the catheter, which was subsequently re

Femoral Artery Injuries in Closed Femur Shaft Fractures: Case Report.

Case 1 and 2  Two young male patients, sustained injury to the superficial femoral artery (SFA) following a closed femur shaft fracture. The arterial injuries were confirmed by computed tomography angiography and both underwent fracture fixation and on SFA exploration; a thrombosed arterial segment was noted at the fracture site, addressed with arteriotomy and thrombectomy to restore the vascularity. At 1-year follow-up, both patients had good union at the fracture site and a well-perfused limb. Conclusion  Thorough clinical examination and appropriate diagnostic studies can diagnose these rar

Open Vs. Closed Reduction in Type 2 Lateral Condyle Fractures.

BACKGROUND: Lateral condyle fractures are associated with high morbidity due to their risk of nonunion and avascular necrosis (AVN). This study aims to assess the outcomes between closed reduction and the more traditional open techniques for operative fractures. METHODS: All lateral condyle fractures that required operative fixation (pins or screws) over a ten-year period were included. We compared open versus closed reduction for OR time, infection rate, AVN, nonunion, premature physeal closure, ulnohumeral angle, and interepicondylar width (IEW). RESULTS: 28 patients were identified in the

Closed reduction of severely displaced radial neck fractures in children.

BACKGROUND: Severely displaced radial neck fractures in skeletally immature children are rare and can be difficult to reduce. The purpose of this study is to evaluate the results using our reduction maneuver. METHODS: From October 2011 to December 2015, 26 children with radial neck fractures(O'Brien type II, III and Judet type III, IV) were treated at our institute. All patients underwent percutaneous K-wire leverage and radial intramedullary pinning in an average surgery time of 35 mins (15-80 min). The injured arm was immobilized at the functional position with plaster for 4-6 weeks, evalua

Closed Reduction and Percutaneous Fixation of Calcaneal Fractures in Children.

Open reduction and internal fixation has been widely used to treat displaced intra-articular calcaneus fractures in children. However, the complications of surgical trauma and the wound created through the extended lateral approach cannot be ignored. This study analyzed the outcomes of displaced intra-articular calcaneal fractures in children treated with closed reduction and percutaneous fixation. Medical records of pediatric patients who had displaced intra-articular calcaneus fractures and underwent closed reduction and percutaneous fixation at the study institution between January 2008 and

Open versus closed reduction: diacapitular fractures of the mandibular condyle.

PURPOSE: The purpose of the study was to review the literature regarding the evolution of current thoughts on management of diacapitular fractures (DFs) of the mandibular condyle. METHODS: An electronic search in PubMed was undertaken in March 2012. The titles and abstracts from these results were read to identify studies within the selection criteria. Eligibility criteria included studies reporting clinical series of DFs, including both animal and human studies, without date or language restrictions. RESULTS: The search strategy initially yielded 108 references. Twenty-eight studies were id

Closed intramedullary fixation for metacarpal fractures using J-shaped nail.

The purpose of this study is to present the results of a new intramedullary fixation technique for metacarpal fractures. A J-shaped nail, which is a curved 2.0 mm diameter Kirschner wire sharply bent at the proximal end, was inserted from the dorsal aspect of the metacarpal base. Twenty-one metacarpal fractures (five transverse shaft fractures and 16 neck fractures) in 19 hands of 18 patients with were operated by this technique. All fractures had successful unions. Clinical and radiographic results were excellent in all ten hands that had excluded concomitant injuries. There was no mechanical

Closed reduction and percutaneous fixation of anterior column acetabular fractures.

OBJECTIVE: The current standard treatment of anterior column acetabular fractures includes formal open reduction with internal fixation (ORIF) through a variety of anterior approaches. These approaches have been associated with significant blood loss, infection, lengthy operative times, and neurovascular complications. It therefore seems reasonable to consider less invasive alternatives to conventional treatment methods. A technique for percutaneous reduction and fixation of a particular acetabular fracture pattern is presented. Execution of this technique has been facilitated by the use of im

Closed reduction of mallet fractures using extension-block Kirschner wire.

Fourteen cases of mallet fingers with a displaced large fracture fragment and/or a subluxated distal phalanx were treated by closed reduction using extension-block Kirschner wire. The follow-up evaluations took place after a mean of 9 months. The results according to Crawford's criteria were eight excellent, four good, one fair, and one poor. This technique is simple, is easier than other techniques for reduction of mallet fractures, and is associated with a low morbidity.

The Lottes nail in the closed treatment of tibia fractures.

The effectiveness of the Lottes nail was evaluated in 63 consecutive cases of tibia fractures treated by closed intramedullary fixation with image intensification. The procedure was offered to all adults except those with Gustillo and Anderson Type III open wounds and with fractures less than 10 cm from the knee or 5 cm from the ankle. The operations were generally performed within a few days in closed fractures and somewhat later in open fractures. Patellar-tendon-bearing casts were applied five to ten days after operation, and weight-bearing to tolerance was emphasized. All fractures united.
