Effect of different dietary fats on inflammation and glucose intolerance in high fructose and high fat fed experimental animals.

OBJECTIVES: Diet is the major modifiable risk factor for the onset of insulin resistance and its progression into diabetes. In the present study the effect of various dietary fats on inflammatory homeostasis and glucose tolerance is investigated in high fat and high fructose fed mice model. METHODS: C57/BL6J mice were divided into four groups and fed a casein-based diet containing high fructose (45%) and high fat (24%) (clarified butter oil [CBO]; safflower oil [SFFO] and lard oil [LO]) for 120 days; oral glucose tolerance (OGTT), plasma lipid profile and plasma & adipose tissue cytokines lev

Small Intestinal Bacterial Overgrowth May Increase the Likelihood of Lactose and Sorbitol but not Fructose Intolerance False Positive Diagnosis.

BACKGROUND: Small intestinal bacterial overgrowth (SIBO) is defined as a bacterial count of more than 105 colony-forming units per milliliter in duodenal aspirate. It shares many symptoms with carbohydrate intolerance, which makes the clinical distinction of the disorders difficult. The aim of the study was to examine the relationship between a positive carbohydrate breath test and the presence of SIBO suggested by a positive lactulose hydrogen breath test. METHODS: The electronic database of the gastroenterology laboratory of a tertiary medical center was searched for all patients clinically

[Hereditary fructose intolerance (HFI) as cause of isolated gamma GT rise in a 5-year old boy with hepatomegaly].

The diagnosis of HFI is easily missed during childhood. It should be suspected in children presenting with hepatomegaly and an isolated increase in GGT. A carefully taken nutritional history forms the basis of the diagnosis of HFI which can be confirmed by molecular analysis with a sensitivity of > 95%. I.v. fructose tolerance tests and liver biopsies often can be omitted.

Glucose intolerance induced by early estrogen deprivation and fructose-rich diet does not impair heart function in female rodents.

It was previously shown that a fructose-rich diet (FRD) induces prediabetes, cardiac dysfunction, and hypertrophy (CH) in males. We assessed FRD and estrogen depletion on female metabolism and cardiovascular function. Females on FRD or control diet (CD) did not develop prediabetes or cardiac dysfunction, although FRD-fed mice showed CH vs. CD. One month of ovariectomy (OVX) did not induce prediabetes, but FRD impaired glucose tolerance in OVX mice without additional metabolic or cardiac changes. Calcium transient amplitude decreased in OVX-FRD vs. SHAM-FRD, with delayed decay, suggesting reduc

When Fruit Turns Harmful: Late Diagnosis of Hereditary Fructose Intolerance in a Pediatric Patient-A Case Report and Literature Review.

INTRODUCTION: Hereditary fructose intolerance is a rare but potentially severe and fatal disorder if it is not recognized promptly. It is caused by biallelic mutations in the ALDOB gene, which encodes the Aldolase B enzyme. Deficiency leads to intracellular accumulation of fructose-1-phosphate, causing secondary inhibition of gluconeogenesis and glucogenolysis and, consequently, hypoglycemia along with hepatic and renal dysfunction. The clinical presentation is variable and often nonspecific, making diagnosis challenging. CASE REPORT: A 12-year-old boy, born small for gestational age, with a

Lipidomics uncovers metabolic manifestations related to liver steatosis and low-grade systemic inflammation in diet-treated hereditary fructose intolerance patients.

BACKGROUND AND AIMS: Hereditary Fructose Intolerance (HFI), a rare autosomal recessive metabolic disorder, has historically been considered benign when treated with a lifelong fructose-, sucrose and sorbitol-restricted diet. However, adverse metabolic manifestations have recently been reported in treated HFI patients. As serum metabolomics offers a valuable tool for assessing metabolic manifestations underlying inherited metabolic disorders, we aim to compare the serum lipidomic profile of HFI-treated patients with age-, gender-, and body mass index-matched healthy controls. METHODS: Long-ter

Extra increase in exhaled hydrogen during fructose malabsorption breath tests is linked to additional food intolerance/malabsorption and/or Helicobacter pylori.

BACKGROUND & AIMS: Food intolerance/malabsorption, including fructose malabsorption (FM), histamine intolerance (HIT), lactose intolerance (LIT), and Helicobacter pylori (H. pylori), may present with symptoms similar to symptoms of the irritable bowel syndrome (IBS) spectrum. We aimed to investigate whether extra food intolerances/malabsorption and H. pylori infection affect the results of hydrogen breath tests in FM patients. METHODS: A hydrogen (H2) breath test was conducted for evaluating FM and LIT. A serum diamine oxidase value determination, a search for H. pylori and antibodies to tiss

Hereditary fructose intolerance mimicking a biochemical phenotype of mucolipidosis: A review of the literature of secondary causes of lysosomal enzyme activity elevation in serum.

We describe a patient with failure to thrive, hepatomegaly, liver dysfunction, and elevation of multiple plasma lysosomal enzyme activities mimicking mucolipidosis II or III, in whom a diagnosis of hereditary fructose intolerance (HFI) was ultimately obtained. She presented before introduction of solid foods, given her consumption of a fructose-containing infant formula. We present the most extensive panel of lysosomal enzyme activities reported to date in a patient with HFI, and propose that multiple enzyme elevations in plasma, especially when in conjunction with a normal plasma α-mannosidas

Comparative study of Streptococcus mutans laboratory strains and fresh isolates from carious and caries-free tooth surfaces and from subjects with hereditary fructose intolerance.

This study was undertaken to investigate and compare some biochemical and physiological properties related to sugar metabolism of 4 laboratory strains and 13 freshly isolated strains of Streptococcus mutans from carious and caries-free tooth surfaces and from subjects with hereditary fructose intolerance. Growth in Trypticase (BBL Microbiology Systems)-yeast extract in the presence of various sugars was almost the same for all of the fresh isolates, which grew generally better than the laboratory strains. This was especially noticeable on sucrose where the fresh isolates (including those isola

Hepatic but not Intestinal FBP1 Is Required for Fructose Metabolism and Tolerance.

Fructose intolerance in mammals is caused by defects in fructose absorption and metabolism. Fructose-1,6-bisphosphatase 1 (FBP1) is a key enzyme in gluconeogenesis, and its deficiency results in hypoglycemia as well as intolerance to fructose. However, the mechanism about fructose intolerance caused by FBP1 deficiency has not been fully elucidated. Here, we demonstrate that hepatic but not intestinal FBP1 is required for fructose metabolism and tolerance. We generated inducible knockout mouse models specifically lacking FBP1 in adult intestine or liver. Intestine-specific deletion of Fbp1 in a

An experimental renal acidification defect in patients with hereditary fructose intolerance. II. Its distinction from classic renal tubular acidosis; its resemblance to the renal acidification defect associated with the Fanconi syndrome of children with cystinosis.

In adult patients with hereditary fructose intolerance (HFI) fructose induces a renal acidification defect characterized by (a) a 20-30% reduction in tubular reabsorption of bicarbonate (T HCO(3) (-)) at plasma bicarbonate concentrations ranging from 21-31 mEq/liter, (b) a maximal tubular reabsorption of bicarbonate (Tm HCO(3) (-)) of approximately 1.9 mEq/100 ml of glomerular filtrate, (c) disappearance of bicarbonaturia at plasma bicarbonate concentrations less than 15 mEq/liter, and (d) during moderately severe degrees of acidosis, a sustained capacity to maintain urinary pH at normal minim

Effects of dietary education, followed by a tailored fructose-restricted diet in adults with fructose malabsorption.

OBJECTIVE: Fructose is absorbed by GLUT transporters in the small intestine. If this process is inadequate, abdominal symptoms because of fructose intolerance may arise. The effect of a tailored fructose-restricted diet on gastrointestinal complaints was assessed in patients with fructose intolerance. MATERIALS AND METHODS: Following an abnormal fructose breath test (50 g), 107 patients (64 also with lactose intolerance) entered three study periods: weeks 0-32 (free diet), weeks 32-36 (progressive increasing amount of fructose up to quantity inducing symptoms, 'trigger dose'), and weeks 36-48

IS THERE A RELATIONSHIP BETWEEN INTESTINAL FRUCTOSE MALABSORPTION, GASTROINTESTINAL SYMPTOMS AFTER FRUCTOSE OVERLOAD, AND BODY ADIPOSITY IN ADOLESCENTS?

BACKGROUND: Over the past several decades, dietary fructose intake has increased significantly, along with the prevalence of obesity. Fructose malabsorption is associated with gastrointestinal symptoms and may lead to fructose intolerance. Evidence suggests a probable association between intestinal fructose malabsorption and increased body adiposity. OBJECTIVE: To analyze the prevalence of intestinal fructose malabsorption and intolerance in asymptomatic adolescents and to investigate their relationship with fructose intake, body adiposity, systemic metabolic indicators, and intestinal permea

Repeatability and effect of blinding of fructose breath tests in patients with functional gastrointestinal disorders.

BACKGROUND: Breath tests are used as measures of sugar intolerance or malabsorption in patients with functional gastrointestinal disorders (FGID), although the repeatability or anticipatory bias have not been adequately studied. We examined the repeatability and anticipatory bias during fructose breath testing using a nocebo-controlled, randomized, cross-over, and double-blind study design. METHODS: Gastrointestinal symptoms and breath concentrations of hydrogen and methane were documented during breath tests with fructose (given open twice and blinded once), water (blind neutral nocebo) and

Lactose and fructose malabsorption in children with recurrent abdominal pain: results of double-blinded testing.

AIM: To investigate malabsorption of lactose and fructose as causes of recurrent abdominal pain (RAP). METHODS: In 220 children (128 girls, mean age 8,8 [4.1-16.0] years) with RAP, hydrogen breath tests (H(2) BT; abnormal if ΔH(2) > 30 ppm) were performed with lactose and fructose. Disappearance of RAP with elimination, recurrence with provocation and disappearance with re-elimination, followed by a 6-month pain-free follow-up, were considered indicative of a causal relation with RAP. For definite proof, a double-blinded placebo-controlled (DBPC) provocation was performed. RESULTS: Malabsorp

Food malabsorption/intolerance complaints triggered by primary epiploic appendagitis.

Primary epiploic appendagitis (PEA) is an uncommon and self-limiting cause of acute or subacute abdominal complaints. The diagnosis of PEA, with its characteristic appearance, is made with computed tomography (CT). This report describes a patient seven months after a CT-confirmed diagnosis of PEA. Because of persistent and recurring, functional, non-specific abdominal complaints, food intolerance/malabsorption was investigated. Fructose malabsorption combined with histamine intolerance was found. A registered dietician helped develop an individually-tailored diet to address the problem. Within

When Long-Lasting Food Selectivity Leads to an Unusual Genetic Diagnosis: A Case Report.

Hereditary fructose intolerance is an autosomal recessive disorder of fructose metabolism caused by catalytic deficiency of aldolase B enzyme [1]. The disease is typically expressed when fructose- and sucrose-containing foods are first introduced in the diet; acute manifestations include nausea, vomiting, abdominal distress, and symptomatic hypoglycemia [1,2]. Chronic fructose ingestion eventually leads to poor feeding, growth retardation and gradual liver and/or renal failure [3,4]. Some patients may remain undiagnosed until adulthood because of a self-protective avoidance of sweet tasting fo
